Lingual Tonsillectomy for Resistant Obstructive Sleep Apnea in Children with Down Syndrome: A Systematic Review and Meta-Analysis.

INTRODUCTION: Obstructive sleep apnea (OSA) affects many children, particularly those with Down syndrome (DS), with prevalence estimates ranging from 50% to 100%. Although adenotonsillectomy is the traditional first-line treatment, individuals with DS frequently have residual OSA (rOSA) because to their complex airway architecture. Lingual tonsil hypertrophy has been found as a significant risk factor for rOSA; however, the role of lingual tonsillectomy (LT) in this population is uncertain. METHODS: We conducted a systematic review and meta-analysis according to PRISMA and Cochrane guidelines

18F-FAPI-74 PET/CT findings of an inflammatory branchial cleft cyst mimicking cervical metastasis from carcinoma of unknown primary: A case report.

Fibroblast-activation protein inhibitor (FAPI) positron-emission tomography (PET) is emerging as a promising alternative to 18F-fluorodeoxyglucose (FDG) PET for head-and-neck cancer. Herein, we describe the case of a 51-year-old man with a cystic level-II neck mass that was intensely FDG-avid and even more avid on 18F-FAPI-74 PET. FDG-PET was performed as standard imaging modality to search for the primary tumor, while 18F-FAPI-74 PET was conducted as part of a clinical trial to detect potential occult lesions missed by FDG. While cytology suggested metastatic squamous-cell carcinoma and FDG P

Vallecular Large B-Cell Lymphoma: An Uncommon Presentation of Follicular-Origin Disease.

The vallecula is a mucosal recess of the hypopharynx situated between the base of the tongue and the lingual surface of the epiglottis and bordered laterally by the glossoepiglottic folds. Lesions in this region are uncommon and encompass a broad differential diagnosis, including vallecular cysts, lingual tonsil hypertrophy, minor salivary gland tumors, squamous cell carcinoma, and, rarely, lymphoproliferative disease. Primary extranodal non-Hodgkin lymphoma of the vallecula is exceptionally rare, and follicular large B-cell lymphoma (FLBCL) has not previously been reported at this site. We pr

Transient radiation-induced severe obstructive sleep apnea.

UNLABELLED: Radiotherapy for head and neck cancer can trigger or worsen obstructive sleep apnea (OSA) due to factors such as neurological impairment, muscle atrophy, edema, and xerostomia. We present a case in which a patient developed severe OSA 3 months after undergoing radiochemotherapy and neck dissection for squamous cell carcinoma of the right lingual tonsil with regional metastasis. Polysomnography confirmed severe OSA. Unfortunately, the patient did not adhere to the recommended treatment. The symptoms persisted for 6 months but then, unexpectedly, resolved completely. A follow-up poly

Kimura disease of the tongue base: a rare case diagnosed through cytological examination of Warthin-Finkeldey-type multinucleated cells.

Kimura disease (KD) is a rare chronic inflammatory condition that primarily affects Asian males and typically presents in the head and neck region. We describe an exceptionally rare case of KD involving the lingual tonsil of Waldeyer's ring in a 39-year-old Japanese man, marking only the second reported instance of lingual involvement and the first specifically affecting the tongue base. The patient presented with a well-circumscribed, 3.5-cm mass extending from the lingual tonsil to the epiglottis. Laboratory findings revealed significant peripheral eosinophilia (13.5%) and elevated serum IgE

Patterns of referral and outcomes in oropharyngeal cancer management.

INTRODUCTION: Cancer care is increasingly centralized, with patients referred to specialized centers. While regionalization may improve outcomes, it can introduce barriers such as reduced access to specialized care. As surgery plays a growing role in oropharyngeal cancer treatment, understanding referral patterns and their impact on treatment modality and outcomes is essential. METHODS: We conducted a retrospective cohort study using the National Cancer Database to identify patients treated for oropharyngeal squamous cell carcinoma between 2010 and 2017. The primary outcome was referred care,

Lingual Tonsil Hypertrophy in OSA: The Role of Prior Tonsillectomy and Associated Clinical Risk Factors.

BACKGROUND: Lingual tonsil hypertrophy (LTH) is increasingly recognized as a contributor to upper airway obstruction in obstructive sleep apnea (OSA). While several etiological factors have been proposed, the potential role of prior palatine tonsillectomy remains understudied in adult OSA populations. OBJECTIVE: To evaluate whether a history of tonsillectomy is independently associated with increased severity of LTH in adults with OSA, and to explore additional anatomical and clinical contributors. METHODS: A cross-sectional observational study was conducted including 117 adult OSA patients

Sociodemographic disparities among Floridians diagnosed with oropharyngeal cancer.

BACKGROUND: Oropharyngeal cancer (OPC) involves the base of tongue, palatine tonsils, lingual tonsil, and soft palate. Established risk factors for OPC include tobacco usage, alcohol usage, and human papillomavirus infection. Although white populations tend to have the highest risk of developing OPC, Black patients are more likely to experience distant stage disease. METHODS: The aim of this study was to elucidate epidemiological factors that are associated with regional and distant stage disease at OPC diagnosis. We performed a retrospective cross-sectional analysis utilizing the Florida Can

Efficacy of Lateral Neck Plain Radiographs in Suspecting an Obstruction at the Base of the Tongue in Children With Sleep Disordered Breathing.

Background Obstructed breathing in children often requires adenotonsillar surgery as the primary treatment. Persistent symptoms may necessitate further diagnostic investigations and surgeries. Base of tongue (BOT) enlargement or lingual tonsillar hypertrophy (LTH), a common source of obstruction in these cases, is detected during flexible endoscopy, usually under sedation in the operating theatre. The role of lateral neck plain radiographs (LNPR) in this context is unknown.  Objectives This study aims to assess the effectiveness of LNPR in diagnosing or suspecting obstruction at the level of B

The tongue of the red-eared slider (Trachemys scripta elegans): morphological characterization through gross, light, scanning electron, and immunofluorescence microscopic examination.

The red-eared slider (Trachemys scripta elegans) is renowned for its remarkable adaptations, yet much of its complex biology remains unknown. In this pioneering study, we utilized a combination of gross anatomy, scanning electron microscopy (SEM), light microscopy, and immunofluorescence techniques to examine the tongue's omnivorous adaptation in this species. This research bridges a critical knowledge gap, enhancing our understanding of this intriguing reptile. Gross examination revealed a unique arrowhead-shaped tongue with a median lingual fissure and puzzle-piece-shaped tongue papillae. SE

Surgical Management of Pediatric Obstructive Sleep Apnea Beyond Tonsillectomy & Adenoidectomy: Tongue Base and Larynx.

Pediatric patients with persistent obstructive sleep apnea (OSA) after adenotonsillectomy often have additional sites of upper airway obstruction such as the tongue base or larynx. Sleep endoscopy and cross-sectional, dynamic imaging can be used to direct surgical management of persistent OSA. The tongue base is one of the most common sites of obstruction in children with persistent OSA, especially for patients with Trisomy 21. Lingual tonsillectomy, tongue suspension, and/or posterior midline glossectomy may be used to address lingual tonsil hypertrophy and tongue base obstruction. Epiglottop
