Sesame Immunotherapy: A Systematic Review and Narrative Synthesis.

Sesame allergy is associated with severe allergic reactions, low rates of spontaneous resolution, and significant psychosocial burden. Current management relies on strict avoidance and emergency treatment. Allergen-specific immunotherapy, particularly oral immunotherapy (OIT), has emerged as a potential disease-modifying approach, but evidence for sesame allergy remains limited. A systematic search was conducted through August 2025. Eligible studies included participants of any age with confirmed sesame allergy treated with allergen immunotherapy. Primary outcomes were desensitization and sust

Diagnosing Nonsteroidal Anti-Inflammatory Drug-Hypersensitivity/Allergy and Nonsteroidal Anti-Inflammatory Drug-Exacerbated or Induced Food Allergy Phenotypes in Children and Adolescents.

BACKGROUND: Nonsteroidal anti-inflammatory drugs (NSAIDs) are widely used in children. Hypersensitivity reactions (HRs) to NSAIDs represent a diagnostic challenge, particularly because NSAIDs may act as aggravating factors or cofactors as in two phenotypes recently named NSAID-exacerbated food allergy (NEFA) and NSAID-induced food allergy (NIFA). OBJECTIVE: To diagnose NSAID hypersensitivity/allergy (NH/A) and NEFA/NIFA phenotypes in children and adolescents, classify HRs to NSAIDs according to updated international guidelines, and identify clinical and immunologic predictors of true NH/A. Incidence, predictors, and treatment outcomes of biphasic anaphylaxis in the emergency department of a tertiary hospital:A 5-year retrospective study.

BACKGROUND: Biphasic anaphylaxis despite successful treatment has an incidence of 4-5% based on NIAID/FAAN criteria. Our study aimed to investigate the frequency and predictive factors associated with biphasic reactions within the emergency department (ED) at Siriraj Hospital. METHODS: This observational study assessed medical records of anaphylaxis and anaphylactic shock patients at Siriraj Hospital's ED from January 2015 to December 2019. Of these, a random sample was reviewed and validated by allergists. Telephone interviews were performed to collect more data. Uni- or biphasic response we

Population-specific transcriptomic shifts underlie secondary metabolic diversification in Aspergillus flavus Link and the domestication of Aspergillus oryzae (Ahlb.).

The genomic signatures of microbial domestication remain poorly understood within the context of natural population variation. Here, we demonstrate that Aspergillus oryzae, the filamentous fungus used in soy sauce and sake production, shares more recent ancestry with a predominantly northern, largely non-aflatoxigenic population of Aspergillus flavus (population C). A. oryzae isolates also overlap with the recently described, clinically enriched A. flavus population D, suggesting multiple domestication events. All isolates tested retained pathogenicity in a zebrafish infection model. At the tr

Omalizumab: First Approved Anti-IgE Therapy for Multiple Food Allergies.

BACKGROUND: Approximately 10% of adults and 7.6% of children in the United States have food allergies. Allergic reactions to foods can be life threatening, which requires patients and caregivers to be constantly diligent in avoiding foods that may trigger an immune-mediated event. Until recently, the only FDA-approved treatment for food allergy was oral immunotherapy (OIT), which involves daily ingestion of small doses of antigen to gradually build up a patient's tolerance. This therapy can be burdensome, requiring multiple office visits, and it is not without serious risk, including anaphylax

Outcome Predictors of Oral Food Challenge in Children.

Background: Food allergy is a leading cause of severe allergic reactions in children and often results in restrictive elimination diets. The oral food challenge (OFC) remains the diagnostic gold standard but is resource-intensive and carries a risk of adverse reactions. This study aimed to identify epidemiological, clinical, and laboratory predictors of OFC outcomes and reaction severity in children with suspected immediate-type food allergies. Methods: We conducted a retrospective review of 148 children who underwent hospital-based, open OFCs due to suspected immediate-type food reactions. Da

Timing of cow's milk oral immunotherapy and adverse reactions in children: A 15-year experience.

BACKGROUND: Cow's milk allergy (CMA) is one of the most common food allergies in children and may persist into later childhood. Oral immunotherapy (OIT) is an effective treatment option; however, the optimal timing of initiation and the mechanisms underlying age-related differences in safety remain uncertain. OBJECTIVE: To evaluate the safety and efficacy of cow's milk OIT initiated at different ages and to explore the relationship between age, baseline immunologic markers, and adverse reactions in a real-world pediatric cohort. METHODS: We retrospectively analyzed 145 children with IgE-medi

Predictive Value of Molecular Biomarkers for Oral Food Challenge Outcomes in Children with Hazelnut and Peanut Allergies.

Background: Food allergy (FA) is an emerging problem in pediatrics, with tree nuts and peanuts being frequent causes of severe reactions. Oral food challenge (OFC) remains the gold standard for diagnosing FA. However, it is a stressful treatment and not always risk-free. Objectives: To identify potential biomarkers, using component-resolved diagnosis (CRD) associated with OFC outcome in children with tree nut (hazelnut, walnut, almond, and pistachio) and peanut allergy, who live in central and southern Italy. Methods: Eighty-eight (1-18 years) children followed at the Pediatric Allergy Clinic

Progress in allergen immunotherapy via advanced transdermal delivery routes.

Food allergies, particularly peanuts and tree nut allergies, can trigger severe anaphylaxis, prompting the development of novel immunotherapies. Conventional allergen immunotherapies, including therapies delivered by oral, sublingual and epicutaneous routes, show variable efficacy and are limited by adverse reactions and poor adherence. This review focuses on emerging advanced transdermal immunotherapeutic techniques, including the use of jet injectors, iontophoresis, sonophoresis and microneedles, which enable minimally invasive allergen delivery to dermal antigen-presenting cells and thus im

Hypersensitivity to Intravenous Lipid Emulsions and Food Allergies: a Systematic Review of Case Reports.

PURPOSE OF THE REVIEW: Intravenous lipid emulsion (ILE) is essential for patients with prolonged intestinal failure but may contain potential allergens. Unnecessary avoidance of ILE in patients with food allergies could limit nutritional options. Therefore, we systematically reviewed the evidence on ILE use in patients with and without food allergies, including those who experienced hypersensitivity reactions. Searches were conducted in Medline, Scopus, Web of Science, Google Scholar, and the Canada Vigilance Adverse Reaction Database through April 30, 2025. RECENT FINDINGS: Twenty-eight indiv

Poppy seed anaphylaxis: clinical and allergic features.

Poppy seeds are increasingly used for flavoring, for example, in cakes, bread, and fruit salads, and it is gaining popularity globally. Most case reports of poppy seed allergy are in children. Ingestion of cake, in which ground poppy seed is used, is mostly associated with anaphylactic reactions. Reactions may occur even with minimal amounts or traces. In sera of patients allergic to poppy seeds, four IgE-reactive poppy seed proteins were identified. Cross allergy with sesame and buckwheat are reported. Cross-reaction is described with nuts (cashew and walnut), wheat, rye flour, buckwheat, ric

Delayed-type anaphylactic shock to mannitol: a case report and review of the literature.

BACKGROUND: Mannitol is Food and Drug Administration-approved for the reduction of intracranial pressure associated with cerebral edema and the reduction of intraocular pressure and is a commonly used drug in clinical practice. Hypersensitivity to mannitol is rarely reported, and only nine cases have been documented in English since 1967, all of which exhibited a rapid onset. We report a case of delayed anaphylactic shock induced by mannitol, aiming to alert clinical practitioners to the potential occurrence of such rare and serious adverse drug reactions (ADRs). CASE PRESENTATION: A 62-year-

Immediate and delayed hypersensitivity to biologicals in children-A practical approach: An EAACI task force report.

In the era of precision medicine, biologicals demonstrate how therapies can be personalized and directed against new targets. This type of therapy includes different molecules such as growth factors, immune modulators, vaccines, and monoclonal antibodies (mAbs). In recent years, biologicals have been increasingly developed and authorized, although their use in children is limited compared to that in adults, due to the complexity of the pharmacokinetics and pharmacodynamics of the involved proteins, as well as other factors, such as regulations governing clinical trials. Regardless, biologicals

Sunflower Seed-Induced Anaphylaxis With Immunoglobulin E Reactivity to 2S Albumins and Novel 60 kDa Antigen Identified by Immunoblot Analysis.

Sunflower seed allergy is uncommon, and reports of severe systemic reactions, such as anaphylaxis, are limited, particularly in Japan where sunflower seeds are not routinely consumed. A 38-year-old woman developed throat discomfort, dyspnea, and generalized urticaria within minutes of ingesting cookies containing sunflower seeds and was diagnosed with anaphylaxis. Her medical history included bronchial asthma and pollinosis caused by Japanese cedar and cypress. Skin prick testing (SPT) showed a strong positive (4+) reaction to sunflower seeds, while the results were negative for mugwort, cashe

Peanut Oral Immunotherapy in Children: An Italian Single-Center Retrospective Cohort Study.

Introduction: Peanut oral immunotherapy (P-OIT) is an emerging treatment strategy for peanut allergy (PA). Although a standardized pharmaceutical product, Peanut (Arachis hypogaea) Allergen Powder-dnfp, has been approved in several countries, it is not universally available. In such contexts, real-world protocols using readily utilizable peanut products may represent an alternative approach. This study aimed to describe the feasibility, safety, and clinical outcomes of P-OIT using toasted peanuts in a real-world effort in a pediatric population. Methods: This single-center retrospective cohort

Allergy-Aware Protocol for Blenderized Tube Feeding Initiation in Children With Severe Motor and Intellectual Disability.

BACKGROUND: Blenderized tube feeding (BTF) provides nutritional and psychosocial benefits for children requiring long-term enteral nutrition. However, BTF has been reportedly associated with food allergies, including anaphylaxis. Standardized protocols that consider food allergies in BTF initiation are currently unavailable for children with severe motor and intellectual disabilities (SMID). Thus, this study aimed to develop a structured, allergy-aware BTF initiation protocol tailored for patients with SMID receiving exclusive enteral nutrition. METHODS: This research included eight children
