Fast Analysis of RNA from Transcription and Rho-Dependent Termination Assays Using Automated Capillary Electrophoresis.

Conventional analysis of RNA products synthesized by bacterial RNA polymerase (RNAP) in in vitro transcription assays relies on radioactive labeling, followed by separation of labeled RNAs by denaturing urea-polyacrylamide gel electrophoresis (PAGE). This approach provides excellent resolution across a wide range of RNA lengths (from 2 nt to several hundred nucleotides), but the use of radioisotopes requires specialized facilities, strict waste-disposal procedures, and is often restricted. To circumvent these limitations, we adapted a standard in vitro transcription assay for use with the Agil

State of the global tuberculosis epidemic: burden, progress towards WHO End TB Strategy targets and systemic challenges.

Mycobacterium tuberculosis remains the leading cause of death from a single infectious pathogen globally despite decades of effective chemotherapy. In 2024, an estimated 10·7 million people developed tuberculosis, including approximately 620 000 people living with HIV (PLHIV), and tuberculosis caused an estimated 1·23 million deaths overall, including approximately 150 000 deaths among PLHIV. Men accounted for more than half of the cases, and children represented a substantial burden, reflecting ongoing transmission and diagnostic gaps. Approximately a quarter of the world's population has bee

Minimal supply chain and forecasting testing volumes for clinical bacteriology laboratories in resource-limited settings for individual patient care: a costing study.

BACKGROUND: Inadequate access to clinical bacteriology testing in low-resource settings compromises individual patient care and contributes to antimicrobial resistance. Funders, policy makers, and health administrators working to expand bacteriology testing in sub-Saharan Africa require evidence-based guidance. The aim of this study was to assemble a costed consensus list of minimum equipment and consumables required to implement and operate a functional clinical bacteriology laboratory in secondary and tertiary care hospitals in sub-Saharan Africa, to guide funders and health administrators.

Advances in tuberculosis diagnostics and biomarkers, 2020-26: scientific evidence, recommendations, gaps, and developmental priorities.

Major advances in tuberculosis diagnostics and biomarker research from 2020 to 2025 have informed updated WHO policy guidance, consolidating recommendations for the detection of tuberculosis disease and drug resistance within a single framework. These recommendations align screening, diagnosis, and drug-resistance testing across populations and specimen types. Digital chest radiography, increasingly supported by computer-aided detection, is now a core WHO-recommended screening and triage tool, improving case detection and referral for confirmatory testing at scale. Decentralised molecular diag

Advances in development of tuberculosis vaccines and preventive drug therapies: current pipeline, evidence, and access.

Tuberculosis remains the leading cause of death from a single infectious agent globally. The WHO Global Tuberculosis Report 2025 indicates that progress towards the WHO End TB Strategy targets remains off track despite modest declines in incidence and mortality. In 2024, an estimated 10·7 million people developed tuberculosis and 1·23 million died from the disease. In this Series paper, we examine evidence published between Jan 6, 2020, and Jan 6, 2026, to provide an updated overview of tuberculosis vaccines and tuberculosis preventive treatment (TPT). As of June 4, 2026, 18 tuberculosis vacci

Advances in tuberculosis treatment: novel regimens, new drug pipelines, host-directed therapies, and updated management guidelines.

Tuberculosis treatment has undergone its most profound transformation since the launch of the standardised DOTS strategy in 1994. Since 2020, a series of pivotal randomised trials, including TB-PRACTECAL, ZeNix, Nix-TB, endTB, BEAT-TB, SHINE, and Study 31/A5349, have redefined the management of both drug-susceptible and drug-resistant tuberculosis. These studies have enabled shorter, fully oral regimens with improved efficacy and safety across adult and paediatric populations, including people with HIV, and have driven major updates to WHO treatment guidelines. Despite these advances, tubercul

Diagnostic performance and testing costs of pooled testing for molecular detection of tuberculosis in seven countries: a prospective, observational, diagnostic accuracy study.

BACKGROUND: Globally in 2024, only 54% of people diagnosed with tuberculosis received an initial WHO-recommended rapid molecular diagnostic test. Pooled testing could expand access to molecular detection using existing laboratory capacity. We aimed to evaluate the performance of pooled tuberculosis testing in a multicentre diagnostic accuracy study. METHODS: In this prospective diagnostic accuracy study, we enrolled individuals with presumptive tuberculosis through facility-based case finding (district hospitals and primary health centres) and community-based case finding in Bangladesh, Brazi

Novel Surrogate Indicators of Intracranial Meningioma Consistency and Outcomes After Resection.

BACKGROUND AND OBJECTIVES: Tumor consistency affects resection and outcomes in meningioma surgery. This study aimed to identify predictors of tumor consistency using the Zada grading scale and assess their association with surgical outcomes and prognosis. METHODS: We retrospectively reviewed patients aged 18 years and older who underwent intracranial meningioma resection from 2014 to 2024, with ≥1 year of follow-up. Tumors were graded into 5 consistency types per Zada's criteria. Data on demographics, imaging (T2-weighted MRI), histology, and surgical outcomes were collected. Signal intensity

T2-Weighted MRI Signal Intensity Ratio Predicts Intraoperative Intracranial Meningioma Consistency: A Retrospective Cohort Study.

BACKGROUND AND OBJECTIVES: Meningioma consistency is linked to important clinical and surgical outcomes, and preoperative knowledge of consistency may have significant implications for operative planning and decision making. This study aimed to determine whether intraoperative meningioma consistency is associated with preoperative T2-weighted imaging (T2WI) and diffusion-weighted imaging. METHODS: Tumor consistency was prospectively assigned in 188 meningioma patients at the time of surgery over a twelve-year period (2012-2024) using a 5-point scale with validated interuser reliability. Decision-Making in Intrathecal Therapy for Leptomeningeal Disease.

INTRODUCTION: Guidelines suggest intrathecal (IT) pharmacotherapy for patients with leptomeningeal disease (LMD). However, evidence on the benefit of IT treatment is limited and IT therapy is an invasive procedure in patients with limited life expectancy. Available drugs for IT therapy do not play a role in systemic treatment of the most common cancers related to LMD, and it is unclear to which extent and how the recommendation for IT therapy is implemented in daily clinical practice. METHODS: A panel of 24 dedicated Swiss experts in the field of breast cancer, lung cancer, melanoma, and neur

NICO Myriad Automated Preservation System improves meningioma sample quality for molecular pathology assays.

Meningiomas are central nervous system tumors primarily treated by surgery and/or radiation with large variance in progression-free survival. Emerging molecular diagnostics such as circular RNA (circRNA) profiling show potential for enhancing diagnostic accuracy but are limited by the processing workflows associated with traditional fresh-frozen protocols that may lead to biomarker degradation. The NICO Automated Preservation System (APS) is a surgical tool that aspirates and resects intact tumor cores, quickly storing the removed tissue in a refrigerated environment of Ringer's Lactate fluid.

Automated deep learning-based segmentation and volumetric analysis of meningiomas.

INTRODUCTION: Meningiomas are the most common primary intracranial tumors and are frequently monitored over extended periods. Volumetric assessment typically requires manual segmentation, which is time-consuming and associated with interrater variability. This study aimed to develop and validate a deep learning-based model for the automated segmentation of meningiomas and associated peritumoral edema on preoperative magnetic resonance imaging (MRI). METHODS: We trained a standard nnU-Net deep learning model on contrast-enhanced T1-weighted and FLAIR MRI scans from 100 patients treated at the

Community-Acquired Klebsiella pneumoniae Meningitis With Liver Abscess and Neurological Sequelae in an Immunocompetent Adult: A Case Report.

Klebsiella pneumoniae meningitis (KPM) is a rare but severe form of community-acquired bacterial meningitis that is often associated with high mortality and neurological sequelae. We report a 46-year-old previously healthy male who presented with acute altered mental status and high-grade fever. Cerebrospinal fluid analysis was consistent with bacterial meningitis, and cultures from the cerebrospinal fluid and blood showed Klebsiella pneumoniae (K. pneumoniae) growth. The clinical course of the patient was complicated with seizures and acute right-sided hemiplegia. Persistent fever (despite ap

[Impact of TERT and CDKN2A/B driver mutations on meningioma recurrence: a systematic review and meta-analysis].

BACKGROUND: Evolution of the WHO classification for meningiomas demonstrates a shift towards integrative approach, where molecular markers are essential for risk stratification. However, their prognostic significance depending on histological grade requires further clarification. OBJECTIVE: To evaluate the impact of TERT promoter mutations and CDKN2A/B deletions on meningioma recurrence, stratified by tumor grade. MATERIAL AND METHODS: A systematic review and meta-analysis (PRISMA-P) of 17 cohort studies (n=5867) was conducted. Random-effect models and network meta-analysis were employed to

A novel mycovirus identified in clinical isolates of Cryptococcus neoformans.

Mycoviruses are widespread throughout the fungal kingdom and are known to infect diverse fungal taxa, including fungal species that are important plant and human pathogens. Although many mycoviruses have been found to have minimal effects on their host, several viruses have been found to modulate fungal physiology, and, as a result, impact fungal virulence. Screens for mycoviruses in clinically relevant fungi have identified numerous mycoviruses within several important human pathogens, but mycoviruses remain uncharacterized in the clinically relevant human pathogen Cryptococcus neoformans.
