UNLABELLED: Following traumatic limb amputation it is common clinical practice to maintain the ischemic tissues in a hypothermic state until surgical reimplantation. A number of methods have been used to protect organ systems and cells from the ravages of ischemia and hypoxia. Some have attempted to reduce the metabolic needs, such as by hypothermia, cardioplegia, and slow calcium channel blockers. The Chromotrop-Anilinblue (CAB) trichrome staining method, developed for liver specimens, enables rapid detection of fresh damage to myocardial fibers by staining contraction-bands bright red. Dehiscence of the transverse striation, although yet unexplained in its pathogenetic significance, becomes visible with CAB staining. Overall, the method seems less sensitive to artefacts than former so-called infarction-staining-methods. Contraction-bands are not conclusive evidence for ischemic damage, as they also appear in traumatic heart damage (e.g. knife- and gunshot-wounds).

The unusual occurrence of spasticity at birth with symmetrical thalamic damage was found in a male infant delivered at 36 weeks' gestation following an episode of traumatically induced premature labor at 32 weeks. In 185 patients after different cardiosurgical operations a postoperative follow-up study (after 3 hours) of the activity of MB creatinephosphokinase (CPK) was carried out. The cumulative activity of MB CPK was calculated (Q MB CPK) according to a modified Shell-Sobel formula. A 40-year-old joiner collapsed while windsurfing. After hospitalization the ECG showed a posterior wall infarction (stage 1) of which the patient shortly died. Fifteen compartment syndromes seen in the first few hours have been collected. Fourteen occurred after tibial fracture and one after a simple contusion. In typical cases it is emphasised that ischaemia affects the contents of the compartment in an insular fashion so that distal ischaemia is not present. The main arteries are sound and demonstrated by distal pulses, the vascularity of the foot, Doppler studies and arteriography. To study muscle necrosis due to prolonged limb compression, we measured intramuscular pressure by inserting wick catheters into 10 volar forearms and 10 anterior tibial compartments of adult volunteers. We then placed the subjects in positions in which victims of drug overdose are commonly found. In rabbits a tourniquet shock was produced by 4-hour unilateral ischemia of one hind leg. By measuring the tissue oxygen tension on the surface of the ischemically damaged muscle, the liver and kidney the regional rate of perfusion of these organs in the state of shock was investigated. Additionally morphological examinations of muscle, liver, kidney and heart were carried out. In therapeutic trials with a proteinase inhibitor (Trasylol), prednisolone and streptokinase, Trasylol proved to prolong the time of survival significantly.

Such lesions cannot be expected to resolve without complications in the absence of appropriate treatment. 3. Myositis ossificans should be anticipated in lesions of muscles adjacent to their attachments. 4. Chronic tendonitis and enthesopathies not associated with inflammatory connective tissue disease are primarily degenerative in nature and are probably the result of comparative ischaemia. The crush syndrome has been an accompaniment of crushing injuries following disaster situations. Recognition of the sequence of events and early institution of the appropriate therapy is essential, if life is to be saved in these critically injured persons. Twenty cases of ischemic contracture in the lower limb demonstrate that the age distribution may be higher than that of patients with similar lesions in the upper limbs. Non-traumatic and traumatic causes are encountered equally frequently. Pain is one of the most important symptoms associated with long standing lesions. The ischemic lesion develops most frequently and severely in the anterior compartment of the leg. The functional prognosis is poor when both nerves are affected and when the deformity is severe.

Six cases with cervical cord contusion are described. All were over 50 years old, had an hyperextension injury of their cervical spine, suffered from tetraparesis predominant in the upper limbs and showed no X-ray evidence of injury to the cervical spine. Intra-arterial tolazoline has been used as an adjunct for angiographers and has been investigated for application in certain peripheral vascular disorders secondary to its potent activity as a vasodilator.
