The Dangers of Chest Compression: A Case Report.

Chest injuries after cardiopulmonary resuscitation (CPR) are common, but subsequent infected parasternal collections after CPR are uncommonly described. We present a case of a 49-year-old female with a history of intravenous drug use (IVDU) who developed multiple sternal and rib fractures and a large infected parasternal collection after community CPR following a heroin overdose. She was treated with antibiotics and required surgical evacuation of the collection. This case highlights the importance of recognizing and managing post-CPR chest wall complications early to prevent serious morbidity

Spontaneous Mesenteric Hematoma Following Severe Coughing in a Patient on Dual Antiplatelet Therapy: A Case Report.

Spontaneous mesenteric hematoma (SMH) is a rare cause of acute abdominal pain and may mimic more common intra-abdominal conditions, creating diagnostic challenges. A 67-year-old man with hypertension, hyperlipidemia, coronary artery disease status post four-vessel coronary artery bypass grafting, and chronic dual antiplatelet therapy with aspirin and clopidogrel presented with acute worsening lower abdominal pain. He reported a severe coughing episode during a recent bronchitis-like illness approximately two weeks before presentation, followed by lower abdominal pain in the same general region

From the Thoracic Outlet to the Brachial Plexus: A Rare Presentation of Pediatric Ewing Sarcoma.

Ewing sarcoma of the chest wall (CWES) is a highly aggressive mesenchymal tumor, the incidence of which in children under five years of age remains extremely rare. Primary involvement of the first rib and the apex of the thorax presents specific anatomical challenges, leading to severe compressive complications at an early stage. We report the case of a four-year-and-two-month-old girl admitted for right-sided neuropathic arm pain that had been present for three months and was initially attributed to a minor trauma. The gradual onset of upper extremity muscle atrophy, motor deficit predominant

Modified Suzuki Frame Fixation for the Treatment of Dorsal Proximal Interphalangeal Joint Fracture-Dislocation.

BACKGROUND: The proximal interphalangeal (PIP) joint is critical for hand function, contributing substantially to grasping and overall dexterity1-3. Injuries to this joint, often caused by axial loading, can lead to various fracture patterns and dislocations, resulting in long-term complications such as pain, stiffness, reduced range of motion, arthritis, and instability1,2. Traditional surgical approaches for unstable dorsal PIP joint fracture-dislocation include dorsal block pinning, open reduction and internal fixation (ORIF), hemi-hamate or hemi-capitate arthroplasty, and external fixation

Giant infected splenic pseudocyst complicated by rupture in a patient with chronic Q fever.

Giant infected splenic pseudocysts are rare and may be difficult to diagnose due to overlapping clinical and imaging features with other cystic splenic lesions, including hydatid cysts and abscesses. At our institution, a 59-year-old male with no reported history of splenic trauma presented with weight loss, early satiety, and left upper quadrant pain. Computed tomography demonstrated a large calcified splenic cyst measuring up to 14.6 cm with internal septations and an air-fluid level, concerning for infection and rupture. The cyst fluid measured a median of 12 Hounsfield units, without perce

Children's Surgery: The Missing Link in the Global Child Survival Agenda.

Global child health is at an inflection point. Under-five mortality has fallen substantially over recent decades, yet progress is slowing and preventable deaths and disability remain concentrated in low- and middle-income countries. As mortality from vaccine-preventable diseases and other infections declines, birth defects and other surgically treatable conditions account for an increasing share of the residual burden. For many children, access to timely, safe and affordable surgical, anesthesia and perioperative care is the difference between survival, disability and death. Yet these services

Optimal surgical timing for anterior cruciate ligament reconstruction to minimize osteoarthritis incidence: A systematic review.

BACKGROUND: Anterior cruciate ligament (ACL) injury is a common cause of knee instability and may contribute to the development of osteoarthritis (OA). Whether the timing of ACL reconstruction influences the subsequent risk of OA remains uncertain. OBJECTIVE: To compare the incidence of OA and secondary knee complications following early versus delayed ACL reconstruction. METHODS: A systematic search was conducted in PubMed, the Cochrane Library, Embase, Web of Science, and Scopus. The review was reported according to the PRISMA 2020 guidelines. Randomized controlled trials and cohort studie

Thirty-day outcomes after pediatric microtia and anotia reconstruction: A CPT-coded national cohort study.

BACKGROUND: Microtia reconstruction requires balancing short-term safety, operative burden, staging, donor-site morbidity, esthetic goals, hearing needs, and long-term revision risk. However, national data describing early postoperative outcomes across Current Procedural Terminology (CPT)-defined reconstructive phenotypes remain limited. METHODS: We performed a retrospective cohort study using American College of Surgeons National Surgical Quality Improvement Program Pediatric data from 2012 through 2024. In silico prioritization of potential matrix metalloproteinase-13 inhibitors for cartilage degeneration in knee injury-associated osteoarthritis.

Knee trauma is an important risk factor for post-traumatic osteoarthritis (PTOA), a progressive joint disease characterized by articular cartilage degeneration and limited disease-modifying treatment options. Matrix metalloproteinase-13 (MMP-13/collagenase-3) contributes to cartilage extracellular matrix degradation through cleavage of type II collagen, making it a relevant molecular target for cartilage-protective drug discovery. This study aimed to identify and prioritize potential small-molecule MMP-13 inhibitors using an integrated in silico workflow. The workflow included MMP-13 sequence

Sarcopenia and Muscle Dysfunction in Older Adults with Burn Injuries: An Integrated Clinical Perspective.

Sarcopenia is a progressive skeletal muscle disorder associated with impaired physical function and adverse outcomes in older adults. Burn injury imposes profound hypermetabolic and catabolic stress that may compromise muscle mass, strength, and physical function, particularly in individuals with reduced baseline muscle reserve. This narrative review integrates current evidence on sarcopenia and muscle dysfunction in older adults with burn injuries, addressing baseline vulnerability, burn-related muscle deterioration, diagnostic challenges, clinical implications, and therapeutic strategies. Sequential Kidney-First Retrieval and Early Graft Outcomes in Deceased Donor Kidney Transplantation: A Paired-Donor Comparative Cohort Study.

OBJECTIVES: Prolonged cold ischemia is a principal modifiable determinant of delayed graft function after deceased donor kidney transplant. In conventional multiorgan procurement, kidneys are removed last, prolonging ischemic exposure, a problem magnified in resource-constrained programs. We assessed whether retrieving 1 kidney first, from a heart -beating donor before aortic cross -clamping, improves early graft outcomes. MATERIALS AND METHODS: This retrospective paired-donor cohort study included 88 adult recipients of kidneys from 44 brain dead donors at collaborating centers in central In

Chronic infectious keratitis: A review of etiopathogenesis, clinical presentation and management challenges.

Chronic infectious keratitis represents a distinct clinical entity characterized by indolent progression, diagnostic difficulty, and prolonged therapeutic courses. Unlike acute bacterial keratitis, chronic infections often present with subtle inflammatory signs, recurrent episodes, or poor response to empirical antimicrobial therapy. Among the most important causes are herpetic keratitis, acanthamoeba keratitis, Microsporidial stromal keratitis, and atypical (non-tuberculous) mycobacterial keratitis. Herpes simplex virus keratitis may manifest as recurrent epithelial disease or immune-mediated

A case of neonatal gastric perforation misdiagnosed as neonatal necrotizing enterocolitis: A literature review.

RATIONALE: Neonatal gastric perforation (NGP) is a rare but life-threatening neonatal acute abdominal condition. Its clinical and imaging manifestations may overlap with those of necrotizing enterocolitis (NEC), making accurate preoperative localization of the perforation site challenging. This case highlights the diagnostic challenges and key points in the differential diagnosis of NGP when its initial presentation resembles NEC. PATIENT CONCERNS: A male infant born at 33 weeks and 5 days of gestation, with a birth weight of 2500 g, was admitted on the fourth day of life because of abdominal

Conservative Outcomes of Subtalar Dislocation: A Systematic Review and Meta-Analysis of the Current Literature.

BACKGROUND: Subtalar dislocation is a rare injury accounting for 1%-2% of all dislocations. Although prompt closed reduction is the accepted initial treatment, optimal conservative management protocols remain unclear, and no prior systematic review has synthesised outcomes of purely conservatively managed cases across both medial and lateral dislocations. This review aims to systematically assess the rates of post-traumatic osteoarthritis, functional outcomes, subtalar stiffness, avascular necrosis, and need for subsequent surgery following conservative management of acute isolated subtalar di

Early prediction of childbirth-related posttraumatic stress disorder symptoms.

BACKGROUND: Medically complicated childbirth is associated with increased risk of childbirth-related posttraumatic stress disorder (PTSD), yet mechanisms linking complications to persistent PTSD symptoms remain poorly understood. METHODS: A total of 667 women were followed from pregnancy through 6 months postpartum in a prospective longitudinal study. Acute childbirth-related distress was assessed during the immediate postpartum hospitalization using the Peritraumatic Distress Inventory. Childbirth-related PTSD symptoms were assessed at approximately 10 days, 2 months, and 6 months postpartum

The near extinction of the RAPTOR: a cautionary tale.

SummaryTrauma hybrid operating rooms (THORs) integrate diagnostic imaging with open surgical capabilities to provide both surgical and interventional radiology procedures, such as may be required by exsanguinating patients, in a single physical location. The world's first trauma program-designed THOR, designated the Resuscitation with Angiography, Percutaneous Treatments, and Operative Repair (RAPTOR) suite, opened in Calgary in 2013. It was considered to be paradigm shifting at the time; however, the physical space has become largely obsolete and is now disfavoured by key multidisciplinary us

Femoral de-escalation during revision total hip arthroplasty using a rectangular-taper shortened stem.

PURPOSE: Long stems with diaphyseal fixation are widely used in revision total hip arthroplasty (RTHA) but may increase surgical complexity and compromise femoral bone stock. Primary femoral stems, used as a de-escalation strategy, could simplify RTHA in selected patients as a bone preserving strategy. This study aimed to evaluate the clinical and radiographic outcomes of this technique using a cementless rectangular-taper shortened stem during both components RTHA. METHODS: Between January 2019 and January 2025, 301 both components RTHA were prospectively included in our institutional total

Preliminary insights into systemic biomarkers associated with coagulopathy and endotheliopathy in a swine burn model and the impact of fresh frozen plasma resuscitation strategies.

BACKGROUND: Burn injury is a form of trauma whose management may be further complicated in battlefield settings. Burn injury induces inflammation, coagulopathy, and endotheliopathy; however, the timing and progression of these responses have not been well established. Fresh frozen plasma (FFP) has been shown to modulate these responses in other trauma models, but its effects after burn injury remain poorly defined. This study aimed to profile these responses in a swine model of burn injury using a biomarker-based approach. METHODS: Anesthetized female Yorkshire crossbred swine (n = 15) were s
