BACKGROUND: While clinical benefits of guideline-directed medical therapy in patients with heart failure (HF) are well established, acute hemodynamic changes after initiation of these agents are not well described. Wireless pulmonary artery pressure (PAP) monitoring using implantable sensors is ideally suited to determine changes in pressure following medication titration.
METHODS: We compared PAP from 7 days pre-initiation to the 7-day period following 30 days of newly initiated treatment with core HF therapies: angiotensin receptor-neprilysin inhibitors, mineralocorticoid receptor antagonis

Congestion is the dominant driver of hospitalization in acute heart failure and relief of congestion remains a central therapeutic target. Loop diuretics are first-line therapy for decongestion, yet their dosing and escalation in routine practice often rely on subjective bedside assessment, variable urine output thresholds, weight change, and delayed laboratory trends approaches that incompletely capture the primary pharmacodynamic goal of diuretic therapy: natriuresis. Neurohormonal activation and renal sodium avidity in heart failure can lead to poor diuretic response despite apparently adeq

BACKGROUND: Gamma-aminobutyric acid (GABA), acting through its receptors, is the principal inhibitory neurotransmitter in the brain. It plays a vital role in maintaining neuronal chloride balance by interacting with chloride co-transporters, mainly NKCC1 and KCC2. In addition to the neurons, GABA receptors and chloride co-transporters are found to be expressed by the immune cells also. This present work aimed to study the gene expression patterns of GABAA receptors and chloride co-transporters (NKCC1 and KCC2) in the autism-affected children of West Bengal using peripheral blood as model.

BACKGROUND: Intravenous (IV) loop diuretics are the cornerstone of therapy for acute decompensated heart failure (ADHF), yet appropriate initial dosing in the emergency department (ED), particularly for patients receiving chronic outpatient diuretics, remains challenging. To support guideline-recommended dosing, an electronic clinical decision support (CDS) alert was implemented to prompt clinicians to administer at least twice a patient's home diuretic dose intravenously.
OBJECTIVES: To assess clinician acceptance of an ED-based CDS alert recommending guideline-concordant IV diuretic dosing

Patients with chronic kidney disease (CKD) are at increased risk for masked and nocturnal hypertension, conditions best identified through ambulatory blood pressure (BP) monitoring (ABPM) and associated with adverse cardiovascular outcomes. We evaluated associations between antihypertensive medication classes and ABPM profiles by conducting a cross-sectional analysis of participants from the Chronic Renal Insufficiency Cohort (CRIC) with ABPM data. Antihypertensive medications were categorized as renin-angiotensin system inhibitors (RASis), beta-blockers, calcium channel blockers (CCBs), and t

Inhaled furosemide shows potential for relieving refractory dyspnoea, yet clinical trials have yielded inconsistent results. We hypothesise this failure stems from a "Dyspnoea Paradox", where high inspiratory flows create an aerodynamic barrier. We evaluated furosemide delivery using a vibrating mesh nebuliser under simulated dyspnoeic conditions (tidal volume 1.39 L, frequency 28.6 min⁻¹). The study integrated standard Pharmacopoeial characterisation, in vitro deposition in a realistic human airway replica, and computational modelling (Large Eddy Simulation and Multiple-Path Particle Dosimetr

Both hypertensive and breast diseases impose a significant societal burden. However, the correlation between antihypertensive drug use and breast cancer remains controversial. We aimed to determine their potential correlations and explore their possible mechanisms. We performed summary data-based Mendelian Randomization (SMR) analysis of expression quantitative trait loci (eQTL) data from the blood of patients with breast diseases. We then performed drug-targeting Mendelian Randomization (MR) analysis using blood pressure-related single nucleotide polymorphism (SNPs) as instrumental variables

AIMS: A reduction in loop diuretics need after sodium-glucose cotransporter-2 inhibitors (SGLT2i) initiation has been suggested by retrospective analyses of clinical trials. We aimed to investigate long-term changes in loop diuretic therapy after SGLT2i initiation in an unselected, real-world heart failure (HF) cohort.
METHODS AND RESULTS: Patients with HF with reduced ejection fraction (HFrEF) enrolled in the SwedeHF between January 2020 and October 2021 were considered. Patients initiated with SGLT2i were the cases. Controls did not initiate SGLT2i despite having an indication. Trajectories

AIMS/INTRODUCTION: Sodium-glucose cotransporter 2 inhibitors (SGLT2i) prevent cardiovascular events in patients with diabetic kidney disease (DKD). However, limited data exist regarding the change in B-type natriuretic peptide (BNP) after treatment. This study investigated whether SGLT2i are associated with a greater BNP reduction than other antidiabetic drugs and identified the predictors of BNP reduction following SGLT2i.
MATERIALS AND METHODS: Using the Japanese nationwide database, we compared BNP levels before and 30-365 days after initiation of antidiabetic medication in 1818 patients w

Rhabdomyolysis is a clinical syndrome characterized by skeletal muscle injury and the release of intracellular contents into the circulation. While early hypocalcemia is common, hypercalcemia during the recovery phase is a rare but serious complication. We present a case of a 36-year-old male who developed severe, refractory hypercalcemia following rhabdomyolysis-induced acute kidney injury. Despite standard treatment with loop diuretics, low-calcium hemodialysis, and calcitonin, the hypercalcemia persisted. In this report, we highlighted a rebound hypercalcemia occurring during recovery from

Many patients hospitalized with acute heart failure (AHF) do not respond adequately to initial intravenous loop diuretic therapy, and residual congestion at discharge is associated with adverse outcomes. We conducted a systematic review and meta-analysis of randomized controlled trials comparing loop diuretics alone versus loop diuretics in combination with add-on pharmacotherapy in adults hospitalized with AHF. PubMed, Embase, and CENTRAL were searched from inception to May 7, 2025. Efficacy outcomes included mortality, hospital readmission, hospital length of stay (LOS), change in body weigh

Acute kidney injury (AKI) is a common and clinically important complication in hospitalized adults receiving nephrotoxic therapies. This study aimed to compare the incidence, timing, and severity of creatinine-defined AKI in patients receiving vancomycin plus Na+-K+-2Cl- cotransporter (NKCC) inhibitor only vs vancomycin plus NKCC inhibitor plus piperacillin-tazobactam. A retrospective observational cohort study was performed using MIMIC-IV version 3.1. Adult hospital admissions with qualifying exposure within the first 48 h of hospitalization were included. Group A comprised patients receiving

INTRODUCTION: Tricuspid regurgitation is frequent, and its related systemic venous congestion is associated with mortality. Diuretics are the main therapy but what shapes their effect is not fully known.
AIM AND METHODS: In this single-center cross-sectional study, we aimed to assess the association of right heart metrics of size and function in relation to diuretic dose. In consecutive patients with measurable tricuspid regurgitation regurgitant volume (RVol) by echocardiography, a high diuretic dose (HDD) was considered a furosemide dose (or equivalent) of greater than 40 mg. Parameters of

Olaparib belongs to the PARP inhibitors and is widely used as an anticancer drug. Due to frequent side effects, it is usually co-administered with an antihypertensive, such as propranolol and/or an antidiuretic, such as furosemide. In this study, a highly sensitive HPLC-FLD method was developed for the simultaneous determination of the three APIs in urine. For their analysis, a Nucleosil C8 column (250 mm × 4.6 mm, 5 μm) at 50°C was used as the stationary phase and an isocratic system of methanol-acetonitrile-20 mM NaH2PO4 at pH 3.6, 12:20:68 v/v/v as the mobile phase. OBJECTIVE: Aminoglycoside antibiotics remain essential for treating serious neonatal and pediatric infections, yet carry a well-documented risk of permanent auditory and vestibular toxicity. This review examines the pharmacological mechanisms of ototoxicity in pediatric populations, identifies those at highest risk, and assesses current prevention, monitoring, and management strategies.
METHODS: PubMed, EMBASE, Cochrane Library, Web of Science, and CINAHL were searched for relevant literature from 2000 to 2025. The primary focus was pediatric populations, though mechanistic and translational

Hypokalemia is a rare but important cause of rhabdomyolysis in children. Unlike typical rhabdomyolysis, hyperkalemia may be absent because the triggering factor is hypokalemia, potentially delaying diagnosis. We report a five-year-old boy with hypoplastic left heart syndrome after a Fontan procedure who developed severe hypokalemia and rhabdomyolysis following prolonged diarrhea while receiving loop diuretics. He presented with acute lower extremity weakness and inability to stand. Laboratory evaluation revealed potassium 1.9 mmol/L and creatine kinase 9,013 U/L, peaking at 32,908 U/L. Urinaly
