Missed Early Signs of Tuberculosis in Pregnancy: Insights From a Case Series of Disseminated Life-Threatening Maternal Disease and Adverse Fetal Outcomes.

Tuberculosis (TB) continues to be a global public health issue. The management of tuberculosis in pregnancy is even more challenging in developing countries. Delays in seeking or providing appropriate care and subtle initial presentation in pregnancy lead to delays in diagnosis and treatment. We present case details of five women who reflect the real-world scenario, where women and their primary antenatal care providers are still ignorant and negligent about tuberculosis. All of the women had life-threatening manifestations of the disease. Three out of five patients had extensive pulmonary tub

Broncholith Removal Using a Cryoprobe in Combination With Biopsy Forceps in a Patient With Immune Thrombocytopenia: A Case Report.

Broncholithiasis is a rare respiratory condition usually caused by calcified lymph nodes eroding into the bronchial lumen, often following pulmonary tuberculosis or histoplasmosis infections. Patients may be asymptomatic or present with haemoptysis, recurrent respiratory infections or airway obstruction, for which bronchoscopic or surgical treatment may be required. No internationally standardized guidelines currently exist for broncholith removal. We report a 63-year-old woman with previous pulmonary tuberculosis, recurrent haemoptysis and thrombocytopenia that was subsequently diagnosed as i

BCG at 100: The Paradox of an Indispensable Vaccine.

Bacille Calmette-Guerin (BCG) vaccine, in use for over a century, occupies a paradoxical position in global health. It protects infants against disseminated tuberculosis (TB) with 60-80% efficacy and reduces all-cause infant mortality through non-specific effects mediated by trained immunity. Yet it provides limited and variable protection against pulmonary TB in adolescents and adults who drive transmission. A tuberculin survey from western Uttar Pradesh documented an annual risk of TB infection of 3.5-4.1%, nearly 50 times the national target, in a predominantly BCG-vaccinated child populati

Advances in host-directed therapies for tuberculosis.

INTRODUCTION: Tuberculosis (TB) treatment is lengthy and complex, requiring patients to adhere to multidrug regimens, which may be difficult to tolerate. Importantly, traditional antimicrobial therapy does not prevent infection-associated lung damage. Adjunctive host-directed therapies (HDTs) could address the issues associated with standard antitubercular therapy. We highlight recent advances in pulmonary TB HDT clinical trials and discuss the molecular pathways targeted, focusing on approaches that future research can build on to develop new TB regimens that may include HDTs. AREAS COVERED:

Antivirulence Benzothiophenes Inhibit Intracellular Mycobacterium tuberculosis by Targeting the Sensor Histidine Kinase MprB.

Pulmonary tuberculosis remains one of the most prevalent and threatening infectious diseases. Classical anti-tubercular therapy targets essential enzymes of the pathogen Mycobacterium tuberculosis (Mtb), thereby suppressing bacterial growth. However, the emergence of drug-resistant strains necessitates alternative therapeutic approaches. Virulence inhibition is a promising strategy that blocks Mtb virulence factors required for survival within host cells. We previously identified a hit compound, BTP15, which protected fibroblasts and THP-1 macrophages from Mtb-induced cell death ex vivo by blo

Home non-invasive ventilation in chronic respiratory failure: Evidence-based guidelines.

Home non-invasive ventilation (NIV) is used across a range of causes of chronic respiratory failure, but patient selection and management are disease-specific. These guidelines provide evidence-based recommendations on initiating, titrating, and monitoring long-term home NIV across different disease spectrums. The panel reviewed studies from international databases, relevant guidelines, and reference lists. The panel refined recommendations through two rounds of consensus and graded them using the Indian Chest Society framework. In chronic obstructive pulmonary disease (COPD), home NIV is reco

Diagnostic challenges in WHO classification of thymic epithelial tumors: The clinical value of an international expert pathology panel.

BACKGROUND: Thymic epithelial tumors (TETs) are rare mediastinal tumors classified according to the WHO classification system. Classification can be challenging due to overlapping histological features. Subtyping on biopsy specimens is often inaccurate due to tumor heterogeneity and sampling effect. Since 2012 an international thymic tumor pathology expert panel is available in the Netherlands. This study evaluates the added value of this panel by assessing the discrepancies in diagnosis, inter-observer agreement and concordance on diagnosis between biopsy and the subsequent resection specimen

Equity is a prerequisite for a globally relevant HIV cure.

HIV cure-related research encompasses an expanding range of investigational strategies, but its global impact will depend on whether cure-related interventions are scientifically valid, acceptable and implementable in the populations most affected by the epidemic. Low- and middle-income countries carry a disproportionate HIV burden, yet cure-related research remains concentrated in high-income settings. This imbalance is not only an issue of access or social justice. Differences in viral subtypes, reservoir characteristics, timing of antiretroviral therapy initiation, co-infections and anatomi

High ambipolar mobility fluorinated MXene for ultrasensitive biosensing transistors.

Transition metal carbides and nitrides (MXenes) are emerging as transformative materials for bioelectronics. However, the advancement of MXene-based field-effect transistor (FET) biosensors is hindered by the lack of high-quality aqueous-stable materials and the low signal transduction efficiency intrinsic to their metallic nature. To address these issues, we report an ultrasensitive biosensing platform enabled by an in-situ fluorination strategy of MXene. By substituting unstable oxygen-containing terminations with chemically stable fluorine terminations in a mild reaction, we synthesized hig

BACKGROUND: Rifampicin-resistant tuberculosis (RR-TB) remains a public health challenge, with suboptimal treatment outcomes and challenging management of contacts. Whole genome sequencing (WGS) generates a comprehensive drug resistance profile and transmission inference with a single assay. Limited evidence exists on how healthcare workers (HCWs) perceive the value of WGS, especially for post-exposure management. METHODS: We conducted a qualitative study with 22 purposively sampled HCWs (9 doctors, 9 nurses, 4 community health workers) at 11 public health care facilities in the Western Cape,
