Delayed diagnosis of patients with rifampicin-resistant tuberculosis and its associated factors in a high tuberculosis endemic area Guizhou, China.

BACKGROUND: Rifampicin-resistant tuberculosis (RR-TB) threatens global TB control; diagnostic delays worsen transmission and outcomes. Evidence on delays in high-burden settings is limited. This study quantified patient, health-system, and total diagnostic delays among RR-TB patients in Guizhou Province, China (2019-2023) and identified factors associated with prolonged delays to inform targeted control measures. METHODS: We included 2,677 RR-TB patients diagnosed and registered in Guizhou from 2019 to 2023. The status of three types of delays at different levels was investigated. Group compa

Individualized TB care for drug-susceptible pulmonary tuberculosis: clinical alternatives when HRZE is contraindicated or not tolerated.

INTRODUCTION: While the standard HRZE regimen is the cornerstone of drug-susceptible tuberculosis (DS-TB) treatment, its success is frequently undermined by treatment-limiting adverse drug reactions (ADRs) and complex drug-drug interactions (DDIs). In clinical practice, managing those situations remains a critical challenge often overlooked by guidelines. Effective toxicity management is essential to prevent treatment interruption, loss-to-follow up, relapse, and escalating healthcare costs. AREAS COVERED: We provided a clinical framework for identifying and mitigating toxicities associated w

Near-point-of-care Diagnosis of Tuberculosis With LyocartE MTB Assay: a Multicenter Diagnostic Accuracy Study.

BACKGROUND: Tuberculosis (TB) continues to pose a major threat to global public health. The development of rapid near point-of-care diagnostics utilizing non-sputum specimens is critical for advancing TB control efforts. METHODS: In this multicenter prospective diagnostic accuracy study, 689 consecutive individuals with presumptive TB were enrolled from 3 sites in China. Sputum and tongue swab specimens were collected from all participants for index testing with the LyocartE assay and comparator testing with Xpert Ultra, smear microscopy, and culture. The diagnostic performance of LyocartE fo

PhoPR variants from several phylogenetic lineages of tuberculosis bacilli respond differently to extracellular signals.

The global spread of Mycobacterium tuberculosis (MTB) is facilitated by airborne transmission, a low infectious dose, and the pathogen's ability to persist in a latent state. Despite sharing over 99% genomic identity, MTB strains and lineages show significant variability in transmissibility. We hypothesized that natural polymorphisms in the two-component regulatory system (TCS) PhoP-PhoR (PhoPR), a major virulence regulator, contribute to this variability.First, we identified zinc and cadmium as new inducers of PhoPR. Second, we expressed phoPR variants from strains with varying transmissibili

Prevalence of Drug-Resistant Mycobacterium tuberculosis in Rural Communities of Vhembe District, South Africa.

Drug-resistant tuberculosis (DR-TB) is still a concern for public health and health security, making TB the world's largest cause of death from a single infectious agent. This Chapter was aimed at evaluating the prevalence of DR-MTB in rural communities of South Africa. A cross-sectional study in rural Vhembe District, South Africa, recruited 175 active TB patients. Data on lifestyle behaviour, socioeconomic, and environmental characteristics were collected via a questionnaire, and samples (blood and sputum) were collected. The U-rapid blood test confirmed HIV status. DNA was extracted from sp

Evaluation of the deeplex Myc-TB-targeted next-generation sequencing assay for mycobacterial identification and drug resistance detection.

Accurate differentiation of Mycobacterium tuberculosis complex (MTBC) and nontuberculous mycobacteria (NTM), together with early detection of drug resistance, is essential for guiding therapy and infection control. Targeted next-generation sequencing assays, such as the Deeplex Myc-TB (GenoScreen, Lille, France), allow simultaneous identification of species and detection of drug-resistance mutations. We evaluated Deeplex Myc-TB on 304 primary, culture-positive mycobacterial growth indicator tube (MGIT) broths collected between May 2024 and March 2025. The assay generated determinate results fo

Anchoring bias in a post-tuberculosis patient: delayed diagnosis of Mounier-Kuhn syndrome.

This article describes the case of a male patient in his late 40s with a history of pulmonary tuberculosis (TB) 25 years ago who presented with a decade-long recurrent cough, sputum production and frequent pneumonia. His symptoms were repeatedly wrongly credited to TB reactivation or reinfection, leading to several unnecessary courses of anti-tubercular therapy. A decisive high-resolution CT thorax revealed the pathognomonic features of Mounier-Kuhn syndrome such as marked tracheobronchomegaly and airway diverticulosis. This provided an appropriate explanation for his clinical course. This cas

Exploring generational differences in the impact of stigma on mental health among people affected by leprosy in rural India: a qualitative study.

Leprosy continues to be highly stigmatised in India, resulting in the continuation of poor mental health outcomes for affected individuals and delays in seeking treatment. Whilst previous research suggests that the effectiveness of intervention strategies to reduce stigma and improve mental health can vary across generations, little is known about how leprosy-related stigma specifically impacts the mental health of different generational groups. This study, therefore, explored these generational differences in rural India. Through qualitative in-depth interviews conducted between June and July

"Fragmented care pathway in a layered TB care system": a grounded theory approach to understand care pathways and delays in TB care in Odisha.

INTRODUCTION: Tuberculosis is a significant public health challenge in India, accounting for one-fourth of the global burden. Complex social, economic and behavioural factors among people affected by TB, combined with health system factors, shape care pathways and contribute to delays in TB care. OBJECTIVES: Given this background, the study aimed to develop a constructivist grounded theory explaining how people affected by TB navigate care pathways and experience delays in two contrasting districts of Odisha. METHODS: Adopting a constructivist grounded theory approach, the study was conducte

Genetic clusteringof KPC-2-producing carbapenem-resistant Klebsiella pneumoniae ST5422-KL19 in Shanghai, China.

The globally disseminated high-risk CRKP clone ST5422, though frequently harboring multiple carbapenemase genes, exhibits only sporadic occurrence in China. We report a Shanghai genetic clustering involving seven ST5422 CRKP isolates harboring blaKPC-2. Comprehensive characterization through antimicrobial susceptibility testing, whole-genome sequencing, phylogenetic reconstruction, and virulence/fitness assessments revealed all strains co-harbored blaCTX-M-15 with homogeneous resistance profiles: high-level resistance to carbapenems, ceftazidime/avibactam, and tigecycline, while maintaining su

Immunoregulatory roles of microRNAs in bacterial infections: A focus on tuberculosis and sepsis.

Tuberculosis and sepsis represent opposing extremes of host immune regulation: one characterised by sustained immune adaptation that permits Mycobacterium tuberculosis persistence, the other by rapid immune collapse that precipitates life-threatening inflammation. Examining these divergent outcomes through a shared regulatory lens highlights microRNAs (miRNAs) as pivotal, yet context-dependent, modulators of antimicrobial defence and immunopathology. Rather than acting as linear switches, miRNAs operate within densely interconnected networks, simultaneously shaping inflammatory signalling, met

A cross-sectional study on discordances between Anyplex™ II MTB/MDR and BACTEC MGIT 960 system for assessing susceptibility for Mycobacterium tuberculosis: analysis of clinical specimens from patients with isoniazid-monoresistant, rifampicin-monoresistant, and multidrug-resistant tuberculosis.

BACKGROUND: Rapid molecular tests have facilitated early diagnosis and guided treatment of tuberculosis (TB) by providing faster results than the gold-standard phenotypic drug susceptibility testing (DST). Anyplex™ II MTB/MDR is an RDT available in Thailand that simultaneously detects Mycobacterium tuberculosis (MTB) complex and mutations associated with isoniazid (INH) and rifampicin (RIF) resistance, enabling genotypic DST in various clinical specimens. However, discordance between the Anyplex™ II MTB/MDR and phenotypic DST creates a decision-making dilemma. METHODS: A retrospective review

From systemic disease to the eye: Why senescence deserves attention in ocular infections.

The global increase in life expectancy has led to a growing elderly population, bringing new challenges in managing infectious diseases. Advancing age is accompanied by progressive dysfunction of both innate and adaptive immunity, resulting in impaired responses to pathogens and elevated morbidity and mortality. Cellular senescence, a state of permanent cell-cycle arrest with extensive molecular and phenotypic changes, including chromatin remodelling and the senescence-associated secretory phenotype (SASP), has emerged as a key factor in this process. Though initially protective as a tumor-sup

Untargeted and Targeted Liquid Chromatography-Mass Spectrometry-Based Lipidomic Profiling Revealed a Potential Biomarker Panel to Distinguish Clinical Isolates of M. tuberculosis From Nontuberculous M. kansasii.

The rising incidence of nontuberculous mycobacteria (NTM) infections, particularly Mycobacterium kansasii (M. kansasii), and their overlapping clinical features with Mycobacterium tuberculosis (MTB) are leading to diagnostic ambiguity in tuberculosis-endemic regions. Accurate differentiation remains limited by conventional diagnostic methods, emphasizing the need for molecular- and lipid-based biomarkers. In the present era of "OMICS" sciences, herein we attempted a comprehensive analysis of MTB and M. kansasii lipid profiles to elucidate species-specific lipidomic features as potential biomar

An evaluation of designs for Phase I/IIa dose-finding studies in Tuberculosis.

Early development of tuberculosis (TB) treatments often separates dose-finding in healthy volunteers (Phase I) and safety and early activity in patients (Phase IIa). The BTZ-043 study (NCT04044001) is a recent study that combined these two objectives in patients in a single study. In this work, we describe and compare three different design options which consider safety and activity endpoints differently in the dose escalation process. In simulations we show that the design that incorporates information about activity together with safety in the dose escalation process allows more precise esti

[Rare muscular localization of tuberculosis mimicking a cervical tumor: a case report from Togo].

OBJECTIVE: The detection of tuberculosis in an apparently isolated location, such as the sternocleidomastoid muscle, is exceptional. It can be mistaken for a cervical tumor. This report presents a case with its diagnostic and therapeutic particularities. OBSERVATION: An immunocompetent patient presented with right anterolateral cervical swelling, which was suggestive of a thyroid tumor. Paraclinical investigations revealed tuberculosis of the right sternocleidomastoid muscle and a latent homolateral pulmonary focus. The patient responded well to medical treatment. CONCLUSION: Muscular tuberc
