In treating ear disease every otolaryngologist is familiar with topical medications. The choice of the ideal agent for a given clinical situation may be difficult. In order to make this decision more easy, we overlook the composition of ear-drops. The most important disadvantages are ototoxicity, allergy and pain. The place of ototopical agents in otitis externa, otitis media and otomycosis is discussed.

Interleukin 1 has been implicated as a mediator of both systemic and local responses to infection and injury. Since systemic and local vasodilatation are hallmarks of sepsis and infection, we studied the direct effect of IL-1 on vascular contractility. Twenty-five patients with seborrhoeic dermatitis localised to the face were treated with bifonazole cream once daily for 4 weeks. Twenty-one (84%) were free of lesions, 2 improved and in 2 no improvement was seen. Bifonazole is not only an effective antimycotic agent, it also has anti-inflammatory activities and this combination may be beneficial in the treatment of seborrhoeic dermatitis.

A mouse model of localized candidosis in air-filled subcutaneous cysts imitating thrush has been developed and was used to test the efficacy of various antifungal drugs. Serum binding of 8-methoxypsoralen (8-MOP) was studied by equilibrium dialysis. In therapeutic concentrations, 8-MOP binding in serum was high, 91.4%, and constant, indicating concentration-independent kinetics. Forty patients with skin diseases were treated by the simultaneous application of three creams, the respective bases of which were beclomethasone dipropionate, sodium fusidate and ketoconazole. The treatment yielded positive results in 92.5% of the treated cases with good relief of symptoms, arrest of growth of pathogenic agents, and good local and systemic tolerance. No adverse reactions of any kind were observed.

An investigator blind trial was performed comparing bismuth salicylate, erythromycin ethylsuccinate, and placebo in the treatment of Campylobacter pyloridis associated gastritis in patients without peptic ulceration. Forty-two infants and young children with skin diseases of various kinds were treated by the simultaneous application of a combination of three creams, the respective ingredients of which were beclomethasone dipropionate, sodium fusidate and ketoconazole. The treatment produced good results in 97.6% of cases, with eradication of pathogenic agents and satisfactory relief of symptoms. The combination was well tolerated in almost all cases, both locally and systemically, and no adverse reactions were reported.

Forty out-patients suffering from a variety of skin diseases were treated over a period of 7 to 14 days (mean 9.6 days) with twice-daily applications of an extempore combination of sodium fusidate, clobetasone butyrate and ketoconazole creams. The severity of symptoms was assessed before, after 5 days and at the end of treatment and an overall evaluation made of response to therapy. The findings indicated that there was clinical cure or significant improvement in 37 (92.5%) of the patients, with excellent tolerance and no reports of any adverse reactions.
