The value of a preoperative physical therapy and home evaluation program in total joint arthroplasty.

PURPOSE: The implementation of prehabilitation prior to total joint arthroplasty (TJA) has been recognized to potentially decrease pain, reduce length of stay (LOS), and increase patient satisfaction. With longer wait times for surgery due to the COVID-19 pandemic contributing to deterioration of function, this study aims to assess the benefits of a physical therapy (PT) and home evaluation program to improve outcomes. METHODS: A retrospective chart review of 130 primary THA and 124 primary TKA patients undergoing a new pre-operative PT and home safety evaluation program was performed to asse

Modified lever extraction technique for management of a rare femoral prosthesis fracture at the shoulder-neck junction: A case report.

Fractures involving the shoulder-neck junction of the femoral component in total hip arthroplasty are extremely rare with only 25 reported cases identified in existing literature by Van Doesburg et al. Extraction of a well-fixed fractured femoral component, particularly when cemented, is a technically demanding and time-intensive procedure that carries a considerable risk of damage to the host bone. Despite the availability of various described techniques, removal of a broken femoral stem while preserving host bone integrity remains one of the most challenging aspects of revision hip arthropla

Intraoperative Fat Embolism Syndrome Causing Acute Right Ventricular Failure Successfully Treated With Veno-Arterial Extracorporeal Membrane Oxygenation: A Case Report.

Fat embolism syndrome (FES) is a rare but potentially fatal complication of orthopedic surgery. Although extracorporeal support has been reported in trauma-related FES, the use of veno-arterial extracorporeal membrane oxygenation (V-A ECMO) for FES during elective arthroplasty has not been described. We report a case of a 67-year-old woman undergoing revision total hip arthroplasty. During the procedure, she developed profound hypoxemia and refractory hypotension following an iatrogenic femoral fracture prior to cement fixation. Despite fluid resuscitation and vasopressor and inotropic therapy

Surgical Repair of Morgagni Hernia With Liver Herniation and Rib Fractures in an Adult Following Rhinovirus-induced Coughing.

Morgagni hernias (MH) are rare congenital diaphragmatic defects that often remain undiagnosed until adulthood due to their subtle or nonspecific clinical presentation. While asymptomatic in many cases, MHs may occasionally lead to visceral herniation; among these, atraumatic liver herniation, especially in the absence of prior thoracoabdominal trauma, is exceptionally uncommon and represents a notable diagnostic and clinical challenge. We report the case of a 62-year-old male patient with no history of trauma who presented with right-sided pleuritic chest pain following a rhinovirus infection.

The Use of MRI With Double-Dose Eovist for the Detection of Bile Leaks.

Background and objectives Bile leaks are serious complications that can occur after liver transplant, hepatobiliary surgery, and abdominal trauma and can be challenging to diagnose. Noninvasive diagnostic studies for bile leaks are limited. In this study, we aimed to evaluate the efficacy of MRI with a double-dose of Eovist (gadoxetate disodium) in comparison with other imaging modalities for detecting biliary leaks. Methods This study involved a retrospective review of patients who underwent MRI with double-dose Eovist (dEovist) to evaluate suspected bile leakage from 2016 to 2022. Indication

Retrospective Review of Self-Adhesive Elastic Wrapping in Patients With Facial Burns.

The management of the burn-injured face poses unique challenges for therapy intervention. Standard compression techniques require an intimate fit and are traditionally implemented after wound closure when the patient has minimal or no dressings. Our center developed a method of self-adhesive facial wrapping that can be initiated early in care to provide custom contoured compression, decrease edema, and help maintain definition of facial features. The study aimed to characterize our experience with self-adhesive wrap in adult and pediatric patients with facial burn injuries. We conducted a retr

Short-Interval Two-Stage Treatment of a Comminuted Open Tibial Diaphyseal Fracture.

Open comminuted tibial fractures caused by gunshot injuries remain challenging due to the high risk of infection, bone loss, and prolonged functional impairment. We report the case of a young male patient who sustained a severe open comminuted tibial diaphyseal fracture with a large segmental bone defect following a gunshot injury. The patient was managed using a short-interval two-stage approach, consisting of early distraction osteogenesis with an external fixator followed by timely conversion to intramedullary fixation combined with autologous bone grafting. This strategy allowed effective

Penetrating Neck Injury With Common Carotid Artery Involvement: A Case Report.

Penetrating neck injuries often involve vascular injuries, including the carotid arteries, posing significant morbidity and mortality risks. This report presents a case of a 23-year-old male who sustained a penetrating neck injury from a nail gun, specifically affecting the common carotid artery (CCA). Upon arrival at the emergency department, the patient exhibited hemodynamic stability, with no active bleeding or neurological deficits. Initial imaging, including CT angiography (CTA) of the neck, revealed a metallic foreign body near the CCA and air foci suggestive of a muscle tear. Following

Neonatal Glanzmann's Thrombasthenia Presenting as Refractory Post-circumcision Hemorrhage in a Region of High Consanguinity: A Case Report.

Glanzmann's thrombasthenia (GT) is a rare autosomal recessive disorder characterized by deficient or dysfunctional platelet glycoprotein IIb/IIIa (αIIbβ3 integrin), the principal fibrinogen receptor. In neonates, GT is often clinically silent until unmasked by a hemostatic challenge such as surgery or trauma. Standard coagulation screens are normal, making timely diagnosis difficult and potentially leading to life-threatening complications. We report the case of a seven-day-old Saudi boy from Najran who developed severe, refractory hemorrhage following circumcision. Initial surgical hemostasis

Accidental Hypothermia in Hospitalized Adults: Risk Factors, Management Strategies, and Clinical Outcomes: A Systematic Review.

Accidental hypothermia (AH) in hospitalized adults is an under-recognized condition associated with increased morbidity, mortality, and prolonged hospital stays. Unlike primary AH caused by environmental exposure, inpatient hypothermia is often secondary to acute illness, comorbidities, iatrogenic factors, or medications that impair thermoregulation. We systematically reviewed studies reporting risk factors, management strategies, and clinical outcomes of AH in adults (≥18 years). Five studies, including 1,698 patients, met the inclusion criteria. Common risk factors were advanced age, male se

Conservative Management of Negative Pressure Pulmonary Edema: A Case Report.

Negative pressure pulmonary edema (NPPE) represents a less frequently encountered, albeit critical, etiology of non-cardiogenic pulmonary edema, manifesting subsequent to acute upper airway obstruction. It is most often seen in young, healthy adults with strong inspiratory effort, usually triggered by post-extubation laryngospasm. Although the presentation can be dramatic, most patients recover completely when the condition is recognized and managed early. A young adult man underwent a laparoscopic appendectomy under general anesthesia. During emergence, he developed acute laryngospasm while b

Whole blood resuscitation and post-traumatic organ dysfunction in injured children.

BACKGROUND: Both whole blood (WB) and component therapy (CT) are used for hemostatic resuscitation in injured children. We hypothesize that early WB transfusion compared to CT alone is associated with decreased post-traumatic organ dysfunction. STUDY DESIGN AND METHODS: This single-center observational study included children ages 0-17 years between January 2021 and March 2024 with trauma mechanism and intensive care unit admission. The primary outcome was Pediatric Logistic Organ Dysfunction 2 (PELOD-2) score on post-trauma days 1-7. Data were analyzed using linear regression adjusting for a

Isolated Left Peroneal Neuropathy Following Total Laparoscopic Hysterectomy: A Case Report.

Neuropathy of the lower extremities is a rare surgical complication, usually caused by compression, stretching, or direct injury to the nerve during the procedure. Although such injuries have been documented in relation to laparotomies, there are few reports concerning minimally invasive procedures. Here, we present the case of a 46-year-old woman with a long-standing history of abnormal uterine bleeding. Imaging studies revealed large uterine myomatosis, indicating the need for a total laparoscopic hysterectomy. During the immediate postoperative period, the patient developed a steppage gait

Development and Validation of a Predictive Model for Surgical Site Infection in Open Hand Injuries.

PURPOSE: Surgical site infection (SSI) is a major complication in patients with open hand injuries. However, current clinical risk assessment largely relies on subjective judgment or traditional scoring systems, which often lack predictive precision and generalizability. This study aimed to develop, compare, and externally validate multiple machine learning (ML) models for predicting SSI in open hand injuries using routinely collected clinical indicators. METHODS: A total of 800 patients with open hand injuries were retrospectively enrolled. The primary cohort (n=500) was randomly divided int
