The morbidity and mortality of hemodialysis by internal central venous catheterization in the subclavian and internal jugular positions are reviewed. A follow-up study was performed in our unit over 10 years (786 catheterizations). The most frequent complications were inadequate flow (7.6%) inadvertent withdrawal (5.6%) and bacteremia (5.1%). The overall complication rate was 27.2%. Kinking (p less than 0.001), bleeding (p less than 0.01) and bacteremia (p less than 0.05) occurred more frequently in patients with chronic renal failure, compared to patients with acute renal failure. Previous animal studies in our laboratory revealed that acute cerebral ischemia induced by air embolism is accompanied by a sharp and pronounced rise in both systemic blood pressure and intracranial pressure. A retrospective study was undertaken to analyse and compare the results of Harrington instrumentation with postural reduction and nursing in patients with fractures and fracture-dislocations of the lumbar spine. One hundred thirty-three cervical spine radiographs of 50 patients with rheumatoid arthritis were examined. Arthrosis following rupture of the anterior cruciate ligament has been analysed in two series. The first series was derived from a review of 150 cases of reconstruction of the anterior cruciate ligament with a follow-up of 3 years or more. Arthrosis was seen to have developed in 13.3%. From analysis of 304 clinical cases with trauma of the spine and spinal cord combined with various extravertebral injuries the authors discuss the specific features of the clinical course and diagnosis of the condition and the surgical tactics in the management of this severe contingent of patients. They substantiate the use of emergency instrumental methods for recognizing injuries to the viscera and brain as well as the tendency to conduct early operations for fractures of the spine and limbs whenever indicated.

The clinical picture of 15 patients (10 male, five female) with amyloid arthropathy secondary to chronic renal failure treated with haemodialysis has been studied. The average period of haemodialysis was 10.8 years. Joint symptoms appeared between three and 13 years after starting haemodialysis. No patient had renal amyloidosis. One hundred seventy-two patients with 204 slipped capital femoral epiphyses treated with nailing or pinning were evaluated an average of 28 years after surgery. Follow-up evaluation was obtained by questionnaire in 153 cases (181 hips) and by clinical and radiographic reexamination in 132 cases (157 hips). Sixty eight patients with aneurysms of the thoracic aorta were studied. Forty one had aortic dissection, 24 had dilatation only, and three had transverse aortic rupture. Sixteen had Marfan's syndrome; 17 had hypertension; and in eight there were other causal factors. In 17 the cause of the aneurysm was unknown. The clinical and radiographic findings of 11 patients with a chronic traumatic aneurysm were reviewed. Each patient had been under medical care at the time of injury but acute aortic rupture was not recognized in any, although several signs of mediastinal hemorrhage were present on the immediate posttraumatic radiographs.
