Blood transfusion safety depends on swift blood compatibility testing. Alloantibodies recognizing these blood group antigens determine blood compatibility, and the transfusion of incompatible blood can lead to life-threatening hemolytic transfusion reactions. Recently, the landscape of blood compatibility testing has been complicated due to the interference caused by therapeutic antibodies targeting CD38, a key target in cancer immunotherapy that is also expressed on red blood cells. The presence of anti-CD38 antibodies in blood samples has been found to bind to test or donor red blood cells,

BACKGROUND: The legal shelf life of platelet concentrates (PCs) has been extended from 5 to 7 days in France since 2018. Our study aimed to determine whether this increase in PC storage time has produced a significant impact on transfusion-related adverse reactions (ARs).
METHODS: A retrospective study was conducted in the Grand Est region of France between January 2022 and December 2023 using data from the e-FIT hemovigilance database and the record of PCs transfused held by the "Etablissement Français du Sang" (EFS).
RESULTS: In 2022 and 2023, 270 ARs related to platelet transfusions were

Background Pre-transfusion testing is essential for preventing incompatible red cell transfusions and associated adverse reactions. Despite modern serological advancements, unexpected alloantibodies, autoantibodies, and monoclonal antibody interference continue to contribute to incompatible crossmatches. This study evaluates the serological patterns, clinical associations, and diagnostic considerations of incompatible crossmatches encountered in routine transfusion practice. Methods This cross-sectional study was conducted at a tertiary-care transfusion service of Swami Rama Himalayan Universi

OBJECTIVES: It is not always possible to access ABO-compatible platelet concentrates (PCs) for reasons such as short shelf life and not always being readily available. Therefore, blood centers have to develop strategies for the use of ABO-incompatible PCs. However, there are conflicting data regarding the safety of using ABO-incompatible platelet transfusions, especially concerning transfusion reactions.
METHODS: This multicenter, retrospective cohort study analyzed platelet transfusion records from 3 tertiary-level hospitals over a 9-year period (2016-2024). Data were extracted from institut

BACKGROUND: The Bombay (Oh) phenotype results from homozygous FUT1 loss-of-function (hh), causing complete absence of H antigen on red blood cells and preventing expression of A and B antigens. Because routine forward and reverse grouping can mimic group O, affected individuals may be transfused with standard group O units, risking severe hemolysis due to naturally occurring anti-H.
CASE: A 31-year-old man in Baghdad presented with massive traumatic hemorrhage and received an uncrossmatched leukoreduced group O RhD-negative packed red cell unit. Within 10 min he developed fever, rigors, hypot

Red blood cell (RBC) alloimmunization continues to challenge the foundational promise of transfusion therapy: to provide safe and universally effective support. Although most transfused individuals remain tolerant, a vulnerable minority develop alloantibodies that complicate care and increase the risk of delayed hemolytic transfusion reactions (DHTRs). While high-income nations increasingly mitigate this risk through molecular genotyping, low- and middle-income countries (LMICs) face a distinct landscape defined by antigenic mismatch and resource scarcity. This critical review evaluates alloim

BACKGROUND: The Rh blood group system, of which RHD is the most notable, is one of the most clinically important red blood cell antigen groups next to ABO.
STUDY DESIGN AND METHOD: In this case report, we describe a severe delayed hemolytic transfusion reaction caused by allo-anti-D in a patient with Sickle Beta Zero Thalassemia initially typed O RHD positive via standard serologic type and screen.
RESULTS: Further serologic testing revealed the development of an allo-anti-D, suggestive of a partial D phenotype. The patient was confirmed to carry the DAU3 allele using confirmatory molecular

Red blood cell alloimmunity is an immune response triggered by individuals who contact with red blood cells different from their own blood type antigens during blood transfusion or pregnancy, which can cause cross-matching incompatibility, hemolytic transfusion reaction and hemolytic disease of newborns. Human leukocyte antigen (HLA) gene polymorphism plays an important role in the alloimmune response induced by the blood type antigen on erythrocytes. This review introduces the research status and progress of the genetic background of HLA gene polymorphism and multiple erythrocytic blood type

BACKGROUND: Interpretation of antibody identification panels remains a manual, expertise-intensive step in transfusion testing; interpretive errors and long turnaround times persist even in well-equipped hospitals. Existing commercial software is closed source and often omits enzyme-phase, cold-phase, and dosage-sensitive exclusion rules, limiting third-party verification and local adaptation.
STUDY DESIGN AND METHODS: We developed IrregulAB, an open-source (LGPL) web application that encodes serologic decision rules in two stages. First, a rule-based screening stage applies LISS, enzyme, and

Testing for ABO incompatibility between a potential blood donor and recipient is the cornerstone of pretransfusion testing. Occasionally, weak agglutination reactions occur due to low expression of A and B antigens on the surface of red blood cells. This can lead to inconsistencies in blood group typing, as seen in a case where a patient's forward and reverse typing showed discrepancies. Following a comprehensive analytical assessment, the patient was identified as having a distinctive variant of blood group A, belonging to the less common subgroups-A3, Aend, Ax, Am, Ay, and Ael. Herein, we pr

BACKGROUND: RhD mismatched transfusions in RhD negative women have the potential to impact future pregnancies through alloimmunization and development of hemolytic disease of the fetus and newborn (HDFN). As use of RhD positive emergency-release blood products in pediatric trauma has increased, it has become clear that a significant alloimmunization risk also exists for RhD negative girls, which has not been addressed by existing literature. This article describes a best-practice guideline to manage RhD negative pediatric females who receive RhD positive transfusions.

Massive transfusion protocols are essential in trauma resuscitation but may inadvertently increase the risk of acute hemolytic transfusion reactions (AHTRs), which are typically caused by ABO incompatibility. Recognition is difficult because signs of hemolysis can mimic trauma-related hemorrhage or coagulopathy. Cognitive biases in emergency decision-making may further delay diagnosis. Although ABO-incompatible AHTR remains exceptionally rare in trauma, it is easily overlooked in the context of polytrauma. We describe three cases of ABO-incompatible AHTR admitted to a tertiary trauma center. F

A better understanding of factors influencing red blood cell (RBC) alloimmunization is desirable, given the dangers of this non-infectious serious hazard of transfusion. In a case-control retrospective study utilizing the Recipient Epidemiology and Donor Evaluation study (REDS)-III Biological Specimen and Data Repository Information Coordinating Center (BioLINCC) dataset, we investigated blood donor, component and recipient variables associated with RBC alloantibody specificities. 19 697 controls with no new alloantibody detected 2-16 weeks post-transfusion and 861 cases (with antibodies inclu

BACKGROUND: Prehospital (ambulance) care can reduce morbidity and mortality from trauma. Yet, there is a dearth of effective evidence-based interventions and implementation strategies. Emergency Medical Services Traumatic Shock Care (EMS-TruShoC) is a novel bundle of five core evidence-based trauma care interventions. High-Efficiency EMS Training (HEET) is an innovative training and sensitization program conducted during clinical shifts in ambulances. We assess the feasibility of implementing EMS-TruShoC using the HEET strategy, and feasibility of assessing implementation and clinical outcomes

BACKGROUND: In ABO-incompatible (ABOi) liver transplantation, antibody-mediated rejection (AMR) is a challenging post-transplant complication. In Japan, rituximab was approved in 2016 to prevent AMR in ABOi kidney and liver transplantation and in 2023 to prevent and treat AMR in kidney, liver, heart, lung, pancreas, and intestine transplantation.
METHODS: We conducted an all-case postmarketing surveillance to assess the safety and effectiveness of rituximab in ABOi liver transplant recipients, with a registration period between February 29, 2016 and May 9, 2021. The safety endpoint was the oc

INTRODUCTION: Bombay (Oh) blood group individuals are typically transfused with Bombay group red blood cells because transfusion with other blood groups can lead to life-threatening hemolysis. The mutations in the FUT1 gene prevent the expression of the H antigen on red blood cells.
METHODS: A 35-year-old man presented with severe anemia (hemoglobin, 4.8 g/dL) and was transfused with 3 units of O RhD-positive packed red blood cells at an outside hospital without adverse reaction. One month later, his blood group at our center was revealed as O RhD positive with panagglutination in reverse gro

BACKGROUND: ABO-incompatible (ABOi) kidney transplantation alleviates the shortage of donor kidneys. However, interventions targeting blood group antibodies alone cannot fully prevent antibody-mediated rejection. We investigated how blood group antigens affect prognosis and rejection in ABOi kidney transplantation and explored their regulatory mechanisms.
METHODS: We prospectively enrolled 28 consecutive ABOi kidney transplant recipient-donor pairs. We performed immunohistochemical staining for A/B antigens on intraoperative donor kidney biopsies. Graft A/B antigen expression levels were asse
