Effects of plasma donation on blood rheology and peripheral vascular function.

Plasma donation, the removal of plasma and reinfusion of cellular components, is widely used to produce plasma-derived products; however its physiological effects on blood rheology and peripheral vascular function remain poorly understood. This study investigated the effects of plasma donation (Investigation 1) on blood viscosity, red blood cell (RBC) deformability and aggregation, and brachial artery reactive hyperaemia flow-mediated dilatation (RH-FMD), and compared these responses with those induced by frusemide-mediated haemoconcentration (40 mg; Investigation 2) to isolate the independent

Application of molecularly imprinted polymers on carbon dots for fluorescence detection of bumetanide in adulterated dietary supplements: instrumental and paper-based approaches.

Bumetanide (BUM) is a strong loop diuretic that is commonly abused as an illicit adulterant in weight-loss dietary supplements to create rapid fluid loss and conceal performance-enhancing chemicals. Considering the health hazards linked with its uncontrolled use, there is an urgent demand to find quick and readily available screening instruments. Herein, we have constructed a very selective and sensitive fluorescent probe, whereby we have incorporated water-soluble carbon dots (CDs) into a molecular imprinted polymer (MIP) platform (CDs@MIP). The prepared probe had strong fluorescence quenchin

Association Between Follow-Up Therapeutic Drug Monitoring and Vancomycin-Induced Nephrotoxicity: A Claims Database Study.

BACKGROUND: In vancomycin treatment, follow-up therapeutic drug monitoring (TDM) within 1 week after the initial TDM is recommended in patients with dose change after the initial TDM or high risk of vancomycin-induced nephrotoxicity (VIN); however, the effectiveness of follow-up TDM implementation remains unclear. This study aimed to delineate outcomes of follow-up TDM after the initial assessment under real-world conditions in Japan and clarify its association with VIN. METHODS: Retrospective data analysis was conducted using a Japanese health insurance claims database. Approval was granted

Weight loss as a marker of successful decongestion during acute heart failure hospitalisation.

BACKGROUND: Effective decongestion is a key therapeutic goal during hospitalisation for acute heart failure (HF), but the clinical significance of intravenous loop diuretic-induced weight loss and its relationship with biomarker changes remain uncertain. METHODS: We included patients hospitalised for HF in the nationwide Swedish Heart Failure Registry (2017-2021) who received intravenous loop diuretics, survived to discharge and had body weight recorded at admission and discharge. Weight loss was defined as ≥2 kg between admission and discharge. Associations between weight loss, changes in bi

Comparing Ibuprofen and Loop Diuretics for Protein-Bound Uremic Toxins Clearance During Hemodialysis.

INTRODUCTION: Protein-bound uremic toxins (PBUTs), such as indoxyl sulphate (IS) and p-cresyl sulphate (pCS) are associated with cardiovascular mortality in patients undergoing hemodialysis. Because of their high albumin affinity, conventional dialysis techniques achieve poor clearance. This study aims to evaluate the efficacy of different competitive binding displacement strategies using ibuprofen and loop diuretics during online hemodiafiltration (OL-HDF). METHODS: A longitudinal, cross-over study in 12 patients undergoing chronic hemodialysis was performed. Each patient underwent 6 differe

Bumetanide/Synthetic Saponite Composites: Performant Systems for Enhanced Drug Dissolution Rate.

Bumetanide is an efficient diuretic drug with poor water solubility and dissolution rate. In this study, bumetanide/synthetic saponite systems with enhanced solubility and dissolution rate are developed. Different synthetic strategies (hydrothermal and sol-gel) and aluminum contents are explored to prepare saponites with adjustable stacking. Bumetanide loading is achieved by a solution method. The synthetic route is feasible and scalable. Crystalline and non-crystalline bumetanide co-exist in the hybrids, and the non-crystalline fraction prevails in stacked saponite-based composites. Putative thiazide-like diuretic medicinal plants: an electrolyte fingerprint-based review of preclinical evidence.

OBJECTIVES: Thiazide and thiazide-like diuretics remain first-line antihypertensive agents, exerting their primary renal effect through inhibition of the Na+-Cl- cotransporter (NCC) in the distal convoluted tubule, producing natriuresis and saluresis with relative K+ and Ca2+ sparing. Although numerous medicinal plants possess diuretic properties, most preclinical studies assess only urine volume, preventing mechanistic classification and obscuring whether observed diuresis reflects NCC-mediated tubular activity. METHODS: This focused review applied an electrolyte-based pharmacological framew

Mapping Pharmacogenomic Determinants of Adverse Drug Reactions in Hypertension: A Systematic Review.

Objectives: Adverse drug reactions (ADRs) to antihypertensive medications contribute to poor medication adherence and treatment discontinuation. Pharmacogenomic approaches offer a potential means of identifying genetic variants that predict ADR risks, thereby supporting more tailored antihypertensive therapy and improved tolerability. We systematically identify and synthesize available pharmacogenomic evidence for gene-drug-ADR interaction in hypertension across all major drug classes. Methods: This systematic review follows PRISMA guidelines. Five bibliographic databases were systematically s

Cardiorenal Syndrome Type 1 in Patients with Heart Failure with Preserved Ejection Fraction.

CRS type 1 (CRS-1) is defined as acute kidney injury (AKI) caused by acute decompensated heart failure (ADHF). HF is divided into three subtypes according to the value of ejection fraction (EF). HF with preserved ejection fraction (HFpEF) is an increasingly prevalent subtype of heart failure. A significant number of patients with HFpEF during episodes of acute decompensation (ADHFpEF) develop CRS-1. The objective of this narrative review is to summarize the data about the epidemiology, pathophysiological mechanisms, therapy, and prognostic impact of CRS-1 in patients with ADHFpEF. The most imp

Renal outcomes of sacubitril/valsartan compared with valsartan under triple whammy exposure in heart failure patients: A retrospective observational study.

BACKGROUND: The concurrent use of renin-angiotensin-aldosterone system (RAAS) inhibitors, diuretics, and nonsteroidal anti-inflammatory drugs (NSAIDs), known as the "triple whammy (TW)," increases the risk of acute kidney injury (AKI). Sacubitril/valsartan has demonstrated favorable renal effects in heart failure populations; however, its impact under triple whammy exposure remains unclear. METHODS: This single-center retrospective observational study included patients with heart failure receiving valsartan or sacubitril/valsartan in combination with diuretics who were newly prescribed an NSA

Continuous Infusion and Sequential Nephron Blockade Versus Bolus Furosemide in Acute Heart Failure: A Systematic Review.

Acute heart failure (AHF) is a leading cause of hospitalization worldwide, with congestion as its central pathophysiologic feature. Loop diuretics, particularly furosemide, remain the cornerstone of decongestive therapy, yet the optimal administration strategy, continuous infusion versus bolus dosing, remains debated. Furthermore, sequential nephron blockade through the addition of distal tubule-acting diuretics or sodium-glucose cotransporter-2 (SGLT2) inhibitors has emerged as a strategy to overcome diuretic resistance. This systematic review had two primary objectives: first, to compare con

UMOD Genotype and Clinical Outcomes in Heart Failure Patients Treated with Loop Diuretics: A UK Biobank Pharmacogenetic Cohort Study.

Uromodulin (UMOD) regulates tubular sodium handling and modulates NKCC2, the molecular target of loop diuretics (LD). Although UMOD variants have been associated with blood pressure and hypertension, their pharmacogenetic relevance in heart failure (HF) remains unknown. We assessed whether the UMOD rs13333226 genotype is associated with clinical outcomes among patients with HF who initiated LD. We assembled a cohort of UK biobank (UKB) participants diagnosed with HF who initiated LD. Patients were followed in an intention-to-treat fashion for up to 12 months. Cox models estimated confounder-ad
