Proliferation of CD10+ mature plasmacytoid dendritic cells in lymph nodes of patients with chronic non-tuberculous mycobacterial infection: a case report.

BACKGROUND: Plasmacytoid dendritic cells are essential immune system components, critical in antiviral and antibacterial defense. Although plasmacytoid dendritic cells are typically present in small numbers in the peripheral blood, bone marrow, and lymph nodes, their expansion within the lymph nodes accompanied by CD10 expression is a relatively uncommon phenomenon. CASE PRESENTATION: A 54-year-old Chinese female had experienced recurrent lymphadenopathy for over 2 years. She was diagnosed with Mycobacterium chimaera infection more than 1 year before presentation, and her condition was assess

NMR based human gut metabolomic profiling reveals altered metabolites associated with pulmonary and extra-pulmonary tuberculosis.

INTRODUCTION: Tuberculosis remains one of the world's deadliest infectious diseases. The pathophysiology of the two manifestations of Mycobacterium tuberculosis infection, pulmonary TB (PTB) and extrapulmonary TB (EPTB) is still not fully understood. Understanding the metabolic profile of both disease manifestations in patients is important for developing therapeutic approaches and molecular diagnosis. OBJECTIVE: The current study aimed to elucidate differences in the gut metabolic profile of PTB and EPTB patients compared to healthy controls (HCs). METHOD: We used an untargeted approach thr

Type I interferons in tuberculosis pathogenesis and prevention.

Mycobacterium tuberculosis (Mtb) remains an epidemiological concern due to heterogeneous outcomes in tuberculosis (TB) pathology and the lack of mechanistic understanding of early immune events. Type I interferons (IFNs) in TB are generally described as pathological. However, evidence also suggests a protective role in vaccination and adjunctive to chemotherapy. Understanding Type I IFN signaling in Mtb-host interactions is therefore critical as it is among the earliest signatures of progressive infection. Crosstalk between Type I and II IFN signaling during TB may also be key to disease outco

Disseminated Talaromyces marneffei infection mimicking tuberculosis in an HIV-negative adult with anti-IFN-γ autoantibodies: a case report.

BACKGROUND: Talaromyces marneffei (TM) is an opportunistic fungus causing life-threatening disseminated infections in immunocompromised individuals. While classically associated with HIV, TM is increasingly reported in HIV-negative patients, often misdiagnosed due to nonspecific manifestations. CASE PRESENTATION: A 38-year-old HIV-negative Chinese woman with a history of thyroid cancer presented with a three-month history of fever, cough, weight loss, and subcutaneous masses. Imaging revealed diffuse pulmonary nodules and osteolytic bone destruction. She was initially misdiagnosed with tubercu

miR-29a-3p serves as a potential biomarker for diagnosing spinal tuberculosis: a comprehensive analysis integrating clinical data and molecular markers.

BACKGROUND: Spinal tuberculosis (STB), the most common bone and joint tuberculosis, often causes spinal deformity and impairs quality of life. While microRNAs (miRNAs) regulate various disease pathogenesis, their expression and function in STB remain poorly understood. METHODS: 100 STB patients (observation group) from Ningxia Medical University Affiliated Hospital (Jan 2021-Dec 2024) and 100 healthy controls were enrolled. miRNA high-throughput sequencing of 3 samples per group identified differentially expressed miRNAs, validated via known STB signaling pathways. The role of TGF-β1 C-509T and TGF-β1 T869C polymorphisms in susceptibility to tuberculosis infection in a Sudanese population: a case-control study.

BACKGROUND: Tuberculosis (TB) affects around 10 million people globally every year. The death rate from TB disease is substantial (almost 50%) if treatment is not received. In 2023, Sudan’s tuberculosis incidence is estimated at 134 cases per 100,000 population, with a 95% confidence interval of 125 to 145 cases per 100,000. The current tuberculosis mortality rate in Sudan is estimated at 25.0 deaths per 100,000 individuals. Several previous studies established an association between different cytokine genetic variants and TB across different ethnic groups. To our knowledge, no published studi

Modeling and dynamical analysis of the impact of alcohol consumption on tuberculosis transmission with optimal control strategies.

Alcohol consumption significantly exacerbates tuberculosis (TB) transmission and severity, creating a critical public health problem by increasing susceptibility and reducing treatment efficacy. To address this, our study develops a novel compartmental model that integrates, for the first time, the dynamics of distinct alcohol use patterns with TB disease progression and key interventions like alcohol protection and TB vaccination. The primary objective was to analyze this co-dynamic system and identify optimal control strategies. Using mathematical modeling, sensitivity analysis, and optimal

Policy analysis on drug-resistant tuberculosis in ASEAN member countries using a governance framework approach: a scoping review.

Drug-resistant tuberculosis (DR-TB) heavily contributes to the global challenge of antimicrobial resistance (AMR), resulting in increased morbidity among tuberculosis cases. The Association of Southeast Asian Nations is home to approximately 10% of the world's population, accounting for almost 15% of the annual global TB incidence. Thus, this study aimed to evaluate the existing national policies on DR-TB in the region using the governance framework approach for AMR and the WHO Health System Building Blocks. A thorough search for DR-TB-related national policies was performed on the websites of

Impact of drug-resistance diagnosis based on whole-genome sequencing on the treatment adequacy of patients with drug-resistant pulmonary tuberculosis in the state of São Paulo, Brazil: a protocol for a non-randomised controlled trial (Gen-TB PróCura).

INTRODUCTION: Since 2018, WHO has endorsed the use of whole-genome sequencing (WGS) of Mycobacterium tuberculosis complex isolates to detect drug-resistant tuberculosis (DR-TB). This endorsement was based on the assumption that a faster and more detailed description of the resistance profile would improve treatment prescription for DR-TB by healthcare providers, and hence the treatment outcomes of patients. Nonetheless, this assumption has not been tested in routine clinical practice and different scenarios. In Brazil, WGS is not routinely used for the diagnosis of DR-TB, having been carried o

Optimizing Diagnostic Protocols for Pulmonary Tuberculosis in Children: A Comparison of Diagnostic Yields From One-day, Two-day and Three-day Specimen Collection Strategies.

BACKGROUND: Diagnosing pediatric pulmonary tuberculosis (PTB) remains challenging due to low bacterial load and difficulty obtaining specimens. Current guidelines recommend 3-day acid-fast bacilli (AFB) and culture, with 1 molecular testing. We aimed to compare diagnostic yield across 1-, 2- and 3-day collection protocols. METHODS: We conducted a retrospective study of children <18 years with PTB at a tertiary hospital (2015-2023) who submitted 3 gastric aspirates (GA) or sputum specimens. Patients without molecular results, treated >14 days, or non-tuberculous mycobacterial infections were e

Case Report: An unexpected diagnosis of cutaneous Mycobacterium marinum infection based on potato dextrose agar culture.

Non-tuberculous mycobacteria (NTM) are slender, non-motile, acid-fast bacilli widely distributed in diverse environmental reservoirs across the globe. In recent years, the incidence of NTM-related diseases has risen significantly, leading to their recognition as major emerging human pathogens. Cutaneous NTM infections typically arise following traumatic injury, surgical intervention, or cosmetic procedures. Due to their heterogeneous clinical presentations, these infections often pose diagnostic challenges. Löwenstein-Jensen medium and Middlebrook medium are widely employed in mycobacterial mi

A diagnostic model for NTM disease in HIV-positive patients: a machine learning-based analysis with novel inflammatory markers.

BACKGROUND: Globally, the prevalence of nontuberculous mycobacterial (NTM) co-infection among HIV-positive patients is increasing. The diagnosis of HIV-positive patients co-infected with NTM relies on mycobacterial culture and identification, as well as molecular biology techniques. However, culture-based methods are technically challenging, time-consuming, and costly. Therefore, it is urgent to explore early diagnostic methods for HIV-positive patients co-infected with NTM. To address this issue, the present study had aimed to explore new approaches for the early diagnosis of NTM disease in H

Reprogramming the host: Mycobacterium tuberculosis as a silent architect of the immuno-tumoral.

Pulmonary tuberculosis, caused by Mycobacterium tuberculosis (Mtb), remains one of the leading causes of infectious disease-related mortality worldwide. In parallel, lung cancer represents the most lethal neoplasm, with high mortality rates globally. Emerging studies suggest that chronic Mtb infection may contribute to the development of lung cancer, particularly adenocarcinoma. Several biological mechanisms support this hypothesis. Chronic inflammation from tuberculosis creates a microenvironment enriched in proinflammatory cytokines, reactive oxygen species (ROS), and growth factors that fav

Development and validation of a clinical risk score nomogram for distinguishing nontuberculous mycobacteria from Mycobacterium tuberculosis pulmonary disease.

BACKGROUND: The prevalence of nontuberculous mycobacterial pulmonary disease (NTM-PD) has increased substantially in recent years, positioning it as a major global public health concern. This rise is compounded by its clinical and radiographic similarities to pulmonary tuberculosis (PTB), encompassing shared manifestations such as chronic cough, hemoptysis, and cavitary lesions on imaging, which pose significant challenges in differentiating infections caused by nontuberculous mycobacteria from those attributable to Mycobacterium tuberculosis. As a result, NTM-PD is frequently misdiagnosed as
