The Impact of Concomitant Traumatic Brain Injuries on the Surgical Treatment of Burns: A Long-Term, Monocentric Retrospective Study.

Burn injuries significantly impact morbidity and mortality, with early surgical intervention crucial for improving outcomes. However, concomitant traumatic brain injury (TBI) frequently complicates burn management, potentially delaying timely surgical treatment due to neurological concerns. Optimal timing of burn surgery in patients with concurrent TBI remains uncertain, necessitating clearer insights into their clinical outcomes. This retrospective study reviewed burned adults admitted to a Swiss Burn Center between 2014 and 2023. Patients were grouped as burns with TBI, burns alone, burns wi

What is the association between time to surgery and patient outcome after hip fracture? : a systematic review.

AIMS: Performance indicators are used to evaluate the quality of healthcare provided to hip fracture patients. 'Time to surgery' describes the delay between the patient's injury or presentation to hospital and their hip fracture surgery, and is one of the most commonly reported performance indicators. In this review, we investigated the association between time to surgery and patient outcomes following hip fracture. METHODS: A total of 12 electronic databases and other sources were searched and the methodological quality of studies meeting the inclusion criteria was assessed. The protocol for

Impact of chronic obstructive pulmonary disease on mortality in elderly patients with hip fracture: A retrospective cohort study.

OBJECTIVE: Chronic obstructive pulmonary disease (COPD) is thought to increase mortality risk in elderly patients with hip fracture, but its independent effect remains unclear. This study aimed to determine whether COPD is an independent predictor of mortality in this population. METHODS: A retrospective cohort study was conducted on elderly patients with hip fractures. Through propensity score matching, confounding factors between patients with chronic obstructive pulmonary disease (COPD) and those without COPD were balanced. LASSO and Cox regression methods were used to identify predictors

A Qualitative Exploration of Post-Injury Challenges and the Potential Role of the PTSD Coach Mobile Application to Improve Recovery Among Acutely Injured Patients.

OBJECTIVE: Digital interventions designed to prevent development of chronic pain and comorbid mental health symptoms, specifically targeting the acute injury recovery period, are in the early stages of development. The evidence-based PTSD Coach mobile application is a free resource offering cognitive-behavioral interventions to self-manage posttraumatic symptoms; it is used widely in varied populations, and it is feasible and acceptable to injured Emergency Department patients. However, patients' subjective experience and feedback regarding optimizing the app are lacking. METHODS: We conducte

Cyclosporine versus placebo pretreatment of brain-dead donors and kidney graft function (Cis-A-Rein trial): a multicenter, double-blind, randomized, controlled trial.

PURPOSE: Delayed graft function is the most frequent early complication of kidney transplantation. Pretreatment of kidney donors with cyclosporine has decreased delayed graft function in animal studies by reducing ischemia-reperfusion graft injuries. No randomized clinical trials have assessed the efficacy of cyclosporine pretreatment of brain-dead donors in reducing delayed graft function. METHODS: In this multicenter randomized, double-blind, and placebo-controlled trial, brain-dead donors were randomized (1:1) to receive either 2.5 mg/kg of cyclosporine or a glucose placebo infusion. The k

Risk factors for electrographic seizures in infants after cardiac surgery and their association with outcomes: a retrospective cohort study.

Neonates and infants undergoing cardiac surgery are prone to cerebral injuries. Neuromonitoring with continuous conventional electroencephalography (cEEG) and/or amplitude integrated electroencephalography (aEEG) is able to detect subclinical abnormal cerebral patterns, especially seizures, which have been found to correlate with medium-long term neurocognitive deficits. The endpoints of this study are (i) to define a risk model for electrographic subclinical epileptic seizures (ES) and (ii) to investigate the relationship between ES and neurologic and non-neurologic outcomes. Retrospective an

Optimizing venous thromboembolism prophylaxis timing in ventilated traumatic brain injury patients: a retrospective cohort study using ACS-TQIP data (2017-2021).

BACKGROUND: Traumatic brain injury (TBI) patients requiring mechanical ventilation face a heightened risk of venous thromboembolism (VTE), including deep vein thrombosis (DVT) and pulmonary embolism (PE). While early VTE prophylaxis (VTEp) has been shown to reduce thromboembolic complications, the optimal timing remains unclear, particularly in ventilated TBI patients, where concerns about intracranial hemorrhage progression exist. This study evaluates the association between VTEp timing and mechanical ventilation duration in TBI patients. METHODS: A retrospective cohort study was conducted u

Brain-derived tau for monitoring brain injury in acute ischemic stroke.

A specific and accurate blood test for acute brain injury could help monitor infarct growth in ischemic stroke and serve as a surrogate end point in clinical trials. Using a single-molecule detection assay, we assessed plasma brain-derived tau (BD-tau), a marker selectively quantifying tau protein from the central nervous system, in a prospective cohort of 502 patients with acute ischemic stroke with serial blood sampling from admission to day 7. Higher BD-tau concentrations at admission were associated with more extensive early brain injury on computed tomography and predicted larger final in

Changing the practice setting and organizational environment does not impact an orthopedic surgeon's surgical performance in primary total hip arthroplasty.

BACKGOUND: Some studies have explored the influence of theatre staff familiarity on surgical performance, but none have focused specifically on orthopedic surgery. Therefore, we performed a retrospective study aiming to: 1) compare complication rates following total hip arthroplasty (THA) performed at a surgeon's familiar site versus an unfamiliar one, 2) to compare patient-reported outcomes at one year and procedure-related outcomes as well as evaluating other surgical-related factors such as length of stay, operative time. HYPOTHESIS: Our hypothesis is that performing THA in a familiar vers

Detection Accuracy of Ischemia and Neuroinflammation in Traumatic Brain Injury in Rats Using Amide Proton Transfer-Weighted MRI.

Ischemia and neuroinflammation are two key secondary injury events following traumatic brain injuries (TBIs), but they are difficult to assess in vivo. Early detection of these secondary injuries post-TBI allows for precise severity assessment and enables timely, targeted interventions to reduce adverse outcomes. This study aimed to quantify the diagnostic accuracy of amide proton transfer-weighted (APTw) imaging to detect these two post-TBI endophenotypes. Controlled cortical impact (CCI) at depths of 1 mm (mild), 3 mm (moderate), and 5 mm (severe) was induced in 55 adult rats (28 males, 27 f

Amniotic Membrane (AM) Covered Airway Stent Mitigates Airway Injury and Complications: A Comparative In Vivo Study in a Porcine Model.

BACKGROUND: Airway stenting is a critical intervention to restore airway patency, yet complications such as stent migration, mucostasis, airway injury, and granulation tissue formation remain major concerns. Although various covering materials have been explored, the effectiveness of amniotic membrane (AM) as a stent cover has not been evaluated. This study is the first to assess airway-related complications of a novel AM-covered self-expandable metallic stent (SEMS) compared with a commercially available synthetic polymer-covered SEMS. To compare the incidence of stent-related complications b

Rethinking emergency risk assessment: A single-center look at shock index and its variants in COVID-19.

BACKGROUND: The Shock Index (SI) is a validated prognostic tool in conditions such as severe trauma and obstetric hemorrhage. During the COVID-19 pandemic, it was used to identify patients at higher risk of clinical deterioration, but results have been inconsistent. This study aimed to evaluate the prognostic value of the SI and its variants in predicting mortality, need for mechanical ventilation, and hospital length of stay in patients with moderate COVID-19. METHODS AND FINDINGS: This longitudinal analytical observational study was conducted at a high-complexity hospital in southwestern Co

Is posttraumatic stress disorder specific to drug-resistant epilepsy or a common feature of chronic disease? A comparative study with atrial fibrillation and type 1 diabetes.

OBJECTIVE: Posttraumatic stress disorder (PTSD) is more prevalent in epilepsy than in the general population. However, it remains unclear whether this association is specific to epilepsy or a broader consequence of experiencing unpredictable acute episodes within chronic diseases. This study aimed to (1) compare PTSD prevalence and severity in patients with drug-resistant epilepsy (DRE) and patients with other chronic episodic diseases, namely, type 1 diabetes (T1D) and drug-resistant atrial fibrillation (AF); and (2) examine psychiatric comorbidities, emotion regulation, and quality of life a

Treatment of Friction Injury Using Enzymatic Debridement and Autologous Skin Cell Suspension.

Friction injury occurs when a patient slides across a surface at high velocity, resulting in mechanical abrasion, laceration, and thermal burn. Mechanical abrasion removes tissue to a variable depth depending on surface texture in contact, and thermal burn devitalizes tissue to a variable depth based on contact pressure and transfer of kinetic energy. In these heterogenous wounds, tangential excision can excise healthy tissue alongside devitalized tissue, and other debridement techniques like hydrosurgery or dermabrasion may not reach deeper crevices created by abrasion and laceration. In addi

Primary Extracranial Ewing Sarcoma Complicated by Hemorrhagic Shock and Multimicrobial Infection: A Diagnostic and Therapeutic Challenge.

Ewing sarcoma is a rare malignant neoplasm that is seldom located in the skull or scalp. We describe the case of a 19-year-old previously healthy man presenting with a rapidly growing left parieto-occipital exophytic mass following minor trauma, initially misinterpreted as an abscess. The lesion recurred exponentially with polymicrobial infection and progression to massive extracranial tumoral hemorrhage, culminating in hemorrhagic shock. Surgical excision and histopathological examination confirmed Ewing sarcoma with EWSR1 gene rearrangement. Staging revealed pulmonary, lymph node, and bone m
