Impact of Hypobaria and Hypoxia Exposure on Mortality, Inflammation, and Coagulopathy in an Animal Model of Polytrauma.

INTRODUCTION: Following traumatic injury, both the inflammatory and hemostatic systems are activated. Patients experience a systemic increase in circulating cytokines and coagulation leading to a greater risk of organ dysfunction. Post-trauma aeromedical evacuation exposes patients to hypobaria and potential hypoxia. We sought to assess the impact of hypobaria and hypoxic (H/H) on mortality, coagulation, and inflammation in a mouse model of polytrauma. MATERIALS AND METHODS: Eight- to 12-week-old male C57BL/6J mice were subjected to sham or polytrauma operation with the latter consisting of b

Mouse Models of Muscle Fibrosis: Mechanisms, Methods, and Applications.

Skeletal muscle injuries are common and some are able to regenerate due to satellite cells, the muscle stem cell population. However, in cases of severe muscle injury, complete tears, or muscle loss via trauma, muscles can undergo fibrosis and long-term compromise of their structure and function. The development of animal models has been key to understanding the pathways involved in muscle injury, fibrosis, and repair. In this review, we discuss the animal models currently used, with a focus on those most applicable to studying muscle fibrosis after traumatic injury. We summarize the approach,

Silencing of HAVCR2 Attenuates Skeletal Muscle Ischemia-Reperfusion Injury by Inhibiting Oxidative Stress.

Skeletal muscle ischemia-reperfusion (IR) injury is a common clinical condition associated with oxidative stress and inflammation that leads to muscle damage and multi-organ failure. However, the molecular mechanisms underlying its pathogenesis remain incompletely understood. R software and the limma package were used to analyze mRNA expression profiles from the GSE275811 dataset. Hub genes were identified using the protein-protein interaction (PPI) network, support vector machine-recursive feature elimination (SVM-RFE), and least absolute shrinkage and selection operator (LASSO) regression an

The effect of quercetin on ischemia-reperfusion injury in skeletal muscle in rats.

BACKGROUND: Ischemia-reperfusion (I/R) injury of the lower limbs is a significant clinical challenge that can arise due to surgical procedures, thrombotic events, embolism, or traumatic vascular damage. This study aimed to evaluate the antioxidative and histo-pathological protective effects of quercetin, a potent flavonoid antioxidant, on skeletal muscle subjected to I/R injury. METHODS: Eighteen Wistar Albino rats were randomly assigned into three groups: Control (sham laparotomy), Ischemia-Reperfusion (IR) group (2 hours of ischemia followed by 2 hours of reperfusion), and Ischemia-Reperfus

Transcatheter Intra-Arterial Delivery of a Platelet-Derived Extracellular Vesicle-Enriched Preparation for Attenuating Skeletal Muscle Ischaemia-Reperfusion Injury in a Rodent Forelimb Model.

Extracellular vesicles (EVs) are non-replicating, lipid membrane-bound nanoparticles released by most eukaryotic cells into the extracellular space. When derived from platelets, a subset of EVs can exhibit remarkable anti-inflammatory and antioxidant properties, making them attractive candidates to attenuate sterile inflammatory states such as limb skeletal muscle ischaemia-reperfusion injury (IRI). Peripheral extremity IRI complicating acute limb revascularisation procedures, replantation of major traumatic amputations and limb allotransplants can trigger a life-threatening reperfusion syndro

From guillotine to EEG: Revisiting the neurological controversies of decapitation through modern neurophysiology.

The question of whether consciousness persists after decapitation has fascinated scientists, philosophers, and the public for over two centuries, from 18th-century debates over the guillotine to modern neurophysiological studies of cerebral ischemia. This review synthesizes historical controversies-such as the anecdotal reports of facial movements in severed heads and early galvanism experiments-with modern neurophysiological evidence to reinterpret the mechanisms underlying the loss of consciousness. We trace the evolution of hypotheses from initial philosophical speculations to electroenceph

Clinical and Hematological Characteristics of Rhabdomyolysis-Associated Kidney Injury in Military Personnel With Limb Wounds.

INTRODUCTION: Tourniquet (TQ) use is a standard method of prehospital hemorrhage control in combat trauma. Prolonged limb ischemia with tourniquet syndrome (TS) may lead to severe skeletal muscle injury, traumatic rhabdomyolysis, and acute kidney injury (AKI). In recent armed conflicts, prolonged evacuation intervals have increased the frequency and clinical significance of these complications, while detailed clinical and laboratory descriptions remain limited. MATERIALS AND METHODS: A retrospective analysis was performed of 84 active-duty service members with combat-related extremity injurie

Charting Variant Vascular Pathways in Musculoskeletal Imaging: Clinical and Surgical Implications.

Variant vascular pathways often present a challenge to a radiologist interpreting routine musculoskeletal imaging studies. In this article, the authors focus on the prevalence, anatomic landmarks for both normal and variant anatomy, clinical significance of the aberrant vasculature and anatomic associations, and potential risks of surgical complications. In the upper extremity, the persistent median artery and high division of the radial artery, known as the brachioradial artery, threaten median neuropathy or upper extremity ischemia. Pelvic trauma cases should be scrutinized for corona mortis

Conservative Management of a Traumatic Deep Palmar Arch Pseudoaneurysm with Delayed Ulnar Motor Dysfunction: A Case Report.

Traumatic pseudoaneurysms of the palmar arch are rare vascular lesions and are typically managed with surgical repair or endovascular exclusion. Deep palmar arch involvement is particularly uncommon, and neurological deficits are often considered indications for exploration. We report the case of a 26-year-old male with a penetrating palm injury resulting in a bilobulated deep palmar arch pseudoaneurysm, a thenar hematoma, and delayed ulnar motor dysfunction. Because there was no digital ischemia, ongoing bleeding, or lesion expansion, conservative management with structured clinical and radio

Traumatic Spinal Cord Infarction Associated With Elevated Intracranial and Intraspinal Pressure in a Pediatric Patient: A Case Report and Review of the Literature.

Spinal cord ischemia (SCI) in the pediatric population is rare and not well characterized. Initial diagnosis is often delayed, and there are no standardized, evidence-based management strategies. Risk factors and etiologies vary and include trauma, thrombotic or embolic disease, infection, vasculitis, cerebellar herniation, arteriovenous malformation, cardiovascular interventions, scoliosis correction, sickle cell disease, and idiopathic causes. We present a case of an 11-year-old girl with anterior spinal artery (ASA) syndrome who presented four days after a fall with back pain, bilateral low

Multifocal Skeletal Muscle Mineralisation Following Severe Trauma and Prolonged Critical Illness: A Case Report.

Soft tissue mineralisation is an uncommon complication of severe trauma and critical illness and may occur in association with metabolic derangements, tissue injury and prolonged immobilisation. We present a case of a 26-year-old male patient who developed extensive skeletal muscle calcification following multiple stab wounds, hypovolemic shock, and surgical interventions, including limb amputation and bowel resections. Initial imaging showed no calcification; however, six weeks post-injury, CT demonstrated extensive amorphous calcification in the abdominal wall, chest, iliopsoas, and gluteus

Development and characterization of rat hindlimb ischemia models mimicking peripheral arterial disease severity.

Peripheral arterial disease (PAD) is a progressive atherosclerotic disorder that reduces blood flow to the lower extremities, leading to skeletal muscle degeneration, functional decline, and limb loss in advanced stages. To enable rigorous preclinical evaluation of regenerative therapies across disease stages, we developed and quantitatively characterized three rat hindlimb ischemia models representing graded arterial insufficiency: iliac artery ligation (Iliac), femoral artery ligation and excision (Femoral), and combined femoral plus popliteal artery ligation and excision (Fem/Pop). Laser Do

Pectoralis Major Muscle Necrosis After Selective Arterial Embolization: A Case Report With Reconstructive Considerations.

The pectoralis major (PM) muscle is commonly used in reconstruction because of its robust vascular supply from the thoracoacromial artery, although vascular dominance varies among individuals. Transcatheter arterial embolization is effective for hemostasis but may risk ischemia when a dominant pedicle is compromised. We describe a case of delayed PM muscle necrosis after selective arterial embolization for traumatic chest wall hemorrhage. A 76-year-old man with diabetes mellitus, pancreatic head carcinoma under chemotherapy, and polymyalgia rheumatica treated with prednisolone for a prolonged

[Crush Syndrome - What Should We Know in the Pre-hospital Setting?].

A crush injury (CI) is damage to bone, muscle, subcutaneous tissue, and skin caused by prolonged external pressure applied to soft tissues. In contrast, a crush syndrome (CS) is a multisystem syndrome caused by the release of toxins from injured muscle. The syndrome is also sometimes referred to as traumatic rhabdomyolysis. The pathophysiology of the syndrome involves prolonged ischemia and reperfusion injury. These processes lead to the release of toxic metabolites from the damaged muscle, resulting in hypovolemic shock, acidosis, electrolyte disturbances, acute kidney injury, and even multip

A Systematic Review of the Vascular and Neuronal Role of L-Carnosine: Mechanistic and Pharmacological Perspectives.

INTRODUCTION: L-carnosine is a powerful natural antioxidant found in the brain, muscles, and digestive systems of all vertebrates, including humans, that helps fight Reactive Oxygen Species (ROS) and other harmful byproducts of oxidative stress. L-carnosine has antioxidant properties as well as the ability to bind with metal ions and prevent glycation, a process that can damage cells. This review article highlights the protective effects of L-carnosine against a variety of pathological conditions, such as Alzheimer's Disease (AD), Parkinson's Disease (PD), ischemia‒reperfusion injury, Huntingt

Therapeutic Limb Hypothermia Preserves Neurophysiologic Function and Reduces Tissue Injury in a Novel Porcine Survival Model of Acute Hindlimb Ischemia.

BACKGROUND: Acute limb ischemia carries high morbidity and risk of limb loss. Prior animal models were limited to non-survival designs and biochemical endpoints, precluding assessment of long-term outcomes. We developed a porcine survival model of ischemia-reperfusion injury to evaluate therapeutic limb hypothermia with functional, neurophysiologic, and histopathologic endpoints. METHODS: Ten female Yorkshire pigs underwent unilateral hindlimb ischemia via balloon occlusion of the ipsilateral external and internal iliac arteries for six hours, confirmed by angiography, and were randomized to

INTRAMUSCULAR TERLIPRESSIN AS MONOTHERAPY FOR EARLY MANAGEMENT OF UNCONTROLLED TRAUMATIC HEMORRHAGIC SHOCK: A PRELIMINARY STUDY.

Monotherapy using a single-dose vasopressor to maintain essential organ perfusion could be a viable alternative to restrictive fluid resuscitation in uncontrolled traumatic hemorrhagic shock (UTHS). This study evaluated the impact of intramuscular terlipressin on hemodynamics and organ injuries in UTHS. Adult male Sprague-Dawley rats, with eight rats in each group, were assigned to six distinct groups. Immediately after randomization, the rats in each group were exposed or not exposed to specific interventions over a 60-min period, as follows: sham, shock, intramuscular terlipressin (5 μg/100
