[Higher Brain Dysfunction Caused by Left Frontal Brain Retraction Injury and Infarction of the Left Genu of the Internal Capsule and Anterior Thalamus: A Case Report of Transient Gerstmann Syndrome Due to Multifocal Lesions].

We herein report a case of subarachnoid hemorrhage caused by the rupture of a cerebral aneurysm that was treated by clipping surgery. The patient was a right-handed woman in her 40s who presented with attention disorder and transient Gerstmann syndrome, including left-right disorientation, finger agnosia, acalculia, and agraphia, associated with brain retraction injury in the left inferior frontal gyrus and perforating branch infarction of the horizontal segment of the left anterior cerebral artery. Diffusion tensor tractography was performed to investigate the underlying pathophysiological me

Management of Bilateral Subacute Subdural Hematomas with Concomitant Subarachnoid Hemorrhage in a Diabetic Patient with HIV in Cameroon: A Case Report.

Bilateral subdural hematomas occurring simultaneously with subarachnoid hemorrhage in a traumatic context are rare and pose considerable diagnostic challenges. Comorbidities such as diabetes and HIV may further worsen prognosis and complicate management. This case underscores the vital importance of early diagnosis and multidisciplinary care in patients with subacute subdural hematomas and subarachnoid hemorrhage, particularly when compounded by comorbidities in resource‑limited settings. We report the case of a 44-year-old housewife who was diabetic and living with HIV. She presented with sev

Clustered Regularly Interspaced Short Palindromic Repeats (CRISPR)-Based Genome Editing in Inherited Cardiovascular Diseases: Therapeutic Advances, Emerging Editing Platforms, and Translational Challenges.

Inherited cardiovascular diseases (ICVDs) are a broad category of hereditary disorders resulting from pathogenic mutations that alter cardiac anatomy, electrical conduction, and metabolic pathways. The main types include hypertrophic cardiomyopathy (HCM), dilated cardiomyopathy (DCM), cardiac channelopathies, and familial hypercholesterolemia (FH) and are associated with heart failure, malignant arrhythmias, and sudden cardiac death, particularly in young people. Current treatments, such as pharmacological therapies, implantable cardioverter-defibrillators, and heart transplantation, have sign

Immediate Unrestricted Assisted Full Weight-Bearing After Bicondylar Tibial Plateau Fixation: A Case Series.

High-energy bicondylar tibial plateau fractures of Schatzker types V and VI are conventionally managed, after open reduction and internal fixation, with 6-12 weeks of restricted weight-bearing, out of concern for loss of reduction and articular collapse. Emerging evidence questions whether this restriction is necessary. Reports of immediate weight-bearing specifically after fixation of these most unstable bicondylar patterns remain scarce, as most comparative studies pool this subtype within mixed cohorts. We present three consecutive patients (two men and one woman; mean age: 54 years; all of

Age-adjusted shock index vs. reverse shock index multiplied by Glasgow Coma Scale to evaluate mortality in pediatric trauma: a systematic literature review and meta-analysis.

OBJECTIVE: Early identification of high-risk pediatric trauma patients is essential for timely intervention. The Shock Index, Pediatric Age-adjusted (SIPA) and the reverse Shock Index multiplied by Glasgow Coma Scale (rSIG, including rSIM variants) are commonly studied physiologic scores that have not been systematically compared. This study evaluated and compared the performance of SIPA and rSIG/rSIM for predicting mortality in pediatric trauma, and secondarily, blood product transfusion and other resource-use outcomes. METHODS: This systematic review and meta-analysis followed PRISMA 2020 a

Early fixation of traumatic mandible fractures is associated with decreased morbidity and resource utilization.

BACKGROUND: There is no clear guidance on when isolated mandibular fractures should undergo surgical fixation. Multiple studies have assessed the timing of mandibular fracture repair, with a consensus that earlier repair does not change outcomes. We aimed to challenge this consensus and identify a data-driven time for surgical fixation. We hypothesized that delayed fixation would be associated with worse outcomes. METHODS: Data on adult patients with closed mandible fractures who underwent surgical fixation were abstracted from the Trauma Quality Improvement Program database from 2017 to 2023

Osteoporosis in Pediatric Orthopaedics: Congenital Diseases, Medical Management, and Perioperative Considerations.

PURPOSE OF REVIEW: Pediatric osteoporosis is a heterogeneous group of primary and secondary disorders that creates challenges for orthopaedic surgeons. This review summarizes evidence on conditions commonly encountered in pediatric orthopedic practice, medical management strategies, and perioperative care for children with bone fragility. RECENT FINDINGS: Diagnostic approaches have been evolving toward integrating fracture characteristics, skeletal risk factors, and bone mineral density (BMD). A single low-trauma vertebral compression fracture is now sufficient for diagnosis of osteoporosis i

Venovenous Extracorporeal Membranous Oxygenation for Tracheoparenchymal Trauma: A Case Report.

Penetrating tracheal injuries present unique challenges for airway management, ventilation, and surgical repair due to the risk of rapid airway decline. We present a case of a 34-year-old male who sustained an arrow injury to the neck. Given the risk of airway compromise with induction or movement, venovenous extracorporeal membrane oxygenation (VV ECMO) was initiated prior to anesthesia. Awake bronchoscopy confirmed tracheal injury with an embedded arrow. The patient underwent successful tracheal repair and robotic arrow extraction under ECMO support. He was weaned off ECMO intraoperatively a

Transradial Perilunate Fracture-Dislocation With Lunotriquetral Coalition.

Perilunate fracture-dislocations are devastating injuries, and their occurrence in the presence of a congenital carpal coalition is exceedingly rare. This combination fundamentally alters normal wrist biomechanics and significantly complicates surgical decision-making. We present a case of a 52-year-old female with acute hand ischemia following a high-energy motorcycle collision. Radiographic evaluation revealed a transradial perilunate fracture-dislocation complicated by a unilateral Minnaar type 3 congenital lunotriquetral coalition. Emergent closed reduction was performed, which immediately

Fixation Strategies for Metacarpal Shaft Fractures: A Comparison of Plate-and-Screw, Intramedullary Screw, and Percutaneous Wire Techniques.

PURPOSE: The optimal fixation for metacarpal shaft fractures remains a topic of debate. This study compares clinical outcomes of three fixation methods-closed reduction with percutaneous Kirschner wire pinning, open reduction internal fixation (ORIF), and intramedullary fixation (IMN)-focusing on time to motion, range of motion (ROM), and complications. We hypothesized that IMN would allow earlier and more reliable return of ROM without increasing complications. METHODS: A retrospective review of 84 patients (104 diaphyseal fractures) treated at a level I trauma center between 2018 and 2024 w

Presentation and management of tislelizumab induced toxic optic neuropathy.

Immune checkpoint inhibitors (ICIs) have revolutionised the management of advanced malignancies, but can cause immune-related adverse events (irAEs), including vision-threatening complications such as optic neuropathy. Ocular irAEs have been described with several programmed death-1/programmed death-ligand 1 and cytotoxic T lymphocyte-associated antigen-4 inhibitors, but ocular toxicities of tislelizumab have so far been limited to Vogt-Koyanagi-Harada (VKH)-like uveitis, panuveitis and retinal vasculitis. We describe a woman with metastatic lung adenocarcinoma who was treated with second-line

Post-traumatic Aspergillus niger empyema in an immunocompetent military patient: a case report.

BACKGROUND: Fungal empyema is an uncommon clinical entity, and empyema attributable to Aspergillus species is exceedingly rare. The vast majority of reported cases occur in immunocompromised hosts. Empyema caused specifically by Aspergillus niger has been described only in exceptional circumstances and is virtually absent from the medical literature. We report the first documented case of post-traumatic Aspergillus niger empyema in a young, immunocompetent individual. CASE PRESENTATION: A 22-year-old male active-duty special forces soldier sustained a penetrating gunshot wound to the left tho

Periprosthetic fractures of the knee.

Periprosthetic fractures around total knee arthroplasty are an increasingly common and complex problem driven by rising arthroplasty volumes, aging populations, and longer implant survivorship. Successful management requires integration of trauma principles with arthroplasty-specific considerations, particularly implant stability, bone stock, and patient frailty. Implant stability and bone quality are the primary determinants of treatment strategy, outweighing fracture morphology alone. Classification systems are useful frameworks but must be interpreted in the context of component fixation an

What Are the Surgical Complications Associated With Oncologic Rotationplasty Procedures? A Systematic Review.

BACKGROUND: In the surgical treatment of lower extremity sarcomas, rotationplasty provides a durable biologic reconstruction option. It effectively allows the patient to function with a transtibial amputation, without the burden of a nonbiologically based reconstruction. Despite robust function and durability, rotationplasty may invite its own array of complications. A thorough description of rotationplasty complications and outcomes is lacking in published reports, limiting effective patient and family counseling. Moreover, improving awareness of the potential complications will allow surgeon

Nutritional Supplements in the Treatment of Endometriosis-Associated Symptoms: A Systematic Review.

Cancer is regarded as one of the leading causes of death worldwide, despite the progress of traditional therapies. Chemotherapy, radiotherapy, and surgery are often accompanied by significant side effects and the development of drug resistance contributes to making the fight against cancer even more challenging, which clearly highlights the urgent need to develop new therapeutic molecular approaches. In this context, natural peptides were introduced into the pharmaceutical market in the last decade and have replenished the ranks of effective anticancer agents due to their structural diversity,

Colchicine for the Prevention of Vascular Events After an Acute Intracerebral Hemorrhage: A Placebo-Controlled Trial.

BACKGROUND: There is a need for novel treatments that lower the risk of major adverse cardiovascular events and secondary inflammatory brain injury after an intracerebral hemorrhage (ICH). METHODS: We performed a double-blind, placebo-controlled, pilot randomized clinical trial at 11 centers across Canada to determine the feasibility of testing colchicine after an acute ICH. We recruited adults presenting within 48 hours of ICH onset with vascular neuroimaging evidence or risk factors for atherosclerosis. Participants were randomized to oral colchicine 0.5 mg daily or placebo and followed to

Reversibility of Immune Dysfunction Following Pediatric Thermal Injury.

Pediatric thermal injury induces immune dysfunction, which is associated with adverse clinical outcomes (eg, nosocomial infections [NIs]). As such, it is crucial to identify those most at risk for developing NI and determine immunomodulating therapeutics to augment the immune response. Our hypothesis was that immune suppression after pediatric thermal injury is reversible ex-vivo using the immunomodulators recombinant human granulocyte macrophage-colony stimulating factor (GM-CSF) and varlilumab (CD27-agonist). We enrolled 141 pediatric patients with acute thermal injuries from a single burn c

Use of Continuous Regional Anesthesia Infusion as an Opioid-Sparing Modality in Mechanically Ventilated Patients With Acute Traumatic Rib Fractures-A Retrospective Study.

BACKGROUND: Continuous regional anesthesia (CRA) techniques are used for analgesia in patients with acute rib fractures. However, there is a paucity of evidence supporting the initiation of CRA in patients receiving mechanical ventilation (MV). We therefore performed this retrospective study to assess changes in opioid consumption and the rate of liberation from MV in patients with traumatic rib fractures. METHODS: Patients referred to the Acute Pain Service (January 2022-July 2023) who were mechanically ventilated with acute rib fractures were included in this study. Patients received consul
