Tricuspid valve prosthetic replacement. Early and late results with the Starr-Edwards prosthesis.

The total experience (154 patients) with ball-valve (Starr-Edwards) replacement of the tricuspid valve, alone and in combination, through Dec. 31, 1971, at the Mayo Clinic is reviewed. The early mortality rate with isolated tricuspid replacement was twice that for tricuspid replacement combined with replacement of other valves. Among patients receiving three valves, those with "functional'' tricuspid insufficiency and those who were in New York Heart Association (N.Y.H.A.) Class IV preoperatively had a higher early mortality rate. The early mortality rate for the total group was 28%. Of those

Bone lesions in early congenital syphilis.

A radiological study of bone changes in 112 children with congenital syphilis was undertaken. A similar number of normal children acted as a control group. Radiological examination of 5 syphilitic children showed that their bones were normal. Combined metaphyseal and periosteal lesions were the commonest bone disorders seen and are thus the most convincing radiological evidence of congenital syphilis. Transverse metaphyseal lucencies occur early in the disease, and with Wimburger's sign they are the prime evidence of pathology in syphilitic bone. Other radiographic changes are probably owing t

Outcome of head trauma: age and post-traumatic seizures.

A series of 307 head-injured children (0-14 yr) was followed from the acute phase for up to six years with a separate group of 220 non-injured siblings as control. The neurological, EEG, psychometric and psychiatric findings were analysed. Here we discuss the influence of age at time of injury with respect to post traumatic seizures. Overall incidence of early seizures (during first week) was 15% and of late seizures (one week to six years) was 5%. Youngest children (0-2 yr), however, had a low frequency (3%) of early seizures, but 11% frequency of late seizures. The frequency of early seizure

Traumatic rupture of the diaphragm. Report of eleven cases.

Rupture of the diaphragm is an unusual injury following blunt abdominal trauma. Due to its rather discrete initial symptoms, the diaphragmatic rupture is frequently overlooked in the acute stage. Of 11 patients operated on, only four ruptures were detected in association with the accident. The clinical picture was dominated by the patient's other injuries. Symptoms referable to the diaphragmatic rupture were pain in the upper abdomen and dyspnoea, especially in the recumbent position. The diagnosis was usually established by serial chest roentgenography, in a few cases supplemented by barium c

Scales of gravity in head injury.

From the head injury onward the victim loses control over his own existence and starts a long journey through the various fields of forces determining his destiny until he can reassume control. Scales of gravity applied at different points in time vary and sometimes contradict each other. The vital scale is an immediate emergency scale, determined by the need to save a life, observe and treat a coma, and detect early complications, and the criteria are those of intensive care, surgery, and neurosurgery. The neurological and neuropsychological scale established towards the end of the first hosp

Acute paraplegia caused by a spontaneous extradural heamatoma of the conus medullaris area.

Four patients with a typical clinical picture of spinal extradural heamatoma of the conus area are presented. Initial symptoms were acute low back pains. The development of symmetrical paraparesis took place in a matter of hours, and spread to total paraplegia. The sensory level was as high as the groins and bladder and rectum paralysis developed early. All patients were over 63, mean age 67 years. Two patients had coagulation defects, one was on anticoagulants, and the other had a severe thrombocytosis. Two patients had used salicylates for rheumatic pains; in one of those patients there was

Fat embolism in severely injured patients.

Forty-five patients with severe blunt injuries were examined during eight posttraumatic days. At random, 14 patients were given three doses of methylprednisolone intravenously; 10 mg/kg at 8-hour intervals. Fat embolism syndrome was diagnosed in 13/45 patients, only one of whom had received corticosteroid (p = 0.03). Shock, acidosis and elevated plasma catecholamines showed no correlation with the occurrence of fat embolism syndrome. Platelet counts immediately after trauma were significantly lower in the fat embolism patients than in the other trauma patients, indicative of early platelet agg

A series of tibial fractures treated conservatively.

Two hundred and two fractures of the tibia have been reviewed. They were treated by a standard, conservative method; with a Phemister graft when union was delayed and a pseudarthrosis threatened. The classifications of Charnley (1968) and Nicoll (1964) have proved to be useful in assessing results and these complement each other. The factors that influence bony union include damage to the interosseous membrane, the degree of the displacement and comminution of the fracture, and injury to soft tissues. Infection has a very unfavourable influence on bone union and leads to the majority of bad se

[Hazards of heart catheterization and angiocardiography in the first year of life. A review of 1366 examinations].

1366 infants in the first year of life had cardiac catheterisation with or without angiocardiography in our Department of Cardiology between 1963 and 1973. The overally mortality in the first 24 hrs after the cardiac catheterisation for the first year of life was 1.83%. The subdivision of the first month of life reveals the high mortality rate of the first two weeks of life with the highest percentage of 18.9% in the first week of life and with 10.7% in the second week of life. After that time the mortality is significantly decreasing, but still amounts to 7.6% in the first month of life. Ther

Clinical course and prognosis of acute post-traumatic coma.

The clinical course and prognosis of 282 cases of acute post-traumatic coma have been studied. Death occurred in 140 cases (49 percent), and, of survivors, about two-thirds achieved complete social reintegration, while one-third were partially reintegrated or not at all. The quality of survival did not depend only on the nature of the physical sequelae, but also on other factors, particularly social ones. With regard to early prognosis, the authors believe it is possible to predict the final outcome of the patient by using the indices of age, level of coma, and the nature of the intracranial l

Disruption of the renal pelvis by blunt trauma.

Blunt trauma to the renal pelvis is exceedingly rare and such trauma of the ureter is only slightly more common. A tetrad of radiologic findings should suggest the diagnosis when there is a high index of suspicion. It would seem that early surgical treatment would be the preferred program but in the face of serious associated trauma there is little lost by waiting. Nineteen days elapsed between injury and repair in this case. That duration may be excessive, but as shown in this case the result was excellent and the patient's retroperitoneum was never infected. We do not advocate a purposeful d

Serum immunoglobulins after surgical operation.

We measured immunoglobulins in the sera of 33 patients on days 1, 3, 6, 10, and 17 and three to four weeks after surgical operations (mostly hysterectomy or appendectomy) or (six patients) after spinal injury. In the absence of infection or blood transfusion, IgG usually decreased slightly and transiently after hysterectomy or appendectomy, as did IgA or IgM after hysterectomy. IgD concentrations showed no consistent changes, but in one patient after hysterectomy and with minimal infection IgD concentration decreased sharply, which contrasted with significant and early increases in IgG, IgA, a

The changing pattern of post-traumatic respiratory distress syndrome.

During a one year period, 78 patients at the Denver General Hospital required mechanical ventilation following injury. Thirteen patients were judged to have Respiratory Distress Syndrome. Of these, 9 had classic early onset RDS but, with intravenous fluid restriction following resuscitation, diuretics and careful mechanical ventilation, all recovered. Six patients, all of whom were septic, developed late onset RDS 5 or more days after injury; 5 died. Disparity between early and late onset of RDS is emphasized; the one with good, the other with dismal prognosis. The current need is to improve t

[Osteosynthesis with bone screws in closed oblique and screw-like fractures of the long tubular bones].

295 patients subjected to screw osteosynthesis for closed oblique and screw-shape fractures of crural, brachial and antebrachial bones were under observation. The use of screws of a cylindrical form with a somewhat conus-like head and deep slit seems to be most rational for osteosynthesis. To apply screws properly two drills should be available. One should be equal to the diameter of a screw, the other-0.5 mm less. Following screw ostenosynthesis an adequate plaster dressing is applied for 2-3 months. The results of the treatment were good in 280 patients (95%) and poor-in 15 patients (5%). Th

Silver sulfadiazine for control of burn wound infections.

Silver sulfadiazine (Silvadene, U.S.; Flamazine, U.K.; Flammazine, N.; Sulplata, S.A.) is the newest topical antimicrobial agent available following worldwide clinical trials. Good control of infection is achieved without pain or other demonstrable side effects, using either dressings or the exposure technic. Many burned areas kept free of infection heal without grafting. Where necessary, early preparation for and good take of grafts has been attained by utilizing this new therapeutic agent. Markedly reduced mortality from burn wound sepsis has generally been observed.

Progressive pulmonary insufficiency and other pulmonary complications of thermal injury.

Progressive pulmonary insufficiency appears to be a universal response to the lung to a variety of injuries which damage the pulmonary-capillary emdothelium. Persistent hyperventilation, unresponsive to the administration of oxygen, is the earliest clinical sign of this complication of trauma and should prompt close monitoring of pulmonary function (measurement of arterial blood gas and pH levels, Vd/Vt A-aDo2, minute ventilation, vital capacity and inspiratory force) to assess the severity of the disease, the need for mechanical ventilatory support and the effectiveness of treatment. Other pu

Acute duodenitis and duodenal ulceration after burns. Clinical and pathological characteristics.

The clinical and pathological characteristics of Curling ulcer were defined by early and serial endoscopic examination of the duodenum in 37 burned patients. Duodenal disease was present in 27 patients and occurred only in patients with burns involving more than 38 percent of the total body surface. Erosive "duodenitis" could occur within 12 hours after injury and was usually associated with acute gastric disease. Isolated duodenitis occurred only in patients with pancreatitis. Contrast roentgenograms did not reliably show the superficial mucosal disease. Duodenal ulcerations were present in 1

Advances in management of splenic injuries.

Because there is no such thing as "minor splenic injury", the diagnosis of splenic rupture is a major problem after abdominal trauma. Since it is our policy to explore all penetrating abdominal injuries, the problem of early recognition arises in patients with blunt trauma. When abdominal evaluation is difficult because of associated injuries, we increasingly use peritoneal dialysis. This has been particularly helpful in patients with head injury or drug intoxication and has also contributed to earlier operation in patients with signs of hypovolemia but minimal abdominal findings. Monteggia lesions and their complicating nerve damage.

Monteggia lesions and their equivalents are reviewed on the basis of the literature and 14 cases encountered over a 10-year period. Mention is made of aetiology, classification into types and treatment. For the 14 cases in this study the mean follow-up time was 6 years. Likewise, mention is made of the relationship between anatomy and the neuropathy which is a frequent complication in Monteggia lesions. In particular the ramus profundus of the radial nerve is exposed to injury but other nerves may also be involved. It is concluded that Monteggia lesions and their equivalents are relatively rar

Traumatic left ventricular aneurysm. Two cases with normal coronary angiograms.

Two young males had traumatic left ventricular aneurysm and normal coronary angiograms. Trauma was penetrating (bullet) in one and nonpenetrating (fist blow) in the other. Recurrent ventricular tachycardia in one and a systemic embolus in the other required aneurysmectomy. The patients are free of symptoms 42 and 21 months after surgery. The presence of traumatic ventricular aneurysm and normal coronary angiograms suggests that myocardial necrosis following chest trauma is often caused by direct injury to the myocardium rather than by injury to occlusion of the coronary arteries. Therefore, co
