Patient Navigation Plus mHealth Incentives for Linkage to OUD Care for Adults Who Also Use Methamphetamines: A Pilot Randomized Clinical Trial.

IMPORTANCE: Hospitalization is common among people with opioid use disorder (OUD) and is recognized as a reachable moment to start treatment. Methamphetamine use among people with OUD is associated with decreased linkage to postdischarge OUD care. OBJECTIVE: To determine the feasibility, acceptability, and exploratory clinical outcomes of a pilot randomized clinical trial (RCT) of an adherence-focused mobile health (mHealth) intervention aiming to increase linkage to and retention in outpatient buprenorphine services for patients hospitalized with OUD and methamphetamine use. DESIGN, SETTING

Pharmacological Evaluation of Novel N- and C-Modified Peptide Analogues of VV-hemorphin-5 and VV-hemorphin-7 as Potential Agents With Anti-Seizure Activity.

Recently, a series of N- and C-modified hemorphins have been synthesized, characterized and evaluated for their antibacterial potency. These analogues included amino acids such as cysteine (Cys), glutamic acid (Glu), and histidine (His), as well as 1-adamantanecarboxylic acid (Adam), and niacin (nicotinic acid) (Nic). The current study aimed to explore the anti-seizure potential of these compounds in mice. The role of opioid receptors (ORs) in their mechanism of action was evaluated both pharmacologically and through in silico methods. A battery of tests were used, including the 6-Hz and maxim

Brief report: Meth, tranq, gas station heroin & other drugs complicating addiction treatment.

BACKGROUND AND OBJECTIVES: Patients using fentanyl have worse treatment outcomes; however, little is known about other drugs that complicate treatment. METHODS: A national survey (n = 396) was conducted using a random sample of clinicians waivered to prescribe buprenorphine in the United States. This study reports the results of a single survey item on clinicians' perceptions of other drugs, besides IMF, complicating treatment. RESULTS: Clinicians reported methamphetamine (86.4%), synthetic cannabinoids (42.7%), and xylazine (41.4%) were complicating treatment; reports varied by geographic r

Impact of Pharmacist-Led Risk Mitigation Strategies for Veterans Prescribed Concurrent Benzodiazepine and Opioid Medications with or Without a Stimulant.

INTRODUCTION: Concurrent use of benzodiazepines (BZDs) and opioids increases the risk of overdose by 3-4 times compared to opioids alone. In 2016, boxed warnings and medication guides were added to these medications to highlight the risks of sedation, respiratory depression, coma, and death. Stimulants oppose the central nervous system effects of these agents; thus masking sedation and can lead to potential unintentional overdose. With this knowledge, the objective of this study was to enhance risk mitigation interventions in patients prescribed BZD and opioids with or without stimulants. MET

Stimulant use predicts high-risk substance use behaviors during medication treatment for opioid use disorder.

BACKGROUND: The fourth wave of the overdose epidemic has been characterized by co-utilization of opioids and stimulants. This combined use is associated with high-risk behaviors, including heroin and intravenous drug use. Because research in real-world settings is limited, the objective of this study was to examine the impact of stimulant use before and during treatment for opioid use disorder on high-risk behaviors (intravenous opioid or stimulant use, heroin use). METHOD: The current study included patients who initiated either methadone or buprenorphine from a statewide network of provider

Ketamine enhancement of dexmedetomidine attenuation of methamphetamine-induced agitation in rats.

Methamphetamine (METH)-induced agitation, a major concern in acute METH intoxication, is currently treated with benzodiazepines. Due to current polysubstance use patterns in METH consumption, this treatment may fatally exacerbate respiratory depression produced by opioid adulterants or intentionally co-administered opioids. We previously showed that the α2-agonist dexmedetomidine (DEX), which does not potentiate opioid-induced respiratory depression in clinical practice, can be safely and effectively co-administered with naloxone to attenuate METH-induced agitation following naloxone reversal

Long-term effects of weight-reducing drugs in people with hypertension.

RATIONALE: Long-term weight management drugs have the potential to reduce body weight and blood pressure in obese or overweight individuals. Being overweight or obese is often associated with hypertension, which is linked to an increased risk of cardiovascular mortality and morbidity. The effects of weight-reducing drugs on patient-relevant outcomes, especially in people with hypertension, are currently unclear. This review is the fourth update of one first published in July 2009. OBJECTIVES: Primary objective To assess the effects of drugs approved for long-term weight management on all-caus

Dynamic mechanistic dominance in fentanyl and polysubstance overdose: Implications for next-generation reversal.

High-potency synthetic opioids, such as fentanyl, now dominate the opioid crisis and are often mixed with other active substances. Naloxone, while essential, increasingly produces incomplete or transient reversal-not because the antagonist "fails," but because respiratory depression reflects shifting mechanistic dominance rather than a single μ-opioid receptor pathway. The mechanism currently limiting ventilation-whether μ-opioid receptor, α2-adrenergic, or other central nervous system depressant-determines both clinical trajectory and reversibility of the overdose. Preclinical, clinical, and

Patterns of polysubstance use among people who use drugs on an American Indian reservation of a southern plains Tribal Nation.

BACKGROUND: Polysubstance use is a key driver of overdose and infectious disease risk among people who use drugs (PWUD). Polysubstance use has not been well characterized in Indigenous American communities, which experience a disproportionate overdose burden. OBJECTIVE: To characterize polysubstance use patterns among 192 PWUD on reservation lands of a southern Midwest tribe. METHODS: We conducted a latent class analysis to identify profiles of polysubstance use in the past 6 months and assessed associations between latent classes, sociodemographic correlates, overdose, naloxone receipt, awa

Xylazine reduces methamphetamine-induced hyperlocomotion in rats through κ-opioid and α2-adrenoceptor mechanisms.

Xylazine, a veterinary anesthetic, is an adulterant in fentanyl and heroin that worsens opioid-related adverse effects. Two recent evidences - one epidemiological and the other pharmacological - have emerged regarding xylazine. Epidemiologically, xylazine co-use with methamphetamine (METH) is increasing; for example, in a recent toxicology report, xylazine was detected in 30% of human METH samples. Pharmacologically, xylazine is not only an α2-adrenoceptor agonist but also a full κ-opioid receptor agonist. Here, we determined effects of xylazine (1, 2.5, 5 mg/kg) on METH (1 mg/kg)-induced hype
