Atypical cerebral fat embolism syndrome in an elderly patient with comorbidities following fracture: A case report.

RATIONALE: Cerebral fat embolism (CFE) is a rare but serious complication of fractures, most commonly affecting young males. However, atypical cases in elderly patients with underlying comorbidities are easily misdiagnosed. PATIENT CONCERNS: We report a case of a 61-year-old male patient with diabetes and hypertension who developed impaired consciousness and fever on the 2nd day after sustaining multiple fractures in a traffic accident. His condition deteriorated after receiving local treatment. DIAGNOSES: CFE was diagnosed based on the characteristic "starry sky sign" on brain magnetic reso

Rethinking emergency risk assessment: A single-center look at shock index and its variants in COVID-19.

BACKGROUND: The Shock Index (SI) is a validated prognostic tool in conditions such as severe trauma and obstetric hemorrhage. During the COVID-19 pandemic, it was used to identify patients at higher risk of clinical deterioration, but results have been inconsistent. This study aimed to evaluate the prognostic value of the SI and its variants in predicting mortality, need for mechanical ventilation, and hospital length of stay in patients with moderate COVID-19. METHODS AND FINDINGS: This longitudinal analytical observational study was conducted at a high-complexity hospital in southwestern Co

Is posttraumatic stress disorder specific to drug-resistant epilepsy or a common feature of chronic disease? A comparative study with atrial fibrillation and type 1 diabetes.

OBJECTIVE: Posttraumatic stress disorder (PTSD) is more prevalent in epilepsy than in the general population. However, it remains unclear whether this association is specific to epilepsy or a broader consequence of experiencing unpredictable acute episodes within chronic diseases. This study aimed to (1) compare PTSD prevalence and severity in patients with drug-resistant epilepsy (DRE) and patients with other chronic episodic diseases, namely, type 1 diabetes (T1D) and drug-resistant atrial fibrillation (AF); and (2) examine psychiatric comorbidities, emotion regulation, and quality of life a

Treatment of Friction Injury Using Enzymatic Debridement and Autologous Skin Cell Suspension.

Friction injury occurs when a patient slides across a surface at high velocity, resulting in mechanical abrasion, laceration, and thermal burn. Mechanical abrasion removes tissue to a variable depth depending on surface texture in contact, and thermal burn devitalizes tissue to a variable depth based on contact pressure and transfer of kinetic energy. In these heterogenous wounds, tangential excision can excise healthy tissue alongside devitalized tissue, and other debridement techniques like hydrosurgery or dermabrasion may not reach deeper crevices created by abrasion and laceration. In addi

Science of heat mapping: Thermography in musculoskeletal disorders.

Musculoskeletal injuries are among the most common causes of disability worldwide, with early detection and appropriate intervention critical to minimizing long-term complications. Infrared thermography (IRT) has emerged as a non-invasive, real-time imaging modality that captures superficial temperature changes reflecting underlying physiological processes such as inflammation and vascular alterations. This review explores the fundamental principles of medical thermography, differentiates between passive and active approaches, and outlines key technological advancements including artificial in

Primary Extracranial Ewing Sarcoma Complicated by Hemorrhagic Shock and Multimicrobial Infection: A Diagnostic and Therapeutic Challenge.

Ewing sarcoma is a rare malignant neoplasm that is seldom located in the skull or scalp. We describe the case of a 19-year-old previously healthy man presenting with a rapidly growing left parieto-occipital exophytic mass following minor trauma, initially misinterpreted as an abscess. The lesion recurred exponentially with polymicrobial infection and progression to massive extracranial tumoral hemorrhage, culminating in hemorrhagic shock. Surgical excision and histopathological examination confirmed Ewing sarcoma with EWSR1 gene rearrangement. Staging revealed pulmonary, lymph node, and bone m

Trauma-specific revision of the Japanese Association for Acute Medicine disseminated intravascular coagulation criteria improves outcome prediction in severely injured patients.

BACKGROUND: No universal definition of trauma-induced coagulopathy exists, and no validated scoring system accurately evaluates coagulopathy in patients with severe trauma. Although developed for sepsis and other critical illnesses, the conventional Japanese Association for Acute Medicine (JAAM) disseminated intravascular coagulation (DIC) score's applicability to trauma populations remains controversial. This study aimed to evaluate the JAAM DIC score's diagnostic performance in patients with severe trauma and develop a trauma-specific coagulopathy scoring system based on its original compone

Thrombelastography-estimated functional fibrinogen in healthy volunteers, end-stage renal disease, and trauma: inaccurate when most needed.

BACKGROUND: Depletion of fibrinogen is an early event in trauma-induced coagulopathy (TIC), while hyperfibrinogenemia increases thrombotic risk in end-stage renal disease (ESRD). Viscoelastic tests aim to provide rapid fibrinogen estimates, yet their accuracy to define abnormal fibrinogenemia remains debated. OBJECTIVES: We assessed the accuracy of thrombelastography (TEG)-TEG 5000 and TEG 6s-in measuring normal and abnormal fibrinogen levels across healthy volunteers, patients with ESRD, and trauma patients at risk for TIC. METHODS: We compared the von Clauss functional fibrinogen (gold sta

Technical considerations and early results of magnetic compressive intramedullary nailing for tibial and femoral shaft non-unions: a case series.

BACKGROUND: Non-union of femoral and tibial shaft fractures remains a challenging complication following surgical fixation. Magnetic intramedullary nails (mIMNs) allow controlled compression and distraction, offering a potential alternative to traditional exchange nailing or external fixation. However, limited clinical data exist regarding their efficacy and complications in treating long bone non-unions. This case series presents early outcomes and technical considerations of compressive magnetic intramedullary nailing for femoral and tibial shaft non-unions. CASE DESCRIPTION: We conducted a

Management of a Female Patient With Traumatic Carotid Aneurysm and Depression.

Extracranial carotid aneurysms are rare and may be asymptomatic for long periods, or they may present with symptoms depending on their size and location. Complications such as rupture or stroke can significantly impact outcomes, particularly if the aneurysm is not detected early or treated promptly. This case report illustrates a traumatic carotid aneurysm in a young female patient and its surgical management with successful stent-graft deployment. Patients with undiagnosed intracerebral aneurysms that have not ruptured may present with a variety of neurological and psychiatric symptoms. Manag

Cocaine-Induced Rhabdomyolysis With Acute Kidney Injury in the Absence of Serotonin Syndrome: A Case Report.

Rhabdomyolysis is a clinical condition resulting from skeletal muscle breakdown, leading to the release of intracellular components, such as creatine kinase, myoglobin, and electrolytes, into the circulation. This process can cause a range of complications, most notably acute kidney injury (AKI), due to myoglobin-induced renal toxicity. While trauma, seizures, and hyperthermia are well-known precipitants, stimulant drugs represent an underrecognized yet clinically significant cause. This report describes a middle-aged patient who presented with marked rhabdomyolysis and acute kidney injury (AK

Mathematical modeling of the combined effects of thermal burn and local irradiation.

Radiation combined injury (RCI), resulting from ionizing radiation exposure accompanied by other injuries such as burn, laceration, or fracture, is associated with higher rates of mortality and severe effects. Current available models lack the ability to capture synergistic effects associated with RCI and/or rely on data-driven approaches that are limited in their predictive capabilities. To address this, we developed a mechanistic mathematical model for local radiation exposure combined with burn injury that captures the inflammatory response and early fibroblast activity associated with inju

Mechanical and Biological Treatments for Annulus Fibrosus Repair and Closure: A Review of Clinical Studies.

To address disc reherniation, several annular repair strategies have been developed, including implantable devices, suture-based techniques, and biomaterials. This review evaluates the clinical evidence supporting these strategies. A systematic search of PubMed, Cochrane, and Embase and MEDLINE via Ovid was performed from database inception to July 2025. A combination of keywords was used in the search string. Publications were included if they were clinical studies and described closing the annulus or blocking the annulus defect with biologics, sutures, or physical devices. Exclusion criteria

Are enlarged perivascular spaces a predictor of early post-traumatic seizures after traumatic brain injury?- A pilot study.

OBJECTIVE: Post-traumatic epilepsy (PTE) is a significant long-term complication following traumatic brain injury (TBI), while early post-traumatic seizures (EPTS), occurring within the first week after TBI, are a critical risk factor that can elevate the likelihood of developing PTE later on. Here we investigate the potential of MRI-based enlarged perivascular space (ePVS) analysis as an imaging biomarker for acute in-hospital EPTS. METHODS: Forty patients who experienced a moderate-severe TBI with early post-traumatic seizures (EPTS+) (32.5% female, median age 52 years) and 88 patients with

Evaluating clinical outcomes for very early versus late venous thromboembolism prophylaxis in patients with isolated severe spinal trauma.

INTRODUCTION: Patients with traumatic spinal injuries face a high risk of venous thromboembolism (VTE); however, the optimal timing of VTE prophylaxis to most effectively reduce VTE rates remains unclear. This study aims to assess the association between thromboprophylaxis timing and clinical outcomes in patients with traumatic spinal injuries. METHODS: This retrospective cohort study used the ACS-TQIP database (2017-2023) to evaluate the timing of VTE prophylaxis in adult trauma patients (≥18 years) with isolated acute spinal cord injuries (SCI). Patients were stratified by prophylaxis type,
