BACKGROUND: Given that mineralocorticoid receptor antagonists have the potential to impair renal function and induce hyperkalemia, close monitoring of estimated glomerular filtration rate (eGFR) and serum potassium levels is essential to ensure the safe administration of this therapy.
OBJECTIVES: In this prespecified analysis, the authors evaluated the efficacy and safety of the kidney function-based finerenone dosing strategy used in the FINEARTS-HF trial.
METHODS: In FINEARTS-HF, patients with an eGFR of 25 to 60 mL/min/1.73 m2 at baseline were stratified at randomization to the low-dose g

Diabetic acute kidney disease (AKD) represents a pivotal transitional phase between acute kidney injury (AKI) and chronic kidney disease (CKD), carrying a high risk of progression to end-stage kidney disease. Effective medical management is crucial during this phase. Renin-angiotensin-aldosterone system inhibitors remain the foundation of therapy, and newer agents have expanded the armamentarium. Sodium-glucose cotransporter-2 inhibitors and glucagon-like peptide-1 receptor agonists have been shown to confer significant nephroprotective and cardioprotective effects, including decreased risks o

Non-diabetic kidney disease (NDKD) is increasingly recognized as a significant and often underappreciated problem in people with type 2 diabetes mellitus (T2DM), accounting for a substantial proportion of kidney abnormalities identified through biopsy. Recent evidence shows that many patients do not have purely diabetic kidney changes; instead, a large share have non-diabetic or mixed pathologies. These findings challenge the long-standing assumption that kidney disease in diabetes is always due to diabetic nephropathy. This narrative review synthesizes current evidence on the prevalence, clin

Diabetic kidney disease (DKD) is the leading cause of end-stage renal disease (ESRD), yet its progressive fibrosis cannot be solely attributed to hyperglycemia-induced oxidative stress or glomerular hypertension. Increasing evidence highlights that the bidirectional interaction between metabolic disturbances and immune activation-termed immunometabolic interactions-plays a pivotal role in driving DKD progression. Chronic metabolic stress, encompassing hyperglycemia, lipotoxicity, mitochondrial dysfunction, and gut-derived metabolites, reprograms innate and adaptive immune cells into pro-inflam

BACKGROUND: The efficacy and safety of lorundrostat, a selective aldosterone synthase inhibitor, in patients with uncontrolled hypertension remain unclear. This meta-analysis aimed to evaluate its effects on blood pressure and risk of hyperkalemia.
METHODS: PubMed, Embase, Cochrane Library, and ClinicalTrials.gov were systematically searched through July 2025 for randomized controlled trials comparing lorundrostat with placebo. Pooled mean differences (MD) and risk ratios (RR) with 95% CIs were estimated using a random-effects model with the Restricted Maximum Likelihood estimator. Trial sequ

BACKGROUND: Chronic kidney disease (CKD) is estimated that by 2040, it will be the 5th highest cause of years of life lost globally due to the aging population and the rising prevalence of age-related diseases such as hypertension and diabetes. Therefore, a deep comprehension of the biological and molecular mechanisms underlying kidney fibrosis is urgently needed to facilitate the development of new therapeutic strategies that can hinder disease progression and simultaneously address the comorbidities associated with CKD. MAIN BODY: In the last years numerous studies have elucidated the comple

Patients with diabetes are disproportionately affected by cardiovascular and kidney disease. Mineralocorticoid receptor (MR) antagonists show organ protection against cardiovascular and renal injury; however, major side effects including hyperkalemia and reduced renal function limit their use in individuals with diabetic complications. The nonsteroidal MR modulator balcinrenone may offer end-organ protection with fewer side effects. We compared responses to balcinrenone and eplerenone delivered from 8 weeks postinduction of streptozotocin (STZ)-induced type 1 diabetes in male mice. RNA sequenc

Heart failure (HF) and chronic kidney disease (CKD) frequently coexist, creating a bidirectional relationship where dysfunction of one organ accelerates deterioration of the other. This overlap is associated with high morbidity, mortality, and healthcare costs. Yet, patients with advanced CKD are systematically underrepresented in pivotal HF trials, resulting in major evidence gaps and therapeutic uncertainty. The pathophysiological interplay involves hemodynamic congestion, activation of the renin-angiotensin-aldosterone system (RAAS) and sympathetic nervous system (SNS), oxidative stress, in

CONTEXT: Elucidating the role of the hydrophobic groove in mineralocorticoid receptor (MR)-spironolactone interactions is important for structure-based drug design, receptor modulation, and the development of more selective MR antagonists. Despite the clinical importance of spironolactone, the contribution of the hydrophobic groove, particularly residues M807, F829, M845, C849, and M852, remains underexplored. Here, we demonstrate through molecular dynamic simulations that these hydrophobic residues, together with polar residue N770, stabilize the thioacetyl moiety of spironolactone. Binding f

Background/Objectives: Diabetic kidney disease (DKD) is the most prevalent form of chronic kidney disease and kidney failure globally. In the last decade, RAAS inhibitors, SGLT2 inhibitors, GLP-1-receptor agonists, and non-steroidal mineralocorticoid receptor antagonists have made significant advancements. However, despite the use of these drugs, patients with DKD often show a residual renal risk. In this narrative review, we synthesize current evidence on residual renal risk in DKD, focusing on underlying mechanisms, high-risk clinical phenotypes, and therapeutic gaps. Methods: A narrative re

Atrial fibrillation (AF) and heart failure with preserved ejection fraction (HFpEF) coexist in 40-60% of cases and mutually reinforce each other through adverse electrical, cellular, and functional remodelling. There is considerable overlap in signs and symptoms, and diagnosis may be challenging due to nonspecific clinical presentations and chronic course. AF is clearly linked with worsening morbidity and mortality in HFpEF with higher rates of HF hospitalizations, HF progression, stroke, systemic embolism, and all-cause death. Optimal management of HFpEF-AF patients requires aggressive treatm

Atrial fibrillation represents the most prevalent cardiac arrhythmia and is associated with substantial morbidity, including an increased risk for stroke and heart failure. The pathophysiology of atrial fibrillation involves electrical and structural remodeling of the atria, often referred to as atrial myopathy, that together increase the risk for arrhythmias. However, a specific approach to target the proarrhythmic substrate of atrial fibrillation is still lacking. Aldosterone and the mineralocorticoid receptor are well-known drivers of cardiac remodeling, and recent clinical and experimental

Despite the availability of effective antihypertensive therapies, global blood pressure control rates remain unacceptably low. Contributing factors, such as low treatment adherence, therapeutic inertia, and rising multimorbidity, underscore the need for innovative approaches to improve hypertension care. New antihypertensive drug therapies that act on physiological pathways beyond those targeted by conventional drug classes are emerging. These therapies include small interfering RNA agents that inhibit angiotensinogen synthesis as a novel approach to inhibit the renin-angiotensin system, and n

BACKGROUND: Diabetic kidney disease (DKD) remains the leading cause of chronic kidney disease and kidney failure worldwide. Over the past 3 decades, management has evolved from strict glycemic and blood pressure control to targeted therapies that modify disease progression.
SUMMARY: The DCCT/EDIC (1993) confirmed the impact of intensive glycemic and multifactorial risk factor management. But the early 1990s established the foundation of nephroprotective therapy with the Captopril Trial (1993) in type 1 diabetes and subsequent IRMA-1 (2001), IDNT (2001), and the RENAAL (2001) studies establish

There are limited reports on the prevalence and clinical significance of guideline-directed medical therapy (GDMT) initiation in patients with acute heart failure (HF). We analyzed 2086 patients with acute heart failure with reduced or mildly reduced ejection fraction (HFrEF or HFmrEF) in the KCHF registry. The patients were classified according to the number of GDMT classes at discharge. The primary outcome was a composite of all-cause death or HF hospitalization. There were 181 (8.7%), 508 (24.4%), 791 (37.9%), and 606 (29.1%) patients with GDMT = 0, 1, 2, and 3, respectively. Current smoker

BACKGROUND: Transcatheter aortic valve implantation (TAVI) aims to improve symptoms and prognosis, while minimising adverse outcomes. Available prediction models focus on individual outcomes, but those combining adverse events and symptom improvement in a single prediction model are scarce, and include only few variables and lack external validation. Using machine learning, we developed a clinically relevant model to identify patients at high risk of both adverse events and poor symptom improvement after TAVI.
METHODS: In total, 72 candidate variables including clinical, medication use, bioma
