HMGB-1 Mediates The Exacerbation of Anaphylactic Shock Under Hyperlipidemic Conditions.

Coronary heart disease (CHD) may worsen anaphylactic shock, but the exact mechanism is unknown. This study aimed to investigate the mechanisms by which coronary heart disease exacerbates anaphylaxis.C57BL/6 (WT) mice and LDLR-/- mice were fed a high-fat diet for 20 weeks to develop atherosclerosis. Anaphylaxis was then induced using ovalbumin (OVA). Compared to WT mice, LDLR-/- mice showed lower body temperature, worse pulmonary edema and higher mortality. Pulmonary endothelial cell CD31 (PECAM-1) expression decreased, but serum HMGB-1 levels increased. In vitro experiments found that ox-LDL e

Peanut Allergy and the Basophil Activation Test in the Military Healthcare System: A Case Series of Monozygotic Twins.

Based on current standards for accession into the U.S. military, food allergy represents a significant barrier to military service. Skin prick and serum specific IgE testing exhibit high sensitivity but low positive predictive value, often leading to overdiagnosis when a confirmatory oral food challenge (OFC) is subsequently avoided because of concern for anaphylaxis in the clinic. Basophil activation testing (BAT) offers a unique advantage by measuring in vitro basophil responses to peanut allergen and can provide superior risk stratification for safe OFCs. We report the first clinical applic

Exploring the Immunomodulatory Effects of Agaricus bisporus Strains Using In Vitro and In Vivo Murine Models.

Background/Objectives: This study evaluated the immunomodulatory effects of multiple A. bisporus strains, applying both in vitro and in vivo murine models and aiming to identify specific strains with immune modulatory properties. Methods: Sixteen A. bisporus strains were screened in a dendritic cell (DC)-CD4+ T cell co-culture system, using cells from female C57BL/6 mice, for their capacity to induce Th1- and Th17-associated cytokine production. Based on these cytokine profiles, three potent strains (MES01856, MES01706, MES01637), one non-responder strain (MES01515), and the commercially avail

Outcomes of oral food challenge in children with IgE-mediated food allergy.

BACKGROUND: Oral food challenges (OFCs) are the gold standard for diagnosing IgE-mediated food allergies in children, although they carry an inherent risk of inducing allergic reactions, including anaphylaxis. This study examined clinical outcomes and predictors of positive OFC results in a pediatric cohort from a region with high exposure to sesame and tree nuts. METHODS: We retrospectively analyzed 310 OFCs in 211 children (median age: 21 months [IQR: 14-37.3]) at a tertiary pediatric allergy center from January 1 to December 31, 2023. Reaction patterns and predictors of OFC positivity, inc

Antibody therapeutics with high affinity for FcγRs exacerbate anaphylaxis via FcγR-mediated capture by tumor-associated myeloid cells.

BACKGROUND: Antibody therapeutics have revolutionized cancer treatment, but their use is increasingly associated with adverse events. Among these, anaphylaxis is particularly concerning due to its severity and unpredictability. Our previous studies demonstrated that repeated administration of anti-programmed death-ligand 1 antibodies to tumor-bearing mice induces antidrug antibodies (ADAs) and anaphylaxis. However, the specific characteristics of antibody therapeutics responsible for this effect and the underlying mechanism of ADA production remain poorly understood. This study aimed to identi

Allergy-associated acute coronary syndrome without anaphylaxis in a prospective observational study.

Kounis syndrome is an acute coronary syndrome (ACS) triggered by allergic reactions via mast cell activation, classically accompanied by anaphylaxis. However, ACS associated with mild or subclinical allergic reactions may be underrecognized. We investigated immune markers and autonomic nervous system activity in patients with ACS and characterized allergy-associated ACS without anaphylactic symptoms. Among 375 patients treated for ACS (April 2022 to March 2024), 49 were prospectively enrolled. Patients were classified into non-allergy-associated ACS (ACS-NA, n = 32) and allergy-associated ACS

Hereditary Alpha-Tryptasemia and Mastocytosis: What We Know and What We Need To Learn.

PURPOSE OF REVIEW: This review aims to elucidate the evolving relationship between hereditary alpha-tryptasemia (HαT) and mastocytosis, emphasizing recent advances in understanding shared pathophysiological mechanisms, diagnostic challenges, and clinical implications. RECENT FINDINGS: HαT is a common genetic trait caused by increased copy numbers of the TPSAB1 gene encoding alpha-tryptase. It accounts for most elevated serum baseline tryptase (SBT) levels (> 8 ng/mL) seen in clinical allergy practice. Diagnosis is established through tryptase genotyping—often referred to as “tryptase copy numb

Direct single dose mass delabelling of antibiotic allergy in pediatrics.

BACKGROUND: Unsubstantiated antibiotic allergy labels affect between 8% and 25% of the population worldwide. Current risk stratification tools, derived from adult data, are not validated for children. A simplified, multi-patient protocol with minimal exclusion criteria is required to tackle the scale of this public health issue. METHODS: Patients with possible antibiotic allergy were recruited from the Children's Health Ireland (CHI) allergy waiting list. Exclusion criteria were a serum sickness like reaction (SSLR), severe cutaneous adverse reaction (SCARs), anaphylaxis, or non-allergic symp

Pharmacological Treatments and Adverse Reactions Following Snake Antivenom Therapy: A Collaborative Study by Healthcare Professionals in the Southernmost Region of Thailand.

The administration of specific immunoglobulin G-based antivenoms is a key strategy for treating snakebite envenoming victims. However, serious adverse reactions, such as anaphylaxis or serum sickness, are frequently observed following such administration. In addition, inflammation associated with delayed wound healing considerably drives the irrational use of antibiotics or anti-inflammatory agents, which may be linked to adverse reactions following antivenom treatment. In this study, we evaluated the factors contributing to adverse effects following the administration of snake antivenom, espe

Differentiating Peanut Allergy from Sensitization in Polish Children: A Real-Life Diagnostic Model.

Peanut allergy (PA) remains a major diagnostic challenge in pediatric allergy, largely due to the frequent discrepancy between immunological sensitization and clinically relevant disease. This study aimed to develop a real-life diagnostic prediction model to distinguish true peanut allergy from asymptomatic peanut sensitization in children referred for evaluation of suspected PA. In this cross-sectional study, 80 children aged 1-18 years were assessed in a tertiary allergy center in Poland. Sixty-five children with peanut sensitization underwent detailed clinical history assessment, skin prick

If It Is Not Immunoglobulin E-Mediated Allergy, What Is It?

The term "allergy" is often used ambiguously, frequently applied to a broad spectrum of immune and non-immune reactions, which can create confusion in clinical practice. Whilst there is some controversy, the term "allergy" is now primarily reserved for describing immunoglobulin E (IgE)-mediated (Type I) hypersensitivity. Accurate diagnosis requires distinguishing Type I reactions from their mimics, especially given the clinical overlap with non-IgE-mediated conditions. This review focuses on Type I hypersensitivity, detailing its immunopathogenesis, clinical features, and diagnostic strategies

Venom Anaphylaxis.

Allergy to venoms of stinging insects (bees, yellow jackets, hornets, wasps, and stinging ants) causes large local reactions or systemic reactions, including anaphylaxis. Diagnostic evaluation includes skin testing and/or serum venom-specific IgE and basal serum tryptase (to identify underlying mast cell disorders). Patients with sting anaphylaxis should be prescribed epinephrine and venom immunotherapy which prevents allergic reactions in up to 98% of patients. Most patients may safely discontinue venom immunotherapy after 5 years, although high-risk patients need to continue indefinitely. Fu

Five-Year Real-World Outcomes of Hymenoptera Venom Immunotherapy: Clinical Effectiveness and Immunological Modifications.

Hymenoptera venom allergy is a cause of anaphylaxis, which significantly affects patients' daily lives due to the constant fear of accidental stings. Venom immunotherapy (VIT) is the only treatment capable of preventing severe systemic reactions (SSRs). Limited long-term real-life data are available, integrating both clinical and immunological outcomes. A five-year prospective observational study was conducted on 35 patients with a history of SSR who underwent VIT at a tertiary allergy center in Southern Italy; two of them had a diagnosis of systemic mastocytosis. Most patients were sensitized
