Risk factors for diuretic resistance in hospitalized patients with acute decompensated heart failure: a retrospective study.

OBJECTIVE: To investigate the independent risk factors for diuretic resistance (DR) and its clinical impact in hospitalized patients with acute decompensated heart failure (ADHF). METHODS: This single-center retrospective study consecutively enrolled 387 patients with ADHF admitted between January 2022 and June 2025. Based on an operational definition (intravenous furosemide ≥ 80 mg/day with failure to achieve an average daily body weight reduction ≥ 1 kg or a net negative fluid balance ≥ 1,000 mL within 72 h), patients were classified into a DR group (n = 132) and a non-DR group (n = 255). Real-World Evaluation of Individualised Natriuresis and Diuresis Driven Protocol for Decongestion in Acute Heart Failure.

BACKGROUND AND AIMS: Acute decompensated heart failure (ADHF) results in high rates of hospitalisation, re-admissions, and mortality, straining healthcare systems. Loop diuretics are crucial for fluid management in ADHF, but optimal dosing and monitoring remain unclear. The 2021 European Society of Cardiology guidelines suggest individualised dosing based on home diuretic use and close urinary sodium monitoring for better decongestion. This study evaluates the effectiveness of a new natriuresis- and urine output-guided protocol at Maastricht University Medical Centre+ (MUMC+) based on these re

The NKCC1 Antagonist Bumetanide, Administered After Prenatal Alcohol Exposure, Mitigates Ethanol-Induced Aberrance in Tangential Migration of GABAergic Interneurons in Mouse Medial Prefrontal Cortex.

BACKGROUND: Prenatal ethanol (alcohol) exposure (PreEE) results in abnormal tangential migration of medial ganglionic eminence (MGE)-derived GABAergic interneurons (GINs) in the embryonic mouse medial prefrontal cortex (mPFC); later in life, this is associated with impaired behavioral flexibility commonly seen in fetal alcohol spectrum disorders (FASD). The PreEE-induced abnormal tangential migration involves, in part, ethanol potentiating GABAA receptor-mediated depolarization, the latter due to elevated intracellular chloride sustained primarily by the Na-K-Cl cotransporter NKCC1 in embryoni

Timing and Causes of Systemic Readmissions After Hip Fracture Surgery: An 8-Year Retrospective Cohort Study.

BACKGROUND: Systemic readmissions after hip fracture surgery impose a substantial clinical and economic burden. Hip fracture represents a sentinel orthopedic injury with prolonged postoperative vulnerability extending beyond the index admission. Evidence from the Middle East remains scarce, particularly regarding the incidence, causes, and timing of systemic readmissions in multiethnic populations. This study aimed to determine the one-year incidence, etiologies, and temporal distribution of systemic readmissions following proximal hip fracture surgery, and to explore whether commonly document

Antihypertensive Medications and Risk of Heat-Related Illness During Heatwaves: Self-Controlled Case Series.

PURPOSE: To investigate whether antihypertensive medications modify the risk of heat-related illness during heatwaves among adults with hypertension in Japan. METHODS: We identified adults with hypertension in Tsukuba City administrative claims data between April 2014 and March 2019. Average daily temperature was derived from Japan Meteorological Agency data. We used a self-controlled case series (SCCS) design to estimate incidence rate ratios (IRRs) for heat-related illness during 5-day pre-heatwave, heatwave and 5-day post-heatwave periods compared to baseline within individuals. The main a

Polypharmacy and Bone Health in Patients with Type 2 Diabetes Mellitus: A Narrative Review.

Patients with type 2 diabetes mellitus (T2DM) commonly demonstrate reduced bone quality and elevated fracture risk rates despite having normal or elevated bone mineral density (BMD). Beyond the pathophysiological effect of diabetes, polypharmacy has emerged as a potential, yet underappreciated contributor of skeletal fragility. To explore this association, a thorough search of the literature was conducted using PubMed, Google Scholar, and Web of Science. and Scopus for articles published in English language until October 2025. Studies addressing diabetes, polypharmacy, bone health, and drug-re

Diuretic-Induced Hypomagnesemia May Play a Key Role in the Development of Calcium Pyrophosphate Arthritis.

INTRODUCTION: The objective of this study is to determine the prevalence of hypomagnesemia in patients with calcium pyrophosphate (CPP) arthritis and the role of diuretics on inducing hypomagnesemia. METHODS: This was an observational, cross-sectional, prospectively recruited, case-control study. Cases were confirmed by both chondrocalcinosis in plain X-ray and CPP crystals in synovial fluid. Serum creatinine, magnesium, calcium, phosphate, 25-OH-vitamin D, parathyroid hormone, ferritin and transferrin saturation were measured. Diuretic use, class, and dosage were also recorded. RESULTS: A t

Liraglutide and renal mitochondrial homeostasis: targeting the "metabolism-inflammation-lithogenesis" axis in metabolic syndrome.

Nephrolithiasis associated with metabolic syndrome represents a growing global public health burden with a 5-year recurrence rate of up to 50%. Current first-line preventive strategies, primarily potassium citrate and thiazide diuretics, only correct urinary chemical abnormalities symptomatically, and patient adherence remains below 50% at 1 year. Importantly, these approaches fail to address the core pathological basis of tubular injury driven by metabolic dysregulation. Mitochondrial dysfunction is the central mechanistic hub linking systemic metabolic stress to intrarenal lithogenic suscept

Combined use of Bumetanide and MGE cell transplantation alleviates neuropathic pain and its mechanism after spinal cord injury in mice.

INTRODUCTION: Neuropathic pain (NP) after spinal cord injury (SCI) is intractable with limited efficacy of single treatments. This study investigated the additive analgesic effect and molecular mechanisms of Bumetanide (Bu, a NKCC1 inhibitor) combined with medial ganglionic eminence (MGE) cell transplantation on SCI-induced NP. METHODS: Ninety adult female C57BL/6N mice were randomly divided into 5 groups (Sham, SCI, Bu, Mge, Bu+Mge). A T10 moderate spinal cord contusion model was established, with treatments (Bu intraperitoneal injection and MGE orthotopic transplantation) on day 10 post-sur

Refractory Ascites and the Alfapump.

Ascites is often the first decompensation in patients with cirrhosis and is associated with reduced survival. The formation of ascites is complex and thought to be due to an increase in hydrostatic pressure, a decrease in oncotic pressure, and increased lymph production. Standard of care for ascites includes dietary sodium restriction, use of loop diuretics and aldosterone antagonists, large-volume paracentesis (LVP), and at times transjugular intrahepatic portosystemic shunt (TIPS). Alfapump was designed in response to the need for alternative treatments for patients with refractory ascites,

Decongestion in chronic heart failure.

Congestion drives symptoms and adverse outcomes in heart failure (HF), and loop diuretics remain the cornerstone of decongestion. Yet randomized trials of diuretic strategies and other acute decongestive approaches have not shown mortality benefit, while mechanistic studies demonstrate rapid neurohormonal activation after acute loop diuretic administration. In chronic HF, observational data consistently associate higher loop diuretic requirements-particularly above ∼40 mg/day furosemide equivalent or with dose escalation-with worse prognosis across HF phenotypes, though confounding by disease

Furosemide Administration Enhances Diagnostic Confidence in the Evaluation of Ureteral Ectopia in Dogs Using Ultrasound.

Ultrasonography is frequently utilized in patients with urinary incontinence to screen for structural abnormalities such as ectopic ureters (EUs). Furosemide can be administered during an ultrasound for a better assessment of the ureters and their insertions. Furosemide decreases urine specific gravity (USG), and differences in specific gravity allow a better visibility of ureteral jets on ultrasound; however, it is unknown whether furosemide has utility in patients with hyposthenuria or isosthenuria. The aims of this study were to determine if furosemide administration improves the diagnosis

Diuretic inhibition of experimental myopia implicates retinal ion-driven efflux in the regulation of ocular growth.

INTRODUCTION: Myopia (short-sightedness), characterized by excessive ocular growth, is the most common visual disorder and is the greatest risk factor for blindness later in life, though its etiology is uncertain. We have previously suggested that ocular volume regulation is related to the rate of Retinal Ion-Driven fluid Efflux from the vitreous to the lymphatic sinusoids of the choroid, and predict that common clinically approved diuretics (amiloride, bumetanide, and furosemide) known to act on different ion exchanger and cation-chloride symporter mechanisms in retina and RPE would show diff

Diuretics in Cardiovascular Disease: For How Long and at What Dose?

Diuretics remain indispensable for the treatment of congestion in cardiovascular disease, particularly in acute and chronic heart failure. Their principal clinical value is rapid symptom relief, reduction in filling pressures and facilitation of decongestion; however, a mortality benefit has not been consistently demonstrated. This distinction is clinically important because long-term diuretic exposure in patients without objective congestion may promote hypotension, renal dysfunction, electrolyte instability, neurohormonal activation, falls, cognitive impairment and avoidable polypharmacy. Atenolol or Bisoprolol after Myocardial Infarction without Recorded Heart Failure.

BACKGROUND: Beta-blockers are a cornerstone of therapy following myocardial infarction. The effect of different β-blockers on the outcomes of patients without heart failure after myocardial infarction is not well-studied. METHODS: This observational cohort study used administrative data from French National Health Data System. Eligible patients were 18 years of age or older, hospitalized in the Paris area between January 1, 2008, and December 31, 2018, with a new myocardial infarction, and were dispensed either atenolol or bisoprolol within 2 days of hospital discharge. Reverse Left Ventricular Remodeling in a Patient With Concurrent Wild-Type Transthyretin Cardiac Amyloidosis and Heart Failure With Reduced Ejection Fraction: The Role of Conventional Heart Failure Therapy.

Wild-type transthyretin amyloid (ATTRwt) amyloidosis is a systemic, progressive disease that often affects older adults and is characterized by ATTRwt deposition in multiple organs and tissues. When the heart is involved, this deposition leads to cardiac amyloidosis (CA), which can result in life-threatening heart failure (HF) and arrhythmias. In general, as left ventricular (LV) wall thickening increases because of ATTRwt deposition, the LV cavity decreases in size. Additionally, as the disease progresses, the myocardial ATTR burden increases, and the LV ejection fraction (LVEF) decreases. Th

Acetazolamide versus metolazone as adjunctive therapy to loop diuretics in hospitalized patients with volume overload: A retrospective cohort study.

PURPOSE: Sequential nephron blockade is commonly used to augment diuresis in patients with volume overload receiving loop diuretics. While metolazone is frequently used, acetazolamide has emerged as an alternative adjunctive agent. Real-world data comparing these strategies remain limited. This study evaluated the effectiveness and safety of acetazolamide vs metolazone as adjunctive therapy. METHODS: This single-center, retrospective cohort study included adult patients receiving adjunctive acetazolamide or metolazone with loop diuretics for volume overload. The primary outcome was cumulative
