BACKGROUND: ABO-incompatible (ABOi) kidney transplantation alleviates the shortage of donor kidneys. However, interventions targeting blood group antibodies alone cannot fully prevent antibody-mediated rejection. We investigated how blood group antigens affect prognosis and rejection in ABOi kidney transplantation and explored their regulatory mechanisms.
METHODS: We prospectively enrolled 28 consecutive ABOi kidney transplant recipient-donor pairs. We performed immunohistochemical staining for A/B antigens on intraoperative donor kidney biopsies. Graft A/B antigen expression levels were asse

Although acute hemolytic transfusion reaction (AHTR) due to ABO-incompatible red blood cell transfusion is a rare complication in high-income countries, it is considered a medical emergency because of the severe intravascular hemolysis that can lead to major morbidity or death. AHTR can cause intravascular hemolysis, acute renal failure, disseminated intravascular coagulation (DIC), and shock, and therefore treatment should be prompted early and directed to prevent these complications. We present a case of AHTR due to ABO-incompatible red blood cell transfusion successfully treated with high-d

Alloantibodies, which can arise naturally in early life or after exposure to allogeneic red blood cells (RBCs), can pose a significant risk to blood transfusion. When patients with alloantibodies receive incompatible RBCs, they may experience potentially fatal hemolytic transfusion reactions (HTRs). Transfusion services work to prevent such reactions by identifying compatible blood; however, complications can still occur, especially in patients with complex alloantibody profiles. Given the consequences of these reactions clinically, various preclinical models have been developed to study incom

BACKGROUND: Anti-Diᵃ is a clinically significant alloantibody directed against a low-prevalence antigen in the Diego blood group system. While rare in most populations, its prevalence is notably higher among East Asian and indigenous South American groups. Detection is often challenging due to the limited presence of Dia (+) cells in routine antibody screening panels.
METHODS: We describe a case involving a 62-year-old male inpatient at Asia University Hospital who was evaluated for symptomatic anemia. Routine pretransfusion testing using an automated analyzer revealed a positive antibody scr

BACKGROUND: Reports suggest that children with mobility impairment represent a significant proportion of the population living with a disability. Footwear is considered to be the key extrinsic factor affecting children's gait and footwear modifications have been historically postulated to assist with locomotory difficulty. Although therapeutic footwear has been considered within the literature, there is a lack of consistency on terminology and paucity on the overall understanding. A scoping review was performed to chart the key concepts in children's footwear and to establish the range of stud

BACKGROUND: Prostatic male genital schistosomiasis and prostate cancer co-existence cases are uncommon however, some studies have indicated that schistosomiasis may trigger development of prostate cancer regardless of age. Schistosomiasis is a public health problem in sub-Saharan Africa and may account for some undocumented cases of schistosomiasis prostatic cancer in schistosome endemic rural communities. It is against this background that we investigated the association between schistosomiasis and risk of prostate cancer development in residents of Murehwa Community, a schistosomiasis endemi

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

BACKGROUND: Allocating incompatible platelet components to avoid product wastage must be balanced against the risk of reduced efficacy and adverse outcomes. The impact of platelet compatibility in association with product irradiation or pathogen reduction is unknown. This study aims to determine the combined and independent impact of platelet compatibility and component modification on transfusion reaction rate.
STUDY DESIGN AND METHODS: A retrospective review of all adult platelet transfusions from 2020 to 2022 was performed, including all reported reactions. Logistic regression was performe

BACKGROUND: Prior studies have evaluated transfusion recipient variables impacting red blood cell (RBC) alloimmunization, but few focused on potentially modifiable blood donor or blood component variables.
STUDY DESIGN AND METHODS: Data from the Recipient Epidemiology and Donor Evaluation Study (REDS)-III, which links donor, component, and patient data in an integrated database, were accessed. For any given RBC unit with sufficient blood donor and component data, we determined if the transfusion recipient experienced a new RBC alloimmunization event ("case") within 16 weeks of the transfusion

BACKGROUND AND OBJECTIVES: High-quality ABO antibody titre testing is required for ABO-incompatible haematopoietic stem cell transplantation and kidney transplantation. To assess the feasibility of automated ABO titration as an alternative to manual and semi-automatic titration during the peri-transplant period, a comparative study was conducted internally in a transfusion medicine laboratory.
MATERIALS AND METHODS: This study was performed in two stages. Firstly, the differences in anti-A/B detecting using conventional tube test (CTT) and manual column agglutination technology (CAT) were com

The recipient was a 50-year-old man with blood type O who was referred to our department for a pre-emptive ABO-incompatible kidney transplantation (KTx), with his wife as the donor whose blood was type A. However, both of the T and B cell complement-dependent crossmatch and flow cytometric crossmatch tests showed positive results. Even though donor-specific antibodies were negative by Luminex, we did not recommend transplantation and he had received dialysis therapy for 3 years. We conducted a re-evaluation of crossmatch tests. IgM and anti-B antibody were eliminated from the recipient's sera

BACKGROUND: Resuscitation strategies for massive hemorrhage increasingly involve the use of low-titer group O whole blood (LTOWB) due to evidence suggesting improved patient outcomes. However, the potential risk of incorrect ABO typing following LTOWB administration, possibly leading to mis-transfusion, remains insufficiently explored. This case series aims to highlight the potential risk of ABO mistyping associated with LTOWB transfusions in trauma settings.
STUDY DESIGN AND METHODS: We retrospectively reviewed three cases involving trauma patients who received LTOWB transfusions at a high-v

AIM: Although blood transfusion is a life-saving procedure, it is associated with a number of preventable and unpreventable transfusion reactions. Transfusion errors account for the majority of preventable causes. The aim of our study was to determine the frequency and characteristics of acute transfusion reactions (ATR) and preventable transfusion errors in our center.
METHODS: Data on transfusion reactions and transfusions administered between January 2019 and December 2023 were collected retrospectively from reported transfusion reaction forms and transfusion center records.
RESULTS: Amon

BACKGROUND: This systematic review aims to determine the loads of anti-A and anti-B in plasma-containing blood products that can safely be transfused to adults.
MATERIALS AND METHODS: A comprehensive literature search was performed in PubMed, including studies dating back to 1940 until January 1st, 2023. In total 1,869 articles were screened, describing 86 cases of hemolytic transfusion reactions (HTR) due to anti-A and/or anti-B in ABO incompatible products. Publications in English were included that reported the anti-A and/or anti-B IgM and/or IgG titers in the blood products, the volume of

The Rh blood group system, especially the D antigen, is crucial in transfusion medicine and obstetrics. Weak D phenotypes, caused by mutations in the Rhesus D antigen (RhD) blood group (RHD) gene, result in reduced antigen expression, posing challenges in serological testing and clinical management. Variability in detection methods leads to inconsistent results, making accurate classification difficult. Molecular techniques like polymerase chain reaction and DNA sequencing have significantly improved the identification of weak D variants, offering more reliable transfusion strategies and reduc

ABO-incompatible (ABOi) red blood cell (RBC) transfusions can lead to severe clinical consequences, including patient death. Electronic systems, such as Bedside Electronic Transfusion Checks (BETC), have been developed to lower the risk of these serious incidents occurring due to errors in patient identification at the bedside; however, the benefits for patients have not yet been fully quantified. To address this gap, we aimed to quantify the harms (ie, morbidity and mortality) associated with ABOi RBC transfusions in the UK, enabling us to better understand the benefits of BETC in preventing

BACKGROUND: Blood transfusion is a critical medical intervention, and accurate cross-matching is essential to prevent adverse reactions. However, difficult cross-matching (DCM), often caused by irregular antibodies, poses significant challenges, particularly in patients with prior transfusions, pregnancies, or certain medical conditions.
OBJECTIVE: To analyze the incidence and causes of DCM from 2017 to 2021 and propose strategies to improve transfusion safety and compatibility.
METHODS: A total of 363 patients who underwent difficult cross-matching due to incompatible blood matching between

BACKGROUND: Alpha-gal syndrome (AGS) is caused by IgE antibodies against the alpha-gal oligosaccharide, which is structurally similar to the Group B antigen. Recent case reports of severe allergic transfusion reactions (ATRs) in Group O patients receiving Group B plasma and platelets raise the possibility of a new adverse event, herein called transfusion-related AGS (TRAGS). The primary goal of this study was to assess the frequency of Groups B and AB plasma and platelet transfusions to Group O patients.

BACKGROUND AND OBJECTIVES: Many transfusion centres no longer perform an antiglobulin crossmatch on patients without clinically significant red cell alloantibodies, but instead use an abbreviated crossmatch. This policy brings many benefits but has an acknowledged risk of haemolytic transfusion reactions (HTRs). Historical estimates of reactions caused by undetected antibodies to low-incidence antigens (LIAs) were in the region of 1 reaction per 500,000 red cell transfusions. This meta-analysis of haemovigilance reports compares historical estimates with recorded events.

Patrilineality plays a significant role in determining and confirming an individual's family membership based on their father's lineage. However, despite the importance of patrilineality in some societies, male infertility-which contributes significantly to infertility and accounts for half of all cases-has emerged as a significant concern. Thus, the study explores societal reactions to male infertility in Krokobite, a suburb of Accra, which is a predominantly Ga-speaking and patrilineal society. The study adopted a qualitative approach, utilizing in-depth interviews (24), focus group discussi
