Polypharmacy burden and incident epilepsy among older adults in the United States.

OBJECTIVES: To estimate the prevalence of polypharmacy among older adults with incident epilepsy and to describe the most common combinations of drug classes filled prior to epilepsy diagnosis. Polypharmacy-the concurrent use of multiple medications-is common in older adults with epilepsy, but little is known about its burden and specific patterns at the time of diagnosis. METHODS: This retrospective longitudinal cohort study included 2 768 656 US Medicare beneficiaries aged 67 and older with Part D coverage. Beneficiaries were epilepsy-free from 2016 to 2018; 13 819 (0.5%) were newly diagnos

Efficacy and safety of human albumin combined with furosemide in acute decompensated heart failure with hepatic dysfunction.

BACKGROUND: Congestion is the most common clinical presentation on admission of patients with acute decompensated heart failure (ADHF). Finding effective ways to alleviate congestion has become a pivotal management step. This study sought to investigate the efficacy and safety of intravenous (IV) albumin use in conjunction with furosemide in patients hospitalized for ADHF with hepatic dysfunction in terms of subjective regression of congestion symptoms and worsening renal function. METHODS: This prospective, open-label, randomized-pragmatic trial recruited 241 patients with hepatic dysfunctio

The 'Chloride Theory' for Heart Failure Pathophysiology: An Updated Narrative Review 2025.

Most studies of heart failure (HF) syndromes have focused on fluid regulation, particularly the balance of sodium and water. Approximately 10 years ago, in 2017, the "chloride theory" was proposed as a unifying hypothesis, suggesting that changes in serum chloride concentration are associated with shifts in plasma volume, hemodynamics, and neurohormonal activity during HF progression and treatment. This narrative review updates the chloride theory based on emerging clinical evidence. It further examines diuretic classification from a chloride-centered perspective, recent advances in guideline-

Comparative effectiveness of torsemide vs furosemide in the management of heart failure patients: Win-ratio reanalysis of the TRANSFORM-HF trial.

BACKGROUND: Loop diuretics are widely used for managing congestion in patients with heart failure (HF). The TRANSFORM-HF trial is a multicenter randomized study that enrolled heart failure patients, comparing a strategy of torsemide vs furosemide. The time-to-event analysis demonstrated neutral effects on all-cause death at 30 months and the composite of all-cause death and first rehospitalization at 12 months. We evaluated whether a hierarchical win-ratio (WR) framework integrating mortality, recurrent hospitalization, and patient-reported health status provides additional interpretive insigh

Comparative efficacy of continuous versus intermittent administration of furosemide in acute heart failure: an updated systematic review and meta-analysis of 22 RCTs.

INTRODUCTION: Acute heart failure is defined as the rapid or gradual onset of signs and/or symptoms of heart failure severe enough to warrant unplanned hospital or emergency department care. As of 2019, an estimated 56.2 million individuals across 204 countries and territories were living with heart failure; data gaps suggest the true burden is likely higher. The global number of HF cases doubled from 27.2 million in 1990 to 56.2 million in 2019, with a parallel doubling observed in both sexes. Intravenous loop diuretics-predominantly furosemide-constitute the cornerstone of decongestive thera

Urinary Biomarker Thresholds to Detect Diuretic Resistance and Insufficient Diuretic Response in Acute Heart Failure.

AIMS: Early identification of poor diuretic response is central to acute heart failure (AHF) care, yet the recommended post-diuretic urine sodium (uNa+) thresholds are neither prospectively derived nor validated. We aimed to derive and validate clinically actionable urinary biomarker thresholds to identify poor diuretic response. METHODS AND RESULTS: In this prospective derivation-validation study, 336 AHF patients formed the derivation cohort, and 141 patients formed the validation cohort. Spot urinary biomarkers were measured 2h after loop diuretic administration. Diuretic resistance was de

Acute Versus Prolonged Use of Furosemide on Delayed Graft Function in Kidney Transplant Patients.

IntroductionDelayed graft function in kidney transplant recipients is defined as the need for dialysis within 7 days of transplant and is associated with poor patient outcomes, including acute organ rejection and shortened allograft lifespan. Furosemide has been used with the goal of preventing delayed graft function by decreasing metabolic demand and ischemic injury; however, there is no consensus regarding duration of furosemide use after kidney transplantation.Project AimTo assess the effect of postoperative intravenous furosemide duration on delayed graft function rates in kidney transplan

Towards a standard toolbox for compatibility testing of pediatric drug products: verification of standard vehicles for fruit juice, apple sauce, yogurt, and pudding through comparative biorelevant dissolution testing using pediatric furosemide mini-tablets.

In vitro compatibility assessments are a valuable tool for the early identification of potential drug-vehicle interactions. To standardize such evaluations, a set of standard vehicles, also referred to as simVehicles (simJuice, simApplesauce, simYogurt, and simPudding) because they simulate the key physicochemical properties of commonly used pediatric dosing vehicles, was previously developed using a Design of Experiments (DoE) approach to replicate these properties while ensuring reproducibility under controlled conditions. The present study aimed to validate selected simVehicles through comp

Lost to Follow-Up in Pragmatic Trials With Remote Follow-Up: Insights From CONNECT-HF and TRANSFORM-HF.

BACKGROUND: Pragmatic trials commonly use remote follow-up to reduce participant burden and costs. However, these approaches may increase the risk of lost to follow-up (LTFU). We identified patient- and site-level factors associated with LTFU in 2 large, pragmatic heart failure (HF) trials. METHODS: We analyzed data from 2 pragmatic US trials (CONNECT-HF [Care Optimization Through Patient and Hospital Engagement Clinical Trial for Heart Failure], TRANSFORM-HF [Torsemide Comparison With Furosemide for Management of Heart Failure]) following HF hospitalization. Both trials employed centralized

Clinical factors associated with biased estimation of glomerular filtration rate: a cross-sectional study.

BACKGROUND AND HYPOTHESIS: Equations estimating glomerular filtration rate (GFR) based on creatinine and/or cystatin C incorporate demographic variables such as age and sex. However, clinical determinants may lead to substantial bias in GFR estimation. We aimed to identify clinical factors associated with biased GFR estimation with the Modification of Diet in Renal Disease, Chronic Kidney Disease-Epidemiology Collaboration, and European Kidney Function Consortium equations. METHODS: In this retrospective cross-sectional study, we included patients referred for GFR measurement from March 2008

From Golden Berry To Golden Nanoprobe: Sustainable Fluorescent Sensing of Torsemide For Doping Control in Athletes.

Doping represents a critical threat to the integrity of competitive sports, with diuretics frequently misused as masking agents to facilitate the urinary excretion of other prohibited substances. Torsemide (TSD), a widely misused loop diuretic, requires a sensitive and selective method for its determination in biological matrices to support efficient doping control. Herein, a sustainable fluorescent nanosensor based on multi-self-doped carbon quantum dots was synthesized for the first time from golden berry (GBQDs) via a rapid, one-step microwave-assisted approach at 800 W in just 7 min withou

Efficacy and Safety of Sequential Nephron Blockade Compared to Standard Loop Diuretic for Fluid De-resuscitation in the ICU.

Diuretics are integral in fluid de-resuscitation of ICU patients following initial stabilization. While loop diuretics are commonly used, sequential nephron blockade (SNB) is an alternative diuretic strategy that combines a loop diuretic with at least one diuretic from another pharmacologic class to increase efficacy and limit adverse effects. The objective of this study was to compare the safety and efficacy of SNB to loop diuretics alone for de-resuscitation of critically ill patients. This retrospective study included adult ICU patients who received either a loop diuretic alone or combined
