AI-based Prediction of Neck Metastasis in Early-stage Lip Squamous Cell Carcinoma Using Preoperative Clinical-morphologic Features.

The objective of this study is to evaluate the feasibility of artificial intelligence (AI)-based large language models (LLMs) for predicting cervical lymph node metastasis (pN+) in early-stage lip squamous cell carcinoma (SCC) using preoperative clinical photographs and basic clinical parameters. This retrospective, single-center study included 14 patients (12 males, 2 females; mean age 59.4) with histopathologically confirmed lip SCC who underwent primary tumor excision and selective neck dissection. Preoperative standardized JPEG photographs were analyzed using 2 LLMs-ChatGPT (GPT-5, OpenAI)

Digital vs. Direct Anthropometry with MetiSmile® 3D Face Scanner: A Validation and Reliability Study on a Mannequin Model.

BACKGROUND: Three-dimensional facial anthropometry is increasingly used in orthodontics and orthognathic surgery. Conventional face scanning systems such as Vectra® and 3dMD® are well validated but remain costly and technically demanding. The MetiSmile® 3D face scanner provides a more affordable and portable alternative, yet its accuracy and reproducibility have not been rigorously evaluated. METHODS: Validation was performed on a standardized mannequin head in two phases. Phase 1 assessed mesh reproducibility under artificial lighting (AL), natural lighting (n-AL), and after mesh-refinement

Universal Lip Integrity, Proportion, and Structure: A Framework for Achieving Consistent, Natural-Looking Lip Augmentation Using Hyaluronic Acid Fillers.

The authors of this study present a standardized approach for achieving consistent, natural-looking lip augmentation using cohesive polydensified matrix hyaluronic acid (CPM-HA) fillers. Developed by a panel of 5 global experts, 8 core injection techniques were established to guide procedures, including supporting the oral commissures, enhancing the vermillion border, increasing lateral lip show, increasing vertical height and eversion, improving lip curvature, enhancing lateral structure, and defining the Cupid's bow. The approach outlines a 5-step process: patient interview, documentation, a

Smile Reconstruction for Patients with Congenital Incomplete Facial Paralysis using Modified Lengthening Temporalis Myoplasty.

OBJECTIVES: Treatment of congenital facial paralysis is often postponed because patients exhibit only mild dysfunction and inconspicuous facial asymmetry at rest. Surgical facilitation of smiling has significant implications for the quality of life and the physical and mental development of patients. We describe facial reanimation using modified lengthening temporalis myoplasty (LTM) in patients with congenital incomplete facial nerve palsy who first presented to our institution after the age of 10 years. METHODS: Three patients (age, 12-26 years; two males and one female) underwent facial re

A Case of Congenital Midline Sinus of the Upper Lip.

Congenital lip sinus is known to occur in the upper lip, lower lip, or oral commissure, but a midline sinus of the upper lip is extremely rare. This report describes a 5-year-old boy who was born as a dizygotic twin at a gestational age of 33 weeks and 6 days, weighing 2,118 g. He was noted at birth to have a subcutaneous mass with an opening in the philtrum of the upper lip. Magnetic resonance imaging at 1 year of age revealed a teardrop-shaped cyst with high intensity on T2-weighted images beneath the upper lip. Surgical excision was performed due to the gradual enlargement of the mass. The

An Investigation of Mucosal Leishmaniasis in the Military Health System.

Leishmaniasis is a protozoal infection with an increased risk of transmission to those serving in the U.S. Military due to theaters of operation in endemic regions. There has, in recent decades, been robust experience with old-world leishmaniasis in the Military Health System (MHS); however, new-world leishmaniasis, which may result in mucosal leishmaniasis, has been less studied. A total of 88 patients from 2012 to 2022 with diagnosis codes for "mucocutaneous leishmaniasis" or "leishmaniasis, unspecified" were identified in the Military Data Repository and reviewed. Within this cohort, upon m

Correlation of Positive Signs of Functional Neurological Disorder With Headache Severity: A Prospective Cross-Sectional Study.

OBJECTIVE: Positive signs are clinical signs that are not explained by a structural neurological lesion and are considered the hallmark of functional neurological disorder (FND). In the literature and in clinical experience, positive signs are observed among patients without FND. The aim of the study was to examine the significance of positive signs among headache patients. METHODS: The authors of this prospective study recruited patients in headache consultation at the University Hospital Inselspital, Bern, Switzerland. Inclusion criteria were headache or facial pain in the past 3 months and

Nasal Glioma in a Newborn With Suspected Pai Syndrome: Surgical and Diagnostic Insights.

INTRODUCTION: Nasal glial heterotopias, or nasal gliomas, are rare congenital midline lesions composed of ectopic, non-neoplastic glial tissue. They are often mistaken for encephaloceles due to overlapping clinical and embryologic features. Accurate diagnosis is essential to avoid complications associated with surgical intervention. METHODS: The authors present the case of a newborn female infant born at 35 weeks and 6 days of gestation with a midline nasal mass, respiratory distress, and multiple dysmorphic features. Prenatal and postnatal imaging revealed a pedunculated nasal lesion without

Case Report: Misdiagnosis of cellular cervical adenomyoma.

BACKGROUND: Cellular cervical adenomyoma (CCA) is a rare benign tumor composed of proliferative smooth muscle cells and benign endocervical-type glands. Due to its nonspecific clinical and imaging features, CCA can mimic cervical malignancies, posing a diagnostic challenge. CASE PRESENTATION: A 37-year-old woman presented with irregular vaginal bleeding. Pelvic examination revealed an approximately 3 cm friable mass on the posterior cervical lip. Transvaginal ultrasound (TVUS) showed a well-circumscribed, heterogeneous lesion with mild peripheral vascularity, favoring a cervical leiomyoma. In

Frequency and Characteristics of Craniomaxillofacial Tumors: A Five-Year Retrospective Institutional Study.

Background: Hospital-based data can complement registry estimates for cranio-maxillofacial (CMF) oncology, particularly in under-reported regions. We aimed to describe the institutional case-mix of CMF tumor diagnoses, standardized to ICD-10 sites, and to quantify trends using visit-normalized indicators. Methods: We conducted a retrospective, observational, single-center, hospital-based study of diagnosis-level encounters (2012-2016). Diagnoses were recoded to ICD-10 and restricted to CMF sites (lip, oral cavity, major salivary glands, oropharynx/hypopharynx, nasal cavity/middle ear, paranasa

Oral Lesions With Identical Clinical Presentation and Different Histopathological Diagnoses: A Case Series of Mucocele, Schwannoma, and Hamartoma.

Oral lesions with similar clinical presentation often conceal distinct histopathological profiles, creating diagnostic challenges and potential risks of misdiagnosis. This report presents the cases of three patients whose oral lesions appeared macroscopically alike but revealed markedly different microscopic characteristics. The first case involved a 20-year-old man with a persistent lower lip lesion that had recurred after two prior interventions; histopathological analysis confirmed a mucocele characterized by mucin extravasation and chronic inflammatory infiltrate. The second case was of a

Why do marionette lines appear? Exploring the anatomical perspectives and role of thread-based interventions.

The pathogenesis of marionette lines involves a complex interplay of anatomical, physiological, and age-related factors leading to the development of wrinkles around the oral commissures. This exploration delves into the distinct anatomical predispositions observed among different ethnicities, emphasizing the role of compact modiolus structures and muscle compositions. Notably, individuals of East Asian descent exhibit inherent facial structures that predispose them to pronounced sagging around the oral commissures during aging. The emergence of distinct facial lines, such as the commissural l

Reconstruction of Squamous Cell Carcinoma on Oral Commissure With Hatchet-Shaped Flap.

The mouth is a unique and prominent element of the lower face. Given the complex anatomy, aesthetic appearance, and function of the oral commissure, its reconstruction due to various causes presents a significant challenge for surgeons. Squamous cell carcinoma (SCC) of the lip is the most common type of oral cancer, accounting for approximately 25% to 30% of all oral cancers. Wide excision is the treatment of choice, and the prognosis is generally favorable. We encountered a case of SCC of the right oral commissure in a 69-year-old man. We designed a hatchet-shaped flap to minimize anatomical

Novel Asymmetric Y Design of Fascial Sling for Restoration of Oral Competence and Adequate Mouth Opening in Oral Commissure Defects Post-Malignancy Resection.

Reconstruction of through-and-through defects of cheek and commissure resulting from cancer resection are challenging. The specialized function of oral competence that the oral commissure and lip play is difficult to replicate with flap only reconstruction. Static slings play an important role in improving the functional and aesthetic outcome. The asymmetric "y" fascial sling helps in achieving the goals of oral competence along with adequate mouth opening. A total of 10 patients were operated by this technique. The results were satisfactory in terms of functional and aesthetic outcome postsur
