Fatal Asphyxia in the Head-down Position: An Uncommon Case of Accidental Work-related Death.

Positional asphyxia is a rare but potentially fatal condition where an individual's body assumes a position interfering with normal respiratory movements and leading to asphyxiation. We report a case of an 83-year-old farmer trapped in the tank of a vineyard spray atomizer while attempting maintenance and stuck in the opening of the tank with his upper body inside and legs outside. He was unable to extricate himself, and rescuers, who arrived 2 hours later, found the victim dead. The forensic autopsy showed patterned external injuries corresponding to the edge of the opening of the tank, inter

FBXW11 inhibits tumorigenesis by ubiquitinating YB1 in hepatocarcinoma.

PURPOSE: The study aimed to investigate the role of FBXW11 in hepatocellular carcinoma (HCC) and its underlying mechanism. Specifically, we explored whether FBXW11 inhibits tumorigenesis by regulating YB1 ubiquitination and elucidated the functional significance of the FBXW11-YB1 axis in HCC progression. METHODS: Clinical HCC specimens and cell lines (HCC-LM3, HuH7, Hep3B, SNU-449) were used. FBXW11 and YB1 expression were analyzed via Western blotting and immunohistochemistry (IHC). Gain- and loss-of-function assays (FBXW11 overexpression/knockdown) were performed to assess cell proliferatio

The relationship between cardiopulmonary resuscitation duration and prognosis in patients with out-of-hospital cardiac arrest due to asphyxiation.

AIM: Asphyxial out-of-hospital cardiac arrest (OHCA) is associated with poor outcomes and may have a narrower time window for favourable outcomes compared to cardiac-origin OHCA. However, studies examining cardiopulmonary resuscitation (CPR) duration in OHCA due to asphyxiation are limited. Hence, this study aimed to investigate the relationship between CPR duration and prognosis in patients who experienced OHCA due to asphyxiation. METHODS: This retrospective study used data from the Japanese Association for Acute Medicine's OHCA registry for June 2014 to December 2021. The study population

Update on Newborn Resuscitation in Dogs and Cats.

Resuscitation of newborn puppies and kittens aims to support animals as they transition from intra- to extrauterine life immediately after birth and includes warming and drying, positive pressure ventilation (PPV), oxygen supplementation, reversal drugs and less commonly CPR. PPV is the most important intervention because it facilitates lung aeration, which is essential for successful cardiopulmonary transition at birth. PPV should be initiated as early as possible in apneic or gasping newborns or in those with severe bradycardia. If there is no heartbeat or the HR is < 50/minute, chest compre

Respiratory Gas Shifts to Delay Asphyxiation in Critical Avalanche Burial: A Randomized Clinical Trial.

IMPORTANCE: People who are critically buried by an avalanche typically die of asphyxia within 35 minutes, often making timely rescue impossible. The development of new strategies to delay asphyxiation is crucial to improve survival rates. OBJECTIVE: To investigate the efficacy of a novel, user-carried avalanche safety device that delivers airflow from avalanche debris to the user's airway without requiring supplemental oxygen or a mouthpiece. DESIGN, SETTING, AND PARTICIPANTS: This randomized, blinded, clinical trial was organized by 4 institutions and conducted at 1 field location in Italy

Single-cell multi-omics uncovers CPS1 as a breast cancer immune evasion therapeutic target.

Despite significant advances in early detection and therapeutic interventions, breast cancer persists as the most frequently diagnosed malignancy and the leading cause of cancer-related deaths among women globally. Although multiple prognostic signatures have been proposed, their predictive power and clinical applicability remain limited. In this study, we utilized an integrated approach combining single-cell multi-omics analysis with machine learning to comprehensively examine the clinical relevance of mitochondrial-related gene sets in TCGA-BRCA and developed a mitochondrial gene set scoring

Asphyxiation due to obstructive fibrinous tracheal pseudomembrane after closure of repeated tracheostomy in a case of von Recklinghausen disease.

BACKGROUND: Obstructive fibrinous tracheal pseudomembrane (OFTP) is a rare but potentially fatal complication, most commonly reported after endotracheal intubation. Its occurrence following tracheostomy closure is poorly recognized. CASE PRESENTATION: We report a fatal case of OFTP in a 33-year-old man with von Recklinghausen disease who developed progressive airway obstruction following closure of a repeated tracheostomy. The patient had a history of multiple facial tumor resections requiring repeated tracheostomies due to orofacial deformities. After the eighth tracheostomy and successful t

Management of Medical Emergencies on Psychiatry Inpatient Floors: A Novel Simulation-Based Curriculum for Psychiatry Residents.

Introduction For most psychiatry residents, training in managing the early phases of acute medical emergencies is limited to exposure during medical school and junior off-service rotations. Patients with psychiatric illnesses, however, often have higher rates of medical comorbidities than their age-matched controls. We developed a half-day medical refresher and simulation curriculum to improve the confidence and efficacy with which psychiatry residents can manage medical emergencies on inpatient floors. Methods Based upon a detailed needs assessment conducted with psychiatry residents at vario

Improved percutaneous transtracheal ventilation for airway obstruction in tongue cancer: a case report.

BACKGROUND: Patients with advanced-stage tongue cancer are often confronted with an increased risk of difficult airway management due to anatomical changes caused by prior surgeries and radiation therapy. These alterations may lead to acute airway obstruction and asphyxiation in critical situations, such as massive hematemesis, thereby necessitating the implementation of refined and context-specific airway management strategies. CASE DESCRIPTION: We present the case of a 66-year-old male with recurrent tongue cancer who experienced a sudden episode of severe hematemesis, leading to acute resp

The Association of Birth Defects or Birth Asphyxia with the Risk of Mortality and Morbidity in Premature Infants.

Introduction: Preterm birth is a leading international health issue with morbidity and mortality risks. Premature infants frequently present with congenital anomalies or perinatal asphyxia, both of which contribute to prolonged hospitalization and greater need for respiratory support. This research investigates the effects of birth defects or asphyxia among premature infants on survival and recovery. Methods: This retrospective cohort study was carried out in an NICU in Mecca, Saudi Arabia. We examined 120 medical records of premature infants diagnosed with birth defects or birth asphyxia to d

Cultivation of all-male large yellow croaker (Larimichthys crocea) and evaluation of its temperature tolerance and hypoxia tolerance.

Mono-sex cultures of all-male large yellow croaker (Larimichthys crocea) offers clear advantages for boosting economic returns in aquaculture. The purpose of this study was to breed all-male large yellow croaker. To achieve this goal, we first successfully induced sex reversal of YY supermale fish to produce YY pseudo-female through adding 17β-estradiol (E2, 120 mg/kg). These YY pseudo-female were then crossed with YY supermale fish to establish a pure-bred YY supermale fish population. Subsequently, the YY supermale fish were hybridized with female fish to produce all-male fish. During the br

Research on tracheal stenosis based on molecular biomarkers.

BACKGROUND: Tracheal stenosis is a common respiratory disease characterized by abnormal narrowing of the tracheal lumen, which restricts airflow and can lead to symptoms such as dyspnea and even asphyxiation. Although the widespread use of imaging and bronchoscopy has improved diagnostic efficiency, the molecular mechanisms underlying tracheal stenosis remain unclear, particularly regarding the identification of predictive molecular markers. METHODS: This was a retrospective study involving 43 patient samples. A range of statistical methods-including Spearman correlation analysis, multivariab

Successful Transplantation of Multiple Organs from Donor after Helium Asphyxiation: First Case Report in Japan.

Helium inhalation has increased, but most cases are either minor injuries or deaths; there have not yet been any reported cases of brain death leading to organ donation. We report a patient who attempted helium inhalation and was declared brain dead and became an organ donor without complications. To the best of our knowledge, this is the first reported case of deceased organ donation following helium asphyxiation in Japan. The patient in cardiac arrest was found with a helium-filled vinyl bag sealed around the neck. During emergency medical transport to the hospital, a spontaneous return of c

Positional asphyxia associated with cervical vertebral fractures.

Positional asphyxia refers to asphyxia resulting from an abnormal body posture that obstructs pulmonary ventilation. We report two cases in which individuals were unable to escape from positions that interfered with respiration, and cervical fractures were found upon autopsy. In the first case, a female in her 70s was found dead in a prone position on a bed following a traffic accident. Forensic autopsy findings suggested that she died from respiratory failure due to shallow breathing associated with pain from a rib fracture and prolonged prone positioning, possibly influenced by potential spi
