Surgical correction of failed thoracic endovascular aortic repair.

OBJECTIVE: The number of thoracic aortic endovascular procedures is increasing rapidly, and the clinical outcome largely depends on the underlying aortic pathology. When primary stent grafting is unsuccessful, secondary endovascular solutions are most often feasible. However, in recurrent endovascular failure without further minimally invasive options, conservative treatments or conversion to open surgery are the only remaining therapeutic strategies. METHODS: In our experience, 106 patients received thoracic aortic endovascular treatment. Five of these patients and three from other centers u

Analysis of 13 cases with gastroepiploic vessels used as grafts.

Free flaps are used increasingly in reconstructive surgery. However, many patients lack a satisfactory recipient vessel near the defect because of radiotherapy, post-traumatic infection, or treatment for recurrent tumor. We usually use a venous graft in such cases, but when both long arterial and venous segments are needed, we use the gastroepiploic vessels because they include extensive, viable arterial and venous segments. Between 1998 and 2006, we used gastroepiploic vessels as grafts in the 13 cases analyzed here. All defects lacked satisfactory recipient vessels nearby. Patients included

[Embolization in peripheral artery bleeding with Onyx].

OBJECTIVE: This study describes use of ethylene vinyl alcohol copolymer (EVOH) dissolved in anhydrous dimethyl sulfoxide (DMSO) and mixed with tantalum powder to achieve radiopacity as an effective, safe, and manageable embolic agent in endovascular treatment of artery haemorrhages. MATERIALS AND METHODS: Twenty patients with pseudoaneurysms secondary to biopsy, or surgery were treated, 11 musculoskeletal arteries, 9 visceral (4 gastroduodenal arteries). A diagnostic angiographic procedure that showed the artery lesion was always performed. Embolization was performed immediately, followed by

Endoluminal embolization and revascularization for complicated mesenteric pseudoaneurysms: a report of two cases and a literature review.

Mesenteric artery pseudoaneurysms are a rare and complex complication after abdominal trauma, surgery, or inflammatory disease. These lesions can be associated with compression or erosion into adjacent structures and may result in life-threatening hemorrhage. Traditional management has been open surgical ligation, aneurysm resection with interposition grafts, or resection or partial resection of the end organ involved. We present two cases of endovascular repair of complex mesenteric pseudoaneurysms. One patient presented with a recurrent pseudoaneurysm of the superior mesenteric artery and th

[Imaging findings of muscle traumas in sports medicine].

Traumatic muscular pathology is frequent in the athlete. Usually, these lesions heal spontaneously leaving no sequelae. One must be able to predict how long the healing process will last in order to avoid a long period of inactivity and to protect the patient from a recurrent tear. Medical imaging can define the precise location and severity of muscle traumas and detect critical elements that will delay complete repair. Sonography is the most useful and the least expensive imaging technique for analyzing muscular trauma. It provides a good study of muscle fibers, tendons, and aponeurosis but i

Stress history and breast cancer recurrence.

BACKGROUND: There is mixed evidence regarding the possible association between a history of stressful or traumatic life events and more rapid breast cancer progression. METHOD: Retrospective reports of past experiences of traumatic life events were assessed among 94 women with metastatic or recurrent breast cancer. A traumatic event assessment was conducted using the event-screening question from the posttraumatic stress disorder (PTSD) module of the Structured Clinical Interview for the DSM-IV-TR (SCID; 2002). Each reported event was judged by two independent raters to determine whether it m

Long-term follow-up after endovascular treatment of acute aortic emergencies.

PURPOSE: To investigate the long-term outcome and efficacy of emergency treatment of acute aortic diseases with endovascular stent-grafts. METHODS: From September 1995 to April 2007, 37 patients (21 men, 16 women; age 53.9 +/- 19.2 years, range 18-85 years) with acute complications of diseases of the descending thoracic aorta were treated by endovascular stent-grafts: traumatic aortic ruptures (n = 9), aortobronchial fistulas due to penetrating ulcer or hematothorax (n = 6), acute type B dissections with aortic wall hematoma, penetration, or ischemia (n = 13), and symptomatic aneurysm of the

The role of postoperative patient posture in the recurrence of traumatic chronic subdural hematoma after burr-hole surgery.

OBJECTIVE: Chronic subdural hematoma (CSDH) is one of the most common types of intracranial hemorrhage, especially in the elderly, with a significant recurrence rate ranging from 9.2 to 26.5%. The role of postoperative patient posture in the recurrence of CSDH has not been studied sufficiently. METHODS: A total of 84 consecutive patients with unilateral traumatic CSDH without known risk factors of CSDH recurrence were prospectively enrolled in this study. All patients underwent burr-hole surgery with closed system drainage and were then allocated randomly to either of two groups: Group A (n =

Effects of chondroitin sulfate on alteration of actin cytoskeleton in rats with acute necrotizing pancreatitis.

Chronic Suppurative Otitis Media (CSOM) is a chronic inflammation of middle ear cleft. It presents with discharging ear and decreased hearing. The diagnosis is mostly on clinical examination with otoscope or oto endoscope. Computerised Tomography (CT) of mastoids is done to evaluate the extent of the disease and its complication. High resolution CT is now the investigation of choice for temporal bone disease. This study emphasizes on the importance of CT scan in diagnosis of inflammatory ear diseases and most importantly identifying pitfalls or complications which a surgeon can come across dur

Central pancreatectomy: a technique for the resection of pancreatic neck lesions.

HYPOTHESIS: Central pancreatectomy has been used sparingly because the spectrum of indications is quite narrow. Although historically used for traumatic pancreatic transection and chronic pancreatitis, it currently is reserved for selective management of pancreatic neck lesions that are benign or have low malignant potential. Varying morbidity rates have been published in the literature. Our objectives were to describe the technique and determine the safety and effectiveness of central pancreatectomy in the excision of benign or low-malignant potential lesions of the pancreatic neck. DESIGN:

Invasive papillary carcinoma of the breast presenting as post-traumatic recurrent hemorrhagic cysts.

We report the sonographic features of an intracystic papillary carcinoma of the breast presenting as recurrent hemorrhagic cysts following trauma. A 56-year-old woman presented with palpable breast masses after a traumatic event; sonography showed multiple, well-defined, hemorrhagic cysts. Hemorrhagic fluid was evacuated by fine needle aspiration with no residual lesions. Cytology was negative for malignancy. Five months later, the mass reappeared; sonography demonstrated multiple cysts with solid nodules. US-guided core biopsy and surgery revealed invasive papillary carcinoma. We suggest clos

The use of a proximally based posterior interosseous adipofascial flap to prevent recurrence of synostosis of the elbow joint and forearm.

We report on four children in whom a proximally based, posterior interosseous artery adipofascial flap was used as an adjunct to surgical resection of synostoses of the forearm and elbow. Three traumatic radio-ulnar and one congenital humero-radial synostoses were treated. The postoperative pronation to supination arc of motion was excellent in all of the traumatic cases and fair in the congenital case.

Assessment of an anti-HIV-1 combination gene therapy strategy using the antisense RNA and multimeric hammerhead ribozymes.

BACKGROUND: Head trauma is a known cause of seizures, with about 10% of patients with moderate or severe injuries experiencing seizure events. Early prevention and control of seizures are critical to limit secondary brain injuries. This study aimed to evaluate the frequency, type, and timing of seizures in patients with traumatic brain injury. METHODS: This analytical-descriptive study included all patients with post-traumatic seizures. Exclusion criteria were pre-existing brain disorders, prior epilepsy, or anticonvulsant use. Data were collected from hospital records using a structured chec

Laparoscopic management of Meckel's diverticulum in children.

BACKGROUND: Rectal bleeding, recurrent abdominal pain, nausea, and vomiting in children could present a diagnostic as well as therapeutic challenge. Meckel's diverticulum (MD) is one of the causes. The objective of the current study was to evaluate the feasibility and outcome of laparoscopic management of MD. METHODS: The clinical data of 33 children admitted with rectal bleeding and/or recurrent abdominal pain with no identifiable cause were reviewed over a period of 8 years. There were 23 boys and 10 girls with a mean age of 5.12 +/- 2 years (range, 3-12 years). In 21 cases, MD was an incid

Posttraumatic epilepsy with special emphasis on prophylaxis and prevention.

Posttraumatic epileptic seizures have an incidence of about 10% in series of severe head injuries. Control of "early seizures", i.e. those occurring in the first week after injury, is mandatory. Attacks, especially if recurrent, may add secondary damage to the injured brain: intravenous phenythoin with therapeutic plasma level allows control of the attacks. Seizures occurring months or years after injury are called "late seizures": recurring "late seizures" make up the clinical syndrome of "posttraumatic epilepsy". "Prophylaxis" should mean that drug treatment, given for a more or less prolong

Post-traumatic epilepsy: an overview.

Post-traumatic epilepsy (PTE) is a recurrent seizure disorder secondary to brain injury following head trauma. PTE is not a homogeneous condition and can appear several years after the head injury. The mechanism by which trauma to the brain tissue leads to recurrent seizures is unknown. Cortical lesions seem important in the genesis of the epileptic activity, and early seizures are likely to have a different pathogenesis than late seizures. Anti-epileptic drugs available for treatment are phenytoin, sodium valproate, and carbamazepine. Newer anti-epileptics are helpful, particularly in patient

Superficial siderosis of the central nervous system following cervical nerve root avulsion: the importance of early diagnosis and surgery.

Superficial siderosis (SS) of the central nervous system is an insidious, progressive, irreversible and debilitating neurological disorder caused by recurrent haemorrhage within the subarachnoid space. The subsequent deposition of haemorrhagic breakdown products in the spinal cord and nervous tissues leads to the loss of neurones and myelin, and to the development of a neurological deficit. In a small number of patients, the source of haemorrhage is related to traumatic cervical nerve root avulsion occurring several years prior to the onset of symptoms. Surgical ablation of the source has been
