Differential effect of painful heterotopic stimulation on capsaicin-induced pain and allodynia.

Painful heterotopic stimulation (HTS) may inhibit experimental and clinical pain, an effect known as diffuse noxious inhibitory control (DNIC). This study examined the effect of painful HTS on capsaicin-induced pain intensity, brush-evoked pain intensity and area of brush-evoked pain in humans. Immersion of the foot into painful cold water significantly reduced capsaicin-induced pain intensity and brush-evoked pain intensity in the contralateral forearm, but did not change area of brush-evoked pain. The observed differential effect on the magnitude of pain and hyperalgesia on the one hand and

Ultrastructural study of glomerular capillary loops at different perfusion pressures as revealed by quick-freezing, freeze-substitution and conventional fixation methods.

Wistar rat kidneys were perfused with some fixatives at different pressures of 100, 150, 200 or 250 cm H2O via the aorta and were studied by the quick-freezing and freeze-substitution (QF-FS) or deep-etching (QF-DE) methods, in addition to the conventional immersion or perfusion fixation method. Different parts of glomerular capillary walls were selected for morphometric analyses. It was demonstrated that the widths of glomerular slit diaphragms and the spaces between foot processes were more widely dilated at higher perfusion pressures (200 and 250 cm H2O) than those seen at both normal perfu

Enhancement of analgesia from systemic opioid in humans by spinal cholinesterase inhibition.

Intravenous opioids cause analgesia and increase release of ACh in spinal cord dorsal horn in animals, and these effects are enhanced by intrathecal neostigmine injection. The purpose of the current study was to test whether intrathecal neostigmine enhanced analgesia and increased cerebrospinal fluid concentrations of ACh over those induced by i.v. alfentanil in volunteers, and also to test whether neostigmine enhanced alfentanil-induced side effects. After human studies committee approval, 40 healthy volunteers received an intrathecal injection of saline or neostigmine (50, 100 or 200 microg)

Raynaud's phenomenon in vibration syndrome: the impact of cold feet on skin temperature and vasomotion of the hand after immersion in cold water.

Patients with vibration syndrome, suffering from Raynaud's phenomenon, are sensitive to cold. Rewarming time, after local cooling, is delayed. The present study evaluated whether rewarming of the hand after cooling is influenced by the temperature of the feet. In five Japanese patients (former forest workers) with vibration syndrome, suffering from Raynaud's phenomenon, and in five healthy controls, temperature changes of the hand after cooling were registered under the two test situations (on different days) with the feet immersed in water of 35 degrees C or 20 degrees C, respectively. In bot

Pain relief by various kinds of interference stimulation applied to the peripheral skin in humans: pain-related brain potentials following CO2 laser stimulation.

Pain perception is changed by various kinds of interference stimulation applied to the peripheral skin in humans. We investigated pain-related somatosensory evoked brain potentials (pain SEPs) following CO2 laser stimulation applied to the hand or foot in normal subjects, to elucidate the underlying mechanisms. A pain visual analogue scale (VAS) was also scored to determine the degree of subjective feeling of painful sensation. The following stimulations were applied as the interference: (1) vibration, (2) active and passive movements of the hand or foot, (3) noxious warming by hot water (46 d

Factors affecting the in vivo precision of broad-band ultrasonic attenuation.

The in vivo precision of broad-band ultrasonic attenuation (BUA) was measured in one normal male volunteer over periods of one day and six weeks and in a group of 10 volunteers over a period of two weeks using a Walker Sonix UBA 1001 bone mineral analyser in which the foot is immersed in a water tank. Coefficients of variation ranged from 1-3% and, in general, there was no difference in BUA variance between right and left feet. However, right foot BUA values were consistently greater than those of the left, but there was no difference between mediolateral and lateromedial transmission. The use

Computer-controlled epidural infusion to targeted cerebrospinal fluid concentrations in humans. Clonidine.

BACKGROUND: Pharmacokinetically designed infusions have been demonstrated to achieve rapidly and maintain desired concentrations of drug in plasma after intravenous administration. In this study we tested whether a similar approach, targeting concentrations in cerebrospinal fluid (CSF), could be used with epidural administration of the alpha 2-adrenergic analgesic clonidine. METHODS: After institutional review board approval and informed consent had been obtained, seven healthy volunteers received a clonidine infusion through a lower lumbar epidural catheter. Infusion of clonidine (10 microgr

Skin microcirculation in patients with sequelae from local cold injuries.

The microcirculation in the skin was assessed in 31 patients with sequelae from local cold injuries (LCI) in the extremities. All patients reported cold intolerance 3-4 years after the primary cold injury, which they sustained during military service. The used methods were laser Doppler fluxmetry, transcutaneous oxygen tension (TCpO2) and vital capillaroscopy. Neurovascular reflexes were stimulated by deep inspiration, digital cuff occlusion of venous and arterial circulation, neck cooling with an ice bag and water immersion at 5 and 15 degrees C. Unaffected lower or upper extremities were als

Growth of the body in early fetuses studied by multivariate analysis.

The growth of the body was studied in 30 human fetuses ranged from 10 to 22 weeks of gestation. The fetuses were fixed by immersion in 4 percent formaldehyde and the following dimensions were studied: a) lengths: arm, forearm, hand, thigh, leg, foot and crown-rump (sitting height), b) perimeters: head, thorax and abdomen. A covariance matrix was calculated from natural logarithms of all measurements. The relative growth of these measurement was computed by multivariate allometry using a principal components analysis (PCA). All characters were positively correlated with the first principal comp

The cold pressor test: vascular and myocardial response patterns and their stability.

The purposes of the present study were to compare the cardiovascular response patterns evoked by three versions of the cold pressor test (either forehead stimulation or hand or foot immersion) and to determine the reproducibility of the responses over a 2-week interval. Blood pressure, heart rate, stroke volume, cardiac output, total peripheral resistance, and systolic time intervals were obtained during rest and during the cold pressor test in 42 young men. Across conditions, the pressor response was supported by peripheral resistance increases with concomitant stroke volume decreases. Althou

Muscle sympathetic outflow in Buerger's disease.

One of the pathophysiological features in Buerger's disease, i.e. thromboangiitis obliterans (TAO), contains a sympathetic contribution which relates to vasospastic phenomenon. The purpose of this study is to clarify sympathetic mechanisms in TAO. Muscle sympathetic nerve activity (MSA) was recorded using a microneurographic technique in patients suffering from TAO. The cold pressor test (by immersing the hand of the subject into ice cold water) was used as a test to examine the sympathetic responsiveness to local cold stimulus. The basal level of the MSA in the TAO group was lower than that i

The effect of prenatal stress on adult sexual behavior in rats depends on the nature of the stressor.

It is well known that prenatal stress induces behavioral demasculinization and/or feminization in male offspring during adulthood. In this study, four different stressors were prenatally applied to rats. Pregnant rats were subjected to immobilization, unavoidable electric foot shocks, rapid eye movement sleep deprivation (REMd), or immersion in cold water. During adulthood, male offspring were tested for masculine and feminine sexual behavior. The results corroborated the fact that immobilization induced an impairment of masculine sexual behavior and a facilitation of feminine sexual behavior.

A comparison of cardiovascular and autonomic adjustments to three types of cold stimulation tasks.

Two types of 'cold pressor' tasks are used frequently in research settings: immersing the hand or foot in ice water, and applying an icebag to the forehead. Both tasks have commonly been selected as 'alpha-adrenergic' tasks due to expected increases in blood pressure and peripheral resistance. However, the forehead cooling task has been used by others to produce increased vagal tone due to the elicitation of the 'diving reflex' and subsequent bradycardia. This differs from the prototypical increase in heart rate during hand immersion. The present study directly compared the cardiovascular adju

Modelling magnetic coil excitation of human cerebral cortex with a peripheral nerve immersed in a brain-shaped volume conductor: the significance of fiber bending in excitation.

To help elucidate some basic principles of magnetic coil (MC) excitation of cerebral cortex, a model system was devised in which mammalian phrenic nerve, or amphibian sciatic nerve with its branches was suspended in appropriate Ringer's solution in a human brain-shaped volume conductor, an inverted plastic skull. The nerve was recorded monophasically out of the volume conductor. The site of nerve excitation by the MC was identified by finding where along the nerve a bipolar electrical stimulus yielded a similar action potential latency. MC excitation of hand-related corticospinal (CT) neurons

Physiological responses of the human extremities to cold water immersion.

Five subjects immersed their hands and feet twice during 1 hour sessions in a calorimeter bath, filled with water at 25 degrees C. Before immersion the hand (33.0 +/- 2.2 degrees C) was warmer than the foot (30.6 +/- 2.1 degrees C) and so was the blood flow: respectively 5.8 +/- 5.2 versus 0.7 +/- 0.3 ml.min-1.100 mo tissue-1 as determined by strain gauge plethysmography and 10.8 +/- 5.1 versus 6.7 +/- 5.3 by the doppler method. At the end of the immersion period the blood flow had decreased a factor of 2.9 (doppler) and 4.3 (plethysmography). The local heat transfer, determined by heat flux t

Circulatory disturbances of the foot in vibration syndrome.

Circulatory disturbances of the foot in patients with vibration syndrome were studied by measuring the skin temperature of both index fingers and great toes through a 3-min immersion of the right foot in cold water at 10 degrees C. Subjects included 11 patients with vibration-induced white finger (VWF) [VWF(+) group], 12 patients without VWF [VWF(-) group], and 20 healthy referents not exposed to vibration. Patients were all male chain saw operators who had scarcely been exposed to vibration of the foot. The prevalence of coldness felt in the upper and lower extremities was greater than 90% in

Insidious hypothermia during raft use.

It is commonly thought that increasing protection against cold water results in greater physiological efficiency in maintaining core temperature. To examine the relationship of physiological changes resulting from cold exposures as a function of increased insulation, five subjects (4 males, 1 female) wearing anti-exposure ensembles covering the entire body and head were exposed in a pool to water temperatures (Twater) = 4.4 degrees C and air temperatures (Tair) = 5.6 degrees C. Trials consisted of subjects undergoing either head-out immersions with mean exposure time (t = 150 +/- 9 S.E.M. min)

Pressor tests in clinical pharmacology: response morphology heterogeneity.

Heart rate, blood pressure and venous plasma catecholamine responses to passive upright tilt, immobile erect standing, upright sitting, immersion of hand or foot in cold water, isometric handgrip and delayed auditory feedback were assessed in young healthy volunteers. Each of the tests was characterized by its own typical response morphology. Postural stress caused mainly a rise in heart rate and diastolic blood pressure with little change in systolic blood pressure. The cold pressor test caused a rapid rise in heart rate, systolic and diastolic blood pressure which then was attenuated on furt
