The impact of an adaptive patient-centered web application on medication optimization in patients with HFrEF: Rationale and design of the SmartHF trial.

INTRODUCTION: Heart failure (HF) remains a significant public health burden, with frequent readmissions often driven by a lack of guideline-directed medical therapy (GDMT) optimization. While clinical decision support (CDS) can improve healthcare processes, patient-centered tools that empower individuals to participate in their own care are increasingly needed. We developed SmartHF, an adaptive web application that provides patients with tailored HFrEF GDMT recommendations based on their clinical data. METHODS: This 12-week, multicenter randomized controlled trial enrolled adults with HFrEF (

The apelin/APJ system as a complementary target to RAAS blockade in cardiorenal disease.

BACKGROUND: Cardiorenal disease emerges from reciprocal injury among the heart, vasculature, kidney, and metabolic organs, with maladaptive neurohormonal activation as a central pathobiological driver. Although renin-angiotensin-aldosterone system (RAAS) blockade has transformed cardiorenal care, substantial residual risk persists despite contemporary multidrug therapy, including mineralocorticoid receptor antagonists and sodium-glucose cotransporter 2 inhibitors Objective and scope: This review summarizes mechanistic and translational evidence linking apelin/APJ signaling to RAAS regulation i

Patiromer Efficacy across Kidney Function and Hyperkalemia Subgroups in Heart Failure and Reduced Ejection Fraction: A DIAMOND Post Hoc Analysis.

BACKGROUND: In the DIAMOND (Patiromer for the Management of Hyperkalemia in Subjects Receiving RAASi Medications for the Treatment of Heart Failure) trial, patiromer enabled optimization of renin-angiotensin-aldosterone system inhibitors (RAASi) for patients with heart failure and reduced ejection fraction (HFrEF) and current or recent hyperkalemia. Impaired kidney function substantially increases hyperkalemia risk due to impaired potassium excretion. Therefore, in this post hoc analysis, we examined the efficacy of patiromer across baseline kidney function and hyperkalemia subgroups. METHODS

Finerenone versus Steroidal MRAs in Type 2 Diabetes and Chronic Kidney Disease without Prior Heart Failure.

AIMS: Type 2 diabetes (T2D) and chronic kidney disease (CKD) markedly increase the risk of heart failure (HF) and death. While finerenone improves outcomes in clinical trials, real-world evidence and direct comparisons with steroidal mineralocorticoid receptor antagonists (MRAs) are limited. We evaluated finerenone versus steroidal MRAs (spironolactone and eplerenone) in patients without prior HF. METHODS: Using the TriNetX platform, we conducted a retrospective multi-center observational study. We identified patients aged ≥18 years with T2D and CKD without prior HF and ischemic heart disease

Effects of mineralocorticoid receptor antagonists on coronary microcirculation: a mini-review.

Mineralocorticoid receptor antagonists (MRAs) serve as a foundational therapies in the management of heart failure, hypertension, and chronic kidney disease. Their primary therapeutic efficacy stems from the competitive antagonism of the mineralocorticoid receptor (MR), which mitigates pathological cascades such as endothelial dysfunction, oxidative stress, chronic inflammation, and perivascular fibrosis. Coronary microvascular dysfunction (CMD)-a pivotal pathological driver across a spectrum of cardiovascular diseases-is defined by impaired vasodilation, heightened microvascular resistance, a

Beyond Hemodynamics: Uremic Toxins, Post-Translational Modifications, and Inflammatory-Metabolic Remodeling in Uremic Cardiomyopathy.

Uremic cardiomyopathy has traditionally been attributed to pressure overload, volume overload, hypertension, and neurohormonal activation in chronic kidney disease. However, growing experimental and clinical evidence indicates that nonhemodynamic mechanisms also contribute to myocardial remodeling in uremia. These mechanisms include retention of uremic toxins and protein-bound solutes, mediators of mineral metabolism, gut-derived metabolites, inflammatory signaling, mitochondrial dysfunction, and post-translational protein modifications. In this review, we discuss uremic cardiomyopathy as a sy

Blood Pressure-Lowering Effects of Finerenone: A Systematic Review and Meta-Analysis of Randomized Controlled Trials.

Background: Optimal blood pressure (BP) control remains elusive for millions of patients worldwide, often leading to resistant or secondary hypertension. Overactivation of the mineralocorticoid receptor (MR) drives this systemic pathology. While steroidal mineralocorticoid receptor antagonists (MRAs) are effective, their clinical utility is limited by poor cross-selectivity and risk of hyperkalemia. Finerenone, a novel non-steroidal MRA, offers high selectivity and potent target-organ protection, but a comprehensive pooling of its direct impact on BP is still needed. Methods: Following the PRI

Estimated Glomerular Filtration Rate Slope, Chronic Kidney Disease Progression, and Pillars of Care in Patients With Diabetic Kidney Disease.

Chronic kidney disease (CKD) is a progressive condition that affects >800 million people worldwide and remains underdiagnosed, especially in its early stages. Diabetes and hypertension are leading drivers of CKD, largely through hyperglycemia- and pressure-mediated kidney damage. The estimated glomerular filtration rate (eGFR) is a standard measure of kidney function, and clinicians increasingly track its decline (the eGFR slope) to catch early warning signs and guide treatment decisions. Key clinical trials, including CREDENCE, DAPA-CKD, EMPA-KIDNEY, FIDELIO-DKD, FIGARO-DKD, and FLOW, have de

[Treatment of type 2 diabetes mellitus].

Modern diabetology is undergoing a paradigm shift from glucose-centric management toward risk-adapted precision medicine. Differentiated phenotyping into clusters allows for the early identification of organ-specific risks. Pharmacotherapy now prioritizes cardiorenal protection using sodium-glucose cotransporter 2 (SGLT2) inhibitors, glucagon-like peptide‑1 (GLP-1) receptor agonists, and nonsteroidal mineralocorticoid receptor antagonists (nsMRAs; finerenone), which reduce vascular events beyond glycated hemoglobin (HbA1c) reduction alone. Technological innovations, such as continuous glucose

Finerenone: A Non-Steroidal Mineralocorticoid Receptor Antagonist and Its Emerging Role in Cardiovascular-Kidney-Metabolic Syndrome.

Cardiovascular-kidney-metabolic (CKM) syndrome represents the interconnected pathophysiology of metabolic risk factors, chronic kidney disease (CKD), and cardiovascular disease, leading to multiorgan dysfunction and elevated adverse outcomes. Overactivation of the mineralocorticoid receptor (MR), via aldosterone-dependent and ligand-independent pathways, drives inflammation, fibrosis, and oxidative stress and facilitates CKM progression. Finerenone is a selective non-steroidal MR antagonist (nsMRA) that exhibits high MR affinity, has balanced cardiac and renal distribution, and minimal off-tar

Finerenone Use in Coenzyme Q8B-Related Focal Segmental Glomerulosclerosis.

Genetic focal segmental glomerulosclerosis (FSGS) caused by mutations affecting coenzyme Q (CoQ) biosynthesis is a rare podocytopathy that is associated with poor response to immunosuppression. Finerenone is a nonsteroidal mineralocorticoid antagonist that reduces oxidative stress, glomerular inflammation, and fibrosis, features seen in CoQ deficiency. We report the first case of its use in COQ8B-related FSGS. A 35-year-old woman was evaluated for persistent postpartum proteinuria and underwent a kidney biopsy that showed features of FSGS. She had a strong family history of early-onset kidney

Four Pillars in Type 2 Diabetes and Chronic Kidney Disease G4-G5: Mission Impossible-or Not Too Late to Start?

The 4-pillar therapy-renin-angiotensin system inhibitors, sodium-glucose cotransporter 2 inhibitors, the nonsteroidal mineralocorticoid receptor antagonist finerenone, and glucagon-like peptide 1 receptor agonists-is increasingly applied to improve kidney and cardiovascular outcomes in adults with type 2 diabetes and chronic kidney disease (CKD). However, landmark trials underpinning current recommendations largely enrolled participants down to estimated glomerular filtration rate thresholds of ∼20-30 mL/min/1.73 m2, leaving uncertainty in CKD G4-G5. This uncertainty contributes to an implemen

The investigational non-steroidal MRA balcinrenone in cardiorenal medicine: current evidence and future perspectives.

For almost two decades, renin-angiotensin-system (RAS) blockade was the basic pillar of therapy for cardiorenal protection in patients with chronic kidney disease (CKD) and albuminuria. Novel therapies, such as sodium-glucose co-transporter type 2 inhibitors (SGLT2is) and the non-steroidal mineralocorticoid receptor antagonist (MRA) finerenone, have been added to our therapeutic armamentarium in the last few years. Despite this substantial progress, there is still an unmet need for the development of additional therapies to mitigate the substantial residual risk of CKD patients. Balcinrenone i

Combined SGLT-2 Inhibitor and Mineralocorticoid Receptor Antagonist Therapy: Effects on the Renin-Angiotensin-Aldosterone System in Heart Failure Treatment.

Sodium-glucose cotransporter 2 inhibitors (SGLT2is), originally developed as glucose-lowering agents for type 2 diabetes mellitus, have increasingly demonstrated clinical benefits in patients with chronic kidney disease and heart failure. We retrospectively evaluated 114 patients with heart failure admitted to the Emergency Medicine Unit of Policlinico Umberto I, Sapienza University of Rome, Italy, between March and December 2025. Patients were stratified into three groups: initiation of combined SGLT2i and mineralocorticoid receptor antagonist (MRA) therapy (Group A), initiation of SGLT2i alo

Aldosterone and Residual Cardiorenal Risk: Pathophysiology and Emerging Therapeutic Strategies.

Aldosterone, the terminal effector of the renin-angiotensin-aldosterone system, regulates sodium retention, potassium excretion, and blood pressure. Beyond these classical effects, aldosterone-associated and mineralocorticoid receptor (MR)-mediated signaling is implicated in inflammation, oxidative stress, fibrosis, albuminuria, and progressive cardiorenal injury, while its synthesis is regulated by factors beyond angiotensin II. This is particularly relevant in chronic kidney disease (CKD), where persistent renin-angiotensin system (RAS) activation, impaired potassium handling, reduced Klotho

Intensive titration for elderly patients with heart failure.

BACKGROUND: Guideline-directed medical therapy (GDMT) for heart failure (HF) with reduced ejection fraction (HFrEF) relies on quadruple therapy, including beta-blockers (BB), angiotensin receptor-neprilysin inhibitors (ARNi), mineralocorticoid receptor antagonists (MRA), and sodium-glucose cotransporter 2 inhibitors (SGLT2i). Elderly patients with HF represent a high-risk population and often face therapeutic inertia but also more adverse events. We aimed to assess the efficacy and safety of an intensive titration program (ITP) for elderly HFrEF patients. METHODS: We conducted a prospective,

Oral eplerenone in first-episode acute central serous chorioretinopathy: results of a double-masked randomized trial.

BACKGROUND: Central serous chorioretinopathy (CSCR) is a pachychoroid disorder characterized by serous retinal detachment driven by choroidal vascular dysfunction and mineralocorticoid receptor (MR) overactivation. Eplerenone, a selective MR antagonist, has been proposed as a therapeutic option, although clinical evidence remains inconsistent. METHODS: In a randomized, double-blind, placebo-controlled trial, 44 patients with first-episode acute CSCR received either oral eplerenone (25 mg twice daily) or placebo for three months.
