At present, the main use of Chinese medicine in the treatment of sepsis and sepsis shock is "four patterns and four methods". Although this method is suitable for clinical treatment, it is difficult for doctors to use, and the relationship between "four patterns" has not been clarified. The author introduces the dialectical thinking of "ministerial fire theory", and aims to interpret "four patterns and four methods". Sepsis and sepsis shock can be devided into four stages: Yang excess, Yin insufficient, fire damage, and fire decay, which corresponding to four disease stages: heat syndrome, hea

The study of heat adaptation in military personnel offers generalizable insights into a variety of sporting, recreational and occupational populations. Conversely, certain characteristics of military employment have few parallels in civilian life, such as the imperative to achieve mission objectives during deployed operations, the opportunity to undergo training and selection for elite units or the requirement to fulfill essential duties under prolonged thermal stress. In such settings, achieving peak individual performance can be critical to organizational success. Short-notice deployment to

Hemodialysis (HD) is a lifesaving treatment for patients with end-stage renal disease, which is very efficient in the correction of abnormalities of the internal environment. However, this efficiency also induces significant hemodynamic, thermal, and respiratory stressors. These have parallels with the extreme physiologic demands which are normally mainly experienced by healthy subjects under adverse environmental conditions, with the difference that they must be endured by a vulnerable patient population. Hemodynamic stress induced by ultrafiltration leads to a decline in circulating blood vo

Exertional heat illnesses (EHI) occur in various populations and settings. Within a school setting, there are student athletes who take part in physical activity where the risk of EHI is increased. The National Athletic Trainers' Association (NATA) released an updated position statement on EHI in September of 2015. This article is a summary of the position statement. The sports medicine team, including school nurses and athletic trainers, provides quality health care to these physically active individuals. Thus, it is important for school nurses to understand the prevention, recognition, and t

KEY POINTS: Humans are unique in controlling body temperature in a hot environment by a large amount of skin blood flow; however, the decrease in total peripheral resistance due to systemic cutaneous vasodilatation and the reduction of venous return to the heart due to blood pooling in the cutaneous vein threatens blood pressure maintenance in the upright position, and occasionally causes heat syncope. Against this condition, cutaneous vasodilatation is reportedly suppressed to maintain arterial pressure; however, the nerve activity responsible for this phenomenon has not been identified. In t

BACKGROUND: While antihypertensive therapy is known to reduce the risk for heart failure, myocardial infarction, and stroke, it can often cause orthostatic hypotension and syncope, especially in the setting of polypharmacy and possibly, a hot and dry climate. The objective of the present study was to investigate whether the results of our prior study involving continued use of antihypertensive drugs at the same dosage in the summer as in the winter months for patients living in the Sonoran desert resulted in an increase in syncopal episodes during the hot summer months.
METHODS: All hypertens

BACKGROUND: Guidelines for preventing exertional heat illnesses (EHIs) during extreme heat stress should be specific to regional environments, age, and sport and should be based on evidence of reducing the risk. Each year in the United States, over 1 million high school football players practice in the August heat; however, no published data describe the incidence of EHIs in these athletes.
PURPOSE: To describe the environmental conditions and incidence of EHIs during high school football practices over a 3-month period.

Health and productivity impacts from occupational heat stress have significant ramifications for the large workforce of India. This study profiled occupational heat stress impacts on the health and productivity of workers in select organized and unorganized Indian work sectors. During hotter and cooler seasons, Wet Bulb Globe Temperatures (WBGT) were used to quantify the risk of heat stress, according to International workplace guidelines. Questionnaires assessed workers' perceived health and productivity impacts from heat stress. A total of 442 workers from 18 Indian workplaces participated (

In this summer season (May-June 2014) most of the days temperature was more than 40°C. Our hospital is surrounded by huge slums area. The population which is low in socioeconomic status used to work in such high temperature for more than 8 h daily. Hence, they are very prone to develop heat injuries in the form of heat edema, heat tetany, heat syncope, heat cramps, miliaria rubra, heat exhaustion, and heatstroke. Again it is compulsory to upgrade our knowledge on this life threatening condition.

OBJECTIVE: To present best-practice recommendations for the prevention, recognition, and treatment of exertional heat illnesses (EHIs) and to describe the relevant physiology of thermoregulation.
BACKGROUND: Certified athletic trainers recognize and treat athletes with EHIs, often in high-risk environments. Although the proper recognition and successful treatment strategies are well documented, EHIs continue to plague athletes, and exertional heat stroke remains one of the leading causes of sudden death during sport. The recommendations presented in this document provide athletic trainers and

Exposure to acute heat or cold stress elicits numerous physiological responses aimed at maintaining body temperatures. Interestingly, many of the physiological responses, mediated by the cardiovascular and autonomic nervous systems, resemble aspects of, or responses to, certain disease states. The purpose of this Perspective is to highlight some of these areas in order to explore how they may help us better understand the pathophysiology underlying aspects of certain disease states. The benefits of using this human thermal stress approach are that (1) no adjustments for inherent comparative di

BACKGROUND: Heat stroke contributes considerably to morbidity and mortality in hot weather, but it is unclear whether pre-existing medical conditions increase the risk of heat stroke. The purpose of this study was to assess the association between heat stroke and pre-existing medical conditions in South Korea.
METHODS: A case-control study was conducted using data from a nationwide surveillance system for heat illnesses in 2012. Individuals with heat stroke were identified and compared to control subjects with mild heat illness such as heat exhaustion, heat edema, heat cramps, and heat syncop

Heat illness spans a broad spectrum of disease, with outcomes ranging from benign rash to fatal heat stroke. Heat illness is broadly divided into 2 types: classic and exertional. Both types occur as a result of exposure to elevated temperature with inadequate thermoregulation; however, classic illness occurs without preceding physical activity. Treatment consists of rapid cooling, fluid replacement, and physiologic support. Other milder forms of heat illness include heat fatigue, heat syncope, heat edema, and heat rash. Drugs, drug combinations, drug side effects, and infections can also cause

Exposure to heat disturbs the homeostasis of body water, serum osmosis, and core temperature, resulting in the development of heat cramp, heat syncope, heat exhaustion, and heat stroke. Commonly coexisting risks are humidity, windlessness, infrared radiation, physical exertion, continuous work, chemical protective clothing, and lack of acclimatization. Exposure to cold constricts peripheral arteries and reduces metabolism, resulting in the development of chilblains, frostbite, immersion foot, and hypothermia. Wind, water immersion, and alcohol drinking will aggravate the symptoms. Exposure to

In the type of heat illness, several medical terms such as heat syncope, heat cramp, heat exhaustion, heat stroke are included. But their Japanese medical terms are neither unified nor clearly defined. To eliminate this problem, the new classification for heat illness is proposed. By the severity of heat illness, they are divided into three grades. Grade I is corresponded to heat cramp and heat syncope. Grade III is corresponded to heat stroke, and used for any one of the three following clinical findings, (1) Central nervous system dysfunction, i.e., consciousness disturbance, seizure, ataxia

BACKGROUND: Exertional heat-related injuries are a risk to all physically active individuals in warm or hot environments. Unlike classic heat-related injury, exertional heat-related injuries do not require extreme ambient temperatures to cause injury. Still, exertional heat-related injuries, including heat cramps, heat syncope, heat exhaustion, heat stress, and heat stroke, can result in injuries causing a range of outcomes from minimal discomfort to death.
PURPOSE: The purpose of this paper was to describe the epidemiology of exertional heat-related injuries treated in U.S. emergency departm
