Hereditary alpha-tryptasemia demonstrates relative basophil enrichment without signs of cellular hyperreactivity.

BACKGROUND: Hereditary alpha-tryptasemia (HαT) is an autosomal dominant trait caused by increased tryptase alpha/beta 1 (TPSAB1) copy number, resulting in elevated serum tryptase levels. Although often asymptomatic, HαT is associated with anaphylaxis, flushing, and connective tissue abnormalities. Although mast cells are primarily implicated, basophil involvement in HαT remains poorly defined. OBJECTIVE: Our aim was to compare basophil proportions, MRGPRX2 expression, and responsiveness to IgE-dependent and IgE-independent activation in individuals with HαT, individuals with indolent systemic

High Salt Intake Exacerbates Food Allergy Symptoms by Impairing Intestinal Barrier Function and Activating Mucosal Mast Cells.

INTRODUCTION: Recent studies have shown that salt affects the differentiation and function of T helper cell subsets; however, its influence on food allergies and the underlying mechanisms remains unclear. This study aimed to clarify how high salt intake exacerbates allergic responses by examining its effects on immune function and intestinal barrier integrity in a food allergy mouse model. METHODS: Five-week-old female BALB/c mice were assigned to four groups: Control/AIN-93 G diet group (CA), Control/NaCl diet group (CN), Sensitization/AIN-93 G diet group (SA), and Sensitization/NaCl diet gr

Severe Reactions to Rituximab in Children: A Cohort Study of Rituximab-Induced Serum Sickness and Anaphylaxis.

BACKGROUND/OBJECTIVES: Severe infusion-related reactions to rituximab are rare; we aim to extend our knowledge about them in children, focusing on rituximab-induced serum sickness (RISS) and anaphylaxis. METHODS: We conducted a monocentric retrospective study on children and adolescents who received rituximab. Patients were defined as having RISS if they had fever and at least rash and/or arthralgia, 1 to 30 days following infusion, and without another diagnosis to explain symptoms. Anaphylaxis was defined according to the diagnostic criteria proposed by the World Allergy Organization. RESUL

Demonstration of Gliadin Penetration into the Epidermis by Tape Stripping as Prerequisite for Percutaneous Sensitisation in Wheat Allergy.

Sensitisation leading to food allergy may occur through the skin. Quantitative data on epidermal allergen penetration as a prerequisite for this remain limited. This study quantifies epidermal penetration of gluten and hydrolysed wheat protein (HWP) in 18 patients with challenge-confirmed WALDA (wheat allergy dependent on augmentation factors) and 12 healthy controls (HC). After 1 h of epicutaneous wheat application, 20 consecutive tape strips (TS) were collected, and wheat protein concentration was quantified by gliadin-specific ELISA. Skin barrier status was assessed by electrical impedance

Clinical Spectrum of Drug Hypersensitivity Reactions in Systemic Mastocytosis: Drug-Induced Anaphylaxis as a Unique Clinical Presentation.

Background and Objectives: Systemic mastocytosis (SM) is a clonal mast cell disorder characterized by abnormal mast cell accumulation and activation in multiple organs, leading to mediator-related symptoms, including anaphylaxis. Drug hypersensitivity reactions (DHRs) are a major clinical challenge in SM, but their frequency and characteristics remain undefined. This study aimed to evaluate the frequency of drug allergy, identify high-risk drug groups, investigate reaction characteristics, and examine the relationship between drug reactions, baseline serum tryptase levels, and SM subtypes in p

Patients with or without a clonal mast cell disease undergoing venom immunotherapy differ in their CD8 T memory cell response.

Hymenoptera venom immunotherapy (VIT) is usually effective in inducing long-term tolerance to the allergen. In patients with a clonal mast cell disease (CMD) and Hymenoptera venom-induced anaphylaxis, however, life-long VIT is recommended, as tolerance to the venom is often lost after VIT termination. We compared blood samples from patients with CMD and non-CMD with severe Hymenoptera venom-induced anaphylaxis before and more than 3 years after VIT to identify differences that might explain the lack of long-term tolerance in patients with CMD. Serum tryptase, IgE, IgG4, KIT D816V variation, as

Diagnostics and monitoring in Hymenoptera venom allergy: current and future perspectives.

Hymenoptera stings are a major trigger of IgE-mediated anaphylaxis, yet only a minority of exposed individuals develop systemic reactions, making accurate diagnosis essential but complex. This review synthesizes current evidence across clinical, serologic, cellular and molecular diagnostic modalities in Hymenoptera venom allergy. Clinical history and epidemiologic predictors-such as occupation, cumulative sting exposure, reaction latency, comorbidities and quality-of-life impairment, provide crucial context but lack sufficient predictive power when used alone. Skin prick and intradermal testin

Utility of the immunoglobulin E crosslinking-induced luciferase expression assay in buckwheat allergy diagnosis.

BACKGROUND: Buckwheat allergy carries a high risk of anaphylaxis. Although the oral food challenge (OFC) is the diagnostic gold standard, it can provoke severe reactions, leading many patients to adopt elimination diets. Conventional markers, including buckwheat-specific IgE (Bw-sIgE) and skin prick testing, have limited accuracy. The IgE crosslinking-induced luciferase expression (EXiLE) assay is a serum-based in vitro cell-activation test that has shown high accuracy for other food allergies. We evaluated its utility for buckwheat allergy. METHODS: This multicenter observational study in Ja

Eperisone-induced immediate hypersensitivity reactions.

Eperisone hydrochloride is a centrally acting muscle relaxant commonly prescribed for musculoskeletal pain, particularly in Korea and Japan. It induces relaxation of skeletal and vascular smooth muscle through central nervous system modulation and peripheral mechanisms, although its precise biochemical pathway remains unclear. While generally well tolerated, eperisone has been reported to cause immediate hypersensitivity reactions, including anaphylaxis. Because eperisone is frequently co-prescribed with non-steroidal anti-inflammatory drugs (NSAIDs), these reactions are often misdiagnosed as

A Case Report and Literature Review: Chlorhexidine-induced anaphylaxis in a hemodialysis patient.

Chlorhexidine is a widely used antiseptic in healthcare settings, particularly for catheter site preparation in hemodialysis patients. While generally considered safe, chlorhexidine can rarely cause severe IgE-mediated anaphylactic reactions. Here we report the case of a 47-year-old man on maintenance hemodialysis who experienced two episodes of severe allergic reactions during dialysis sessions. During the first episode the patient presented with dyspnea, hypotension, and pruritus, which we initially attributed to the dialyzer membrane reaction. After changing the dialyzer membrane, the patie

Adverse drug reactions associated with glycyrrhiza-containing Chinese patent medicines: a literature-based analysis of individual case reports and clinical trials.

BACKGROUND: Licorice, a commonly used traditional Chinese medicine in clinical practice, and pharmaceuticals with glycyrrhiza or glycyrrhiza extract as the main component play an important role in the treatment of hepatic and biliary system diseases and other conditions. Its dual identities as a traditional Chinese material medica and a natural phytocomponent have led to the underestimation of its latent adverse drug reactions (ADRs) in clinical and research. OBJECTIVE: This study adopted a retrospective literature analysis method and stratified subgroups based on the inherent attribute diffe

Acute Pancreatitis Associated With Anaphylactic Shock: A Case Report and Literature Review.

Acute pancreatitis is most commonly caused by gallstones or alcohol abuse, although rare etiologies such as drug-induced anaphylaxis have been described. Pancreatic involvement during anaphylactic shock remains exceptional and is likely underdiagnosed. We report the case of a 58-year-old woman who developed acute circulatory shock shortly after the topical application of a hemorrhoidal ointment (TriAnal). She presented with hypotension, tachycardia, pruritus, and erythematous urticarial lesions. Laboratory investigations revealed elevated serum lipase and tryptase. Abdominal computed tomograph

The REMA Score Revisited: An Optimized Algorithm for Early Prediction of Clonal Mast Cell Disease.

BACKGROUND: The Spanish Network on Mastocytosis (REMA) score was developed to predict clonal mast cell (MC) activation disorders (MCAD) in patients presenting with anaphylaxis without mastocytosis in the skin (MIS) and guide selection of patients for bone marrow (BM) examination. This study reassessed the diagnostic accuracy of the REMA score after 15 years of use and proposes refinements to optimize its predictive value. METHODS: Overall, 1204 patients with severe MC mediator-related symptoms without MIS referred to the REMA were retrospectively analyzed. All underwent clinical and allergolo

Identification of antibiotic allergies in children using prescribing patterns in the electronic medical record.

BACKGROUND: By age three, 5%-75% of children have a documented antibiotic allergy (AL). Despite this prevalence, recognition remains low because of limited clinician knowledge and time. Our study evaluates a novel approach to identifying AL using electronic medical record (EMR) prescription patterns. METHODS: An EMR report was developed to identify paediatric encounters where adrenaline and/or an antihistamine was administered within 48 h of the last antibiotic dose. The first 100 cases were reviewed to refine exclusion criteria. Exclusions included diagnoses of eczema, or rash; antihistamine

Refractory anaphylactic shock complicated by acute right heart failure during anesthetic induction in a patient with a left ventricular assist device: a case report.

BACKGROUND: Anaphylaxis in patients with a left ventricular assist device (LVAD) may critically reduce LVAD flow, particularly when the right ventricular function is impaired. CASE PRESENTATION: A 51-year-old man with a HeartMate III LVAD developed bronchospasm, profound hypotension, and decreased LVAD flow during anesthetic induction for endoscopic sinus surgery. The shock remained refractory despite epinephrine boluses and continuous infusions of epinephrine, norepinephrine, and vasopressin. Transesophageal echocardiography revealed severe right ventricular dilatation, leftward septal shift

Neutrophil-to-Lymphocyte Ratio in Pediatric Anaphylaxis: A Potential Indicator of Cardiovascular Involvement.

INTRODUCTION: The diagnostic utility of the neutrophil-to-lymphocyte ratio (NLR) in anaphylaxis is often obscured by the rapid immunomodulatory effects of epinephrine and corticosteroids. We evaluated NLR as a marker of reaction severity and clinical phenotype in pediatric anaphylaxis, employing a strict sampling protocol to minimize treatment-induced leukocytosis. METHODS: This retrospective cohort study analyzed 81 children meeting EAACI anaphylaxis criteria. To isolate the intrinsic inflammatory signal, blood samples were included only if obtained within 30 min of epinephrine and 1 h of sy
