Contact allergy to various components of topical ophthalmological preparations was tested in 27 patients suffering from prolonged or chronic conjunctivitis, keratoconjunctivitis or blepharoconjunctivitis. Our results reveal relevant allergy towards ophthalmological preparations in 44.4% of patients tested. Allergic reactions to topical antibiotics, especially to neomycin (18.5%) and bacitracin (7.4%) was encountered as well as allergy to preservatives such as benzalconium chloride (3.7%). Commercial preparations of two topical ophthalmic vasoconstrictors were evaluated for whitening ability, duration of action, tolerance, and rebound vasodilation in 11 normal volunteers. Drug treatment containing boric acid, has not been permitted with the exception of ophthalmological preparations, since 1.6.1981. Attention is brought to possible complications during treatment with the well-known boric acid containing Wittmaack'sche Nasensalbe. In preparing a new unguent for the nose we took Chlorhexidindiacetat as an antiseptic. The antibacterial and toxicological features of Chlorhexidindiacetat are mentioned. Different therapeutical possibilities of this new nose unguent are listed.

The cardiovascular effects of topical ophthalmological preparations of 2% epinephrine (EPI), 0.1% dipivalyl epinephrine (DPE), and placebo were studied in double-blind fashion in 20 patients with glaucoma. Fifty-eight patients with keratitis sicca were enrolled in a long-term study to test the effectiveness of slow-release artificial tears (SRAT). The average follow up period of 39 weeks, with a range of 9 to 136 weeks. Forty-six (79%) are still being followed. The aphakic rabbit is a useful model for studying the ocular penetration and pharmacological action of topically applied drugs at the posterior pole, but the use of this model is complicated by the need for specialized preparative surgery. This paper describes a simplified procedure for planned extracapsular lens surgery which utilizes inexpensive instruments commonly available in an ophthalmology department or ophthalmic research laboratory.

Clobetasone butyrate has been formulated as a new steroid preparation of use in ophthalmology and has been compared with prednisolone phosphate and betamethasone phosphate in the treatment of anterior uveitis. The results from 4 double-blind, between-patient studies have shown that all 3 treatments are effective in reducing the signs and symptoms of this intraocular disease. 87% of those patients receiving clobetasone butyrate had a good or satisfactory response, but no differences in therapeutic efficacy were observed between these 3 steroid treatments. This study was designed to evaluate a low-concentration, cycloplegic-mydriatic mixture of phenylephrine and tropicamide in a routine office examination. Tears of patients and laboratory animals on systemic medications have been analyzed for the presence and influence of these agents on tear constituents. Antibiotic penetration into tears, drugs stimulating or retarding lacrimation, and effects on tear electrolytes, lysozyme, and immunoglobulins are reviewed from the literature. Important applications to clinical practice such as contact lens wear, general anesthesia, eye infections, and epiphora or dry eye symptoms are discussed.

A survey of the membership of the South Carolina Ophthalmological Society revealed an average of 3 1/2 patient drug reactions per year per physician using topical diagnostic drugs in the office. The reactions to the drugs included topical allergic responses, acute glaucoma, cardiovascular collapse, and one death. All of the reactions had previous been attributed to these drugs in the literature. Suggestions are presented to prevent these reactions from occurring or to better prepare the ophthalmologist and his staff to manage these problems when they do arise.

An experimental study, by angiography of the anterior segment of rabbit's eye, estimates the permeability of the hemato-aqueous barrier, using fluorescein. A trauma of the eye (contusion) leads to a rapid and significant increase in this permeability.
