Hydrogen peroxide is widely used for irrigation of surgical wounds. However, its administration has been associated with gas embolism. We report a case of gas embolism after wound irrigation with hydrogen peroxide in a 11-year-old boy undergoing extraction of the extra-traumatic splint under general anesthesia. When 3% hydrogen peroxide 12 ml was applied to wound of the left femur after extraction of the splint, the patient showed clinical signs of pulmonary embolism. Symptomatic treatment was initiated immediately. When the patient awoke from anesthesia, he showed tonic convulsion. But he rec

PURPOSE: To illustrate a new airway and ventilatory management strategy for patients with unilateral penetrating lung injury. Emphasis is placed on avoiding positive pressure ventilation (PPV)-induced systemic air/gas embolism (SAE) through traumatic bronchiole-pulmonary venous fistulas.
CLINICAL FEATURES: A 14-yr-old male, stabbed in the left chest, presented with hypovolemia, left hemopneumothorax, an equivocal acute abdomen, and no cardiac or neurological injury. In view of the risk of SAE, we did not ventilate the left lung until any fistulas, if present, had been excised. After pre-oxyge

UNLABELLED: The pediatric patient is to be found in hyperbaric facilities throughout the world, receiving hyperbaric oxygen (HBO) therapy for both life-threatening and chronic diseases.
OBJECTIVE: To review the experience accumulated at the Israel Naval Medical Institute in the treatment of pediatric patients.
DESIGN: A retrospective analysis and review of all records of patients younger than age 18 years.
RESULTS: Between 1980 and 1997, 139 pediatric patients age 2 months to 18 years (mean, 7.7 years) received HBO treatment at the Israel Naval Medical Institute. Of the children, 111 (79%)

Although the sitting position offers advantages for posterior fossa surgery, it is accompanied by complications such as air embolism and pneumatocephalus. Subdural and epidural haematomas are less common postoperative complications of posterior fossa surgery. To the best of our knowledge, however, anosmia is not a known sequela of surgery in the sitting position. It has been described following aneurysm surgery in the rostral part of the circle of Willis and is, of course, well known in traumatic brain injury.

The world literature, including hospital and medicolegal case records, was reviewed to collate cases of venous air embolism resulting from the increasing number of operative hysteroscopies being performed. Seven women undergoing operative hysteroscopy for five different indications had clear-cut evidence of venous air embolism early in the course of the procedure. Five of these patients died. This complication is rare but devastating, resulting from traumatic opening into large uterine sinuses, especially with the patient in the Trendelenburg position, when the heart is below the level of the

Central venous catheters are often utilized in the acute care management of the traumatic brain injury (TBI) patient. While in the agitated phase of recovery, TBI patients often focus on removing indwelling tubes or venous access lines. Air embolism is among the most serious complications reported in the use of central venous catheters. We present a case of air embolism in an agitated patient, and discuss prevention methodologies. A 38-year-old male was involved in a fall, with an initial Glasgow Coma Scale of 12. CAT scan imaging revealed a left frontal intracerebral haematoma. He required ce

Embolization is a well-recognized phenomenon in medicine and forensic pathology and generally involves formed blood elements, bone, air, amniotic fluid, or more exotic items such as bullets. We report the embolization of a chain-saw link. The case demonstrates how, without complete autopsy and investigation, a penetrating sharp-force injury could be misdiagnosed and a false conclusion as to the commission of a crime could occur. Although the forensic literature is replete with examples of bizarre emboli, from bullets to suspected crystals of methylmethacrylate, a literature review failed to fi

A case of post-traumatic cerebral air embolism is presented. In the chest-X-radiography fractures of many ribs and hemo-pneumothorax were visible on the right side associated with intra-alveolar, interstitial hemorrhages and mediastinal emphysema. At the end of a CT scan of the head appeared features of arterial air embolism. The patient was diagnosed as brain dead and cardiac arrest occurred one hour after.

Traumatic air embolism is rare in Australia, but must be treated promptly if patients are to survive. A single fatal case of traumatic air embolism due to penetrating trauma is described with attention to the presenting symptoms and signs, the unsuccessful attempts at resuscitation and correlation with the post-mortem findings. Patients with penetrating chest trauma are at high risk of traumatic air embolism and positive pressure ventilation of the affected lung will cause rapid death if the condition is not immediately recognized. Early aggressive treatment is therefore necessary for survival

An effective method has been developed for prevention of air embolism in cardiopulmonary bypass surgery on the open heart without heart isolation from intergrown tissues. The method consists in filling and washing heart cavities with carbon dioxide before removing the clamps from the aorta. This method does not involve routine prophylactic measures such as heart dislocation and puncture of left-ventricular apex, heart ventricles massage to remove air, etc. Operation without heart isolation from intergrown tissues helps markedly reduce blood loss, make the operation less traumatic and shorter.

A retrospective study was performed of 100 consecutive acute traumatic spinal injury patients with neurological deficit admitted to the Yorkshire regional spinal injuries centre prior to May 1990. Ninety-seven of these patients received prophylactic low dose subcutaneous heparin and 3 patients were excluded because of noted complications. Twenty-six patients developed thromboembolic complications. Apart from the fact that acute spinal cord injury patients are considered to be in an hypercoaguable state, the occurrence of deep vein thrombosis/pulmonary embolism (DVT/PE) in our study, even thoug

A 34-year-old man developed a nonfatal venous air embolism during arthrography of his post-traumatically contractured knee joint. From the arthrographic findings, injected air was assumed to have entered the great saphenous vein via the venous opening in the knee joint. Venous air embolism is an extremely rare complication of knee arthrography, but can occur. As it is potentially fatal, prompt recognition and appropriate positional and supportive therapy are essential.

Air within the cavernous sinus resulting from fracture of the wall of the sphenoid sinus was identified by cranial CT in a patient with head injury. Careful search of the skull base is important in post-traumatic pneumocephalus to identify a potential site of CSF leak.

The morbidity and mortality of hemodialysis by internal central venous catheterization in the subclavian and internal jugular positions are reviewed. A follow-up study was performed in our unit over 10 years (786 catheterizations). The most frequent complications were inadequate flow (7.6%) inadvertent withdrawal (5.6%) and bacteremia (5.1%). The overall complication rate was 27.2%. Kinking (p less than 0.001), bleeding (p less than 0.01) and bacteremia (p less than 0.05) occurred more frequently in patients with chronic renal failure, compared to patients with acute renal failure. Previous animal studies in our laboratory revealed that acute cerebral ischemia induced by air embolism is accompanied by a sharp and pronounced rise in both systemic blood pressure and intracranial pressure. To investigate possible approaches to the treatment of severe intracranial hypertension, we produced cerebral air embolism in anesthetized, ventilated cats by the infusion of air into the vertebral artery. Untreated animals consistently experienced severe acute intracranial hypertension. Pretreating the animals with lidocaine greatly attenuated the intracranial hypertension caused by air
