Atypical waves: nonsporotrichoid Mycobacterium marinum skin infection in an immunocompromised scuba diver.

A 58-year-old man presented with an enlarging painful ulcer on his left medial knee. He had a history of psoriatic arthritis, treated with methotrexate and adalimumab for the last 5 years. He had travelled to over 20 countries in the past 2 years, and his hobbies included scuba diving and seawater/freshwater swimming. He recalled freshwater swimming in a cave lake in Brazil 1 year prior. Subsequently, he developed small erythematous papules on each knee with rapid resolution on the right knee. The left knee lesion slowly enlarged, and he sought medical attention in Ireland 9 months later. He r

Inconsistent standard of care for tuberculosis screening and preventive therapy before initiating systemic psoriasis treatment.

BACKGROUND AND OBJECTIVES: Preventive tuberculosis (TB) therapy before initiating MTX or IL-17/IL-23/IL-12/23p40 inhibitors for latent tuberculosis infection (LTBI) is supported by indirect evidence of TB reactivations with TNF inhibitors. However, direct evidence for MTX or IL-17/IL-23/IL-12/23p40 inhibitors is limited. To better evaluate the risk of TB reactivation, data on LTBI patients exposed to these medications without preventive TB therapy are necessary. This study was conducted as part of the update of the European and German psoriasis guidelines aimed to assess current LTBI screening

Analysis of the drug target of the anti-tuberculosis compound OCT313: phosphotransacetylase is a potential drug target for anti-mycobacterial agents.

Tuberculosis (TB) is one of the most common infectious diseases caused by bacteria worldwide. The increasing prevalence of multidrug-resistant TB (MDR-TB) and latent TB infection (LTBI) has intensified the global TB burden. Therefore, the development of new drugs for MDR-TB and LTBI is urgently required. We have reported that the derivative of dithiocarbamate sugar derivative, 2-acetamido-2-deoxy-β-D-glucopyranosyl N,N-dimethyldithiocarbamate (OCT313), exhibits anti-mycobacterial activity against MDR-MTB. Here, we identified the target of OCT313. In experimentally generated OCT313-resistant ba

Diagnostic accuracy of the BD MAX MDR-TB assay on sputum and tongue swabs for Mycobacterium tuberculosis complex detection in adults under investigation for TB in South Africa.

UNLABELLED: Despite the availability of molecular diagnostics, only 48% of newly diagnosed tuberculosis (TB) cases were confirmed using nucleic acid amplification tests in 2023. The MAX Multi Drug Resistant Tuberculosis (MAX MDR-TB) assay detects Mycobacterium tuberculosis complex (MTBC) and resistance to rifampicin (RIF) and isoniazid (INH), but data on clinical performance are limited. This study assessed the assay's performance on sputum and tongue swabs (TSs). The assay was evaluated for MTBC detection and RIF and INH resistance profiling on sputum using liquid culture as the reference and

A novel diagnostic strategy of differential diagnosis of tuberculous meningitis and non-tuberculous meningitis: a retrospective observational cohort study.

Tuberculous meningitis (TBM) leads to severe disability and mortality rates, making early diagnosis critical. However, it is challenging to distinguish it from other common forms of non-tuberculous meningitis (non-TBM), including bacterial meningitis, cryptococcal meningitis, and viral meningitis. This study aims to construct a diagnostic model between TBM and non-TBM. A total of 543 patients were enrolled, and 405 subjects remained and were subsequently divided into a training set and a validation set. Basic information, laboratory results, and imaging results of patients were collected, and

[Report of two cases of Mycobacterium abscessus infection: clinical and therapeutic challenges].

Nontuberculous mycobacterial (NTM) infections, particularly those caused by Mycobacterium abscessus, are of increasing clinical importance due to their ability to cause a variety of infectious manifestations. Diagnosis is difficult due to the ambiguity between contamination and infection and the no specificity of symptoms. Treatment is complex, requiring multiple drug regimens with a risk of toxicity. The need for a structured diagnostic and therapeutic approach, including a high level of clinical suspicion and antimicrobial susceptibility testing to guide drug selection, is emphasized. Two cl

Distribution, prevalence of non-tuberculous mycobacteria on Hainan Island and antibiotic resistance of Mycobacterium abscessus.

UNLABELLED: Non-tuberculous mycobacteria (NTM) infections are increasingly recognized as a public health concern, yet their distribution and antibiotic resistance patterns on Hainan Island, China's tropical hotspot, remain understudied. This study analyzes 158 clinical respiratory samples from 2015 to 2018 across 14 cities, identifying 118 NTM isolates from 24 species. Predominant species include Mycobacterium abscessus (23.73%), M. intracellulare (22.03%), and M. simiae (14.41%). Spatial mapping reveals elevated prevalence in coastal areas, with no significant north-south divergence on the is

Detection, treatment and prevention of tuberculosis.

Mycobacteria tuberculosis (Mtb) infects millions yearly in many countries. Numerous multi-generational genetic factors have been identified that account for high host susceptibility to Mtb. Diagnostic tests based on quantification of interferon-γ generation by Mtb antigen-stimulated blood mononuclear leukocytes detect infection accurately and show far lower false-positive and false-negative results than Mtb antigen skin tests. Antibiotic resistance of Mtb has limited effective treatment. Now new tests for antibiotic resistance of Mtb based on genetic mutations characteristic of resistance to i

Pharmacogenomics and mutation informatics: correlation of NAT2 mutations and isoniazid acetylation rate.

Tuberculosis (TB) drug resistance poses a major global health challenge. The first-line antitubercular prodrug isoniazid (INH) is metabolized by N-acetyltransferase 2 (NAT2) and activated by catalase peroxidase (KatG) to inhibit enoyl-acyl carrier protein reductase (InhA) in the mycolic acid biosynthesis pathway. Genetic variations in NAT2 are associated with the formation of slow and fast acetylators, influencing drug efficacy and toxicity. Despite significant advances that have clarified key aspects of NAT2-mediated isoniazid metabolism, the complete spectrum of mechanisms governing isoniazi

Rheumatoid arthritis and risk of non-tuberculous mycobacterial pulmonary disease: a nationwide longitudinal cohort study.

BACKGROUND: Previous literature suggested that rheumatoid arthritis (RA) might be a risk factor for nontuberculous mycobacteria pulmonary disease (NTM-PD). However, longitudinal studies did not comprehensively consider potential confounders such as body mass index and smoking status. In addition, the impact of RA seropositivity has not been well elucidated. In this study, we aimed to evaluate the risk of incident NTM-PD in subjects with RA versus age-matched and sex-matched controls, while focusing on the impact of RA serologic status on this association. METHODS: From the Korean National Hea

Genome graphs reveal the importance of structural variation in Mycobacterium tuberculosis evolution and drug resistance.

Structural variants (SVs) are increasingly recognized as key drivers of bacterial evolution, yet their role has not been explored thoroughly. This is due to limitations in traditional short-read sequencing and linear reference-based analyses, which can miss complex structural changes. Tuberculosis (TB), a disease caused by Mycobacterium tuberculosis (Mtb), remains a major global health concern. In this study, we harness long-read sequencing technologies and genome graph tools to construct a Mtb pangenome reference graph (PRG) from 859 high-quality, diverse, long-read assemblies. To enable accu

Primary tuberculous mycobacterial granulomas provide a niche for superinfecting Mycobacterium abscessus.

Prior and concurrent Mycobacterium tuberculosis infection are among the most important susceptibility factors for nontuberculous mycobacterial infection in Asia. Here we model this process in zebrafish with a primary Mycobacterium marinum infection followed by a secondary M. abscessus infection. We demonstrate preferential growth of secondary M. abscessus infection inside primary M. marinum granulomas. Granuloma-resident secondary M. abscessus is protected from macrophage-mediated immune control and antibiotic therapy. We find other opportunistic pathogens Mycobacterium smegmatis, Aspergillus

Single-cell RNA sequencing reveals immune cell dysfunction and rewired interaction networks in the peripheral blood of active tuberculosis patients.

Tuberculosis (TB), caused by Mycobacterium tuberculosis (Mtb), remains a global health threat with poorly understood immune mechanisms. This exploratory study employed single-cell RNA sequencing (scRNA-seq) on peripheral blood mononuclear cells (PBMCs) from 3 active TB (ATB) patients and 2 healthy controls (HC) to dissect immune cell dysfunction and communication networks. Data were processed using CellRanger, Seurat, and Harmony for quality control, batch correction, and clustering, with cell types annotated via marker genes and SingleR. Functional analyses included cytotoxicity, antigen pres

The role of TRIM proteins in the pathogenesis of mycobacterium tuberculosis.

Mycobacterium tuberculosis (Mtb) is an intracellular pathogen responsible for tuberculosis (TB), a disease that remains the leading infectious cause of death worldwide. Several host cell proteins have been reported to play a significant role in regulating Mtb infection and replication. Among these, TRIM proteins are a large family of E3 ubiquitin ligases that have been described as being involved in host defence against Mtb, by controlling two important cellular processes: autophagy and the inflammatory response. Autophagy is a highly conserved cellular process deputed to the degradation of da

Evidence of mycobacteriaemias and mycobacterial co-infections uncovered in cattle at slaughter using a novel phage-based PhMS-qPCR assay for viable Mycobacterium bovis and Mycobacterium avium subsp. paratuberculosis.

Accurate diagnosis of Bovine Tuberculosis (bTB) in cattle is important for success of eradication programmes, but the current reliance on the single intradermal comparative cervical tuberculin (SICCT) test is not achieving disease eradication in all jurisdictions, including Northern Ireland (NI). In this study, a novel Phagomagnetic separation (PhMS)-qPCR assay to rapidly detect viable Mycobacterium bovis in bovine blood samples was evaluated. A total of 149 heart blood samples were collected from cattle at exsanguination point of the slaughter line at a NI abattoir between June and August 202
