Definitive Chemoradiation for Unresectable Hyalinizing Clear Cell Carcinoma of the Base of the Tongue: A Molecularly Confirmed Case.

Hyalinizing clear cell carcinoma (HCCC) is a rare malignancy of the minor salivary glands, most often managed by surgical resection. We report a case of a 63-year-old woman with an unresectable base-of-tongue tumor initially presumed to be squamous cell carcinoma. Histopathologic evaluation and molecular testing ultimately confirmed HCCC with an EWSR1-ATF1 fusion. Given the tumor's extent, she was treated with definitive chemoradiation using weekly cisplatin and 70 Gy in 35 fractions. Her course was complicated by pulmonary embolism, neutropenia, and severe mucositis requiring percutaneous end

Risk factors for dysphagia following total laryngectomy.

BACKGROUND: Dysphagia is a common and challenging long-term sequela following total laryngectomy (TL) that can negatively impact patient quality of life. Following TL, many patients require repeat esophageal dilations to maintain swallowing function. This study aimed to identify surgical and non-surgical risk factors associated with dysphagia following TL, defined as clinically diagnosed postoperative dysphagia based on patient-reported symptoms and dysphagia requiring dilation. METHODS: We conducted a retrospective single-institution cohort study of patients who underwent total laryngectomy

Plasma Cell-Free Human Papillomavirus DNA and Oral Gargle HPV DNA in Patients with HPV-Related Oropharyngeal Cancer Treated with Radiotherapy.

UNLABELLED: Dynamic biomarkers that guide de-escalation strategies in human papillomavirus (HPV)-related oropharyngeal squamous cell carcinoma (OPSCC) remain an unmet need. In this study, we evaluated the kinetics of plasma cell-free HPV (cfHPV) DNA and oral gargle HPV DNA during radiotherapy in patients with low-risk HPV-related OPSCC. Data were obtained from a trial evaluating an adaptive model optimizing radiation fractionation in patients with low-risk (T0-2N0-1M0) HPV-related OPSCC undergoing radiotherapy. The primary objective was to determine whether week 4 plasma cfHPV DNA or oral garg

Advancements in immunotherapy for oropharyngeal cancer: Current landscape and future prospects.

Oropharyngeal cancer (OPC), affecting the tonsils, base of the tongue, and soft palate, has witnessed a notable increase in incidence, particularly among cases linked to human papillomavirus (HPV) infection. This epidemiological shift has led to changes in treatment strategies, with immunotherapy emerging as a promising alternative to conventional modalities such as surgery, radiation, and chemotherapy, which are often associated with significant toxicity. This systematic review aims to evaluate the current landscape of immunotherapeutic interventions in OPC, including immune checkpoint inhibi

Incidence of HPV-independent second primary malignancies following treatment of HPV-associated malignancy.

Second primary tumors after HPV-associated oropharyngeal squamous cell carcinoma (HPV + OPSCCa) are generally assumed to share the same HPV status as the index lesion, and current pathology guidelines discourage repeat HPV testing in recurrent disease. We retrospectively reviewed records from a tertiary academic center (2010-2024) to identify patients with a history of HPV + OPSCCa who subsequently developed an HPV-independent second primary head and neck squamous cell carcinoma. HPV status was determined by p16 immunohistochemistry, high-risk HPV DNA PCR, or RNA in situ hybridization. Eleven

Circulating cell free HPVDNA in HPV positive head and neck cancer reveals fast responders and early peakers during treatment.

The incidence of human papillomavirus-positive (HPV+) head and neck cancer (HNC), particularly oropharyngeal squamous cell carcinoma (OPSCC), is rising globally. Although radio- or chemo-radiotherapy yields better outcomes for patients with HPV+ tumors compared to those with HPV-negative tumors, these treatments do not cure all cases and are associated with significant side effects. This underscores the need for more personalized therapy. To help individualize treatment, we evaluated the presence of cell-free HPV DNA (cfHPV-DNA) in plasma collected before, during, and after therapy in patients

Tumor Upstaging With MRI for HPV-Associated Oropharynx Cancer: Implications for De-Escalation Trials.

OBJECTIVES: Human papillomavirus (HPV)-associated oropharynx cancer (OPC) requires accurate staging to guide treatment and de-escalation clinical trial enrollment. MRI provides superior soft tissue contrast and assessment of tumor depth of invasion compared with CT with contrast and FDG-PET/CT. This study aims to evaluate the prevalence of HPV-associated OPC tumor upstaging and newly identified retropharyngeal lymph node (RPLN) metastases with MRI. METHODS: Fifty consecutive patients with newly diagnosed, previously untreated HPV-associated OPC planned for treatment with primary radiotherapy

Quality of Life After Neoadjuvant Chemotherapy and Transoral Robotic Surgery for Oropharynx Cancer.

IMPORTANCE: Efforts are underway to deintensified treatment protocols for patients with human papillomavirus virus-associated oropharyngeal squamous cell carcinoma (HPV-OPSCC) to achieve similar excellent oncologic outcomes while reducing treatment-related adverse effects. Transoral robotic surgery (TORS) as primary treatment often requires adjuvant therapy due to the high incidence of nodal metastasis. Treatment with neoadjuvant chemotherapy followed by TORS and neck dissection (NECTORS), reserving radiation therapy for salvage, yields excellent oncologic outcomes. OBJECTIVE: To assess patie

Step Serial Sectioning in Head and Neck Squamous Cell Carcinoma of Unknown Primary.

IMPORTANCE: Patients with suspected head and neck squamous cell carcinoma of unknown primary (HNSCCUP) may undergo tonsillectomy and tongue base mucosectomy (TBM) to help identify clinicoradiologically occult primary disease. It is hypothesized that when these diagnostic specimens are analyzed, conventional histopathological (CH) techniques risk missing small primary tumors that may be hidden in the tissue blocks. OBJECTIVE: To establish the outcomes of a step serial sectioning (SSS) histopathological technique vs CH when analyzing diagnostic tissue specimens from TBM and tonsillectomy perfor

Differential Regulation of the STING Pathway in Human Papillomavirus-Positive and -Negative Head and Neck Cancers.

UNLABELLED: Squamous cell carcinomas, which arise from the cells that line the mucosal surfaces of the head and neck, represent the most common type of head and neck cancers (HNSCC). Human papillomavirus (HPV) infection has been strongly associated with the development of oropharyngeal cancers, which are cancers that occur in the back of the throat, including the tonsils and base of the tongue. HNSCCs with and without HPV infection have distinct pathology, with HPV-positive patients having higher levels of immune infiltration, activation in the tumor microenvironment and better response to rad
