Early prediction of childbirth-related posttraumatic stress disorder symptoms.

BACKGROUND: Medically complicated childbirth is associated with increased risk of childbirth-related posttraumatic stress disorder (PTSD), yet mechanisms linking complications to persistent PTSD symptoms remain poorly understood. METHODS: A total of 667 women were followed from pregnancy through 6 months postpartum in a prospective longitudinal study. Acute childbirth-related distress was assessed during the immediate postpartum hospitalization using the Peritraumatic Distress Inventory. Childbirth-related PTSD symptoms were assessed at approximately 10 days, 2 months, and 6 months postpartum

The near extinction of the RAPTOR: a cautionary tale.

SummaryTrauma hybrid operating rooms (THORs) integrate diagnostic imaging with open surgical capabilities to provide both surgical and interventional radiology procedures, such as may be required by exsanguinating patients, in a single physical location. The world's first trauma program-designed THOR, designated the Resuscitation with Angiography, Percutaneous Treatments, and Operative Repair (RAPTOR) suite, opened in Calgary in 2013. It was considered to be paradigm shifting at the time; however, the physical space has become largely obsolete and is now disfavoured by key multidisciplinary us

Femoral de-escalation during revision total hip arthroplasty using a rectangular-taper shortened stem.

PURPOSE: Long stems with diaphyseal fixation are widely used in revision total hip arthroplasty (RTHA) but may increase surgical complexity and compromise femoral bone stock. Primary femoral stems, used as a de-escalation strategy, could simplify RTHA in selected patients as a bone preserving strategy. This study aimed to evaluate the clinical and radiographic outcomes of this technique using a cementless rectangular-taper shortened stem during both components RTHA. METHODS: Between January 2019 and January 2025, 301 both components RTHA were prospectively included in our institutional total

Preliminary insights into systemic biomarkers associated with coagulopathy and endotheliopathy in a swine burn model and the impact of fresh frozen plasma resuscitation strategies.

BACKGROUND: Burn injury is a form of trauma whose management may be further complicated in battlefield settings. Burn injury induces inflammation, coagulopathy, and endotheliopathy; however, the timing and progression of these responses have not been well established. Fresh frozen plasma (FFP) has been shown to modulate these responses in other trauma models, but its effects after burn injury remain poorly defined. This study aimed to profile these responses in a swine model of burn injury using a biomarker-based approach. METHODS: Anesthetized female Yorkshire crossbred swine (n = 15) were s

Mechanism matters with traumatic rectal injuries: Multicenter analysis of blunt versus penetrating traumatic rectal injury management and associated outcomes.

OBJECTIVE: Examine the impact of injury mechanism and management strategies on outcomes after traumatic rectal injury (TRI). BACKGROUND: TRI are uncommon, with most data derived from penetrating injuries. Whether management and outcomes differ between blunt (BR) and penetrating (PR) injuries remains unclear. METHODS: A multicenter study across 22 trauma centers compared management patterns, use of the "4 Ds" adjuncts (drain, diversion, distal washout, direct repair), and outcomes between BR and PR. RESULTS: Among 725 patients (511 PR, 214 BR), BR had higher injury severity, more abdominal c

Biliary Complications After High-Grade Blunt Liver Trauma in Children: Impact of Early Recognition on Management Burden.

BACKGROUND/PURPOSE: Biliary complications (BIC) are rare after high-grade blunt liver trauma in children, but a recognized source of morbidity. The main clinical issue is whether they are recognized in time to limit treatment burden. This study evaluated the incidence, presentation, and management of BIC after high-grade blunt liver trauma in a pediatric referral cohort. METHODS: All consecutive children younger than 18 years referred to a quaternary pediatric hepatobiliary center between January 2006 and December 2025 for severe blunt liver injury were retrospectively reviewed. Demographic d

Surgical management of periorbital non-traumatic subcutaneous emphysema during continuous positive airway pressure.

A 47-year-old male requested treatment for recurrent, rapid left upper eyelid swelling due to subcutaneous emphysema (SE) with use of continuous positive airway pressure (CPAP) for treatment of obstructive sleep apnoea. There was no previous history of facial trauma or surgery. Computerised tomography revealed pneumatisation of the lacrimal sac fossa and nasolacrimal duct with a prominent agger nasi air cell medial to the lacrimal sac extending down the nasolacrimal duct. He underwent endoscopic surgery to reduce the pneumatisation superomedial to the lacrimal sac. The subcutaneous emphysema s

Management of Bilateral Subacute Subdural Hematomas with Concomitant Subarachnoid Hemorrhage in a Diabetic Patient with HIV in Cameroon: A Case Report.

Bilateral subdural hematomas occurring simultaneously with subarachnoid hemorrhage in a traumatic context are rare and pose considerable diagnostic challenges. Comorbidities such as diabetes and HIV may further worsen prognosis and complicate management. This case underscores the vital importance of early diagnosis and multidisciplinary care in patients with subacute subdural hematomas and subarachnoid hemorrhage, particularly when compounded by comorbidities in resource‑limited settings. We report the case of a 44-year-old housewife who was diabetic and living with HIV. She presented with sev

Clustered Regularly Interspaced Short Palindromic Repeats (CRISPR)-Based Genome Editing in Inherited Cardiovascular Diseases: Therapeutic Advances, Emerging Editing Platforms, and Translational Challenges.

Inherited cardiovascular diseases (ICVDs) are a broad category of hereditary disorders resulting from pathogenic mutations that alter cardiac anatomy, electrical conduction, and metabolic pathways. The main types include hypertrophic cardiomyopathy (HCM), dilated cardiomyopathy (DCM), cardiac channelopathies, and familial hypercholesterolemia (FH) and are associated with heart failure, malignant arrhythmias, and sudden cardiac death, particularly in young people. Current treatments, such as pharmacological therapies, implantable cardioverter-defibrillators, and heart transplantation, have sign

Immediate Unrestricted Assisted Full Weight-Bearing After Bicondylar Tibial Plateau Fixation: A Case Series.

High-energy bicondylar tibial plateau fractures of Schatzker types V and VI are conventionally managed, after open reduction and internal fixation, with 6-12 weeks of restricted weight-bearing, out of concern for loss of reduction and articular collapse. Emerging evidence questions whether this restriction is necessary. Reports of immediate weight-bearing specifically after fixation of these most unstable bicondylar patterns remain scarce, as most comparative studies pool this subtype within mixed cohorts. We present three consecutive patients (two men and one woman; mean age: 54 years; all of

Limb Salvage Using Negative Pressure Wound Therapy and Split-Thickness Skin Grafting for Polymicrobial Ecthyma Gangrenosum in a Patient With Acute Lymphoblastic Leukemia: A Case Report.

Ecthyma gangrenosum (EG) is a rare and potentially life-threatening cutaneous infection predominantly affecting profoundly immunocompromised patients. Although classically associated with Pseudomonas (P.) aeruginosa, polymicrobial infections may occur and can result in extensive tissue necrosis and limb-threatening defects. Reconstruction in severely immunocompromised patients is particularly challenging when exposed tendons are present. We present the case of a 32-year-old woman with relapsed B-cell acute lymphoblastic leukemia undergoing re-induction chemotherapy who developed profound neutr

Age-adjusted shock index vs. reverse shock index multiplied by Glasgow Coma Scale to evaluate mortality in pediatric trauma: a systematic literature review and meta-analysis.

OBJECTIVE: Early identification of high-risk pediatric trauma patients is essential for timely intervention. The Shock Index, Pediatric Age-adjusted (SIPA) and the reverse Shock Index multiplied by Glasgow Coma Scale (rSIG, including rSIM variants) are commonly studied physiologic scores that have not been systematically compared. This study evaluated and compared the performance of SIPA and rSIG/rSIM for predicting mortality in pediatric trauma, and secondarily, blood product transfusion and other resource-use outcomes. METHODS: This systematic review and meta-analysis followed PRISMA 2020 a

Early fixation of traumatic mandible fractures is associated with decreased morbidity and resource utilization.

BACKGROUND: There is no clear guidance on when isolated mandibular fractures should undergo surgical fixation. Multiple studies have assessed the timing of mandibular fracture repair, with a consensus that earlier repair does not change outcomes. We aimed to challenge this consensus and identify a data-driven time for surgical fixation. We hypothesized that delayed fixation would be associated with worse outcomes. METHODS: Data on adult patients with closed mandible fractures who underwent surgical fixation were abstracted from the Trauma Quality Improvement Program database from 2017 to 2023

Osteoporosis in Pediatric Orthopaedics: Congenital Diseases, Medical Management, and Perioperative Considerations.

PURPOSE OF REVIEW: Pediatric osteoporosis is a heterogeneous group of primary and secondary disorders that creates challenges for orthopaedic surgeons. This review summarizes evidence on conditions commonly encountered in pediatric orthopedic practice, medical management strategies, and perioperative care for children with bone fragility. RECENT FINDINGS: Diagnostic approaches have been evolving toward integrating fracture characteristics, skeletal risk factors, and bone mineral density (BMD). A single low-trauma vertebral compression fracture is now sufficient for diagnosis of osteoporosis i

Venovenous Extracorporeal Membranous Oxygenation for Tracheoparenchymal Trauma: A Case Report.

Penetrating tracheal injuries present unique challenges for airway management, ventilation, and surgical repair due to the risk of rapid airway decline. We present a case of a 34-year-old male who sustained an arrow injury to the neck. Given the risk of airway compromise with induction or movement, venovenous extracorporeal membrane oxygenation (VV ECMO) was initiated prior to anesthesia. Awake bronchoscopy confirmed tracheal injury with an embedded arrow. The patient underwent successful tracheal repair and robotic arrow extraction under ECMO support. He was weaned off ECMO intraoperatively a

Transradial Perilunate Fracture-Dislocation With Lunotriquetral Coalition.

Perilunate fracture-dislocations are devastating injuries, and their occurrence in the presence of a congenital carpal coalition is exceedingly rare. This combination fundamentally alters normal wrist biomechanics and significantly complicates surgical decision-making. We present a case of a 52-year-old female with acute hand ischemia following a high-energy motorcycle collision. Radiographic evaluation revealed a transradial perilunate fracture-dislocation complicated by a unilateral Minnaar type 3 congenital lunotriquetral coalition. Emergent closed reduction was performed, which immediately

Fixation Strategies for Metacarpal Shaft Fractures: A Comparison of Plate-and-Screw, Intramedullary Screw, and Percutaneous Wire Techniques.

PURPOSE: The optimal fixation for metacarpal shaft fractures remains a topic of debate. This study compares clinical outcomes of three fixation methods-closed reduction with percutaneous Kirschner wire pinning, open reduction internal fixation (ORIF), and intramedullary fixation (IMN)-focusing on time to motion, range of motion (ROM), and complications. We hypothesized that IMN would allow earlier and more reliable return of ROM without increasing complications. Presentation and management of tislelizumab induced toxic optic neuropathy.

Immune checkpoint inhibitors (ICIs) have revolutionised the management of advanced malignancies, but can cause immune-related adverse events (irAEs), including vision-threatening complications such as optic neuropathy. Ocular irAEs have been described with several programmed death-1/programmed death-ligand 1 and cytotoxic T lymphocyte-associated antigen-4 inhibitors, but ocular toxicities of tislelizumab have so far been limited to Vogt-Koyanagi-Harada (VKH)-like uveitis, panuveitis and retinal vasculitis. We describe a woman with metastatic lung adenocarcinoma who was treated with second-line
