Continuous Versus Intermittent Loop Diuretics Step-by-Step Protocol in Acute Heart Failure (DIUR-AHF): A Propensity-Matched Analysis.

BACKGROUND: Loop diuretics are used to solve congestion in acute heart failure. However, a clear indication about the best infusion modality, dose, and duration of the treatment has not yet been established. In this analysis of the DIUR-AHF (Different Loop Diuretic Dosing and Administration in Acute Heart Failure) study, we aimed to investigate the effects of different diuretic administration modalities (ie, intermittent versus continuous furosemide infusion) and dose (high dose [HD] versus low dose) on congestion, renal function, and outcome. METHODS: Patients received intermittent or contin

Systemic inflammatory response index is associated with acute kidney injury following cardiac surgery: A retrospective cohort study using the MIMIC database.

PURPOSE: This study aimed to investigate the association between the Systemic Inflammatory Response Index (SIRI) and acute kidney injury (AKI) following cardiac surgery using the Medical Information Mart for Intensive Care (MIMIC) database, and to evaluate whether SIRI could serve as a potential risk marker associated with post-cardiac surgery AKI. METHODS: We conducted a retrospective cohort study of 2,884 cardiac surgery patients from the MIMIC-IV database. SIRI was calculated as (neutrophil count × monocyte count)/ lymphocyte count. The primary outcome was AKI occurrence within seven days

Minimizing in Source Fragmentation for Mass Spectrometry Based NDSRI Analytical Procedures: A Case Study of Nitroso-bumetanide.

Liquid chromatography-mass spectrometry (LC-MS) is the standard instrumental procedure for quantitating nitrosamine drug substance-related impurities (NDSRIs) due to its superior specificity and sensitivity. Electrospray (ESI) is the most used ionization source in LC-MS. However, analytes can undergo fragmentation directly within the ESI source before reaching the collision cell. This phenomenon is known as in source fragmentation (ISF). To our knowledge, the impact of ISF on analytical procedure performance for NDSRI measurements has not been explored. Thus, here, we present a case study on a

Predictive value of FST for renal replacement therapy in patients with acute kidney injury: a meta-analysis.

The furosemide stress test (FST) has been used as a predictor for renal replacement therapy (RRT), yet its reported performance varies across studies. This meta-analysis aimed to evaluate the predictive value of FST for RRT in patients with acute kidney injury (AKI). We searched PubMed, Embase, Cochrane, CNKI, and Wanfang Medical databases up to December 13, 2025, for prospective or retrospective cohort studies reporting the predictive performance of FST for RRT. This trial has been registered in PROSPERO (CRD42024570956). A total of 14 studies with 3506 AKI patients were included. The pooled

Uncovering the Untapped Potential of Furosemide in Sepsis: Real-World Evidence on Coagulopathy and Organ Outcomes.

BACKGROUND: Sepsis-related coagulopathy is a major driver of organ failure and mortality, yet effective adjuncts remain limited. We therefore examined whether early furosemide use is associated with reduced coagulopathy and improved organ outcomes in real-world practice. METHODS: Using Taiwan's National Health Insurance Research Database (2000-2013), we assembled a population-based matched cohort of 9,934 adults with incident sepsis. Exposure was receipt of furosemide during the index hospitalization; comparators were other diuretics (e.g., acetazolamide, hydrochlorothiazide, and spironolacto

Mineralocorticoid Receptor Antagonist and Its Combinations in Heart Failure.

Mineralocorticoid receptor antagonists (MRAs) are strongly recommended by various guidelines for the management of patients with heart failure. Present and emerging clinical evidence also supports the beneficial role of MRAs in lowering the risk of heart failure-associated hospitalization and mortality. Loop diuretics play a crucial role in the management of edema associated with heart failure; however, their use has been associated with electrolyte abnormalities, activation of the renin-angiotensin-aldosterone and sympathetic systems, and diuretic resistance. Combined use of loop diuretics al

A rapid review and narrative synthesis of the evidence for oral sodium chloride supplements in the management of heart failure.

AIMS: Intravenous (i.v.) hypertonic saline alongside i.v. loop diuretics is sometimes used to enhance diuresis in people hospitalized with heart failure (HF) but is challenging to administer. Oral sodium chloride (NaCl) supplements might be a practical alternative, but little is known about their effects in patients with HF. We performed a rapid review of the relevant evidence. METHODS AND RESULTS: A rapid systematic review was registered (PROSPERO: CRD420250618965) and reported following Preferred Reporting Items for Systematic Reviews and Meta-analyses guidelines. Acute declines in kidney function with thiazide plus loop diuretics vs loop diuretics alone in acute decompensated heart failure.

BACKGROUND AND HYPOTHESIS: The association between acute declines in estimated glomerular filtration rate (eGFR) among individuals admitted for acute decompensated heart failure (ADHF) and cardiovascular outcomes has been inconsistent. Our objective was to examine whether eGFR decline, and the timing of these declines, are associated with mortality and a composite outcome of mortality or heart failure (HF) hospitalization. METHODS: We used data from the CLOROTIC Trial, which randomized patients admitted for ADHF to thiazide versus placebo. We examined %eGFR change at 2-days (n = 225) and at 4

Polysaccharide-based suspensions: a novel pediatric-friendly formulation for administering furosemide through nasogastric tubes.

OBJECTIVE: Our study aimed to develop extemporaneous furosemide suspensions using a polysaccharide as the suspensor agent and to evaluate their stability and drug loss during tube administration. SIGNIFICANCE: Extemporaneous oral furosemide liquid formulations are often necessary for managing neonatal cardiovascular disorders, as commercial formulations are typically limited to solid preparations. However, the practice involves syrup as the vehicle, and data on the stability of these formulations are missing. METHODS: Suspensions were prepared by crushing tablets and using xanthan gum as a t

SGLT2 Inhibitors as a Novel Therapeutic Strategy in SIADH-Induced Hyponatraemia: Emerging Evidence and Clinical Implications.

Syndrome of inappropriate antidiuretic hormone secretion (SIADH) is a leading cause of euvolaemic hyponatraemia and remains challenging to manage due to limitations of existing therapies, including poor adherence to fluid restriction and safety concerns associated with vasopressin receptor antagonists and demeclocycline. Recent mechanistic and clinical evidence suggests that sodium-glucose cotransporter-2 (SGLT2) inhibitors may offer a novel therapeutic approach by promoting osmotic diuresis and increasing electrolyte-free water clearance. This narrative review synthesises current pathophysiol

Differential pulse anoding voltammetric determination of torasemide using carbon paste electrode.

Torasemide electrochemical behavior at a carbon paste electrode was investigated and optimized using cyclic and differential pulse voltammetry in 0.04 mol/L Britton-Robinson (BR) buffer pH 2.8. Cyclic voltammetry showed that the oxidation process was irreversible and primarily controlled by diffusion. The responses of differential pulse were linear over the torasemide concentration range of 0.35-4.13 µg/mL, with detection and quantification limits of 0.110 and 0.367 µg/mL, respectively. The suggested voltammetric method was successful in detecting torasemide in its pharmaceutical preparation (

Acetazolamide effects on natriuresis and diuresis in acute heart failure treated with furosemide and SGLT2i (SANDI).

AIMS: Early adjunctive therapy with intravenous acetazolamide in combination with loop diuretics has been shown to enhance decongestion in acute heart failure (AHF). However, patients receiving sodium-glucose cotransporter-2 inhibitors (SGLT2i) were excluded from previous acetazolamide trials, and the efficacy of this combination remains unknown. This study aims to evaluate the natriuretic and diuretic effects of acetazolamide in patients with AHF treated with intravenous furosemide and concomitant SGLT2i therapy. METHODS: The SANDI study is a prospective, multicentre, observational study enr

Denosumab-induced hypocalcemia: detection of drug-drug interactions using disproportionality analysis in VigiBase®.

BACKGROUND: Denosumab-induced hypocalcemia has been reported more frequently in real-world studies than in pivotal clinical trials; this is probably due to a closer monitoring of medication regimens in trials. Here, we sought to detect signals for drug-drug interactions (DDIs) between denosumab and other hypocalcemia-inducing drugs. METHODS: We performed a disproportionality analysis of data in VigiBase® between January 1st, 2011, and February 7th, 2024. The frequency of hypocalcemia reports for all drugs known to induce hypocalcemia was compared with that for all other drugs and was quoted a

Synergistic Sugaring-Out Microextraction Coupled With Synchronous Fluorescence for Multiplexed Diuretic Detection in Pharmaceuticals and Biofluids.

An environmentally friendly approach was developed for the precise determination of bumetanide and amiloride, two diuretics with complementary therapeutic applications. The method employs an integrated sugaring-out homogeneous liquid-liquid microextraction (SULLME) technique in conjunction with synchronous fluorescence spectroscopy. This approach eliminates toxic organic solvents typically used in liquid-liquid extraction. The two drugs exhibit distinct synchronous fluorescence signatures at a Δλ of 140 nm, facilitating baseline-resolved signals and thereby reducing analysis time and costs. Th
