Morel-Lavallée lesions (MLLs) are closed soft-tissue degloving injuries most often reported in the pelvis and lower extremity after high-energy shear trauma. Upper extremity (UE) involvement is exceedingly rare and frequently misdiagnosed as cellulitis or abscess. We report a 49-year-old Black male with a history of psychiatric disorders and intravenous drug use (IVDU) who presented to the Hospital of Central Connecticut's Emergency Department (ED) with a swollen, erythematous, and painful right forearm one week after blunt trauma. Contrast-enhanced computed tomography (CT) was inconclusive, b

OBJECTIVE: Decompressive craniectomy (DC) relieves refractory intracranial hypertension but produces a cranial defect that mandates secondary cranioplasty and carries substantial morbidity. Bone-flap-preserving alternatives exist but are constrained by limited decompressive volume or the need for a second repositioning operation. We describe a modified expansion-floating craniotomy (EFC) and report its preliminary feasibility and safety.
METHODS: We retrospectively reviewed 9 consecutive patients with refractory intracranial hypertension treated with a modified EFC between April 2025 and May

A man in his 20s developed chronic osteomyelitis following surgical fixation of a compound distal humerus fracture. He presented with pain, discharging sinuses from the operated site, and restricted elbow movement. A staged limb salvage strategy was employed. The first stage involved implant removal, extensive bone and soft tissue debridement with antibiotic cement bead placement, and primary skin closure. Post-surgery, he developed wound dehiscence and infection, not treated by antibiotic therapy. A secondary wound debridement along with negative pressure therapy was done. Three weeks later,

BACKGROUND: Post-traumatic Edwardsiella tarda (E. tarda) infections usually have an acute course. Chronic infection is rare. We report a chronic refractory ulcer caused by E. tarda after a freshwater stingray injury.
CASE PRESENTATION: A previously healthy 32-year-old male diver sustained a stingray barb puncture wound to the right knee. The wound remained inflamed despite initial treatment and progressed to a deep ulcer with an 8-cm sinus tract. Bacterial culture confirmed E. tarda. Treatment included repeated surgical debridement, negative pressure wound therapy, and intravenous cefuroxime

OBJECTIVE: To identify the virulence factors responsible for toxic shock syndrome in causative strains submitted by field laboratories throughout the Czech Republic.
METHODS: To detect the production of TSST-1 and staphylococcal enterotoxins, the micro-slide gel diffusion test was initially used and later followed by reverse passive latex agglutination. Currently, the polymerase chain reaction is the preferred tool for the detection of toxin genes as well as genes for other toxins of S. aureus strains: exfoliatins A, B, and D, Panton-Valentine leukocidin, and the mecA and mecC genes responsib

Chronic osteomyelitis of the distal tibia following failed fracture fixation is a challenging condition because of limited soft-tissue coverage, compromised vascularity, persistent infection, and difficulty achieving stable reconstruction. We report the case of a 34-year-old female who developed chronic post-traumatic osteomyelitis of the distal tibia after failed initial management of a distal tibial fracture. She presented with persistent pain, swelling, inability to bear weight, and a discharging sinus. Radiographic evaluation showed persistent distal tibial fracture pathology and failure o

Eikenella corrodens is a slow-growing, fastidious gram-negative bacillus that is often overlooked in routine diagnostic procedures. We report a case of a nine-year-old girl who developed a surgical site infection five days after debridement and suturing for craniofacial trauma. Gram staining of purulent material found gram-negative coccobacilli. The isolate was presumptively identified as Eikenella spp. by conventional culture and biochemical testing, and later confirmed as E. corrodens by matrix-assisted laser desorption/ionization time-of-flight mass spectrometry. Based on the literature, am

Lower extremity reconstruction in elderly patients with post-traumatic osteomyelitis presents a formidable challenge. We report a 70-year-old female with hypertension, dyslipidaemia, and bilateral knee osteoarthritis who sustained a Gustilo-Anderson type IIIB open right ankle fracture with exposed calcaneus following a fall. The course was complicated by chronic osteomyelitis, managed with partial talectomy and total calcanectomy, antibiotic-impregnated cement spacer placement, cement exchange and prolonged culture-directed intravenous antibiotics. Following infection eradication, definitive h

Liposomal amphotericin B (AmBisome) is a lipid-associated formulation of the broad-spectrum polyene antifungal agent amphotericin B. It is active against clinically relevant yeasts and moulds, including Candida spp., Aspergillus spp. and filamentous moulds such as Zygomycetes, and is approved for the treatment of invasive fungal infections in many countries worldwide. It was developed to improve the tolerability profile of amphotericin B deoxycholate, which was for many decades considered the gold standard of antifungal treatment, despite being associated with infusion-related events and nephr

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Post-traumatic wound infection is commonly observed in trauma patients and has a poor prognosis. Vitamin D is a significant modulator of the immune system and may play a role in preventing wound infection. However, the inherent limitations and inconsistent results of previous observational studies restricted them from inferring causal effects. We employed Mendelian randomization design to estimate the causal effects of serum 25-hydroxyvitamin D levels on the risk of post-traumatic wound infection. Data in the analysis were extracted from large-scale genome-wide association studies with 417,580

Uncontrolled hemorrhage and infection are leading causes of post-traumatic deaths in both clinical and battlefield settings. Although several wound dressings have been developed, rapid and effective hemostasis and wound healing promotion in irregular and non-compressible wounds remain serious challenges. In this study, a multifunctional hydrogel (HPCS-C/ODEX/CC hydrogel) was developed using two natural polysaccharide derivatives, catechol-modified hydroxypropyl chitosan (HPCS-C) and oxidized dextran (ODEX), as well as Crinis Carbonisatus (CC) nanoparticles for rapid hemostasis and wound healin

Vibrio alginolyticus is a facultatively anaerobic, Gram-negative bacillus that is a common component of marine flora. Infections caused by Vibrio alginolyticus are rare and typically occur following exposure to seawater or marine animals. This report details the clinical presentation and follow-up of a 65-year-old immunocompetent male who developed a wound infection due to Vibrio alginolyticus. Advanced diagnostic tools, such as MALDI-TOF mass spectrometry, can enhance the identification of these bacteria. Sport clinicians need to recognise Vibrio infections in seawater-contaminated wounds, as

BACKGROUND: Total knee arthroplasty (TKA) in post-traumatic osteoarthritis (PTOA) presents challenges that are not encountered in primary osteoarthritis (OA) cases. Given the inconsistent evidence regarding the safety and efficacy of TKA after PTOA, this meta-analysis, seeks to provide a clear understanding of the additional risks that are associated in PTOA patients.
METHODS: Following the PRISMA guidelines, PubMed, Cochrane, and Google Scholar (pages 1-20) were accessed and explored until January 2025. The extracted data consisted of complications (overall complications, mechanical complica

Tactical Combat Casualty Care (TCCC) guidelines have his-torically recommended antibiotics for combat wounds due to potential delays in evacuation and wound contamination. The currently recommended agents, moxifloxacin (oral) and ertap-enem (parenteral), have not been recently reviewed. This paper documents the findings of a multidisciplinary panel convened in 2023 to re-evaluate TCCC antibiotic recommendations con-sidering current antibiotic options, emerging data regarding multi-drug resistance (MDR), and evolving combat wound microbiology. The panel addressed four key questions through lite
