Real-Time, Objective Assessment of Facial Paralysis Using a Mobile Tool (FaceADE): Feasibility Case-Control Study.

BACKGROUND: Patients with facial paralysis require detailed clinical assessment and long-term follow-up to monitor facial function. The current standard of care for evaluating facial symmetry and movement uses validated clinician scoring tools such as the House-Brackmann facial paralysis score or the Electronic Clinician-Graded Facial Function Scale (eFACE). Existing tools are difficult to use in normal clinic workflows and do not provide real-time facial movement tracking, representing an unmet need. Therefore, we developed FaceADE, a novel iOS app leveraging native 3D image acquisition capab

Bilateral Pedicled Mucosal Sliding Advancement Flap for Moderate Lower Lip Vermilion Defects Following Ablative Defects: A Technical Innovation.

Reconstruction of moderate lower lip vermilion defects remains challenging. We present a novel single-stage technique, the bilateral pedicled mucosal sliding advancement flap (BPM-SAF), which utilizes a horizontal relaxing incision above the vestibular sulcus to advance adjacent mucosa laterally. This retrospective single-center case series evaluated 10 subjects undergoing BPM-SAF reconstruction following tumor resection. Over a mean follow-up of 13.3 months (range, 1 to 31 months), all mucosal flaps survived completely. Donor sites healed smoothly via secondary epithelialization without infec

Facial Electrical Muscle Stimulation for Soft-tissue Lifting and Symmetry Enhancement.

BACKGROUND: Facial neuromuscular electrical stimulation (NMES) has been used to support facial muscle recruitment in functional settings; however, evidence for aesthetic applications remains limited. This exploratory case series descriptively documents soft-tissue appearance changes and treatment tolerance following a standardized anatomical treatment approach with individualized stimulation settings targeting the lip elevator complex and orbicularis oculi. METHODS: Seven adults (5 women, 2 men; age range 24-39 y) underwent 10 NMES sessions at 1-2 week intervals using a commercially available

"Optimizing Injection Sites of Botulinum Toxin Type A for Improving Oral Commissure Position: A Two-Stage, Prospective, Double-Blind, Randomized Trial".

BACKGROUND: Drooping oral commissures can impart a displeased appearance, negatively affecting psychosocial interactions and well-being. Botulinum toxin type A (BTX-A) is an efficacious treatment for mouth corner lift. However, the optimal injection strategy is controversial. Therefore, this study aims to compare the efficacy and safety of BTX-A injection at different sites of depressor anguli oris (DAO) muscle and to assess the added benefit of BTX-A injection into the platysma (PLT). METHODS: This two-stage, single-center prospective study included a randomized, double-blind randomized tria

Minor Salivary Gland Biopsy in Suspected Sjögren Disease: Diagnostic Value of Fluorescein-assisted Preoperative Gland Evaluation.

PURPOSE: To predict the diagnostic value of the fluorescein-assisted labial salivary gland test (FAST) in diagnosing Sjögren disease (SjD) and its correlation with biopsy positivity. METHODS: Consecutive patients presenting with aqueous-deficient dry eye, that is, dry eye symptoms plus Schirmer less than 10 mm in 1 eye and positive ocular staining, were investigated for SjD using serology and labial minor salivary gland (MSG) biopsy. Using the FAST technique, the number of actively secreting MSGs and the flow rate were calculated in μL/min. Receiver operating characteristic curves, sensitivit

Bilateral Nasolabial Tunnel Flaps for Single-stage Oral Commissure Reconstruction.

Nasolabial flaps are a reliable option for oral commissure reconstruction due to their robust vascularity and minimal donor site morbidity. However, conventional methods that involve folding of the flap into the oral cavity usually lead to bulky commissures and restricted mouth opening. Here, we report a case of bilateral oral commissure defects in an older patient with suspected verrucous carcinoma. Bilateral inferiorly pedicled nasolabial tunnel flaps were used for reconstruction after tumor resection. The flaps were transferred intraorally via subcutaneous tunnels to preserve oral function

Transverse facial cleft repair using a small triangular flap technique.

Transverse facial cleft is a rare congenital malformation frequently associated with anomalies of the first and second branchial arches. We report a case of transverse facial cleft repaired using a small triangular flap technique designed to improve aesthetic and functional outcomes while minimizing scar formation. A 5-day-old infant presented with a right transverse facial cleft and bilateral accessory auricles without additional anomalies. Surgical repair was performed at 5 months of age. A 2-mm triangular flap was designed at the commissure, followed by excision of redundant skin and mucosa

Classifying Synkinesis Smile Dynamics: The UCL Classification System.

OBJECTIVES: Patients with synkinesis struggle to emote through smiling, as their smiles tend to convey negative emotions rather than happiness. There is a wide variation in anatomic makeup and severity of smile asymmetry in patients with synkinesis. Despite this, a classification system does not exist to characterize these variations. This research presents a reliable, repeatable, and easy-to-use synkinesis smile classification system based on simple visual assessment: the "UCL System." METHODS: Two facial reanimation experts classified smile dynamics according to upper lip slope (U), paralyz

The Role of Artificial Intelligence Models in Predicting Post-Prosthetic Facial Esthetics in Edentulous Patients: Clinical and Anthropometric Comparative Study.

OBJECTIVES: Artificial intelligence (AI) is increasingly used in esthetic dentistry; however, its accuracy in predicting post-prosthetic facial outcomes in edentulous patients remains unclear. This study aimed to evaluate the ability of AI models to simulate post-denture facial esthetics compared with actual clinical outcomes. MATERIALS AND METHODS: A prospective within-subject observational study was conducted on 14 completely edentulous patients receiving new complete dentures. Standardized facial photographs were obtained pretreatment (T0) and posttreatment (T1). AI-generated images were p

Rapid Restoration: Suture Stabilization of the Central Facial Landmarks.

BACKGROUND: Reinnervation procedures for patients with flaccid facial paralysis (FFP) often require a lengthy delay until facial symmetry is restored. We have incorporated a simultaneous suture-based "rapid restoration" (RR) to symmetrize the mid- and lower face in the early postoperative period. METHODS: We studied patients with FFP who underwent facial reanimation from March 2022 to February 2025 where dynamic changes were not expected for at least 4 months. We used Emotrics to measure nasolabial fold (NLF) angle, upper lip slope, and oral commissure (OC) height and calculated symmetry indi

Microstomia Correction in an 11-Year-Old Girl With Freeman-Burian Syndrome.

INTRODUCTION: Freeman-Burian syndrome (FBS) is a congenital myopathic craniofacial condition that often includes features outside the craniofacial region. Among the craniofacial manifestations of FBS, the severely reduced oral aperture stands out as one of the most functionally limiting features of the condition. This marked constriction compromises nutritional intake, speech clarity, oral health maintenance, and airway access. Additionally, the distinctive "pursed-lip/whistling" facial appearance imposes considerable psychological and social challenges. CASE PRESENTATION: An 11-year-old girl

Repeatability of Phoneme-Guided Versus Conventional Acquisition of Smiling and Rest-Related Facial States Using 3D Stereophotogrammetry: A Within-Participant Study.

Repeatable acquisition of smiling and rest-related facial states is essential for longitudinal three-dimensional (3D) facial imaging, yet the effect of task instructions may depend on the state and measurement. This within-participant study compared conventional (CONV) and state-specific phoneme-guided (PHON) acquisition in 162 adults. For SMILE, CONV elicited a posed smile, whereas PHON used /tʃ/ articulation followed by maintenance of the resulting perioral configuration; for the REST-related condition, CONV elicited relaxed rest, whereas PHON used sustained /m/ to define a closed-lip refere

Lymphoma in Meckel cave mimicking trigeminal schwannoma - analysis of misdiagnosis causes and insights into imaging: Case report.

RATIONALE: Central nervous system lymphoma typically arises in the deep cerebral regions, while occurrence in the Meckel cave at the skull base is exceedingly rare, with fewer than 1 hundred cases reported globally. Lesions in this area are often misdiagnosed as trigeminal schwannomas or meningiomas. This report aims to enhance clinical recognition of this rare entity by presenting a meticulously analyzed case that was initially misdiagnosed as a trigeminal schwannoma due to overlapping radiological features. Through detailed clinicopathological correlation, we seek to identify key diagnostic

Unusual Presentation of Orofacial Granulomatosis Leading to the Incidental Diagnosis of Idiopathic Pulmonary Fibrosis: A Case Report.

Orofacial granulomatosis (OFG) is a rare chronic inflammatory disorder characterized by non-caseating granulomatous inflammation involving the oral and maxillofacial region. Its heterogeneous clinical presentation and overlap with systemic granulomatous diseases often make diagnosis challenging. We report the case of a 54-year-old woman presenting with a four-month history of progressive mandibular mucosal hypertrophy mimicking an epulis-like lesion. Clinical examination revealed multiple pedunculated, firm, painless mucosal lesions localized to the anterior mandibular ridge in a fully edentul

Primary diffuse large b-cell lymphoma of the lip: a case report and literature review.

BACKGROUND: Primary diffuse large B-cell lymphoma (DLBCL) of the lip is a rare subtype of extranodal non-Hodgkin lymphoma that presents a diagnostic challenge due to its infrequent location and nonspecific clinical symptoms. The early suspicion of this diagnosis is often delayed because of its overlap with benign inflammation/infection, such as abscesses and mucoceles. CASE PRESENTATION: This report describes a 65-year-old male with worsening swelling of the upper lip that was initially diagnosed as a maxillary space infection. Imaging studies, including ultrasound and MRI, showed a poorly de

Co-Occurrence of En Coup de Sabre, Parry-Romberg Syndrome, and Hemimasticatory Spasm: A Case Report.

INTRODUCTION: En coup de sabre (ECDS) and Parry-Romberg syndrome (PRS) are rare, localised scleroderma subtypes, often considered to be overlapping variants of morphea. Hemimasticatory spasm (HMS), a distinct rare neuromuscular disorder characterised by unilateral paroxysmal contraction of the jaw muscles, has rarely been reported in association with these conditions. CASE PRESENTATION: A 20-year-old man presented with a linear, cicatricial alopecic patch over the left parietal scalp, along with indurated and bound-down skin over the left cheek. This was followed by progressive depression and

Efficacy of rTMS in Post-Stroke Bilingual Aphasia: A Single Case Experimental Design.

BACKGROUND: The application of non-invasive brain stimulation in the rehabilitation of bilingual individuals with aphasia following a stroke is understudied. Recent studies show benefits of anodal cerebellar transcranial Direct Current Stimulation in bilinguals, while in monolinguals, repetitive Transcranial Magnetic Stimulation (rTMS) targeting the motor cortex of the lips seems to be effective in aphasia recovery. OBJECTIVE: We aimed to investigate the effects of inhibitory continuous theta burst rTMS stimulation targeting the right motor cortex of the lips during picture naming task in the

Plasma Cell Gingivitis With Cheilitis in an Adolescent: A Case Report.

Plasma cell gingivitis (PCG) is an uncommon plasma-cell-rich inflammatory disorder of the gingiva that may clinically resemble plaque-associated enlargement, granulomatous disease, hematologic disease, or plasma-cell neoplasia. Its association with cheilitis is rare, and diagnosis requires clinicopathologic correlation. An 18-year-old female presented with a 1-year history of progressive gingival enlargement and upper-lip swelling. Intraoral examination showed generalized erythematous marginal and papillary gingival enlargement with loss of stippling, bleeding on probing, pseudopocketing, and
