Oxidative stress drives disuse-induced chronic pain through sex-divergent immune-neural mechanisms.

Physical disuse is a major risk factor for complex regional pain syndrome type I. Using a rat chronic post-cast pain model, we investigated how oxidative stress modulates pain processing across the neuraxis and evaluated sex as a biological variable. Following immobilization, both sexes developed widespread mechanical hyperalgesia, which was markedly attenuated by the antioxidant Tempol. However, we uncovered a striking phenotypic dissociation: whereas male rats also developed tactile and cold allodynia, female rats completely lacked tactile allodynia. Peripherally, Tempol reduced neurogenic i

Exploratory Analysis of APOE ε4-Stratified Associations Between Plasma Alzheimer's Biomarkers and Retinal Imaging Metrics in Cognitively Normal Adults.

PURPOSE: Retinal imaging offers a noninvasive window into microvascular and neurodegenerative changes, yet its relationship to circulating plasma biomarkers of Alzheimer's disease (AD) remains incompletely understood. We evaluated associations between plasma AD biomarkers and retinal imaging metrics in cognitively normal adults and whether these differ by apolipoprotein E (APOE) ε4 status. METHODS: This cross-sectional analysis included cognitively normal participants prospectively enrolled from the Duke/UNC Alzheimer's Disease Research Center. Eighty-three eyes from 44 participants (20 APOE

Axial Length-Adjusted Normative Reference Values for Retinal Layer Thickness in a Population-Based OCT Study.

PURPOSE: To establish axial length-adjusted normative reference values for retinal layer thickness using population-based optical coherence tomography (OCT) data and evaluate their clinical implications in highly myopic eyes. METHODS: This cross-sectional study used Korea National Health and Nutrition Examination Survey 2017-2021 data. Participants aged ≥19 years with reliable OCT and axial length measurements and without major ophthalmic disease were included. Sampling-weighted quantile regression adjusted for age and sex estimated axial length-specific percentiles for peripapillary retinal

Dopamine suppresses pathological retinal oscillations and enhances the signal-to-noise ratio.

Infantile nystagmus is a debilitating involuntary eye movement disorder often associated with retinal diseases such as congenital stationary night blindness (CSNB). The oscillating eye movements of infantile nystagmus come with reduced visual acuity and strongly impair quality of life. No cure exists for this condition. Previously, we demonstrated that nystagmus in the CSNB mouse model Nyxnob has a retinal cause. Specifically, we found that synchronized oscillations of retinal ganglion cells (RGCs) are transmitted to the accessory optic system, triggering compensatory eye movements. The RGC os

Small Bowel Obstruction Following Laparoscopic Sacrocervicopexy Involving Barbed Sutures: Case Report and Focused Review.

INTRODUCTION AND HYPOTHESIS: Barbed sutures are increasingly used in minimally invasive pelvic reconstructive surgery but have been associated with rare postoperative complications. We present a case of delayed small bowel obstruction (SBO) due to barbed-suture entrapment after laparoscopic sacrocervicopexy, and review the current literature to characterize the presentation, mechanisms, and management of this complication. METHODS: We retrospectively reviewed the clinical course, imaging, operative findings, and outcomes of a patient who developed SBO following manual laparoscopic sacrocervic

[The "simple" laparoscopic cholecystectomy as a hybrid DRG in 2026 : Critical appraisal and practical orientation].

BACKGROUND: With approximately 175,000 procedures per year (IQTIG, reporting year 2024), laparoscopic cholecystectomy is among the most common visceral surgical procedures in Germany. Since 2026, "simple" laparoscopic cholecystectomy (H08M) has been included in the cross-sectoral hybrid diagnosis-related groups (DRG) scheme under § 115f of the German Social Code Book V (SGB V). OBJECTIVE: To critically analyze the hybrid DRG classification with respect to the underlying evidence, coding/grouping logic, real-world care patterns and economic implications, including a model calculation of the ex

Stage-Based Model of User Engagement Patterns in an Online Health Community for Cardiovascular Disease Management: Qualitative Interview Study.

BACKGROUND: Online health communities (OHCs) provide vital peer support and health information to individuals managing chronic conditions. However, sustained user engagement remains challenging, with many users reducing activity over time despite the ongoing benefits these platforms offer. While some individuals may improve or become more knowledgeable, sustained engagement remains critical because ongoing participation fosters trust, peer support, and continuous access to evolving health information that may persist beyond initial recovery or learning. Hence, it is important to consider how c

Brain structural covariance networks in co-occurring PTSD and alcohol use disorder: An ENIGMA PTSD and ENIGMA addiction working group collaboration.

High rates of comorbid posttraumatic stress disorder (PTSD) and alcohol use disorder (AUD) may stem from a shared neurobiological basis; however, research exploring this overlap remains limited. We examined whole-brain structural covariance (SC) across PTSD, AUD, comorbid PTSD & AUD, trauma-exposed controls, and trauma-naïve controls to identify shared and unique SC profiles. Structural brain scans were acquired from individuals across 14 sites (N = 1565), and cortical thickness estimates were extracted to compute SC. Group differences in SC between pairs of cortical regions were examined usin

Automated artificial intelligence-enabled measurement of cardiac structures on point-of-care ultrasonography: a prospective multicenter validation study.

INTRODUCTION: Point-of-care ultrasonography (POCUS) enables clinicians to obtain critical diagnostic information at the bedside, especially in resource-limited settings. This information may include 2D cardiac quantitative data, although measuring the data manually can be time-consuming and subject to user experience. Artificial intelligence (AI) can potentially automate this quantification. This study assessed the interpretation of key cardiac measurements on POCUS images by an AI-enabled device (AISAP Cardio V1.0). METHODS: This retrospective diagnostic accuracy study included 200 POCUS cas

CD5-Positive Mature B-Cell Neoplasm With Large-Cell Morphology, Predominant Bone Marrow Involvement, and a Leukemic Peripheral Blood Presentation: A Case Report.

A 65-year-old man presenting with pancytopenia and circulating large atypical cells was initially suspected of having acute leukemia. Flow cytometric analysis demonstrated a mature clonal B-cell phenotype, and the integrated findings supported a diagnosis of a CD5-positive mature B-cell neoplasm with large-cell morphology, predominant bone marrow involvement, and a leukemic peripheral blood presentation. Based on the available staging data, primary bone marrow DLBCL was suspected but could not be definitively confirmed. The neoplasm was characterized by CD5 expression, a BCL6 rearrangement, an

Altered gut microbial functional landscape in children with pediatric inflammatory multisystem syndrome following SARS-CoV-2 infection: an exploratory metagenomic study.

BACKGROUND: Pediatric Inflammatory Multisystem Syndrome (PIMS), also known as MIS-C (Multisystem inflammatory syndrome in children), is a severe post-infectious inflammatory condition associated with SARS-CoV-2 in children. While coronavirus disease 2019 (COVID-19), caused by SARS-CoV-2, primarily affects the respiratory system, growing evidence highlights gastrointestinal involvement and the relevance of the gut-lung axis in systemic inflammation. However, the taxonomic and, particularly, the functional landscape of the gut microbiome in children with PIMS remains insufficiently characterized

Nurse led titration of guideline-directed medical therapy for heart failure: an operative flow chart.

BACKGROUND AND AIMS: Titration of guideline-directed medical therapy (GDMT) in heart failure (HF) is recommended by current guidelines. Recent data show that titration is not performed in most patients because of barriers related to healthcare systems, patient factors, and therapeutic inertia, leading to worse patient outcomes. METHODS AND RESULTS: Nurse-led titration of GDMT for HF may increase the uptake of recommended HF therapies and doses. In Italy, only a few seminal experiences regarding multi-professional management of HF therapy have been reported, and huge disparities regarding this

Early Lower Pulmonary Arterial Diastolic Pressure Monitored With Pulmonary Arterial Catheters Is Associated With Improved Survival During VA ECMO for Patients With Acute Cardiogenic Shock: A Propensity-Matched Study.

BACKGROUND: Venoarterial (VA) extracorporeal membrane oxygenation (ECMO) is used to support refractory cardiogenic shock (CS) but may increase peripheral afterload. Invasive hemodynamic measurement and cardiopulmonary congestion during VA ECMO may correlate with survival, but the precise mechanisms underlying this benefit are uncertain. OBJECTIVES: The aim of this study was to determine whether invasive hemodynamic monitoring with a pulmonary arterial catheter (PAC) and early on-support pulmonary artery diastolic pressure (PAD) are associated with survival during VA ECMO. METHODS: Adult pati

Quality of life in hyperkalemia: baseline analysis of a cohort study of management of hyperkalemia in patients with chronic kidney disease or heart failure in Japan.

BACKGROUND: Hyperkalemia is common in chronic kidney disease (CKD) and chronic heart failure (CHF), often leading to treatment dilemmas regarding renin-angiotensin-aldosterone system (RAAS) inhibitors. Although potassium binders and dietary restrictions are central to chronic management, their quality-of-life (QOL) impact remains insufficiently described. This study aimed to characterize real-world treatment patterns and evaluate treatment impact on QOL. METHODS: We analyzed baseline data from a prospective cohort in Japanese nephrology and cardiology outpatient clinics. Participants were adu

Pediatric-to-adult transition of care in patients with surfactant metabolism disorders.

Genetic disorders of surfactant metabolism, caused by pathogenic variants in the surfactant related genes (SRG) SFTPB, ABCA3, SFTPC and NKX2-1, were long considered as diseases of the neonate and the infant, with an outcome measured in weeks or months. Advances in neonatal intensive care, increased awareness, earlier molecular diagnosis, structured management and pediatric lung transplantation have generated a growing population of adolescents and young adults living with a surfactant metabolism disorder. In the largest European survey of childhood interstitial lung disease (chILD) survivors,
