Recurrences and Survival in Oropharyngeal Squamous Cell Carcinoma According to p16 Status and Extranodal Extension.

OBJECTIVE: Extranodal extension (ENE) is a recognized adverse prognostic factor in oropharyngeal squamous cell carcinoma (OPSCC). However, most evidence concerns clinical ENE, whereas data on pathologic ENE (pENE) are limited. With the renewed role of surgery in the management of OPSCC, this study evaluates the prognostic significance of pENE according to HPV status. STUDY DESIGN: Multicenter retrospective cohort conducted from 2010 to 2021. SETTING: Specialized otolaryngology-head and neck surgery European departments. METHODS: Patients aged 18 years or older who underwent primary surgery

Mutational signature-based classification uncovers emerging oral cancer subtypes with distinct molecular patterns.

Tobacco use, alcohol consumption, and infection with human papilloma virus (HPV) are well-established risk factors for head and neck squamous cell carcinomas (HNSCC). However, the incidence of oral cancer, particularly in the mobile tongue, has been rising in non-smoker/non-drinker and HPV-negative patients, suggesting the emergence of a new clinical entity. To understand in molecular terms this subtype of oral cavity squamous cell carcinomas (OCSCC) with no-identified risk factor (NIRF), we analyzed the available public head and neck cancer multi-omics data. We identified mutational signature

A Rare Epithelioid Hemangioendothelioma of the Tongue: A Case Report and Review of Published Cases.

AIM: Epithelioid hemangioendothelioma (EHE) is a rare vascular tumor caused by the proliferation of endothelial cells. The purpose of this study was to describe a rare case of EHE of the tongue and to perform a systematic review of the literature reporting oral EHE. METHODS: A 58-year-old woman was referred to our Oral Medicine Department for evaluation of a slowly growing ulcerative lesion of the tongue. Her medical history was unremarkable. Intraoral examination revealed a soft ulcerated mass with hypertrophic margins and a yellowish base located on the right lateral border of the tongue, m

Rising incidence of oral tongue squamous cell carcinoma in young women: Emerging non-traditional etiologic factors and clinical implications: A structured narrative review.

Oral squamous cell carcinoma (OSCC) accounts for the majority of oral malignancies worldwide, with oral tongue squamous cell carcinoma (OTSCC) considered one of its most aggressive subtypes. Historically, OSCC has predominantly affected older males with substantial tobacco and alcohol exposure. However, emerging epidemiologic evidence demonstrates a rising incidence of OTSCC among young females, particularly patients aged ≤ 45 years, many of whom lack these traditional risk factors, suggesting distinct etiologic pathways. A structured narrative review using a comprehensive literature search st

Transoral robotic surgery using the da Vinci single-port system: current evidence and clinical indications - a systematic review and meta-analysis.

The DaVinci Single-Port (SP) transoral robotic surgery (TORS) platform was developed to overcome spatial constraints and instrument collisions encountered with multiport systems in head and neck surgery. We systematically reviewed and meta-analyzed perioperative performance, safety, functional recovery, and oncologic outcomes of SP TORS. Following PRISMA 2020, we searched PubMed/MEDLINE, Embase, Cochrane Library, Web of Science, and the first 25 pages of Google Scholar through January 1, 2026. Peer-reviewed English-language clinical and cadaveric studies evaluating SP TORS were included. Three

Neoplastic and non-neoplastic lesions in the upper digestive tract associated with the ingestion of Pteridium esculentum subsp. arachnoideum by 3 water buffalo in Brazil.

Upper digestive tract neoplasms associated with ingestion of Pteridium spp. (bracken fern) have been documented in cattle but not in buffalo. Here, we describe upper digestive tract lesions associated with the ingestion of P. esculentum subsp. arachnoideum in 3 water buffalo (Bubalus bubalis) in Brazil. Gross inspection revealed proliferative and ulcerative lesions in the mucosa of the base of the tongue, oropharynx, and esophagus in all 3 cases. Proliferative lesions were well-demarcated, white-to-brown, exophytic nodules on the lingual and oropharyngeal mucosa (papillomas, cases 1-3); a well

MRI-Based Radiomics and Artificial Intelligence for Prediction of Recurrence and Prognostic Outcomes in Oral Tongue Squamous Cell Carcinoma: A Systematic Review with Functional Meta-Synthesis.

BACKGROUND/OBJECTIVES: Oral tongue squamous cell carcinoma (OTSCC) remains clinically challenging because conventional clinicopathological markers do not fully explain variability in recurrence and survival. This systematic review and functional meta-synthesis aimed to identify and critically appraise studies using preoperative magnetic resonance imaging (MRI)-based radiomics, artificial intelligence (AI), deep learning, or quantitative MRI-derived models to predict recurrence and prognostic outcomes in OTSCC. METHODS: PubMed, Scopus, and Embase were searched from inception to March 2026. Eli

Oral malignant melanoma of the tongue with suspected intralingual in-transit metastasis.

Oral malignant melanoma (OMM) is a rare and aggressive malignancy, and involvement of the tongue is particularly uncommon. We report a case presenting with two isolated pigmented tongue lesions clinically resembling benign conditions. Excisional biopsy of the anterior lesion confirmed melanoma, and the tongue base lesion was clinically suspected to represent possible in-transit metastasis. To our knowledge, in-transit metastasis in OMM is not well documented. The patient underwent wide local excision followed by adjuvant pembrolizumab based on cutaneous melanoma guidelines. Final pathological

The continuous evolution of concepts in compartmental resections for oral cavity cancer: principles, indications, and outcomes.

PURPOSE OF REVIEW: This paper is aimed at synthetizing the conceptual basis of compartmental surgery (CS) for tumours of the anterior (floor of mouth and oral tongue) as well as posterior (base of tongue, superior and inferior gingival-buccal sulcus extending into the masticator space and infratemporal fossa) oral cavity, its indications, and outcomes. RECENT FINDINGS: In spite of numerous bioendoscopic, radiological, and surgical advancements in management of oral cancer, its oncologic surgical control continues to be hampered by a considerable risk of local recurrence rate. Primary adenocarcinoma of the base of tongue: a case report and review of the literature.

BACKGROUND: Primary adenocarcinoma of the base of the tongue is an exceptionally rare malignancy, accounting for only a small proportion of oropharyngeal tumors. Diagnosis is challenging due to nonspecific symptoms and overlap with more common squamous cell carcinomas and salivary-type neoplasms. CASE PRESENTATION: We report the case of a 59-year-old Moroccan woman presenting with progressive dysphagia and referred otalgia. Nasofibroscopy revealed a right-sided base of the tongue mass extending to the vallecula. Magnetic resonance imaging confirmed a 28 × 19 mm lesion with ipsilateral cervica

Disability-adjusted life-years from human papillomavirus-related oral cavity and pharynx cancers in US men, 2017-2021.

BACKGROUND: Squamous cell oropharyngeal cancer (OPC) incidence in men has surpassed cervical cancer in women as the most common cancer linked to high-risk human papillomavirus (HPV). However, vaccine uptake remains suboptimal in both sexes. The objectives of this study were to quantify disability-adjusted life-years (DALYs) because of HPV-associated OPC in US men from 2017 to 2021 and to highlight the potential impact of widespread vaccination. METHODS: Crude incidence counts and 12-month survival rates were extracted from the National Cancer Institute's Surveillance, Epidemiology, and End Re

HPV-Associated Sarcomatoid (Spindle Cell) Carcinoma of the Base of the Tongue with HRAS and TERT Promoter Mutations.

BACKGROUND: Sarcomatoid carcinoma is an aggressive and uncommon variant of squamous cell carcinoma that is rarely encountered in the oropharynx. Distinguishing this entity from true sarcomas or reactive stromal proliferations can be challenging, particularly in cases where the conventional squamous component is minimal or not readily apparent. Molecular characterization of head and neck sarcomatoid carcinomas remains sparse. METHODS: A 62-year-old male with no significant past medical history presented with an exophytic mass involving the right base of tongue and floor of mouth. The lesion ex

Immature squamous metaplastic epithelium of Weber's glandular duct: a significant origin of HPV-independent oropharyngeal squamous cell carcinoma in the base of the tongue.

PURPOSE: This study investigated the anatomical and pathological mechanisms of immature squamous metaplastic epithelium in Weber's glandular ducts at the tongue base in human papillomavirus (HPV)-independent oropharyngeal squamous cell carcinoma (OPSCC). METHODS: An analysis of 80 tongue base carcinoma cases clarified the anatomical connection to Weber's glands. Histological and immunohistochemical studies of these glandular ducts identified areas susceptible to cancer. A mouse model using 4-nitroquinoline-N-oxide was employed to simulate carcinogenic development. RESULTS: 42.5% of cases wer
