Heat exhaustion during mass pilgrimage--is there a diagnostic role for pulse oximetry?

Every year over 2 million pilgrims (Hajjis) gather from different countries to perform the sacred ritual, the fifth pillar of Islam, Hajj. Several nationalities from different climates come to Saudi Arabia which is located in a subtropical area with a hot and humid climate during the long summer season. This undertaking is characterised by several days of continuous physical, spiritual, and emotional exertion following their homeland. Several factors predispose them to heat exhaustion, such as the hot climate, excessive physical exercise, lack of acclimatisation, overcrowding, illiteracy, old

Integrated responses to exhaustive exercise and recovery in rainbow trout white muscle: acid-base, phosphogen, carbohydrate, lipid, ammonia, fluid volume and electrolyte metabolism.

White muscle and arterial blood plasma were sampled at rest and during 4 h of recovery from exhaustive exercise in rainbow trout. A compound respiratory and metabolic acidosis in the blood was accompanied by increases in plasma lactate (in excess of the metabolic acid load), pyruvate, glucose, ammonia and inorganic phosphate levels, large elevations in haemoglobin concentration and haematocrit, red cell swelling, increases in the levels of most plasma electrolytes, but no shift of fluid out of the extracellular fluid (ECF) into the intracellular fluid (ICF) of white muscle. The decrease in whi

Kidney function during exercise in healthy and diseased humans. An update.

Exercise induces profound changes in renal haemodynamics and protein excretion. The rate of ultrafiltration across the glomerular capillary is determined by the imbalance between the transcapillary hydraulic and colloid osmotic pressure gradients. Despite a major reduction in the renal plasma flow, the filtration fraction can double with maximal exercise, preserving the transfer of metabolites or substances through the glomerulus. Tubular processes and excretion rates are modified by exercise. Despite large increases in plasma lactate during strenuous exercise, renal excretion plays a limited

The roles of insulin and catecholamines in the glucoregulatory response during intense exercise and early recovery in insulin-dependent diabetic and control subjects.

Intense exercise is associated with a marked stimulation of glucose production (Ra), a somewhat smaller increment in its utilization (Rd) (and therefore hyperglycemia), large increases in plasma catecholamines, and moderate hyperglucagonemia. The hyperglycemia increases in recovery and is accompanied by hyperinsulinemia. Because these adaptations are unique to intense exercise, we tested the physiological significance of the hyperinsulinemia by exercising six fit, postabsorptive young male subjects with insulin-dependent diabetes mellitus (IDDM) after overnight glycemic normalization by iv ins

Effect of sodium bicarbonate ingestion on exhaustive resistance exercise performance.

Six weight trained males were studied prior to, during, and in recovery from exhaustive resistance exercise, 105 min after ingesting 300 mg.kg-1 of either a placebo or NaHCO3. The exercise test consisted of four sets of 12 repetitions with a fifth set to volitional fatigue on a Universal leg press machine at a resistance equaling approximately 70% of the subjects 1-repetition maximum. Arterialized venous blood was analyzed for lactate concentration, blood gas, and acid-base parameters. The ingestion of NaHCO3 produced a significant increase in resting pH (7.39 to 7.46), HCO3- (22.9 to 28.3 mEq

Metabolic demands and perceived exertion during cardiopulmonary resuscitation.

Emergency situations often require continuous execution of one-person cardiopulmonary resuscitation (CPR) for periods of time in excess of 30 minutes. The limited research which has examined the demands of the procedure has focused on central physiological measures, despite (1) the use of a subjective end-point for termination of CPR (i.e., exhaustion) as stated in professional guidelines, and (2) significant peripheral involvement in the form of muscular exertion, a phenomenon more closely linked to Ratings of Perceived Exertion (RPE) than to central factors. To examine subjective responses t

Pulmonary gas exchange dynamics and the tolerance to muscular exercise: effects of fitness and training.

Oxygen uptake (VO2) kinetics are generally agreed to be first-order for moderate work rates with a time constant (tau VO2) that is thought to reflect the kinetics of intramuscular creatine phosphate depletion. However, when there is a concomitant lactic acidosis, tau VO2 is appreciably longer, reflecting an additional, delayed and slowed component that leads to VO2S greater than the aerobic equivalent of that work rate and which therefore invalidates current techniques for O2 deficit estimation. This "excess" VO2 is no more than approximately 250-300 ml/min at work rates for which [lactate] an

Gas exchange, metabolite status and excess post-exercise oxygen consumption after repetitive bouts of exhaustive exercise in juvenile rainbow trout.

Juvenile rainbow trout (approximately 6 g) were exercised to exhaustion in two 5 min bouts given 6 h apart. Resting levels of whole-body lactate and glycogen were restored prior to the second bout. The rate of O2 consumption increased about threefold 5 min after each bout of exercise, while recovery time decreased from 4 h after the first bout to 2-3 h after the second. The excess post-exercise oxygen consumption, i.e. 'oxygen debt', was significantly reduced by 40% after the second exercise bout, despite almost identical rates of lactate clearance and glycogen resynthesis. The rates of CO2 an

Neuromuscular function in polio survivors.

Although there is no documented, objective evidence that symptomatic post-polio subjects are rapidly losing strength, they have a number of neuromuscular deficits related to a more severe poliomyelitis illness that may explain why they complain of problems with strength, endurance, and local muscle fatigue. Symptomatic post-polio subjects were hospitalized longer during the acute poliomyelitis, recovered more slowly, and had electromyographic evidence of greater loss of anterior horn cells. Additionally, recent assessment demonstrated that they were weaker, had a reduced work capacity, and rec

Multi-organ damage in exertional heat stroke.

Exertion-induced heat stroke is a relatively rare disorder in the moderate maritime climate of The Netherlands. Serious complications of excessive physical activity rarely occur. We describe a marathon runner with multi-organ failure after exertion-induced heat stroke. The patient developed shock, diarrhoea, coma, rhabdomyolysis, acute renal failure, liver cell damage and disseminated intravascular coagulation but recovered completely.

The effects of a glycogen loading regimen on acid-base status and blood lactate concentration before and after a fixed period of high intensity exercise in man.

Six healthy male subjects exercised after an overnight fast for a fixed 3 min period at a workload equivalent to 100% of their maximal oxygen uptake (VO2max) on 3 separate occasions. The first test took place after subjects had consumed a mixed diet (43 +/- 3% carbohydrate (CHO), 41 +/- 5% fat and 16 +/- 3% protein) for 3 days, and was followed 2 h later by prolonged cycling to exhaustion at 77 +/- 3% VO2max to deplete muscle glycogen stores. Following this, subjects consumed a low CHO diet (4 +/- 1% CHO, 63 +/- 5% fat and 33 +/- 6% protein) for the remainder of the day and for the subsequent

Influence of a 24 h fast on high intensity cycle exercise performance in man.

The influence of a 24 h fast on endurance performance and the metabolic response to maximal cycle exercise was investigated in 6 healthy men (mean +/- SD: age = 27 +/- 7 years; weight = 73 +/- 10 kg; VO2max = 46 +/- 10 ml.kg-1.min-1). Subjects performed in randomised order two exercise bouts to exhaustion separated by one week. Effect of L-tryptophan supplementation on exercise performance.

The performance of strenuous physical exercise is associated with discomfort and pain, the tolerance for that being modulated by the activity of the endogenous opioid systems. As 5-hydroxy-tryptamine (5-HT) affects nociception through its effects on the enkephalin-endorphin system, we have analyzed the effects of a moderate supplementation with L-tryptophan, the immediate precursor of 5-HT, on endurance and sensation of effort. Twelve healthy sportsmen were subjected to a work load corresponding to 80% of their maximal oxygen uptake on two separate trials, after receiving a placebo and after r

The effects of dietary manipulation on blood acid-base status and the performance of high intensity exercise.

The effect of a pattern of exercise and dietary modification, which is normally used to alter muscle glycogen content, upon the acid-base status of the blood and the ability to perform high intensity exercise was studied. Eleven healthy male subjects cycled to exhaustion on an electrically braked cycle ergometer at a workload equivalent to 100% of their maximal oxygen uptake (VO2max) on three separate occasions. The first exercise test took place after a normal diet (46.2 +/- 6.7% carbohydrate (CHO)), and was followed by prolonged exercise to exhaustion to deplete muscle glycogen stores. The s

[Endurance bicycle ergometry at 0 degrees C: cardio-respiratory and metabolic course].

The purpose of the present study is to determine the physiological and metabolic variations during and after an exhaustive exercise in different thermal conditions (0 degree C and 20 degrees C). For the eleven experimental subjects, the maximal power output is the same at 0 degrees C and 20 degrees C: 330 +/- 30 W (Mean +/- s). The heart rate is lower (bradycardia) at rest, exhaustion, and recovery. However, these differences are only significant (p less than 0.05) at exhaustion. The oxygen consumption and pulmonary ventilation is higher at 0 degree C in comparison with the measurements at 20

Tissue intracellular acid-base status and the fate of lactate after exhaustive exercise in the rainbow trout.

Exhaustive 'burst-type' exercise in the rainbow trout resulted in a severe acidosis in the white muscle, with pHi dropping from 7.21 to a low of 6.62, as measured by DMO distribution. An accumulation of lactate and pyruvate, depletions of glycogen, ATP and CP stores, and a fluid shift from the extracellular fluid to the intracellular fluid of white muscle were associated with the acidosis. The proton load was in excess of the lactate load by an amount equivalent to the drop in ATP, suggesting that there was an uncoupling of ATP hydrolysis and glycolysis. Initially, lactate was cleared more qui

Effect of high intensity interval training on 2,3-diphosphoglycerate at rest and after maximal exercise.

The purpose of this study was to examine the effect of intense interval training on erythrocyte 2,3-diphosphoglycerate (2,3-DPG) levels at rest and after maximal exercise. Eight normal men, mean +/- SE = 24.2 +/- 4.3 years, trained 4 days X week-1 for a period of 8 weeks. Each training session consisted of eight maximal 30-s rides on a cycle ergometer, with 4 min active rest between rides . Prior to and after training the subjects performed a maximal 45-s ride on an isokinetic cycle ergometer at 90 rev X min-1 and a graded leg exercise test ( GLET ) to exhaustion on a cycle ergometer. Excessive intracellular acidosis of skeletal muscle on exercise in a patient with a post-viral exhaustion/fatigue syndrome. A 31P nuclear magnetic resonance study.

A patient with prolonged post-viral exhaustion and excessive fatigue was examined by 31P nuclear magnetic resonance. During exercise, muscles of the forearm demonstrated abnormally early intracellular acidosis for the exercise performed. This was out of proportion to the associated changes in high-energy phosphates. This may represent excessive lactic acid formation resulting from a disorder of metabolic regulation. The metabolic abnormality in this patient could not have been demonstrated by traditional diagnostic techniques.

Applied physiology of rowing.

Elite oarsmen and oarswomen possess large body dimensions and show outstanding aerobic and anaerobic qualities. Oarsmen have VO2max values of 6.1 +/- 0.6 L/min and have incurred O2 debts of between 10 and 20 litres. The caloric expenditure of rowing estimated from the O2 cost of a 6-minute rowing ergometer exercise was calculated at 36 kcal/min, one of the highest energy costs so far reported for any predominantly aerobic-type sport. Aerobic and anaerobic calculations show that 70 to 75% of the energy necessary to row the standard 2000m distance for men is derived from aerobiosis while the rem

Acid-base balance during repeated bouts of exercise: influence of HCO3.

Ten men and one woman were studied before, during, and following five 1-min cycling bouts (100% VO2 max) 1 h after consuming either 0.2 g/kg B.W. of NaHCO3 or a placebo drink (NaCl). The fifth exercise bout was performed until exhaustion and the time recorded to judge the effects of the NaHCO3 drink on performance. Blood samples taken from a forearm vein revealed that the intake of NaHCO3 increased resting pH (7.34 to 7.41), HCO3 (27.5 to 31.0 mM), and base excess (1.1 to 4.6 mM). During and following the exercise, blood pH and HCO3 were always higher (P less than 0.05) in the NaHCO3 than plac
