Medical interventions for traumatic hyphema.

BACKGROUND: Traumatic hyphema is the entry of blood into the anterior chamber (the space between the cornea and iris) subsequent to a blow or a projectile striking the eye. Hyphema uncommonly causes permanent loss of vision. Associated trauma (e.g. corneal staining, traumatic cataract, angle recession glaucoma, optic atrophy, etc.) may seriously affect vision. Such complications may lead to permanent impairment of vision. Patients with sickle cell trait/disease may be particularly susceptible to increases of elevated intraocular pressure. If rebleeding occurs, the rates and severity of complic

Spinal epidural hematoma: not always an obvious diagnosis.

Spinal epidural hematoma (SEH) is a rare neurosurgical emergency. SEH is characterized by an archetypal clinical presentation including abrupt spinal pain followed more or less rapidly by various degrees of neurological deficit. The diagnosis of SEH, often based on a clinical presumption, represents a clinical challenge. Several reports have outlined missed or delayed diagnosis due to unusual and confusing onsets or unawareness of this diagnosis by physicians. Therefore, physicians should keep in mind the possibility of SEH in their differential diagnosis when confronted with patients complain

Value of percutaneous embolotherapy for the management of traumatic vascular limb injury.

BACKGROUND: Arterial injuries to the extremities may result in severe hemorrhagic complications. Open surgical repair has been the standard of care for these patients, but endovascular management may be a valuable, less invasive alternative. PURPOSE: To evaluate the safety, efficacy and long-term durability of embolotherapy for the management of hemorrhagic, traumatic vascular injuries in upper and lower limbs. MATERIAL AND METHODS: Patients with traumatic or iatrogenic hemorrhagic vascular injuries to the extremities and managed with endovascular embolotherapy in the authors' institution be

Traumatic intracranial aneurysms due to blunt brain injury-a single center experience.

BACKGROUND: Traumatic intracranial aneurysms (TICAs) have previously been described in literature. However, the evidence of TICAs secondary to blunt brain injury have not been elucidated well, with most of the conclusions coming from isolated case reports. We have attempted to examine the epidemiology, classification, clinical presentation, therapeutic modalities, and outcomes of TICAs with our series of patients at the neurosurgery department of Xuanwu Hospital, China. METHODS: We reviewed our aneurysm database from January 1, 2005 to December 31, 2011. In particular, patients with TICAs sec

[Local fibrinolysis in treatment of non-traumatic intracerebral hematomas and ventricular hemorrhages].

Local fibrinolysis and aspiration in treatment of spontaneous intracerebral hematomas (SIH) and ventricular hemorrhages (VH) have become a wide spread technique in dedicated cerebrovascular centers. Forty four patients treated in Burdenko NSI in 2007-2011 were evaluated. Local fibrinolysis for SIH were perfomed in 30 pt., for isolated VH in 14. Puroplazan, a prourokinaze based derivative (mean dose - 50 000 ME) were used in 36 cases, Actilyse (tPA) (2.0 g mean) in 8 cases. Status at discharge was improved in 66.7% of patients with SIH and 57.1% of patients with isolated VH. Mortality comprised

A multicenter experience of the management of collapsed thoracic endografts.

OBJECTIVES: Thoracic endograft collapse after thoracic endovascular aortic repair (TEVAR) is a potentially devastating complication. This study evaluates the management of thoracic stent graft collapse. METHODS: A multicenter review of thoracic stent graft collapse was performed from 2005 to 2009. Diagnosis and preoperative planning was performed by computed tomography angiography (CTA). Outcome measures included success of endovascular salvage, postoperative complications, and conversion to open repair. RESULTS: Eleven patients (10 men) with thoracic endograft collapse were identified. Mean

Open versus endovascular repair for patients with acute traumatic rupture of the thoracic aorta.

OBJECTIVE: The study objective was to compare the outcome between open and endovascular repair of acute traumatic rupture of the thoracic aorta. METHODS: Seventy-five patients (mean age 38.6 ± 10.7 years) with an acute traumatic aortic rupture were referred to the Arnaud de Villeneuve Hospital between January 1990 and January 2010. Between January 1990 and December 2000, 35 patients (33 men, mean age 35.8 ± 11.3 years) underwent surgical repair using cardiopulmonary bypass. From January 2001, an endovascular approach was deliberately chosen; 40 patients (30 male, mean age 41 ± 10.1 years) und

Expert opinion on emerging drugs in childhood arterial ischemic stroke.

INTRODUCTION: Vascular stroke syndromes in childhood (arterial ischemic stroke (AIS), cerebral sinovenous thrombosis and non-traumatic intracranial hemorrhage) are increasingly recognized as important causes of mortality and morbidity, with significant personal and societal economic costs. The disease mechanisms underlying childhood AIS differ significantly from adults; in particular, atheroma plays an extremely minor role in children. However, to date, treatment principles in children have been based on extrapolation from adult data, usually by consensus expert opinion. AREAS COVERED: This r

Conservative treatment of an aortoesophagial fistula after endovascular stent grafting for a thoracic aortic aneurysm.

BACKGROUND: Aortoesophageal fistula (AEF) is an uncommon condition that presents a problem in therapy because of the high rate of morbidity and mortality associated with its surgical management and the uniformly fatal outcome of medical treatment. In this article we describe a case of secondary AEF after endoluminal stent grafting of the thoracic aorta, which was observed by only conservative management and followed up for 14 months with no signs of recurrent hemorrhage or chronic mediastinitis. CASE REPORT: A 54-year old man with hepatocellular carcinoma (HCC) was admitted to our hospital be

[Ultrasonic dissection in operative treatment of scars].

The authors have developed a technique of disconnection of soft tissues using an ultrasound dissector for ablation of the changed by scar skin covering. The ultrasound dissector splits the subcutaneous adipose tissue and loose connective tissue without injuring the vascular-nervous bundles. This method substantially reduces the intraoperative blood loss. The complex treatment combining the little traumatic ultrasound dissector with conservative and physiotherapeutic methods of treatment and prophylactics allows considerable reduction of probability of recurrent pathological scars.

Cubital tunnel syndrome - a review and management guidelines.

Cubital tunnel syndrome (CuTS) is the second most common peripheral nerve compression syndrome. In German-speaking countries, cubital tunnel syndrome is often referred to as sulcus ulnaris syndrome (retrocondylar groove syndrome). This term is anatomically incorrect, since the site of compression comprises not only the retrocondylar groove but the cubital tunnel, which consists of 3 parts: the retrocondylar groove, partially covered by the cubital tunnel retinaculum (lig. arcuatum or Osborne ligament), the humeroulnar arcade, and the deep flexor/pronator aponeurosis. According to Sunderland ,

Subdural haematoma due to dural metastases from bronchogenic carcinoma in a previously well patient: an unusual cause of non-traumatic recurrent intracranial haematomata.

Subdural haematomata (SDH) are usually traumatic in aetiology. Non-traumatic instances of SDH are uncommon, and can rarely be due to metastases involving the dura. Computed tomography or magnetic resonance imaging can be misleading, as the underlying aetiology may be masked by the SDH, or the appearance can simulate meningiomas. A high index of suspicion for SDH is thus required. Under such circumstances, when no overt cause is identified, dural tissue should be sent for histological analysis and blood clot for cytology, even if the appearances are grossly normal at surgery. We present a rare

Management of distal traumatic median nerve painful neuromas and of recurrent carpal tunnel syndrome: hypothenar fat pad flap.

The hypothenar fat pad flap receives segmental blood supply from the ulnar artery in Guyon's canal. It is easy to elevate and mobilize and can provide protective covering and a gliding surface for the median nerve during secondary nerve surgery in the distal forearm and carpal tunnel. It is locally available and can be used without making additional surgical incisions.

Aura attacks from acute convexity subarachnoid haemorrhage not due to cerebral amyloid angiopathy.

BACKGROUND AND PURPOSE: Convexity subarachnoid haemorrhage (cSAH) has recently been recognised as a cause of recurrent aura-like symptoms, mimicking transient ischaemic attacks (TIAs). Subarachnoid haemorrhage and recurrent aura-like episodes can occur in patients with cerebral amyloid angiopathy (CAA), which has been the presumed cause in the majority of reported cases. However, this syndrome can occur following cSAH secondary to other conditions, and it is important for clinicians to investigate and manage such patients appropriately. METHOD: Case series. RESULTS: We describe two patients

Hemorrhage from a cavernous hemangioma with fatal blood aspiration: A rare cause of sudden unexpected death.

Blood aspiration may be fatal, even when caused by a minor hemorrhage, and then it is important to localize and identify the source of bleeding in order to determine its origin as traumatic, spontaneous or iatrogenic. The present case deals with the clinically unexpected sudden death of a 73-year-old woman who underwent pulmonary artery catheterization (PAC) 11 days before because of known pulmonary hypertension. The forensic autopsy revealed a previously undiagnosed cavernous hemangioma of the right lung which had led to a local intrapulmonary hemorrhage with secondary bleeding into the bronc

Methods of model calibration: observations from a mathematical model of cervical cancer.

Continuing with comparative genomic exploration of worldwide butterfly fauna, we use all protein-coding genes as they are retrieved from the whole genome shotgun sequences for phylogeny construction. Analysis of these genome-scale phylogenies projected onto the taxonomic classification and the knowledge about butterfly phenotypes suggests further refinements of butterfly taxonomy that are presented here. As a general rule, we assign most prominent clades of similar genetic differentiation to the same taxonomic rank, and use criteria based on relative population diversification and the extent o

[Outpatient treatment of deep vein thrombosis with low-molecular-weight heparins. Experience from an open, prospective study in daily practice].

BACKGROUND AND PURPOSE: Clinical studies have shown the safety and efficacy of low-molecular-weight heparin (LMWH) for initial treatment of deep vein thrombosis (DVT) in outpatients. Given the limited evidence for the application of LMWH in this indication in daily practice, the authors conducted an open, prospective study in primary care. Primary endpoint of this study was a clinical sign of recurrent thrombosis and/or pulmonary embolism 14 days after the end of DVT treatment with 1 mg/kg body weight enoxaparin twice daily. Secondary endpoint were bleeding complications during and up to 48 h

Favorable response to intrathecal, but not oral, baclofen of priapism in a patient with spinal cord injury.

STUDY DESIGN: Case report. OBJECTIVE: To retrieve data about the utility of intrathecal baclofen for the treatment of otherwise refractory priapism that may occur in patients with spinal spasticity. SUMMARY OF BACKGROUND: Baclofen exerts well-known inhibitory effects on sexual function both in animals and humans. This observation led to the introduction of oral baclofen in the treatment of refractory and recurrent idiopathic priapism. METHODS: We report a 41-year-old male patient who sustained a severe traumatic spinal cord injury in a motor vehicle accident. On clinical examination he pres
