Variety and complexity of fluorine-18-labelled fluoro-2-deoxy-D-glucose accumulations in the oral cavity of patients with oral cancers.

OBJECTIVES: To elucidate the points that require attention when interpreting fluorine-18-labelled fluoro-2-deoxy-d-glucose ((18)F-FDG)/positron emission tomography (PET) images by demonstration of (18)F-FDG accumulation in various areas of the oral cavity other than primary lesions in patients with oral cancers. METHODS: (18)F-FDG accumulations with a maximal standardized uptake value of over 2.5 in various areas of the oral cavity other than primary lesions were identified in 82 patients with oral cancers. RESULTS: (18)F-FDG/PET-positive areas, excluding primary tumours, included the front

Adenotonsillectomy in obese children with obstructive sleep apnea syndrome: magnetic resonance imaging findings and considerations.

OBJECTIVE: The reasons why adenotonsillectomy (AT) is less effective treating obese children with obstructive sleep apnea syndrome (OSAS) are not understood. Thus, the aim of the study was to evaluate how anatomical factors contributing to airway obstruction are affected by AT in these children. METHODS: Twenty-seven obese children with OSAS (age 13.0 ± 2.3 y, body mass index Z-score 2.5 ± 0.3) underwent polysomnography and magnetic resonance imaging of the head during wakefulness before and after AT. Volumetric analysis of the upper airway and surrounding tissues was performed using commerci

A role for Waldeyer's ring in immunological response to allergens.

OBJECTIVE: Adenoids, tubal tonsil, palatine tonsil, and lingual tonsil are immunological organs included in the Waldeyer's ring, the basic function of which is the antibody production to common environmental antigens. Adenoidal hypertrophy (AH) is a major medical issue in children, and adenoidectomy is still the most used treatment worldwide. The response of adenoids to allergens is a good model to evaluate their immunological function. This report assessed the immunological changes in adenoid tissues from children with allergic rhinitis (AR) undergoing sublingual immunotherapy (SLIT). METHOD

A case-control comparison of lingual tonsillar size in children with and without Down syndrome.

OBJECTIVES/HYPOTHESIS: Lingual tonsillar hypertrophy is a common cause of persistent obstructive sleep apnea following adenotonsillectomy in the pediatric population and may be more prevalent in patients with Down syndrome (DS). We sought to quantify lingual tonsil size in pediatric DS patients and compare these findings to those of nonsyndromic children using cervical spine (c-spine) radiographs. STUDY DESIGN: Case control study. METHODS: Retrospective review of c-spine radiographs from 105 pediatric DS patients and 89 age- and gender-matched non-DS controls was conducted. Films performed f

Pediatric lingual tonsillectomy.

Tonsillectomy and adenoidectomy are an effective surgical treatment of pediatric obstructive sleep apnea; however, up to 20% of these patients can have persistent disease. In this select patient population, the lingual tonsil may be an occult source of obstruction. Lingual tonsillectomy can be a challenging procedure due to poor access and visualization, airway edema, postoperative pain and hemostasis during tissue removal. In this chapter, we describe our preferred technique for lingual tonsillectomy including surgical pearls for success.

Complicated airway due to unexpected lingual tonsil hypertrophy.

We report an unexpected failed laryngeal mask airway in a patient with unrecognized lingual tonsil hypertrophy (LTH). A 19-year-old obese woman presented for extraction of multiple teeth via intravenous general anesthesia. Surgery was interrupted due to a laryngospasm midway through the procedure. The laryngospasm required the existing laryngeal mask airway to be removed so the patient could be suctioned. Although it is unclear the extent of obstruction caused by LTH, the surgery had to be postponed due to the discovery of enlarged lingual tonsils, which prevented endotracheal intubation. One

Non-Hodgkin lymphomas of Waldeyer's ring.

Lymphomas represent malignant lymphoproliferative diseases and they are generally classified as Hodgkin's (HL) or non-Hodgkin malignant lymphomas (NHML). Head and neck lymphomas represent one of the most common sites of extranodal lymphomas, second after the gastrointestinal tract. Waldeyer's ring structures include the palatine tonsils, the nasopharyngeal lymphatic tissue, and the lingual tonsil. We investigated 38 patients with malignant lymphoma with ages ranging from 21 to 95 years, all localized in the Waldeyer's ring. Good knowledge of the clinical characteristics of these lymphomas and

Difficult airway in a pediatric patient with Klippel-Feil syndrome and an unexpected lingual tonsil.

Klippel-Feil Syndrome (KFS) is a congenital defect characterized by the fusion of at least two cervical vertebrae. This article presents the case of a 12-year-old girl with KFS planned for scoliosis surgery. A short, rigid neck and limited mouth opening were predictors of difficult airway access. After failing multiple intubation attempts employing several devices, the case was postponed. Imaging tests showed lingual tonsil hypertrophy and a deviation of the tracheal axis, clarifying the causes of the failed intubation and suggesting new approaches for airway management. As far as we know, the

Systematic review of endoscopic airway findings in children with gastroesophageal reflux disease.

OBJECTIVES: We performed a systematic review of published literature correlating findings on endoscopic evaluation of the larynx and trachea in the pediatric population with the incidence of gastroesophageal reflux disease. METHODS: Eight articles were identified through a structured PubMed search of English-language literature using the key terms laryngopharyngeal reflux, extraesophageal reflux, and gastroesophageal reflux. A systematic review was performed relating the presence of reflux in the pediatric population to findings on endoscopic airway evaluation. A covariant analysis was perfor

Massive lymphoid hyperplasia presenting with obstructive sleep apnea secondary to lingual and palatine tonsil hyperplasia.

Lingual tonsil hyperplasia is a rare condition that may cause obstructive sleep apnea (OSA). In the management of OSA, the lingual tonsils should be evaluated during the otorhinolaryngologic examination. We report the case of a 66-year-old man with findings of upper airway obstruction secondary to excessive lingual and palatine tonsil hyperplasia and with MRI findings of bilateral cervical lymphadenopathy. We review the clinical, radiologic, and histopathologic aspects of this case, and we discuss the surgical options for treating massive reactive lymphoid hyperplasia in conjunction with OSA.

Distribution of the lingual lymphoid tissue in domestic ruminants.

The distribution and organisation of the intralingual lymphoid tissue was studied in sheep, goat and cattle. For each species, the tongues of two animals were harvested and divided in sample blocks extending over the total surface of the tongue. With 2.5 mm intervals, ten serial histological sections were made for conventional histological staining (haematoxylin-eosin, Van Gieson, Masson's trichrome) and immunohistochemical staining of lymphoid cells (anti-CD3, anti-CD21, anti-CD45). Lymphocytes were scattered in the subepithelial propria-submucosa and in the connective tissue cores of the lin

Excessive reactive lymphoid hyperplasia in a child with persistent obstructive sleep apnea despite previous tonsillectomy and adenoidectomy.

Both lingual tonsil and adenoid are the lymphoid tissue members of Waldeyer ring. Enlargement of the lingual tonsil and adenoid occurs in children with persistent obstructive sleep apnea despite previous tonsillectomy and adenoidectomy relatively frequently. In this article, we present a case with upper airway obstruction and infection findings because of excessive lingual tonsil and adenoid tissue hyperplasia and, additionally, bilateral multiple cervical lymphadenopathies. The patient had had tonsillectomy and adenoidectomy 8 years ago. Etiopathogenetic mechanisms, clinical, radiologic, histo
