Limb Salvage Using Negative Pressure Wound Therapy and Split-Thickness Skin Grafting for Polymicrobial Ecthyma Gangrenosum in a Patient With Acute Lymphoblastic Leukemia: A Case Report.

Ecthyma gangrenosum (EG) is a rare and potentially life-threatening cutaneous infection predominantly affecting profoundly immunocompromised patients. Although classically associated with Pseudomonas (P.) aeruginosa, polymicrobial infections may occur and can result in extensive tissue necrosis and limb-threatening defects. Reconstruction in severely immunocompromised patients is particularly challenging when exposed tendons are present. We present the case of a 32-year-old woman with relapsed B-cell acute lymphoblastic leukemia undergoing re-induction chemotherapy who developed profound neutr

Post-traumatic Aspergillus niger empyema in an immunocompetent military patient: a case report.

BACKGROUND: Fungal empyema is an uncommon clinical entity, and empyema attributable to Aspergillus species is exceedingly rare. The vast majority of reported cases occur in immunocompromised hosts. Empyema caused specifically by Aspergillus niger has been described only in exceptional circumstances and is virtually absent from the medical literature. We report the first documented case of post-traumatic Aspergillus niger empyema in a young, immunocompetent individual. CASE PRESENTATION: A 22-year-old male active-duty special forces soldier sustained a penetrating gunshot wound to the left tho

Marjolin's ulcer developing in a post-traumatic scar: management with negative pressure wound therapy and split-thickness skin grafting.

Marjolin's ulcer is a rare but aggressive malignant transformation arising in chronic wounds or scars. We report the case of a man in his 60s who developed a large, non-healing ulcer at the site of a previous traumatic injury and reconstructive surgery performed three decades earlier. Clinical suspicion of malignancy led to wide local excision with sentinel lymph node biopsy, followed by staged reconstruction. Negative pressure wound therapy (NPWT) was used to optimise the wound bed, which was complicated by a local infection requiring targeted antibiotic therapy. Subsequent split-thickness sk

A quaternized chitosan-based biomimetic hydrogel: Integrating barrier and regulatory functions for post-traumatic analgesia and accelerated burn repair.

Burn wound management is plagued by inadequate pain control and high infection risks. Conventional smart hydrogels often suffer from compromised mechanical integrity, unstable adhesion, and burst drug release upon stimulation. Inspired by the sucker morphology and secretion-based adhesion of remora that withstands turbulent marine environments, we design a novel remora-inspired hydrogel dressing. It integrates quaternized chitosan (QCS), sodium p-styrene sulfonate (VBS), hydroxyethyl methacrylate (HEMA), N-vinyl-2-pyrrolidone (NVP), and a newly synthesized mimicry monomer (EI) to confer enviro

Managing Refractory Infection and Suicide Risk After Auricular Reconstruction.

RATIONALE: Multidrug-resistant organism (MDRO) infection following auricular reconstruction is uncommon, but it can severely compromise reconstructive outcomes, especially when associated with an increased risk of suicide. PATIENT CONCERNS: A 39-year-old man developed a refractory MDRO infection following post-traumatic autologous costal cartilage auricular reconstruction, experiencing severe depressive symptoms with suicidal ideation (peak PHQ-9 score: 25). DIAGNOSES: Postoperative MDRO infection and major depressive disorder with high suicide risk. INTERVENTIONS: An integrated, safety-cen

Major Traumatic and Severe Thermal Injuries Lead to Immediate and Persistent Elevations in Circulating Concentrations of Resistin That Are Associated with Poor Clinical Outcomes and Impaired Innate Immune Responses.

Major trauma induces innate immune suppression, yet the underlying mechanisms are poorly understood. Resistin is an immunosuppressive molecule that is systemically elevated post-injury. However, its role in trauma-induced immune dysfunction and clinical outcomes is poorly defined. Here, we acquired blood samples from 147 adult trauma patients (≤1, 4-12, 48-72 h post-injury) and 95 burns patients (days 1, 3, 7, 14, 28 post-burn). We measured plasma resistin concentrations, studied resistin gene expression in peripheral blood mononuclear cells (PBMCs) and neutrophils, and measured resistin produ

Machine Learning-Based Prediction Model for Infectious Complications in Trauma and Its Association With In-Hospital Mortality.

BACKGROUND: Infectious complications, such as sepsis or catheter-related infections, are common and serious sequelae after trauma. Despite their clinical significance, existing risk-prediction models are limited by reliance on in-hospital data that fail to capture complex physiological interactions. Thus, this study aimed to develop and validate an interpretable ensemble machine learning (ML) model integrating both prehospital and in-hospital clinical data to predict infectious complications after trauma. METHODS: We used data from the Korean Trauma Data Bank, comprising patients admitted to

Risk factors for post-traumatic lymphedema after open fractures of the lower leg: a retrospective cohort study.

BACKGROUND: Post-traumatic lymphedema (PTL) is an underrecognized complication of severe limb trauma and is characterized by persistent limb swelling. Patients with open fractures of the lower leg may be more susceptible to PTL, and clinical factors such as extensive soft-tissue injury, wound infection, and repeated surgical procedures may be associated with lymphatic injury and local inflammation. However, the clinical factors associated with PTL in this population remain unclear. This study aimed to identify factors associated with PTL after open fractures of the lower leg. METHODS: We retr

Perioperative Glucagon-Like Peptide-1 Receptor Agonist Therapy and Postoperative Outcomes in Adult Foot and Ankle Surgery: A Scoping Review.

Glucagon-like peptide-1 receptor agonists (GLP-1RAs) are emerging as a cornerstone in the management of type 2 diabetes and obesity. The perioperative implications of GLP-1RAs in orthopaedic surgery are a growing area of interest; however, their effects in patients undergoing foot and ankle surgery remain uncertain. This scoping review aims to map the available evidence on the use of GLP-1RAs in adults undergoing foot and/or ankle surgical procedures. We aim to examine the impact of perioperative GLP-1RA use on postoperative outcomes in adults undergoing foot and/or ankle surgery. We systemati

Effect of early compared with late weightbearing on the outcomes of subtalar joint arthrodesis : a systematic review and meta-analysis of single proportions.

AIMS: Although end-stage osteoarthritis (OA) of the subtalar joint is routinely managed surgically with arthrodesis, postoperative weightbearing regimens vary widely from immediate weightbearing through to prolonged (≥ six weeks) non-weightbearing. It has been assumed that immediate weightbearing may compromise the rate of union and be associated with an increased rate of complications, but these concerns have not been substantiated. This systematic review and meta-analysis of single proportions aimed to assess the effect of early compared with late weightbearing regimens on the outcomes after

Case Report: Post-traumatic Aeromonas infection causing severe lower limb soft tissue infection-a report of two surviving cases.

We report two cases of severe lower limb soft tissue infection caused by Aeromonas following traumatic injury. A 26-year-old male sustained propeller injury with fractures and developed progressive infection complicated by septic shock and necrotizing fasciitis, requiring left femoral mid-shaft amputation. A 58-year-old male suffered an open tibial plateau fracture and presented with wound infection that resolved after antimicrobial treatment but was followed by tetanus. Both patients had positive cultures for Aeromonas hydrophila (and Aeromonas caviae in the second case) identified via matrix

[Our Experience with Trabecular Metal Total Ankle System].

PURPOSE OF THE STUDY: One of the surgical treatment options for advanced ankle joint destruction with various etiologies is the total joint replacement. Its significant upside is the preservation of range of motion of the ankle joint and less stress on forefoot joints compared to ankle arthrodesis. Since 2022, we have been using the Zimmer Trabecular Metal Total Ankle inserted via a lateral transfibular approach. This study aims to evaluate the initial outcomes and experience with this implant. PATIENTS AND METHODS: Between 2022 and 2024, 65 total ankle replacements were performed in 63 patie

In Situ Coenzyme-Based Transformable Polymers Powder With Instant-Tough-Adhesive Properties for Emergency Hemostasis, Antibiosis, and Wound Repair.

Current tissue adhesives are challenged by uncontrolled post-traumatic hemorrhage and infection due to inadequate wet adhesion, prolonged setting time, potential cytotoxicity, and insufficient integration of key biological functionalities-specifically antimicrobial and pro-healing capabilities-for effective wound management. Herein, this work proposes a self-stabilizing, water-responsive coacervate powder composed of coenzyme-based polymers (α-lipoic acid/sodium α-lipoate, PLA/Na), tannic acid (TA), and ε-polylysine (PLys), abbreviated as PLA/Na@TA/PLys, to provide a one-stop solution for rapi

Post-Traumatic Chest Wall Abscess Associated with Gordonia sputi in an Immunocompetent Patient: A Case Report.

INTRODUCTION: Gordonia species are rare aerobic Gram-positive actinomycetes and opportunistic pathogens. Their clinical manifestations and optimal management remain poorly defined. Gordonia sputi is an especially uncommon cause of soft-tissue and chest wall infections, and diagnosis is often delayed because of slow growth and difficulties in identification. CASE PRESENTATION: We report the case of a 45-year-old previously healthy man who presented with a progressively enlarging and painful mass on the left chest wall following blunt trauma. He was afebrile and had no systemic symptoms. Contra

Reconsidering Surgical Intervention for Infection After Cranioplasty in Older Adults: A Case Report.

Post-cranioplasty infection with scalp breakdown in older adults poses a difficult reconstructive problem, particularly when frailty and cognitive impairment may limit tolerance for staged procedures. Although debridement, infection control, and delayed cranioplasty are often considered standard management, the extent of reconstruction that should be pursued in elderly patients remains uncertain. We report the case of a woman in her late 80s who developed scalp loss with exposure of a synthetic cranial implant eight months after cranioplasty following surgery for acute subdural hematoma. Application of a posterior tibial artery perforator-based fasciocutaneous flap with a broad fascial pedicle in reconstruction of complex lower-leg wounds: a retrospective clinical study.

BACKGROUND: Complex soft-tissue defects of the lower leg with exposed bone, tendon, or internal fixation material remain difficult to reconstruct, particularly when the surrounding wound bed has been affected by infection, scar formation, or previous surgery. Free flaps are reliable for extensive defects but require microsurgical expertise and suitable recipient vessels. This study evaluated the clinical outcomes of a posterior tibial artery perforator-based fasciocutaneous flap with a broad fascial pedicle for selected complex lower-leg wounds, while preserving the main posterior tibial arter

Surgical Approach to a Rare Forearm Morel-Lavallée Lesion: Diagnostic and Management Challenges in the Upper Extremity.

Morel-Lavallée lesions (MLLs) are closed soft-tissue degloving injuries most often reported in the pelvis and lower extremity after high-energy shear trauma. Upper extremity (UE) involvement is exceedingly rare and frequently misdiagnosed as cellulitis or abscess. We report a 49-year-old Black male with a history of psychiatric disorders and intravenous drug use (IVDU) who presented to the Hospital of Central Connecticut's Emergency Department (ED) with a swollen, erythematous, and painful right forearm one week after blunt trauma. Contrast-enhanced computed tomography (CT) was inconclusive, b
