Partial Immersion Using Body Bags for Exertional Hyperthermia: A Randomized Crossover Study.

INTRODUCTION: Recent recommendations suggest emergency medical personnel should consider rapid whole-body cooling for heat stroke patients. Cold-water immersion maximizes cooling rate and subsequent outcomes following heat stroke. Body bags filled with ice and water may provide portable, accessible partial cold-water immersion for this setting. The purpose of this study was to determine whole-body cooling rates when using body bags to facilitate partial cold-water immersion to treat exertional hyperthermia. METHODS: A total of 9 healthy participants (8 male; mean [SD] age, 24 [4] years; heigh

Metabolic reprogramming of NK cells in chronic brucellar arthritis: role of the HIF-1α/mTOR axis.

BACKGROUND: Brucellar arthritis is the most common complication of brucellosis, and its chronic progression is closely associated with immune cell dysfunction. Natural killer (NK) cells play a central role in anti-infective immunity; however, their functional status and regulatory mechanisms in brucellar arthritis remain poorly understood. METHODS: Peripheral blood and synovial tissue samples were collected from healthy controls, as well as from patients with acute-phase and chronic-phase brucellar arthritis. Single-cell RNA sequencing (scRNA-seq), flow cytometry, quantitative real-time PCR (

Inhaling cooler air reduces thermal, cardiorespiratory, and perceptual strain during prolonged exercise in the heat and extends exercise duration in young men.

OBJECTIVES: In hot environments, hyperthermia and the associated physiological and perceptual strain potentially limit prolonged exercise capacity. We tested the hypothesis that during prolonged exercise in the heat, lowering the temperature of inspired air would attenuate elevations in core temperature, cardiorespiratory strain, cerebral hypoperfusion, and perceptual stress, thereby enhancing exercise capacity. DESIGN: Randomized crossover design. METHODS: Twelve healthy males cycled at 50% of peak oxygen uptake in the heat (38 °C, 50% relative humidity) until esophageal temperature (an ind

Self-management strategies for menstrual cycle symptoms: a systematic review.

PURPOSE: Menstrual cycle symptoms, such as discomfort, fatigue, and mood swings, are prevalent among women and can significantly diminish quality of life. Many women employ non-clinical, self-management strategies to mitigate symptoms, yet the supporting evidence for these strategies is equivocal. This systematic review sought to identify, evaluate and synthesise existing evidence from randomised controlled trials (RCTs) for non-clinical, self-management strategies for menstrual cycle symptoms. METHODS: A systematic literature search was performed in PubMed, Scopus, Web of Science, and Cochra

Occupational heat stress and adaptation among outdoor workers: a narrative review of global evidence and policy responses.

Occupational heat stress is an escalating global health concern, particularly for outdoor workers exposed to rising temperatures due to climate change. This narrative review synthesizes evidence from 42 peer-reviewed articles and 13 policy documents published between 2014 and 2025, focusing on the health impacts, determinants, and adaptation strategies for outdoor workers. The review identifies a high prevalence of heat-related illnesses-including heat exhaustion, dehydration, kidney disorders, and productivity loss-across sectors such as agriculture, construction, and informal labour. Key vul

Temperature-attributable mortality projections under scenarios of climate change for Oslo, Norway.

BACKGROUND & AIM: Climate change and evolving of population dynamics, including ageing and changes in population size, are reshaping temperature-attributable mortality patterns. However, there is limited evidence on the prospective trajectory of heat- and cold-attributable mortality in Oslo, particularly under combined scenarios of global warming and population development. This study aims to project heat- and cold-attributable mortality in Oslo and assess the distinct contributions of each of these drivers, utilising high-resolution data. METHODS: We conducted a two-step approach with time se

Extreme heat and physical and mental health: A qualitative study with low-income women in South Texas.

Heat waves in the US are becoming more frequent, longer, and more intense due to climate change. South Texas is especially vulnerable because it is experiencing the highest increases in heat "danger days" nationally and has a high number of residents living in unincorporated neighborhoods known as colonias, which often lack basic infrastructure. The purpose of this qualitative study is to explore how extreme heat affects the physical and mental health of Latina women who live in South Texas colonias. Focus groups with 18 women ages 20-70 revealed a cycle of physical and mental health consequen

No Effect of High-Amylose Maize Starch on Pre- or Postexercise Hydration Status, Thermophysiological Strain, or Exhaustive Exercise Performance During Exertional Heat Stress in Endurance Runners.

High-amylose maize starch (HAMS), containing fermentable resistant starch (RS), is suggested to improve fluid absorption/retention in athletes but lacks supporting evidence and may increase gastrointestinal symptom (GIS) risk. This study investigated the use of HAMS in pre- and postexercise hydration protocols on hydration status and GIS before, during, and after exertional heat stress (EHS), and thermophysiological strain and exhaustive exercise performance during EHS. Ten endurance-trained runners (eight men and two women) completed two trials in randomized order, consuming a HAMS beverage (

Environmental health during extreme heat: challenges for the practice of safe physical activity in childhood and adolescence.

This essay discusses evidence and evidence-based strategies to ensure the safety, well-being, and continuity of physical activity for children and adolescents during extreme heat. It emphasizes the adaptation of environments, professional training, and equity in public policies. High temperatures and sedentary behaviors are not isolated challenges, and they create a critical scenario for the health of children and adolescents when combined, with impacts ranging from thermal regulation to mental health, motor behavior, and risk of obesity. The increasing frequency of extreme heatwaves imposes a

Repeated Sprint Training in Heat Improves Repeated Sprint Ability Under Temperate Conditions Similarly in Active Males and Females.

The aim of this study was to compare the adaptive responses to repeated sprint training in heat (RSHT) on exercise performance under temperate conditions between males and females. Active males and females completed 7 weeks of repeated sprint training twice a week in temperate (CON, 20°C, 55% relative humidity (RH), males: n = 12, females: n = 13) or hot (HEAT, 30°C, 60% RH, males: n = 12, females: n = 14) conditions. Before and after training, a repeated sprint ability test (RSA) was performed (10-s cycle sprints with 20-s recovery between sprints, until exhaustion), and aerobic and anaerobic

Case-control study of water and agricultural exposures among women in CKDu-endemic Sri Lanka.

Chronic kidney disease of unknown etiology (CKDu) disproportionately affects men in agricultural regions; few studies parse exposures among affected and unaffected women. We conducted a case-control study among women in Sri Lanka, identifying 15 cases, who experienced progressive kidney disease in a prospective study in a CKDu-endemic region. We selected age-matched controls from the same endemic and neighboring non-endemic region. We then assessed differences in self-reported exposures, and analyzed water samples for inorganic and organic chemicals. Women who reported heat exhaustion (OR = 19

Clinically oriented immune heterogeneity in prostate cancer: emerging targets and strategies.

Prostate cancer (PCa) has long been viewed as an immunologically "cold" malignancy because immune checkpoint inhibitors (ICIs) show limited benefit in unselected patients, particularly after progression to metastatic castration-resistant PCa (mCRPC) or treatment-related neuroendocrine PCa (NEPC). Single-cell and spatial profiling now reveal immune heterogeneity across patients, between lesions, and along the path from localized disease to metastasis. Primary tumors form mosaics of immune-excluded glands, myeloid-suppressed stromal borders, and focal lymphocyte-rich niches with B-cell aggregate

Differential Intestinal Epithelial Injury Following Passive and Exertional Hyperthermia.

BACKGROUND: Passive and exertional hyperthermia can compromise gastrointestinal (GI) integrity, contributing to systemic complications and heat stroke. Intestinal epithelial injury, a potential early event in this cascade, is likely multi-factorial and context-dependent, influenced by thermal, metabolic and mechanical stress. We compared the effects of passive hyperthermia (PaH) and exertional hyperthermia (RUN) at matched peak body core temperature (Tc) on intestinal epithelial injury and endotoxin translocation to delineate the relative contributions of these mechanisms. A prolonged brisk wa

Acute Heat Exposure-Related Illness: A Unified Emergency Medicine Framework for Hot Baths, Hot Springs, and Saunas-A Narrative Review.

Hot springs, hot-water bathing, and saunas are widely practiced forms of acute heat exposure and are often perceived as health-promoting. However, emergency clinicians frequently encounter patients in whom these exposures precipitate syncope, hypotension, drowning/aspiration, heat-related illness, and renal or electrolyte disturbances. This narrative review integrates these modalities within a unified "acute heat exposure" framework and summarizes pathophysiology and clinical implications from an emergency medicine perspective. We searched PubMed/MEDLINE from inception to January 2026 using co
