Medico-legal interpretations of gas on postmortem CT: a scoping review.

The use of postmortem computed tomography (PMCT) in forensic investigations has increased during recent years. PMCT differs from clinical CT due to the inevitable manifestation of postmortem changes that varies significantly between cases. However, differentiating between gas with an antemortem origin (e.g. gas embolism) vs. gas with a postmortem (putrefactive gas) origin presents a critical challenge in forensic medicine. This scoping review evaluated how gas distribution patterns on PMCT may aid in determining gas origin. In accordance with PRISMA-ScR guidelines, searches were conducted in P

Thrombotic and thromboembolic events were not associated with tranexamic acid in three large randomized controlled trials.

BACKGROUND: Thrombotic and thromboembolic events are a common and potentially preventable complication in multitrauma patients, and substantial quality improvement efforts are directed at prevention. The results of several randomized controlled trials (RCTs) related to the association between tranexamic acid (TXA) and thrombotic/thromboembolic events have demonstrated conflicting results. We aimed to address this by examining whether prehospital TXA was associated with higher rates of thrombotic/thromboembolic events in a harmonized data set from three large multicenter RCTs. METHODS: We anal

Forensic evidence from a 30-year study of non-thrombotic pulmonary embolism.

Non-thrombotic pulmonary embolism (NTPE) represents a heterogeneous and underrecognized group of conditions characterized by the obstruction of the pulmonary vasculature by non-thrombotic material, often associated with complex pathogenetic mechanisms and non-specific clinical presentations. Despite their potential to cause sudden and unexpected death, comprehensive forensic studies on NTPE remain limited. This study presents a 30-year retrospective forensic analysis (1993-2024) of autopsy cases performed at the Institute of Legal Medicine of Milan, aimed at identifying and characterizing deat

Traumatic cerebral venous air embolism associated with open skull fracture: Radiographic differentiation from pneumocephalus for clinical decision-making.

Cerebral venous air embolism (CVAE) is a rare condition associated with various etiologies, including trauma. While intracranial air is a recognized finding in clinical practice, the pathway of its entry is rarely documented. An 84-year-old male presented following a head trauma after a fall. Computed tomography (CT) imaging revealed an open parietal skull fracture without associated intracranial hemorrhage. It also demonstrated that intracranial air was strictly restricted to the superior sagittal and cavernous sinuses. Consequently, a CVAE was diagnosed based on the air distribution, ruling

Fatal Massive Air Embolism Following Penetrating Infraorbital Trauma From a High-Pressure Industrial Pump Component: A Forensic Autopsy Case Report.

Air embolism is an uncommon but well-recognized cause of sudden death, most frequently encountered in iatrogenic settings. Traumatic air embolism is rare and typically associated with injuries of the head, neck, or thorax. We report an exceptional occupational fatality caused by massive systemic air embolism following penetrating facial trauma. A high-pressure industrial pump component ruptured and was propelled into the worker's right infraorbital region, producing fractures of the orbital floor and skull base with laceration of the right cavernous and sigmoid venous sinuses. Autopsy revealed

[Anaesthesiological Concepts in Paediatric Neurosurgery].

Paediatric neurosurgery presents a unique set for the anaesthesiologist, necessitating a patient tailored approach for a diverse population, ranging from neonates to adolescents and from healthy to chronically ill children. Understanding the intracranial compartment as the sum of brain tissue, blood volume and cerebral spinal fluid is essential. Given that approximately a quarter of a child's cardiac output is dedicated to cerebral perfusion, intracranial bleeding might cumulate in haemorrhagic shock. Anaesthesiological measures to maintain homeostasis and their influences both on cerebral aut

ARDS management in trauma patients.

Acute respiratory distress syndrome (ARDS) is a frequent and severe complication in trauma patients, arising from a complex interplay between direct tissue injury, systemic inflammation, and resuscitation-related factors. Although trauma accounts for a minority of ARDS cases overall, its incidence remains high in patients with severe thoracic injury. The pathophysiology is best described by a "multi-hit" model, in which the initial traumatic insult is followed by a dysregulated inflammatory response and additional secondary insults, including transfusion, infection, or fat embolism, ultimately

Cerebral air embolism after implantation of a leadless pacemaker via the right internal jugular vein: a case report.

BACKGROUND: Leadless pacemaker (LP) implantation is associated with fewer device-related complications than conventional transvenous pacing systems. While traditionally performed via the femoral vein, the right internal jugular (RIJ) approach has recently emerged as an alternative strategy, potentially improving catheter control. However, safety data for this approach remain limited. CASE SUMMARY: A 77-year-old man with precapillary pulmonary hypertension, diabetes mellitus, and recurrent traumatic syncope underwent LP implantation via the RIJ vein following documentation of infra-Hissian con

Scuba-related injuries presenting to United States emergency departments: paediatric and adult patterns.

INTRODUCTION: Scuba diving is a popular sport that carries risks unique to the underwater environment. Paediatric emergency department presentations for scuba diving injuries are incompletely characterised, particularly in comparison with adults. METHODS: We retrospectively analysed scuba-related injuries recorded in the National Electronic Injury Surveillance System (NEISS) from 2005-2024. We estimated national paediatric emergency department presentations using NEISS weights and compared injury categories and hospitalisation rates between paediatric and adult patients. RESULTS: We identifi

Unexpected Air on Trauma Computed Tomography: Iatrogenic Intravascular Air After Peripheral Venous Cannulation.

Small amounts of intravascular air are occasionally detected on computed tomography (CT), most commonly after intravenous contrast administration. However, intravascular air may also be introduced during routine peripheral venous access before contrast injection. In the trauma setting, this finding may be misinterpreted as traumatic mediastinal or thoracic pathology, particularly in elderly patients. We report the case of an 89-year-old man who presented after a ground-level fall with mild head trauma. A 20-gauge peripheral intravenous catheter was inserted into the left median cubital vein be

Fat embolism syndrome following severe blunt chest trauma treated with veno-venous extracorporeal membrane oxygenation: A case report.

BACKGROUND: Fat embolism syndrome (FES) often occurs following long bone fractures and presents with respiratory symptoms in 90 % of cases. Although rare, the mortality rate ranges from 5 % to 20 %. Its pathophysiology involves fat obstruction in the pulmonary arteries or free fatty acids that damage the alveoli. Treatment primarily involves supportive care, including lung-protective ventilation strategies such as acute respiratory distress syndrome (ARDS) management. We report a case of severe FES that required veno-venous extracorporeal membrane oxygenation (VV-ECMO). CASE PRESENTATION: A m

Extracorporeal cardiopulmonary resuscitation in trauma patients: An analysis of the ELSO registry.

BACKGROUND: Cardiac arrest in trauma patients can occur from traumatic or non-traumatic aetiologies. Traditionally, trauma has been regarded a contraindication for extracorporeal life support, particularly extracorporeal cardiopulmonary resuscitation (ECPR). METHODS: In this retrospective study, the Extracorporeal Life Support Organisation registry was screened for adult trauma patients receiving ECPR (01/01/2020-01/12/2024). We reported characteristics, reasons of cardiac arrest, complications and survival of trauma patients receiving ECPR because of a traumatic cardiac arrest and trauma pat

Resuscitative thoracotomy: What you need to know.

Resuscitative thoracotomy (RT) is a time-critical procedure performed in the setting of traumatic cardiac arrest, primarily to address cardiac tamponade, control intrathoracic bleeding, and reduce infradiaphragmatic blood loss. Advances in trauma care have refined the indications and improved the outcomes of RT in select patient populations. This review focuses on current techniques, indications, outcomes, and future directions. The physiologic objectives are to release pericardial tamponade, repair cardiac injuries, control intrathoracic hemorrhage, perform open cardiac massage, and provide a

Postmortem diagnosis of fatal venous air embolism due to sigmoid sinus rupture associated with a basilar skull fracture.

BACKGROUND: Venous air embolism (VAE) secondary to basilar skull fractures is rare but potentially fatal. While VAE is a known complication in central venous catheter management, traumatic VAE-particularly in forensic contexts-remains underreported. In forensic medicine, few case reports have detailed the use of postmortem computed tomography (PMCT) to diagnose VAE. CASE PRESENTATION: A man in his 30 s sustained a head injury at a construction site when his head was caught between a 500-kg steel plate and the ground. Despite resuscitation attempts, spontaneous circulation was not restored, an

Traumatic air embolism; a case report and literature review of this rare complication of trauma.

Trauma management forms a significant component of any health system. It can affect any body system, and as such knowledge of the wide possible presentations and sequalae are critical. Systemic air embolism (SAE) is a rare presentation in trauma, though it can be associated with significant morbidity and mortality. We present the case of a 23-year-old gentleman with isolated penetrating trauma to the chest who developed widespread neurological insult as a result of SAE, and review historical and proposed contemporary management of SAE.

Fatal Consequences of Head Trauma: A Case Report of Venous Air Embolism Complicated by Substance Abuse and Liver Disease.

Venous air embolism is a rare but potentially fatal complication arising from traumatic injuries or medical procedures, characterized by the intravascular introduction of air leading to the formation of gas emboli within the pulmonary vasculature and cardiac chambers. Here, we present the case of a 34-year-old female with a history of substance abuse who sustained a head injury during an altercation. Despite initial resistance to medical assistance due to intoxication, she eventually received treatment for a significant head wound. A postmortem examination revealed a triangular wound penetrati
