Sudden Collapse in a Child Revealing a Malignant Coronary Anomaly: A Case of Congenital Anomaly of Left Coronary Artery Origin.

Chest pain in children can be a common presentation with causes including idiopathic, musculoskeletal and respiratory conditions, but is rarely cardiac in origin. However, in rare cases, it may indicate life-threatening conditions such as anomalous origin of coronary arteries. Among these, anomalous origin of the left coronary artery from the right sinus (ALCRSV) with an intramural and inter-arterial course is particularly concerning due to its association with myocardial ischemia and sudden cardiac death. We present the case of a previously healthy seven-year-old girl who experienced an episo

Can Body Mass Index Values and/or Player Position be Viable Predictors for Exertional Heat Illness in American Football Players?

BACKGROUND: Exertional heat illnesses (EHIs) affect thousands of high school athletes each year. Although there are clear risk patterns for fatal EHIs based on player characteristics, there is less information on these associations for nonfatal EHIs. HYPOTHESIS: Prediction of nonfatal EHI risk in American high school football participants can be determined using commonly available data on body mass index (BMI) values and/or player position. STUDY DESIGN: Epidemiological study. LEVEL OF EVIDENCE: Level 3. METHODS: Participants from 22 high schools in the state of Georgia in the United State

Heat acclimation alleviates the heat stress-induced impairment of vascular endothelial cells.

Heat acclimation (HA) is found to help decrease the incidence of heat-related illnesses such as heat syncope and exertional heat stroke. However, the response of vascular endothelial cells to HA remain to be elucidated. In this study, mouse brain microvascular endothelial cells (bEnd.3), human umbilical vein endothelial cells (HUVEC), and human aortic endothelial cells (HAEC) were selected. The cells were first subjected to HA at 40 ℃ for 2 h per day for 3 days, and then subjected to heat stress at 43 ℃ for 2 h or 4 h. After heat stress, HA-pretreated cells showed a significant increase in cel

The Clinical Profile of Heatstroke Patients Admitted to the Intensive Care Unit: A Case Series From a Tertiary Care Center in New Delhi, India.

Heat-related illnesses include a spectrum of disorders ranging from heat syncope, muscle cramps, and heat exhaustion to heat emergencies such as heatstroke. Severe heatwaves can lead to extremely high environmental temperatures and a spurt of cases of heat-related illnesses and heatstroke. The incidence of such heat-related medical emergencies was much higher in 2024 compared to previous years throughout North and Central India. Here, we are describing a case series of five patients admitted to the intensive care unit of a tertiary care center in New Delhi during the month of June 2024.

[Heat illness].

Thermoregulation keeps the normal body temperature of humans at approximately 37 °C. However, as a result of heat load - both endogenous and exogenous heat - it can occur that the body is unable to dissipate excess heat, leading to an increase in the core body temperature. This can result in various heat illnesses, ranging from mild, non-life-threatening conditions, such as heat rash, heat edema, heat cramps, heat syncope and exercise associated collapse to life-threatening conditions, namely exertional heatstroke and classic heatstroke. Exertional heatstroke is the result of strenuous exercis

Improvements in Orthostatic Tolerance with Exercise Are Augmented by Heat Acclimation: A Randomized Controlled Trial.

INTRODUCTION: Heat adaptation is protective against heat illness; however, its role in heat syncope, due to reflex mechanisms, has not been conclusively established. The aim of this study was to evaluate if heat acclimation (HA) was protective against heat syncope and to ascertain underlying physiological mechanisms. METHODS: Twenty (15 males, 5 females) endurance-trained athletes were randomized to either 8 d of mixed active and passive HA (HEAT) or climatically temperate exercise (CONTROL). Before, and after, the interventions participants underwent a head up tilt (HUT) with graded lower bo

Heatwaves and human sleep: Stress response versus adaptation.

The World Meteorological Organization considers a heatwave as "a period of statistically unusual hot weather persisting for a number of days and nights". Accompanying the ongoing global climate change, sharp heatwave bouts occur worldwide, growing in frequency and intensity, and beginning earlier in the season. Heatwaves exacerbate the risk of heat-related illnesses, hence human morbidity and mortality, particularly in vulnerable elderly and children. Heat-related illnesses present a continuum from normothermic (prickly heat, heat edema, heat cramps, heat tetany) to hyperthermic syndromes (fro

Exertional heat illnesses in marching band artists: a case series.

Marching band (MB) artists frequently spend many hours engaged in outdoor physical activity. Anecdotal evidence and small studies have indicated that MB artists do experience heat-related health problems. Yet, unlike athletes, military personnel, or workers, there is very little research on heat-related hazards among this unique population. Here, we seek to understand the incidence and circumstances under which exertional heat illnesses (EHIs) occur among MB artists over a 31-year period (1990-2020) across the USA. Using an on-line news dataset, we identified 34 separate events and at least 39

Fatal sickling-associated microvascular occlusive crisis in a young with sickle cell trait.

Sickle cell trait (SCT), a heterozygous state characterized by hemoglobin AS, occurs commonly in sub-Saharan Africa, South America, Central America, India, and the Mediterranean countries. SCT is compatible with a normal lifespan and is not commonly regarded as a cause of morbid illness or death compared to its homozygous counterpart. We describe a case of fatal sickling-associated microvascular crisis, identified on post mortem evaluation in a previously undiagnosed 21-year-old military recruit with sickle cell trait. The individual presented with repeated syncope episodes during his training

Patient-related factors associated with severe heat-related illnesses in Karachi: A hospital perspective.

INTRODUCTION: Corrective surgery for Dupuytren's disease represents a significant proportion of a hand surgeon's workload. The decision to go ahead with surgery and the success of surgery requires measuring the degree of contracture of the diseased finger(s). This is performed in clinic with a goniometer, pre- and postoperatively. Monitoring the recurrence of the contracture can inform on surgical outcome, research and audit. PATIENTS AND METHODS: We compared visual and computer software-aided estimation of Dupuytren's contractures to clinical goniometric measurements in 60 patients with Dupu
