'Leukaemic pernio': perniosis-like presentations of chronic lymphocytic leukaemia.

Chronic lymphocytic leukaemia (CLL) is the most common type of leukaemia in adults. Skin involvement by malignant CLL cells, or leukaemia cutis, typically presents as localized or generalized papulonodular lesions. We report a series of five patients with CLL who presented with perniosis-like skin findings on the ears, nose and/or digits. Histopathological analysis of seven skin specimens from these five patients showed a relatively dense, nodular to diffuse, pandermal infiltrate of monomorphic lymphocytes, compatible with CLL. The typical histopathological findings of perniosis, such as a rob

Immune-Based Cytokine Signatures of Prolonged Pandemic-Associated Chilblains.

Most pandemic-associated chilblains (PAC) resolve spontaneously, but a subset of patients develops persistent or recurrent disease. We analyzed peripheral cytokine profiles in 17 patients with prolonged PAC and identified elevated IFN-γ, CXCL10, CX3CL1, IL-16, CCL22, and S100A8. In contrast to the transient type I interferon response described in acute PAC, prolonged disease was characterized by a type II interferon-skewed inflammatory signature with T-cell activation and endothelial involvement. Frequent autoimmune comorbidities suggest baseline immune dysregulation contributing to chronicity

Physiological responses of individuals with idiopathic chilblains during cold exposure and rewarming: a nonrandomized clinical trial.

Idiopathic chilblains (IC) is a localized inflammatory disorder affecting acral areas, with unclear pathophysiology and pathogenesis. This study aimed to investigate the vasomotor, autonomic, and inflammatory responses of adults with (HIC+) and without (HIC-) a history of IC during cold exposure and rewarming. Sixteen healthy adults (8 HIC+, 8 HIC-; 10 females, 6 males; aged 19-60 yr) underwent a controlled environmental protocol involving progressive cooling (22-10°C) followed by rewarming (22°C). Skin blood flow (SkBF); finger and toe temperatures (Tf and Tt); mean skin temperature (Tsk); mi

Quantitative Assessment of the Correlation Between 'COVID Toes' Search Volume and COVID-19 Case Incidence and Mortality Dynamics: A Longitudinal Data-Driven Approach.

COVID-19, caused by the SARS-CoV-2 virus, has become a global public health crisis with diverse clinical manifestations affecting multiple organ systems, including the integumentary system. One notable cutaneous manifestation, referred to as "COVID toes," involves the development of pernio-like chilblains, characterized by red-to-violet macules, plaques, or nodules, primarily on toes and fingers. This characteristic clinical feature gained significant attention due to its apparent association with COVID-19, especially during the early stages of the pandemic when individuals with mild or asympt

Two cases of therapy-refractory chilblain lupus erythematosus successfully treated with deucravacitinib.

Chilblain lupus erythematosus is a rare skin manifestation of lupus erythematosus (LE), which has proven particularly difficult to treat and therefore often presents a great challenge for patients and physicians. We treated two patients with cases of long-lasting, refractory chilblain LE with the selective, allosteric TYK2-inhibitor deucravacitinib. Both patients showed rapid improvement of their respective skin lesions and symptoms. The treatment was generally well tolerated with no severe adverse events. One of the patients experienced an intermittent occurrence of acne lesions. We therefore

Endothelial cell expression of STINGV154M: Gain-of-function mutation delays the resolution of UVB-induced skin injury.

Gain-of-function mutations in STING (STimulator of IFN Genes) cause STING-associated vasculopathy with onset in infancy (SAVI), a rare autoinflammatory disease characterized by debilitating inflammatory lung disease and hallmark skin manifestations, such as chilblains and progressive, nonhealing ulcers. Mice expressing the most common SAVI-associated variant, STINGV154M (denoted as VM), recapitulate many clinical features of SAVI, including inflammatory lung disease, but do not develop spontaneous skin lesions. In this study, we show that a single low-dose of UVB irradiation, which induces onl

Persistent Hyperprocalcitoninemia as the Initial Diagnostic Clue to Sporadic Medullary Thyroid Carcinoma.

UNLABELLED: Medullary thyroid carcinoma (MTC) is a rare neuroendocrine tumour arising from parafollicular C cells, and serum calcitonin remains its most reliable biochemical marker. Procalcitonin (PCT), widely used in the evaluation of bacterial infections, is usually undetectable under non-infectious conditions. Persistent elevation of PCT in the absence of clinical or microbiological evidence of infection is uncommon and may represent an early clue to underlying endocrine malignancy. We report the case of a 48-year-old woman who presented with markedly elevated PCT levels (>20 ng/ml) detecte

A homozygous splice-site variant in SAMHD1 shows variable expressivity of Aicardi-Goutières syndrome type 5: a case report and literature review.

BACKGROUND: Aicardi-Goutières syndrome type 5 (AGS5) is a rare pediatric-onset monogenic interferonopathy caused by loss-of-function variants in the SAMHD1 gene. Affected individuals typically present in infancy with microcephaly, leukodystrophy, intracranial calcifications, developmental delay, and spasticity. However, phenotypic heterogeneity and atypical systemic manifestations have been increasingly recognized. CASE PRESENTATION: We report a consanguineous Pakistani family with two siblings affected by AGS5. The affected individuals (9-year-old female and 7-year-old male) presented early-

Type I Interferonopathies in the Differential Diagnosis of Vasculitis: A Comprehensive Review.

Type I interferonopathies are a heterogeneous group of monogenic autoinflammatory disorders characterized by dysregulated type I interferon (IFN-I) signaling due to pathogenic variants that affect nucleic acid sensing, processing, or downstream signaling pathways. Mutations in genes including TREX1, RNASEH2A/B/C, SAMHD1, ADAR1, STING1 (TMEM173), PSMB8, COPA, and DNASE1L3 lead to persistent activation of innate immune pathways, particularly the cGAS-STING, MDA5, and Toll-like receptor pathways, with subsequent JAK-STAT signaling and sustained overexpression of interferon-stimulated genes. Hand Lesions as a Diagnostic Clue in Unexplained SIADH.

In apparently idiopathic syndrome of inappropriate antidiuretic hormone secretion (SIADH), chilblain-like hand plaques may reveal autoimmune connective tissue disease. Skin examination, biopsy, and serology can uncover discoid lupus erythematosus with Sjögren's syndrome overlap. Serum sodium improvement after skin treatment requires caution because spontaneous stabilization cannot be excluded.

Development and implementation of "Mazboot Bunyad": a digital health intervention for the screening and management of undernutrition among children aged 6-59 months, at private provider clinics in urban slums of Karachi, Pakistan.

BACKGROUND: Although factor Xa inhibitors have become a popular choice for chronic oral anticoagulation, effective drug reversal remains difficult due to a lack of specific antidote. Currently, 4-factor prothrombin complex concentrate (4F-PCC) is considered the treatment of choice for factor Xa inhibitor-related major bleeding. However, safety of 4F-PCC and its risk of thrombosis when used for this off-label purpose remain unclear. The purpose of this retrospective study is to determine the rate of thromboembolism when 4F-PCC is used for the emergent reversal of factor Xa inhibitors. METHODS:

Towards credible digital twins for basic and preclinical research.

Atomoxetine (Strattera(R)) is a selective norepinephrine (noradrenaline) reuptake inhibitor that is not classified as a stimulant, and is indicated for use in patients with attention-deficit hyperactivity disorder (ADHD). Atomoxetine is effective and generally well tolerated. It is significantly more effective than placebo and standard current therapy and does not differ significantly from or is noninferior to immediate-release methylphenidate; however, it is significantly less effective than the extended-release methylphenidate formulation OROS(R) methylphenidate (hereafter referred to as osm

Idiopathic chilblain: a case series from Injibara General Hospital, Ethiopia.

BACKGROUND: Idiopathic chilblains are inflammatory lesions on the skin that typically appear after being in a cold, wet environment. The condition manifests itself as burning, soreness, and pruritis in the extremities (fingers and toes), usually occurring 12 to 24 h following a triggering event. Conservative measures are the mainstay of treatment for idiopathic chilblain; however pharmaceutical therapy may be necessary in cases that are severe or persistent. This is the only case series in Ethiopia concerning idiopathic chilblain. CASE PRESENTATION: From June to August of 2024, 12 individuals

Enhanced TLR7-dependent production of type I interferon by pDCs underlies pandemic chilblains.

Outbreaks of chilblains were reported during the COVID-19 pandemic. Given the essential role of type I interferon (I-IFN) in protective immunity against SARS-CoV-2 and the association of chilblains with inherited type I interferonopathies, we hypothesized that excessive I-IFN responses to SARS-CoV-2 might underlie the occurrence of chilblains in this context. We identified a transient I-IFN signature in chilblain lesions, accompanied by an acral infiltration of activated plasmacytoid dendritic cells (pDCs). Patients with chilblains were otherwise asymptomatic or had mild disease without seroco

An Evidence-Based Review of Perniosis (Chilblains): Therapeutic Strategies and Integration With Raynaud's Syndrome Management.

Perniosis (chilblains) is an uncommon inflammatory condition characterized by erythematous to violaceous papules or plaques typically affecting acral surfaces. The pathogenesis remains poorly understood, and its management remains challenging due to the limited evidence available. This comprehensive review examines evidence-based therapies for primary and secondary perniosis, informed by observational studies, case series, and trials on similar conditions such as Raynaud's phenomenon. We synthesize treatment options for managing inflammation, alleviating symptoms, and preventing recurrence, in

New Phenotypes Associated With Pathogenic RNASEH2B and SAMHD1 Variants.

Pathogenic variants in nine genes (AGS1-9) were mainly reported in patients with Aicardi-Goutières syndrome (AGS), which is a genetic disorder of the innate immune response associated with improper induction of Type I interferon (IFN). These variants led to a broad range of clinical manifestations ranging from congenital type of AGS displaying congenital microcephaly, severe developmental delay, spasticity, basal ganglia calcification, white matter abnormalities and early lethality up to infantile or juvenile onset with a broader phenotypic spectrum of AGS with severe to mild disease, includin
